Endosalpingiosis is a condition that causes the non-neoplastic proliferation of ectopic tubal epithelium. Florid cystic endosalpingiosis is an atypical subtype that is very rarely reported. It presents as a mass-like feature and therefore needs to be differentiated from tumorous conditions. Here, we report the imaging findings of a case of multicentric florid cystic endosalpingiosis in the extraperitoneal pelvic cavity and the retroperitoneal spaces.

There has been no report about aortic dissection due to cardiopulmonary resuscitation (CPR). We present here a case of acute aortic dissection as a rare complication of CPR and propose the potential mechanism of injury on the basis of transesophageal echocardiographic observations. A 54-year-old man presented with cardiac arrest after choking and received 19 minutes of CPR in the emergency department. Transesophageal echocardiography (TEE) during CPR revealed a focal separation of the intimal layer at the descending thoracic aorta without evidence of aortic dissection. After restoration of spontaneous circulation, hemorrhagic cardiac tamponade developed. Follow-up TEE to investigate the cause of cardiac tamponade revealed aortic dissection of the descending thoracic aorta. Hemorrhagic cardiac tamponade was thought to be caused by myocardial hemorrhage from CPR.

4. A tool to measure the impact of inaction toward elimination of hepatitis C: A case study in Korea.

BACKGROUND AND AIMS:Hepatitis C virus (HCV) and its sequelae present a significant source of economic and societal burden. Introduction of highly effective curative therapies has made HCV elimination attainable. The study used a predictive model to assess the clinical and economic impact of implementing national screening and treatment policies toward HCV elimination in Korea. METHODS:A previously validated Markov disease progression model of HCV infection was employed to analyze the clinical and economic impact of various strategies for HCV diagnosis and treatment in Korea. In this analysis, the model compared the clinical and economic outcomes of current HCV-related interventions in Korea (7,000 patients treated and 4,200 patients newly diagnosed annually, starting in 2017) to four elimination scenarios: 1) initiating sufficient diagnosis and treatment interventions to meet the World Health Organization's GHSS elimination targets by 2030, 2) delaying initiation of interventions by one year, 3) delaying initiation of interventions by two years and 4) accelerating initiation of interventions to meet elimination targets by 2025. Modelled historical incidence of HCV was calibrated to match a viremic HCV prevalence of 0.44% in 2009. Elimination scenarios required 24,000 treatments and 34,000 newly diagnosed patients annually, starting in 2018, to reach the 2030 targets. RESULTS:Compared to current "status quo" interventions, elimination (or accelerated elimination by 2025) would avert 23,700 (27,000) incident cases of HCV, 1,300 (1,400) liver-related deaths (LRDs) and 2,900 (3,100) cases of end-stage liver disease (ESLD) over the 2017-2030 time period. Postponing interventions by one (or two) years would avert 21,100 (18,600) new HCV infections, 920 (660) LRDs and 2,000 (1,400) cases of ESLD by 2030. Following elimination or accelerated elimination strategies would save 860 million USD or 1.1 billion USD by 2030, respectively, compared to the status quo, requiring an up-front investment in prevention that decreases spending on liver-related complications and death. CONCLUSIONS:By projecting the impact of interventions and tracking progress toward GHSS elimination targets using modelling, we demonstrate that Korea can prevent significant morbidity, mortality and spending on HCV. Results should serve as the backbone for policy and decision-making, demonstrating how aggressive prevention measures are designed to reduce future costs and increase the health of the public.

5. Clinical outcomes of adverse cardiovascular events in patients with acute dapsone poisoning

Objective Adverse cardiovascular events (ACVEs) account for a large proportion of the morbidities and mortalities associated with drug overdose emergencies. However, there are no published reports regarding outcomes of ACVEs associated with acute dapsone poisoning. Here, the authors retrospectively analyzed ACVEs reported within 48 hours of treatment in patients with acute dapsone poisoning and assessed the significance of ACVEs as early predictors of mortality. Methods Sixty-one consecutive cases of acute dapsone poisoning that were diagnosed and treated at a regional emergency center between 2006 and 2014 were included in the study. An ACVE was defined as myocardial injury, shock, ventricular dysrhythmia, cardiac arrest, or any combination of these occurring within the first 48 hours of treatment for acute dapsone poisoning. Results Nineteen patients (31.1%) had evidence of myocardial injury (elevation of serum troponin-I level or electrocardiography signs of ischemia) after dapsone overdose, and there were a total of 19 ACVEs (31.1%), including one case of shock (1.6%). Fourteen patients (23.0%) died from pneumonia or multiple organ failure, and the incidence of ACVEs was significantly higher among non-survivors than among survivors (64.3% vs. 21.3%, P=0.006). ACVE was a significant predictor of mortality (odds ratio, 5.690; 95% confidence interval, 1.428 to 22.675; P=0.014). Conclusion The incidence of ACVE was significantly higher among patients who died after acute dapsone poisoning. ACVE is a significant predictor of mortality after dapsone overdose, and evidence of ACVE should be carefully sought in these patients.

Although respiratory infections, such as pneumonia, have long been recognized as precipitators of exacerbation in patients with acute decompensated heart failure (ADHF), identifying signs of concomitant pneumonia in ADHF is a clinical diagnostic challenge. We evaluated the predictive value of the delta neutrophil index (DNI), a new indicator for immature granulocytes, for diagnosing superimposed pneumonia in patients presenting with ADHF in the emergency department (ED).This was a retrospective and observational study of consecutive patients (>18 years old) diagnosed with an ADHF in the ED over a 7-month period. Patients were categorized into either the ADHF group or the ADHF with pneumonia group. DNI, serum white blood cell (WBC), C-reactive protein (CRP), and β-natriuretic peptide (BNP) were measured upon ED arrival.The ADHF with pneumonia group included 30 patients (20.4%). We have investigated hydrogen plasma induced by intense extreme ultraviolet (EUV) radiation by measuring the plasma parameters and population density of radicals, which is expected to clean the tin contamination on the optics in EUV sources, using a time-resolved optical emission spectroscopy. The obtained electron density was n e = (2 ± 0.4) × 1013 cm−3, and the electron temperature was T e = 1 ± 0.2 eV. The electron density was five orders of magnitude higher than that of previous study. The radical population density determined by the experiment was consistent with that calculated with a collisional radiative model, showing excitation and recombination are dominant production processes.

Logistics had only been perceived as a supporting means or au xiliary of corporate activity, but is now surfacing as a priority sector for competitiveness in the infin ite competition of the global market. This study looks into the current status of logistics and set s forth the improvement tasks by analyzing the problems of the returned goods logistics. 2016Purpose: Pepper prices have risen continuously because of a decrease in cultivation area; therefore, mechanical harvesting systems for peppers should be developed to reduce cost, time, and labor during harvest. In this study, a screw type picking head for a self-propelled pepper harvester was developed, and the optimal working conditions were evaluated considering helix types, winding directions of helix, and rotational speeds of the helix. Methods: The screw type was selected for the picking head after analyzing previous studies, and the device consisted of helices and a feed chain mechanism for conveying pepper branches. A double helix and a triple helix were manufactured, and rotational speeds of 200, 300, and 400 rpm were tested. The device was controlled by a variable speed (VS) motor and an inverter. Both the forward and reverse directions were tested for the winding and rotating directions of the helix. An experiment crop (cultivar: Longgreenmat) was cultivated in a plastic greenhouse. The test results were analyzed using the SAS program with ANOVA to examine the relationship between each factor and the performance of the picking head. Results: The results of the double and triple helix tests in the reverse direction showed gross harvest efficiency levels of 60–95%, mechanical damage rates of 8–20%, and net marketable portion rates of 50–80%. The dividing ratio was highest at a rotational speed of 400 rpm. Gross harvest efficiency was influenced by the types of helix and rotational speed. Net marketable portion was influenced by rotational speed but not influenced by the type of helix. Mechanical damage was not influenced by the type of helix or rotational speed. Conclusions: Best gross harvest efficiency was obtained at a rotational speed of 400 rpm; however, operating the device at that speed resulted in vibration, which should be reduced. Supply units in each command are multi-tiered and each supply unit keeps a supply level independently, which can cause excess stock, leading false reports to the Logistics Command and increasing difficulties in managing user needs. This causes excess assets and the excess then causes deformation in demand. therefore, the supply support systems of our armed forces have become high-cost/low-efficiency and are insufficient to meet the needs of users in combat units. Civilian corporations and the US Department of Defense are downsizing the aforementioned multi-tiered supply systems thus revolutionizing efficient and effective logistics by adopting Supply Chain Management(SCM), and Prime Vendor policies. Prime Vendor policy is a logistics support method that allows users to directly request and receive supply items from suppliers, based on supply contracts between suppliers and central maintenance organizations like KDA. In other words, it is a system that allows for users to make orders to suppliers directly and suppliers to deliver goods to the users directly, cutting out the middle stage, thus allowing an efficient supply. This is a way forward in finance that cuts costs in net supplies and allows an efficient utilization of civilian assets. which is also known to fasten the speed of logistical support and stripping down the logistical structure. therefore, this report will explore Prime Vendor policies adopted in certain number of units for medical supplies that were taken in consideration of improvements in stock management in civilian organizations and the US Army, and aims to apply such policies for repair parts.

The purpose of this study is to let the entire logistics system be managed in cyberspace by effectively sharing the logistics information between companies involved in the process, changing conventional commerce to electronic commerce. This produces many problems in shipping and delivery and the leads an inefficient logistics system. Another problems is that there is no sharing of information that process the logistics flow and no systematic management of pick-up and delivery information. As a result, rise in logistics cost, longer delivery period and poor service quality are inevitable and these results are becoming an obstacle in the widespread usage of the electronic commerce. PURPOSE: A gas detection system using a smart phone and a gas detection method are provided to solve problems which are generated by an artificial manipulation or a wrong input by using a attachable/detachable sensor module and an application to the smart phone, and are provided to enhance the reliability of the data by enabling the measurement data, which is measured from the sensor module which performs the gas detection, processed as the expertise data about the combustible gas or the toxic gas, by sending to an operation server. CONSTITUTION: A gas detection system using a smart phone comprises a sensor module(20) and a gas detection APP(100). The sensor module is attached to the smart phone and detects and measures the combustible gas among the LNG, LPG, CNG, and Freon, and the toxic gas between the carbon monoxide, and the carbon dioxide. The gas detection APP has the following properties: installed inside the smart phone; a tag which is built in the smart phone is read by an RFID or NFC(Near Field Communication) which indicates a user or location for the gas detection by using a connection unit which stores an RFID reader or an NFC reader within the smart phone; and detects the combustible gas or the toxic gas and obtains the measurement data through the measurement by driving a sensor module after recognizing the user information and location information automatically. The smart phone transmits the user information, the location information, and the measurement data which are obtained from the gas detection APP to an operation server(40) through a communication network(30). The smart phone includes a database(50) which stores the user information, the location information, and the measurement data received from the operation server. Total industrial accident rate in Korea is facing a record 0. 7% for 10 years since 2000. After entering the continuous versus 0.6% (0.69%) in 2010. In 2011, t he decrease was reduced to 0.65%. Despite the ongoing disaster reduction efforts to reduce disaster (share) of foreig n workers has been increasing rapidly Survey, Whilst the provision of disaster prevention system stat us and problem analysis and identify measures that can be established through such disaster reductio n measures are urgently needed.Keywords: foreign workers, Disaster Reduction efforts 1. 서 론 우리나라 전업종의 산업재해율은 2000년 이후 10여년 간 0.7%대를 기록하고 있다. 2010년 이후 0.6%대(0.69%)로 진입 지속적으로 감소하여 2011년에는 0.65%까지 감소하였다. 이와는 대조적으로 외국인 근로자의 재해 점유율은 2000년 이후 10여년간 급격히 증가하여 2014년에는 0.5%대 재해율 진입에 어려움이 있을 것으로 예상된다.한편 외국인의 재해 점유율은 2001년 1.9%이던 것이 2011년에는 7.0%로 증가하였고, 2012년3월에는 8.7%(120명)로 증가하였다. 참고로 내국인은 -1.1%(-211명)감소하였다. 외국인근로자 건설현장의 재해분석 및 위험성 평가연구를 통하여 급격히 증가되고 있는 외국인근로자의 재해(점유율)를 감소하는 것은 건설업 전체 재해율 감소에 매우 의미 있는 일이다. 재해감소 노력에도 불구하고 지속적으로 급격히 증가되고 있는 외국인근로자의 재해(점유율)를 감소시키기 위해 현황조사, 그간의 재해예방현황 및 문제점 분석 등을 통한 재해감소 대책 수립을 파악할 수 있는 체계와 대책의 마련이 시급하다. 급격히 증가되고 있는 외국인근로자의 재해(점유율)를 감소시키기 위해 현황조사, 그간의 재해예방사업 추진현황 및 문제점 분석 등을 통한 재해감소 종합대책 수립하여 외국인근로자의 재해예방을 위해 실행 가능한 대책을 모두 도출한 내용으로 이를 실행하기 위해†Corresponding Author : Kyung-Sik Kang, Industrial and Engineering, Myongji University, Yongin 449-728, Korea, E-mail : kangks@mju.ac.kr Received April 20, 2015; Revision Received June 11, 2015; A ccepted June 17, 2015.

The history of Korea Qualification system began after the Korea War. The Qualification was recognized to it`s value with economic growth until the 2000. But the value of qualifications has to be re-evaluated under the current global environments. Almost Previous research has not to work for the value of qualifications all of Korea. So, This study presents a evaluation structure frame to work and some evaluation factors for the qualification value. In addition, the methodology proposed to obtain results using the AHP weights based on the evaluation methodology and questionnaire. The analysis results are used as a basis for selecting the future policy direction of the national qualification.
