Objective: Sphincter of Oddi dysfunction (SOD) has been used to describe patients with RUQ abdominal pain without an etiology. We conducted a systematic review and meta-analysis to evaluate the efficacy and safety of ES (endoscopic sphincterotomy) for SOD., Methods: The study methodology follows the PRISMA guidelines. A comprehensive search was conducted using MEDLINE and EMBASE databases for RCTs with ES in patients with SOD. The primary outcome assessed was the improvement of abdominal pain after ES/sham. A random effects model was used to calculate pooled estimates for each outcome of interest., Results: Of the initial 55 studies, 23 were screened and thoroughly reviewed. The final analysis included 3 studies. 340 patients (89.7% women) with SOD were included. All patients had a cholecystectomy. Most included patients had SOD type II and III. The pooled rate of technical success of ERCP was 100%. The average clinical success rate was 50%. The pooled cumulative rate of overall AEs related to all ERCP procedures was 14.6%. In the sensitivity analysis, only one study significantly affected the outcome or the heterogeneity., Conclusion: ES appears no better than placebo in patients with SOD type III. Sphincterotomy could be considered in patients with SOD type II and elevated SO basal pressure., (© 2023. Nonalcoholic fatty liver disease (NAFLD) affects nearly one third of the population worldwide. Mexico is one of the countries whose population has several risk factors for the disease and its prevalence could surpass 50%. If immediate action is not taken to counteract what is now considered a national health problem, the medium-term panorama will be very bleak. This serious situation prompted the Asociación Mexicana de Gastroenterología and the Asociación Mexicana de Hepatología to produce the Mexican Consensus on Fatty Liver Disease. It is an up-to-date and detailed review of the epidemiology, pathophysiology, clinical forms, diagnosis, and treatment of the disease, whose aim is to provide the Mexican physician with a useful tool for the prevention and management of nonalcoholic fatty liver disease., (Copyright © 2019 Asociación Mexicana de Gastroenterología. Introduction and Aims: In Mexico, complications of cirrhosis are the third leading cause of death in adult males. In recent decades, the incidence of hepatocellular carcinoma has increased worldwide. The aim of this study was to determine the characteristics of patients with hepatocellular carcinoma at two Mexican tertiary care hospitals., Material and Methods: An observational, cross-sectional, retrospective study was conducted between January 2008 and April 2014. We described the clinical features, epidemiologic characteristics, diagnosis, and treatment of patients with hepatocellular carcinoma., Results: One hundred and forty-eight patients were included. There was a predominance in males and disease manifestation in the sixth decade of life. Liver disease was associated in 87% of subjects and was mainly attributed to alcohol abuse, hepatitis C infection, and nonalcoholic steatohepatitis. Sixty percent (60%) of cases were classified as Child-Pugh stage A cirrhosis, 75.5% harbored a single tumor at diagnosis, 27.7% had normal alpha-fetoprotein values, and only 39.2% of patients with known liver disease were under a surveillance program. Tumors were larger than 5cm at diagnosis in 64.3% of patients, and well-differentiated lesions were most frequently detected. Over 70% of patients were diagnosed at a non-curative stage. By the 2014 study cutoff point, 77.7% of patients had died. Treatment was determined by the means available at each center and followed the therapeutic recommendations in international guidelines in 45.3% of cases, clearly impacting survival., Conclusions: Better surveillance methods are required to diagnose the disease at its early stages, but treatment still requires individual adaptation to each center's available resources., (Copyright © 2018 Asociación Mexicana de Gastroenterología.
