Abstract

Summary Livers from donation after circulatory death (DCD) donors are an increasingly more common source of organs for transplantation. While there are few high‐level studies in the field of DCD liver transplantation, clinical practice has undergone progressive changes during the past decade, in particular due to mounting use of postmortem normothermic regional perfusion (NRP). In Spain, uncontrolled DCD has been performed since the late 1980s/early 1990s, while controlled DCD was implemented nationally in 2012. Since 2012, the rise in DCD liver transplant activity in Spain has been considerable, and the great majority of DCD livers transplanted in Spain today are recovered with NRP. A panel of the Spanish Liver Transplantation Society was convened in 2018 to evaluate current evidence and accumulated experience in DCD liver transplantation, in particular addressing issues related to DCD liver evaluation, acceptance criteria, and recovery as well as recipient selection and postoperative management. This panel has created a series of consensus statements for the standard of practice in Spain and has published these statements with the hope they might help guide other groups interested in implementing new forms of DCD liver transplantation and/or introducing NRP into their clinical practices.

More experience needs to be accumulated in order to conclude whether transplanted patients represent or not a group of risk for SARS-CoV-2 infection and determine the best way to handle immunosuppression treatment. Abstract

El trasplante hepático constituye hoy en día el único tratamiento definitivo para muchas de las enfermedades hepáticas graves, algunas de ellas mortales en un gran número de casos, como la Atresia de vías biliares, el Síndrome de Alagille, la colestasis familiar intrahepática, los tumores hepáticos, y otras enfermedades crónicas del hígado. El desarrollo de programas de trasplante hepático ha incrementado no sólo la supervivencia, sino también la mejora de la calidad de vida de estos pacientes. Las alteraciones bucodentales, aunque no graves, tienen repercusiones por las molestias y las alteraciones tanto estéticas como funcionales y psicológicas que pueden producir en el paciente odontopediátrico. Esta tesis doctoral estudia el tratamiento y seguimiento de estos pacientes, prestando especial atención a las necesidades odontológicas, tanto antes como después del procedimiento quirúrgico, y a las complicaciones que a corto, medio y largo plazo puedan surgir debido a la instauración de un tratamiento inmunosupresor durante toda la vida3. Se estudian las hepatopatías susceptibles de trasplante hepático, las condiciones de higiene oral en estos pacientes, y las manifestaciones bucodentales específicas de algunas de estas enfermedades, tales como hipoplasias dentarias y tinciones entre otras, así como las alteraciones que algunos fármacos pueden producir, como agrandamiento gingival y retrasos en la erupción. Para ello se ha estudiado un grupo de pacientes infantiles sometidos a trasplante de hígado en un hospital de referencia nacional y se han comparado su estado de salud bucodental con el existente en un grupo de niños sanos. Este trabajo ha concluido que existen lesiones orales específicas de estos niños, tanto debidas a patologías previas al trasplante, como al tratamiento inmunosupresor instaurado, prestando especial atención a los efectos secundarios de algunos de ellos. Abstract

Aim:  The onset of inflammatory bowel disease (IBD) in patients younger than the age of 5 years is rare. Moreover, when there is exclusive colonic inflammation, diagnosis can be challenging. Our aim was to describe the difficulties and pitfalls in the diagnosis of early-onset IBD (eoIBD) and their repercussions in treatment decisions.Patients and Methods:  Patients with eoIBD (Crohn's disease [CD], ulcerative, and unclassified colitis) treated at our center between 1990 and 2016 were studied retrospectively. Demographic, clinical, medical, and surgical treatment data were analyzed.Results:  A total of 19 patients were diagnosed with eoIBD at 21 (1-46) months of age. Bloody diarrhea, growth failure, and abdominal pain were present in 90, 42, and 16%, respectively. After 9 (1-62) months from the onset, patients were classified as ulcerative colitis (nine), CD (two), and unclassified colitis (eight). Unresponsiveness to medical treatment was observed in 10 and prompted surgical assessment. A partial colectomy was performed in one, and nine underwent a total colectomy (one end stoma and eight ileoanal anastomosis [IAA]). At least one surgical complication occurred in 80% and ultimately six patients with an IAA required an end stoma. Overtime, final diagnosis of 5 out of the 10 surgical patients changed due to biopsy findings, unresponsiveness to medical treatment, or extraintestinal and perianal manifestations. After a 12-year (1-22) follow-up, 57.9% of the diagnoses of all patients were modified.Conclusion:  EoIBD poses a challenge due to ambiguous presentation and absence of specific diagnostic tests. Surgical evaluation is often needed and surgeons must be mindful of possible initial misdiagnosis, in addition to short- and long-term outcomes before deciding aggressive surgical measures as well as intestinal reconstruction. Abstract

Cholestasis constitutes one of the most common and severe manifestations of acquired or inherited liver disease. When manifest in early infancy, it is often life-threatening and usually requires surgical management. In many cases, liver transplantation is the only effective therapy. Extensive knowledge about the molecular mechanisms underlying several pediatric cholestatic disorders has been gained in recent years from studies in both experimental models and clinical forms. In this review, we focus on recent contributions to the knowledge of molecular basis of main pediatric cholestatic disorders, such as biliary atresia, Alagille syndrome, and familial intrahepatic cholestasis. For some of them, putative targets of therapeutic interest, such as interferon-γ and Farnesoid X receptor, have been proposed. [Copyright &y& Elsevier]
