Abstract

The magical idiom of evil occupies an important position in numerous Christian societies. Cosmological capture refers to a historicized process through which Christian narratives and institutions attempt to integrate evil into dualist and oppositional cosmological schemas. This article begins by addressing the way that biblical stories of defeated magicians contribute to modern dynamics of cosmological capture. It then proceeds to address the role of evil in Cypriot society through narratives and descriptions of everyday rituals and events. As these narratives and rituals show, capture remains incomplete, and as evil extends beyond the limits of dualist categorization, the result is a situation of 'magical disorder': a cosmological arrangement in which evil manifests as an indifferent and inhuman force, which nevertheless conditions everyday experience and social relations. 3. Activating illness : tactics from patient activism and the politics of thalassaemia in Cyprus

Abstract

Thalassaemia is a blood disorder prevalent amongst the Cypriot population because of genetic, ecological, and social reasons. Although a successful prevention system has been in place since the early 1980s, approximately 650 thalassaemia patients still live on the island whose births preceded the given system. For my fieldwork I spent a year in Cyprus with the PanCyprian Thalassaemia Association (PTA) – a patients group which acts as the main channel of politicisation for thalassaemia patients in Cyprus. By organising events such as conferences, fundraisers and workshops, the PTA strives to maintain the awareness of thalassaemia in the Cypriot public sphere. The association also maintains an agonistic yet healthy relationship with the Cypriot state. Thalassaemia treatment in Cy-prus is provided by public healthcare and, since its foundation in 1973, the PTA has won several skirmishes against the state on issues such as a more reliable blood supply, better provision of medicines, and more hospital space for patients. In addition, the PTA has forged numerous alliances with national and international organisations, patient associa-tions and scientific research bodies which have a decisive say in how thalassaemia comes to be enacted on a Cypriot and global level. Throughout the thesis I focus on the tactics the PTA uses to politically activate thalassaemia. As I argue, activating illness entails mak-ing discernible political dimensions of illness which previously evaded, or were left unac-counted, by public and governmental perceptions. In addition, through the anthropologi-cal analysis of PTA case studies, I develop tactics of my own by which patient associa-tions can activate illness. Through an ethnography and at the same time conceptual de-velopment of tactics, the thesis aims to fruitfully reconcile the ontology and politics of illness.

Abstract

Background: Transthyretin amyloidosis (ATTR amyloidosis) is a rare, life-threatening disease caused by the accumulation of variant or wild-type (ATTRwt amyloidosis) transthyretin amyloid fibrils in the heart, peripheral nerves, and other tissues and organs. Methods: Established in 2007, the Transthyretin Amyloidosis Outcomes Survey (THAOS) is the largest ongoing, global, longitudinal observational study of patients with ATTR amyloidosis, including both inherited and wild-type disease, and asymptomatic carriers of pathogenic TTR mutations. This descriptive analysis examines baseline characteristics of symptomatic patients and asymptomatic gene carriers enrolled in THAOS since its inception in 2007 (data cutoff: August 1, 2021). Results: This analysis included 3779 symptomatic patients and 1830 asymptomatic gene carriers. Symptomatic patients were predominantly male (71.4%) and had a mean (standard deviation [SD]) age of symptom onset of 56.3 (17.8) years. Val30Met was the most common genotype in symptomatic patients in South America (80.9%), Europe (55.4%), and Asia (50.5%), and more patients had early- versus late-onset disease in these regions. The majority of symptomatic patients in North America (58.8%) had ATTRwt amyloidosis. The overall distribution of phenotypes in symptomatic patients was predominantly cardiac (40.7%), predominantly neurologic (40.1%), mixed (16.6%), and no phenotype (2.5%). In asymptomatic gene carriers, mean (SD) age at enrollment was 42.4 (15.7) years, 42.4% were male, and 73.2% carried the Val30Met mutation. Conclusions: This 14-year global overview of THAOS in over 5000 patients represents the largest analysis of ATTR amyloidosis to date and highlights the genotypic and phenotypic heterogeneity of the disease. Methods Established in 2007, the Transthyretin Amyloidosis Outcomes Survey (THAOS) is the largest ongoing, global, longitudinal observational study of patients with ATTR amyloidosis, including both inherited and wild-type disease, and asymptomatic carriers of pathogenic TTR mutations. This descriptive analysis examines baseline characteristics of symptomatic patients and asymptomatic gene carriers enrolled in THAOS since its inception in 2007 (data cutoff: August 1, 2021). Results This analysis included 3779 symptomatic patients and 1830 asymptomatic gene carriers. Symptomatic patients were predominantly male (71.4%) and had a mean (standard deviation [SD]) age of symptom onset of 56.3 (17.8) years. Val30Met was the most common genotype in symptomatic patients in South America (80.9%), Europe (55.4%), and Asia (50.5%), and more patients had early- versus late-onset disease in these regions. The majority of symptomatic patients in North America (58.8%) had ATTRwt amyloidosis. The overall distribution of phenotypes in symptomatic patients was predominantly cardiac (40.7%), predominantly neurologic (40.1%), mixed (16.6%), and no phenotype (2.5%). In asymptomatic gene carriers, mean (SD) age at enrollment was 42.4 (15.7) years, 42.4% were male, and 73.2% carried the Val30Met mutation. Conclusions This 14-year global overview of THAOS in over 5000 patients represents the largest analysis of ATTR amyloidosis to date and highlights the genotypic and phenotypic heterogeneity of the disease. Abstract

Our research explores the modern status of magic in two different European island settings: Cyprus and Orkney. By historically and ethnographically addressing the gradual dissolution of magical traditions in our respective fieldsites, our aim is to explore the means and instances by which magic continues to manifest in narratives and everyday practices. By exploring the current epistemological and social standing attributed to magic in Cyprus and Orkney societies, we wish to enrich anthropological understandings of the manner by which magic emerges from the conditions of unknowing that permeate processes of modernity whilst nevertheless remaining external to it. We approach such objectives by critiquing the implicit suggestion contained, but often not articulated or theorised within anthropological literature, that humans ‘know’ magic. Through our ethnography, we attempt to show that magic in these two islands does not persist through the ability of people to understand or rationalise magic as a thing of the past, but rather emerges through an awareness of historical, social, and cognitive incompleteness, accumulated and contained within dormant magical memory. We hence wish to identify and further theorise a central, modern tenet of magic: the unknowability contained and nourished in social and cognitive processes, through which the possibility - future and past - of magic is thought of and narrated in disenchanted worlds. As we suggest, not-knowing, in such sense, does not merely signal lack of knowledge but a mode of thinking and modernity specific to Cypriot and Orcadian modernity. Postprint

Abstract

Background Hereditary transthyretin-mediated amyloidosis is a rare, inherited, progressive disease caused by mutations in the transthyretin (TTR) gene. We assessed the safety and efficacy of long-term treatment with patisiran, an RNA interference therapeutic that inhibits TTR production, in patients with hereditary transthyretin-mediated amyloidosis with polyneuropathy. Methods This multicentre, open-label extension (OLE) trial enrolled patients at 43 hospitals or clinical centres in 19 countries as of Sept 24, 2018. Patients were eligible if they had completed the phase 3 APOLLO or phase 2 OLE parent studies and tolerated the study drug. Eligible patients from APOLLO (patisiran and placebo groups) and the phase 2 OLE (patisiran group) studies enrolled in this global OLE trial and received patisiran 0.3 mg/kg by intravenous infusion every 3 weeks with plans to continue to do so for up to 5 years. Efficacy assessments included measures of polyneuropathy (modified Neuropathy Impairment Score +7 [mNIS+7]), quality of life, autonomic symptoms, nutritional status, disability, ambulation status, motor function, and cardiac stress, with analysis by study groups (APOLLO-placebo, APOLLO-patisiran, phase 2 OLE patisiran) based on allocation in the parent trial. The global OLE is ongoing with no new enrolment, and current findings are based on the interim analysis of the patients who had completed 12-month efficacy assessments as of the data cutoff. Safety analyses included all patients who received one or more dose of patisiran up to the data cutoff. This study is registered with ClinicalTrials.gov, NCT02510261. Findings Between July 13, 2015, and Aug 21, 2017, of 212 eligible patients, 211 were enrolled: 137 patients from the APOLLO-patisiran group, 49 from the APOLLO-placebo group, and 25 from the phase 2 OLE patisiran group. Abstract

In this essay I attempt to draw some crucial theoretical parallelisms between ancient Greek cosmology and the Anthropocene. Taking inspiration from Marcel Detienne and Timothy Morton's work, I deploy the figure of Dionysus as a conceptual persona that can help us think of strangeness as a non-human trait that human societies in the Anthropocene must urgently engage with. Events such as the ongoing Covid-19 epidemic, through which non-humans are brought to the forefront of politics and social relations, traditionally result in attempts of sublating strangeness through human modes of knowledge. As I argue, epidemics instead demand the creation of mythic practices, collectives and techniques through which strangeness is not eliminated or understood but rather elevated to a fundamental feature of social relations. In such sense, Greek antiquity presents a critical vector of ethical and ecological intervention to the current state of the Anthropocene, because it showcases a cosmos in which human life and society are constantly embedded and negotiated amid non-human strangeness. 44. An unusual case of suprascapular nerve neuropathy: a case report

Abstract

Abstract Introduction Suprascapular nerve neuropathy constitutes an unusual cause of shoulder weakness, with the most common etiology being nerve compression from a ganglion cyst at the suprascapular or spinoglenoid notch. We present a puzzling case of a man with suprascapular nerve neuropathy that may have been associated with an appendectomy. The case was attributed to nerve injury as the most likely cause that may have occurred during improper post-operative patient mobilization. Case presentation A 23-year-old Caucasian man presented to an orthopedic surgeon with a history of left shoulder weakness of several weeks' duration. The patient complained of pain and inability to lift minimal weight, such as a glass of water, following an appendectomy. His orthopedic clinical examination revealed obvious atrophy of the supraspinatus and infraspinatus muscles and 2 of 5 muscle strength scores on flexion resistance and external rotation resistance. Magnetic resonance imaging showed diffuse high signal intensity within the supraspinatus and infraspinatus muscles and early signs of minimal fatty infiltration consistent with denervation changes. No compression of the suprascapular nerve in the suprascapular or spinoglenoid notch was noted. Electromyographic studies showed active denervation effects in the supraspinatus muscle and more prominent in the left infraspinatus muscle. The findings were compatible with damage to the suprascapular nerve, especially the part supplying the infraspinatus muscle. On the basis of the patient's history, clinical examination, and imaging studies, the diagnosis was suspected to be associated with a possible traction injury of the suprascapular nerve that could have occurred during the patient's transfer from the operating table following an appendectomy. Conclusion Our case report may provide important insight into patient transfer techniques used by hospital personnel, may elucidate the clinical significance of careful movement of patients following general anesthesia, and may have important implications for patient safety techniques, including those outlined in the World Health Organization Surgical Safety Checklist program.

Abstract

In this work, multi-scale amplitude modulation-frequency modulation (AM-FM) features are extracted from surface electromyographic (SEMG) signals and they are used for the classification of neuromuscular disorders. The method is validated on SEMG signals recorded from a total of 40 subjects: 20 normal and 20 abnormal cases (11 myopathy, and 9 neuropathy cases), at 10%, 30%, 50%, 70% and 100% of maximum voluntary contraction (MVC), from the biceps brachii muscle. For the classification, three classifiers are used: (i) the statistical K-nearest neighbor (KNN), (ii) the self-organizing map (SOM) and (iii) the support vector machine (SVM). For all classifiers, the leave-one-out methodology is used to validate the classification of the SEMG signals into normal or abnormal (myopathy or neuropathy). A classification success rate of 78% for the AM-FM features and SVM models was achieved. These results also show that SEMG can be used as a non-invasive alternative to needle EMG for differentiating between normal and abnormal (myopathy, or neuropathy) cases. © 2012 Elsevier Ltd. 7 3 265 269 Cited By :7
