Background: Palliative care focuses on the prevention of worsening health, improving the quality of the patient’s life, and the relief of suffering, and therefore has a considerable impact on both the patient suffering from a life-threatening or potentially life-threatening illness and on their family. Spirituality, as the dimension of human life involving the search for meaning, purpose, and transcendence, and connection with oneself, others, and the sacred, could be essential in supporting these patients. The aim of this study was to synthesise the scientific evidence describing the interventions and/or activities undertaken to meet the spiritual needs of the palliative patient. Methods: A literature search was carried out across the following databases: PubMed, LILACS, Scopus, and Web of Science. The PRISMA statement was used to guide this review. Results: Twenty-four articles were included. The thematic categories included spiritual needs at the end of life, the influence of music and dance as palliative care, care for family caregivers, and the comparison between counselling and dignity therapy. Conclusions: Interventions in the biopsychosocial–spiritual spheres impact on the patient’s peace of mind and promote the acceptance of a “good death”. Healthcare personnel play an essential role in the way their patients prepare for the moment of death, and the meaning and values they convey help them to accompany and welcome patients. Last but not least, universities can play a crucial role by training nurses to integrate spiritual interventions such as music and dance, or by considering the family as a unit of care. Introducción. El parto prematuro es un problema médico, social y económico importante, causa gran mortalidad y morbilidad neonatal, tiene un impacto importante en el sistema de salud y afecta la calidad de vida de las familias. El peso de los recién nacidos de madres con enfermedad periodontal es significativamente menor en comparación con los de madres no afectadas por esta enfermedad bucal. Este resultado adverso se considera un problema de salud pública global según los datos epidemiológicos. Objetivo. Determinar la asociación entre la prevalencia de parto prematuro y la enfermedad periodontal en Bolivia, Chile y Colombia entre el 2000 y el 2020. Materiales y métodos. Este estudio ecológico consideró las poblaciones de mujeres de Bolivia, Chile y Colombia, y la prevalencia de partos prematuros y enfermedad periodontal, discriminadas por grupos de edad. El estudio abarcó el período entre el 2000 y el 2020. La estrategia de búsqueda con la herramienta de investigación del Institute for Health Metrics and Evaluation incluyó prevalencia, grupos de edad, años entre 2000 y 2020, causas de parto prematuro y enfermedad periodontal, contexto y ubicaciones, mujeres y tasas. El análisis estadístico incluyó una regresión lineal simple entre parto prematuro y enfermedad periodontal para cada grupo de edad dentro de cada país. Resultados. Las tasas de partos prematuros fueron mayores en el grupo de 15 a 19 años (Bolivia: 697.563, Chile: 844.864, Colombia: 804.126). La prevalencia de la enfermedad periodontal aumentó con la edad, particularmente en el grupo de 45 a 49 años (Bolivia: 22’077.854, Chile: 34’297.901, Colombia: 32'032,830). Según los grupos de edad, la regresión lineal fue estadísticamente significativa (p < 0,001) para todos los grupos evaluados de la población boliviana, en los grupos mayores de 30 años para las colombianas y solo en el grupo de 15 a 19 años para las mujeres chilenas. Conclusión. Se encontró asociación entre el parto prematuro y la enfermedad periodontal en todos los grupos de edad en Bolivia, solo en el grupo de 15 a 19 años en Chile, y de 30 años y más en Colombia en el período evaluado de 20 años.

In clinical research, it is crucial to make an appropriate selection of the research design. Each type of design meets different objectives, from examining prevalences through cross-sectional designs to analyzing exposures over time in cohort studies. Research designs can be classified as primary, such as randomized clinical trials, or secondary, such as reviews. They are also divided into experimental, which evaluate interventions, and observational, which simply observe without intervening. When choosing the most appropriate design, the research question, available resources, and ethical considerations should be taken into account. A correct choice guarantees the validity and applicability of the results, which is beneficial for both medicine and patients.

Objetivos: Estudiar la evolución a lo largo plazo, así como los factores que influyen en la misma, de la población con fibrilación auricular no valvular (FANV) que ingresa en los servicios de Medicina Interna. Pacientes y método: Estudio observacional y retrospectivo de la evolución durante cinco años, de los pacientes ingresados con FANV en los servicios de Medicina Interna gallegos, entre enero-2016 y enero-2017. Para este fin se cuantificaron las visitas a urgencias, los ingresos hospitalarios y la supervivencia. Se estudiaron los factores que más influyeron en estas variables. Resultados: Se incluyeron 1342 pacientes y se realizó un seguimiento durante 5 años. Se contabilizaron 3691 ingresos hospitalarios, y 8687 visitas al servicio de urgencias (SU). Tuvieron una supervivencia del 66,6%, con una mediana de supervivencia de 1034,57 días. En el análisis univariante la edad, el tratamiento antitrombótico al alta y el índice de Barthel influyeron en la supervivencia, no así el sexo. En el análisis multivariante, el índice de Barthel fue la única variable independiente que influyó en la supervivencia. Conclusiones: Los pacientes con FANV ingresados en los servicios de Medicina Interna, suponen una subpoblación de alto riesgo de reingreso hospitalario y visitas al SU. Es necesario un cambio en el modelo de transición al alta y de seguimiento ambulatorio, mediante programas proactivos adaptados, capaces de reducir eventos hospitalarios y mejorar la calidad de vida de estos pacientes y sus cuidadores. Objectives: The objective of this study was to investigate the long-term evolution and influencing factors of patients with non-valvular atrial fibrillation (NVAF) admitted to internal medicine services. Patients and method: This is an observational and retrospective study of the evolution during five years of the patients admitted, between January-2016 and January-2017, with FANV in the Galician Internal Medicine services. For this end, it was quantified the emergency room visits, hospital admissions and survival. The factors with more influence over these variables were studied. Results: It was included 1342 patients and followed for 5 years. There were 3691 hospital admissions, and 8687 visits to the emergency department (ED). They had a survival of 66.6%, with a median survival of 1034.57 days. The univariate analysis found that age, antithrombotic treatment at discharge and Barthel's index influenced survival, but not sex. However, in the multivariate analysis only Barthel’s index was found to be independent variable that influence survival. Conclusions: Patients with NVAF admitted to internal medicine services constitute a subpopulation at high risk of hospital readmission and visits to the ED. A change in the model of transition to discharge and outpatient follow-up is necessary, through adapted proactive programs, capable of reducing hospital events and improving the quality of life of these patients and their caregivers.

La diabetes es una enfermedad metabólica de alta prevalencia internacional. La hiperglicemia y sus complicaciones derivadas son la principal característica de la diabetes. El desarrollo de fármacos para el control glicémico y sus complicaciones ha sido del interés de la comunidad científica por muchos años y las estrategias para obtener evidencia de los efectos biológicos de las sustancias y extractos de plantas son diversas. Los modelos murinos son herramientas experimentales que permiten comprender los mecanismos fisiopatológicos de la diabetes, así como para evaluar los efectos de compuestos obtenidos de fuentes naturales, así como sintéticos sobre la hiperglicemia, el estrés oxidativo, la inflamación y la cicatrización de heridas en ratas y ratones diabéticos. En esta revisión se describen los modelos murinos para el estudio de la diabetes más frecuentemente reportados en la literatura científica actual y las estrategias que en ellos se utilizan para llevar a cabo el estudio de la actividad biológica de extractos de plantas o compuestos sintéticos. Background: Type 2 diabetes (T2D) is a complex metabolic ailment marked by a global high prevalence and significant attention in primary healthcare settings due to its elevated morbidity and mortality rates. The pathophysiological mechanisms underlying the onset and progression of this disease remain subjects of ongoing investigation. Recent evidence underscores the pivotal role of the intricate intercellular communication network, wherein cell-derived vesicles, commonly referred to as extracellular vesicles (EVs), emerge as dynamic regulators of diabetes-related complications. Given that the protein cargo carried by EVs is contingent upon the metabolic conditions of the originating cells, particular proteins may serve as informative indicators for the risk of activating or inhibiting signaling pathways crucial to the progression of T2D complications. Methods: In this study, we conducted a systematic review to analyze the published evidence on the proteome of EVs from the plasma or serum of patients with T2D, both with and without complications (PROSPERO: CRD42023431464). Results: Nine eligible articles were systematically identified from the databases, and the proteins featured in these articles underwent Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis. We identified changes in the level of 426 proteins, with CST6, CD55, HBA1, S100A8, and S100A9 reported to have high levels, while FGL1 exhibited low levels. Conclusion: These proteins are implicated in pathophysiological mechanisms such as inflammation, complement, and platelet activation, suggesting their potential as risk markers for T2D development and progression. Further studies are required to explore this topic in greater detail.

Abstract Background Falls are among the most common and serious adverse events for hospitalised patients. In-hospital falls pose a major medical and economic challenge for public health worldwide. Nevertheless, the issue is often addressed without regard to certain relevant variables such as the time of the fall. The aim of this study was to determine the effect of the implementation of a nurse-led intervention based on the temporal patterns of falls and their aetiology on the occurrence of falls. Methods A mixed-method research design was carried out in three phases: a) a longitudinal prospective study (audits, chronobiological analyses and implementation of a multicentre nurse-led intervention based on temporal patterns of falls); b) a retrospective study of fall records; and c) a qualitative study based on focus groups. Piedra de sol (1957) del poeta mexicano Octavio Paz es un poema extenso. Desde su publicación hasta la actualidad ha sido objeto de diversas lecturas. En el presente artículo se hace un análisis del poema y, sobre todo, una descripción, análisis y clasificación de las lecturas aportadas por la crítica especializada con el objetivo de trazar una directriz que explique la lógica y evolución de tales interpretaciones. Se utilizan los conceptos teóricos de inmanencia y trascendencia para exponer cronológicamente aquellas lecturas que observan en el texto de Paz un manifiesto poético de una estructura verbal o un discurso (filosófico, religioso, antropológico, etcétera) extratextual, o la fusión de estas dos lecturas. Todo esto para proponer que la noción de ritmo (encontrada en las últimas lecturas del poema) es una clave analítica e interpretativa que cohesiona lo inmanente y lo trascendente, y representa, por lo tanto, un punto cúspide de la crítica.

Para los fines de esta investigación científica, en este caso cuasi experimental, el método comunicativo o CLT fue utilizado, con el fin de lograr que los alumnos del grupo experimental puedan incrementar significativamente el nivel de conversación en inglés. Al final del estudio, otras áreas del inglés también mejoraron, tales como: lectura, escritura, gramática y vocabulario. En lo referente a la población, se dividió en dos grupos: control y experimental. Cada grupo estuvo conformado de 10 alumnos, los integrantes de ambos grupos tenían, originalmente, un nivel de inglés de pre-alfabetización (‘pre-literacy’, en inglés), es decir que sus niveles eran inferior a básico; además es importante mencionar, que todos los participantes fueron adultos profesionales de postgrado de maestría en Gerencia de Proyectos y Programas Sociales Ciclo III 2018- I de la Universidad Nacional Santiago Antúnez de Mayolo - UNASAM, Huaraz-2018. Es primordial enfatizar que los alumnos del grupo experimental priorizan la comunicación de forma oral, es decir habla, y a la vez el de comprender, es decir escucha, para así poder interactuar con profesionales extranjeros los cuales utilizan estas dos habilidades del inglés en sus centros de trabajo. Por otro lado, el grupo de control necesita lograr aprobar el examen de suficiencia de inglés de su universidad para poder graduarse. Tomando en cuenta las razones mencionadas líneas arriba, se logró diseñar un programa de 16 sesiones, el cual fue utilizado para lograr el objetivo esperado; es decir, aprender el idioma inglés básico para al final lograr una diferencia significativa en: habla y escucha en el grupo experimental. Finalmente, el resultado fue exitoso, el estudio logro el objetivo de acuerdo a la hipótesis planteada.

BackgroundInappropriate medication in older patients with multimorbidity results in a greater risk of adverse drug events. Clinical decision support systems (CDSSs) are intended to improve medication appropriateness. One approach to improving CDSSs is to use ontologies instead of relational databases. Previously, we developed OntoPharma—an ontology-based CDSS for reducing medication prescribing errors. ObjectiveThe primary aim was to model a domain for improving medication appropriateness in older patients (chronic patient domain). The secondary aim was to implement the version of OntoPharma containing the chronic patient domain in a hospital setting. MethodsA 4-step process was proposed. The first step was defining the domain scope. The chronic patient domain focused on improving medication appropriateness in older patients. A group of experts selected the following three use cases: medication regimen complexity, anticholinergic and sedative drug burden, and the presence of triggers for identifying possible adverse events. The second step was domain model representation. The implementation was conducted by medical informatics specialists and clinical pharmacists using Protégé-OWL (Stanford Center for Biomedical Informatics Research). The third step was OntoPharma-driven alert module adaptation. We reused the existing framework based on SPARQL to query ontologies. The fourth step was implementing the version of OntoPharma containing the chronic patient domain in a hospital setting. Alerts generated from July to September 2022 were analyzed. ResultsWe proposed 6 new classes and 5 new properties, introducing the necessary changes in the ontologies previously created. An alert is shown if the Medication Regimen Complexity Index is ≥40, if the Drug Burden Index is ≥1, or if there is a trigger based on an abnormal laboratory value. A total of 364 alerts were generated for 107 patients; 154 (42.3%) alerts were accepted. ConclusionsWe proposed an ontology-based approach to provide support for improving medication appropriateness in older patients with multimorbidity in a scalable, sustainable, and reusable way. The chronic patient domain was built based on our previous research, reusing the existing framework. OntoPharma has been implemented in clinical practice and generates alerts, considering the following use cases: medication regimen complexity, anticholinergic and sedative drug burden, and the presence of triggers for identifying possible adverse events.

Con el objetivo de estudiar algunos problemas de ética relacionados con situaciones clínicas que involucran a la familia, se presenta una revisión sistemática de este término para entenderla y definirla. Concebida como un cuerpo orgánico en su constitución, estructura y organización, en el presente artículo se analizan las variadas circunstancias en las que existe una relación entre la ética clínica y la familia, principalmente haciendo énfasis a la diferencia entre la autonomía individual y la autoridad familiar.

The study, synthesis, and application of nanomaterials in medicine have grown exponentially in recent years. An example of this is the understanding of how nanomaterials activate or regulate the immune system, particularly macrophages. In this work, nanoparticles were synthesized using Rumex hymenosepalus as a reducing agent (AgRhNPs). According to thermogravimetric analysis, the metal content of nanoparticles is 55.5% by weight. The size of the particles ranges from 5–26 nm, with an average of 11 nm, and they possess an fcc crystalline structure. The presence of extract molecules on the nanomaterial was confirmed by UV-Vis and FTIR. It was found by UPLC-qTOF that the most abundant compounds in Rh extract are flavonols, flavones, isoflavones, chalcones, and anthocyanidins. The viability and apoptosis of the THP-1 cell line were evaluated for AgRhNPs, commercial nanoparticles (AgCNPs), and Rh extract. The results indicate a minimal cytotoxic and apoptotic effect at a concentration of 12.5 μg/mL for both nanoparticles and 25 μg/mL for Rh extract. The interaction of the THP-1 cell line and treatments was used to evaluate the polarization of monocyte subsets in conjunction with an evaluation of CCR2, Tie-2, and Arg-1 expression. The AgRhNPs nanoparticles and Rh extract neither exhibited cytotoxicity in the THP-1 monocyte cell line. Additionally, the treatments mentioned above exhibited anti-inflammatory effects by maintaining the classical monocyte phenotype CD14++CD16, reducing pro-inflammatory interleukin IL-6 production, and increasing IL-4 production.

The fruits of the nopal (Opuntia spp.), cactus pears, are considered functional foods due to their content of nutritive and bioactive substances. Its pulp is generated by numerous seeds that limit their consumption due to their size and hardness and detract from their quality. Other undesirable fruit characteristics are its inedible peel and its short shelf life. In the case of the cactus pear cactus (Opuntia spp.), no cultivar has been reported that produces quality parthenocarpic fruits, nor have they been obtained by artificially inducing parthenocarpy. The objectives of this research were to evaluate the response of 11 genotypes to the induction of parthenocarpy, to characterize the fruits of the genotypes with the best response, and to determine their postharvest life. To induce parthenocarpy, floral buds were used in the pre-anthesis stage, from which the style-stigma, stamens, and tepals were removed, generating a cavity in which a solution of growth regulators was applied (250 mg L−1 AG3 + 75 mg L −1 BA + 15 mg L−1 of AIB). A similar number of buds was used as a control, without treatment, and in free pollination. Only two genotypes, MX CP-30 Red and MX CP-40 Yellow, produced parthenocarpic fruits since their empty integuments produced pulp, remaining small, empty, and soft. Their peel was sweet (10–14 °Brix) and edible, and they had a longer shelf life than the corresponding fruits with seeds.

Objetive: To compare the stresses and deformations generated on the surrounding bone of the zygomatic implants when using an intra sinusal and extra-maxillary approach, through the finite element method. Material and Methods: Computer aided designs (CADs) were constructed using SolidWorks Software of a skull with bone resorption to be rehabilitated through a fixed hybrid prosthesis using two zygomatic and two conventional straight implants. For the boundary conditions (load conditions), symmetry in the sagittal plane was assumed and that all the materials were isotropic, homogeneous and linearly elastic. Two zygomatic implantation techniques were simulated: intra sinusal (Is) and extra maxillary (Em). Vertical and lateral loads of 150 N and 50 N were applied to the finite element models to obtain Von Mises equivalent stress and strain (displacement). Results: The average measurement of the Von Mises stress (MPa) recorded were as follows: Approach of the implant body (Is: 0.24- Em: 0.28,) effort of implant body with vertical load: Is: 0.69 - Em: 0.96; effort of peri-implant surface under horizontal load: Is: 2.11 - Em: 0.94. Average displacement under vertical load of peri-implant surface Is: 0.35 - Em: 0.40, and of implant body Is: 1.34 - Em: 2.04. Average total deformation in approach Is: 2.23 mm - Em: 0.80mm, and average total deformation in the implant body under horizontal load was Is: 0.14 - Em: 0.21. Conclusion: The results of this study indicate that despite the differences that occurred in both stress and strain (displacement) between the intra-sinus and extra-maxillary approaches, the static strength of the bone, which is approximately 150 MPa in tension and 250 MPa in compression was not exceeded. Considering the limitations of finite element analysis, there seems to be no biomechanical reason to choose one approach over the other.

El curso de Habilidades Informacionales de Bibliotecas se inserta como un taller de tres lecciones dentro de un curso de corte investigativo en la malla de Teología, cuyo fin es desarrollar en el estudiante competencias informacionales e incorporarlas al trabajo de investigación realizado durante el curso. Como resultado, se ha observado en los estudiantes una mejora tanto en la búsqueda de información como en el uso y manejo de gestores de referencias bibliográficas, y un conocimiento/aplicación de normas específicas de citación. En los académicos, se valora el uso de las competencias informacionales en su quehacer docente, así como la sistematización del trabajo colaborativo con el bibliotecólogo en el desarrollo del curso. Se observa además un incremento en la demanda de consultas recibidas por el bibliotecólogo de la Biblioteca de Teología. Se espera realizar un análisis de los resultados para evaluar su alcance y pertinencia en el currículo de Teología, en aras de fomentar un perfil de investigador en los egresados.

Early life history traits of small pelagic fish are usually affected by environmental conditions. Patagonian sprat, Sprattus fuegensis, is a small pelagic fish that inhabits the continental shelf, fjords and channels of Patagonia. Their larvae are usually exposed to large variations in physical and biological conditions. We evaluated the effects of environmental conditions on early life history traits of this key species encompassing over 20 years of information. The characterization of oceanographic features, larval abundance and distribution was carried out in western Patagonia between 1996 and 2019, while the growth and mortality rates were estimated for the austral spring 2019 between 49°54′S and 53°55′S. Larval abundance decreased three to ten times from 1996 to 1997 to the rest of the study period (2008–2010 and 2014–2019), with larger abundances being found over the shelf. In spring 2019, the growth rate was 0.20 ± 0.05 mm d−1 for larvae between 9 and 19 days after hatching. The growth rate was similar in partially stratified (7–50 J m−3) and stratified waters (50–100 J m−3), but slower in mixed waters (< 7 J m−3). GAM modelling showed that the faster larval growth occurred in partially stratified waters with temperatures above 7.5°C. Natural mortality rates were higher in partially stratified waters (24% of daily loss), lower in stratified areas (14%) and the lowest in the mixed water column (12%). Accordingly, larvae living in channels had faster growth rates but higher daily mortality, while those that hatched or were advected to water parcels on the continental shelf had slower growth but lower daily mortality. Background: Intensive care unit (ICU) patients are at an increased risk of ocular surface injuries because of various factors such as reduced tear production and impaired protective mechanisms. Despite the significance of ocular care in ICU settings, there is a lack of consensus on effective interventions, leading to inadequate prevention of ocular surface disease (OSD). Aim: This systematic review aimed to assess the effectiveness of nursing eye care in preventing OSD in ICU patients. Secondary objectives included identifying primary risk factors for ocular injuries and examining the most effective preventive methods. Study Design: A systematic review following PRISMA guidelines was conducted, encompassing a literature search, article selection, quality assessment and data synthesis. Studies meeting inclusion criteria were observational studies and clinical trials, focusing on adults admitted to ICUs under sedation and receiving mechanical ventilation. Results: Of 3545 initially identified articles, 12 studies met inclusion criteria. These studies involved a total of 1853 participants. Various interventions were assessed, including saline rinsing, lubricating drops, gel lubricants, occlusion with polyethylene dressing, passive blinking and eyelid closure with tape. Moist chamber occlusion every 6 h combined with gel lubrication emerged as the most effective method in preventing OSD. Conclusions: Gel lubrication along with moist chamber occlusion proved to be the most effective strategy in preventing ocular injuries in ICU patients. Conversely, the routine use of physiological saline was associated with increased severity of corneal lesions. Properly defined protocols and well‐trained nursing teams are crucial for reducing ocular injuries in ICU settings. Relevance to Clinical Practice: The findings underscore the importance of implementing evidence‐based eye care protocols in ICUs, emphasizing the use of gel lubrication and ocular surface protection to mitigate the risk of OSD. This highlights the need for comprehensive training programmes for ICU nursing staff to ensure optimal ocular care delivery. [ABSTRACT FROM AUTHOR]

Background: Palliative care focuses on the prevention of worsening health, improving the quality of the patient's life, and the relief of suffering, and therefore has a considerable impact on both the patient suffering from a life-threatening or potentially life-threatening illness and on their family. Spirituality, as the dimension of human life involving the search for meaning, purpose, and transcendence, and connection with oneself, others, and the sacred, could be essential in supporting these patients. The aim of this study was to synthesise the scientific evidence describing the interventions and/or activities undertaken to meet the spiritual needs of the palliative patient. Methods: A literature search was carried out across the following databases: PubMed, LILACS, Scopus, and Web of Science. The PRISMA statement was used to guide this review. Results: Twenty-four articles were included. The thematic categories included spiritual needs at the end of life, the influence of music and dance as palliative care, care for family caregivers, and the comparison between counselling and dignity therapy. Conclusions: Interventions in the biopsychosocial–spiritual spheres impact on the patient's peace of mind and promote the acceptance of a "good death". Healthcare personnel play an essential role in the way their patients prepare for the moment of death, and the meaning and values they convey help them to accompany and welcome patients. Last but not least, universities can play a crucial role by training nurses to integrate spiritual interventions such as music and dance, or by considering the family as a unit of care. En este artículo se analizan algunos textos del poeta y ensayista mexicano Gilberto Owen, específicamente “Poesía —¿pura?— plena. Ejemplo y sugestión” y “Poesía y Revolución (Introducción a un Baedeker de la poesía mexicana actual, [Escrito en 1931])”. Se propone que tales textos son dos núcleos en el pensamiento literario y político de Owen, pues permiten observar cómo el autor se halla en medio del cosmopolitismo y el nacionalismo. Además, se conceptualiza a la generación de escritores mexicanos llamada Contemporáneos, a la cual Owen perteneció y de la cual es tanto una muestra representativa como un caso sui generis. This article examines two texts by Mexican poet and essayist Gilberto Owen: “Poesía —¿pura?— plena. Ejemplo y sugestión” and “Poesía y Revolución (Introducción a un Baedeker de la poesía mexicana actual, [Escrito en 1931])”. I suggest that these texts are two nuclei in Owen’s literary and political thought, since they allow us to observe how the author is in the midst of cosmopolitanism and nationalism. In addition, this article approaches Mexican literary generation known as Contemporáneos, to which Owen belonged and of which he is both a representative and a sui generis case.
