Abstract

The most severe complications in transarterial closure of the ductus arteriosus persistens are problems with placing the plug into the ductus (misfit plug size/ductus size) and the flow of closure materials into the aorta after instable fixation of the plug. In such cases surgical intervention by arteriotomy or aortotomy for the removal of the plug becomes necessary. Transvenous dilatation of the ductus arteriosus persistens after recognizing unstable plus positions or difficulties in fixing the plug prevents the mentioned complications.

Abstract

Safe and permanent closure of patent ductus arteriosus (PDA) can be accomplished with an ivalon plug introduced by catheterization from the femoral artery. The authors' experience thus far obtained from treating more than a hundred patients suggests that the technique described in this paper is a feasible alternative to the surgical approach. A prerequisite has been the size and shape of the PDA, the lumen of which had to be conical in shape and smaller than the lumen of the femoral artery. Now safe closure of oversized PDA complicated by pulmonary hypertension has become possible, using a plug designed with a special mechanism for anchorage to the PDA. The technique of this closure is described in detail in this report of its successful use in a 50-year-old woman patient with an oversized, calcified PDA complicated by pulmonary and systemic hypertension and cystic lung disease. Treating such high-risk cases with transfemoral closure means real improvement in therapy over surgical treatment.

Abstract

In 16 patients suffering from occlusions of the pelvic and/or femoral arteries a combined treatment was performed: percutaneous transluminal angioplasty (PTA) and surgical reconstructive measures. Indications for this procedure are: 1. Stenoses of the pelvic arteries and occlusion of the ipsilateral superficial femoral artery with PTA for the iliac artery and endarterectomy for the femoral artery. 2. Occlusion of the iliac and the femoral arteries with a decreased run-off: PTA and lumbar sympathectomy. 3. Endarterectomy on one side and PTA contralaterally.
