Introduction: This study reviews malpractice, also called medical professional liability (MPL), claims involving adult patients cared for in emergency departments (ED) and urgent care settings. Methods: We conducted a retrospective review of closed MPL claims of adults over 18 years, from the Medical Professional Liability Association’s Data Sharing Project database from 2001–2015, identifying 6,779 closed claims. Data included the total amount, origin, top medical specialties named, chief medical factors, top medical conditions, severity of injury, resolution, average indemnity, and defense costs of closed claims. Results: Of 6,779 closed claims, 65.9% were dropped, withdrawn, or dismissed. Another 22.8% of claims settled for an average indemnity of $297,709. Of the 515 (7.6%) cases that went to trial, juries returned verdicts for the defendant in 92.6% of cases (477/515). The remaining 7.4% of cases (38/515) were jury verdicts for the plaintiff, with an average indemnity of $816,909. The most common resulting medical condition cited in paid claims was cardiac or cardiorespiratory arrest (10.4%). Error in diagnosis was the most common chief medical error cited in closed claims. Death was the most common level of severity listed in closed (38.5%) and paid (42.8%) claims. Claims reporting major permanent injury had the highest paid-to-closed ratio, and those reporting grave injury had the highest average indemnity of $686,239. Conclusion: This retrospective review updates the body of knowledge surrounding medical professional liability and represents the most recent analysis of claims in emergency medicine. As the majority of emergency providers will be named in a MPL claim during their career, it is essential to have a better understanding of the most common factors resulting in MPL claims.

Non-cell-autonomous signals often play crucial roles in cell fate decisions during animal development. Reciprocal signaling between endoderm and mesoderm is vital for embryonic development, yet the key signals and mechanisms remain unclear. Here, we show that endodermal cells efficiently promote the emergence of mesodermal cells in the neighboring population through signals containing an essential short-range component. The endoderm-mesoderm interaction promoted precardiac mesoderm formation in mouse embryonic stem cells and involved endodermal production of fibronectin. In vivo, fibronectin deficiency resulted in a dramatic reduction of mesoderm accompanied by endodermal expansion in zebrafish embryos. This event was mediated by regulation of Wnt signaling in mesodermal cells through activation of integrin-β1. Our findings highlight the importance of the extracellular matrix in mediating short-range signals and reveal a novel function of endoderm, involving fibronectin and its downstream signaling cascades, in promoting the emergence of mesoderm. © 2013. The objective of this experiment was to investigate the effect of dietary supplementation of arginine (Arg), tryptophan (Trp), and taurine (Tau) on productive performance, egg quality, liver visual characteristics, antioxidant status, immune response, and stress indicator of laying hens raised under heat stress conditions. A total of two hundred eighty 47-wk-old Hy-Line Brown laying hens were randomly allotted to 1 of 4 dietary treatments with 7 replicates consisting of 10 cages per replicate. A basal diet (BD) was prepared to meet or exceed nutrient requirement estimates. Two additional diets were formulated to increase either digestible Arg or Trp by 50% greater than the BD. Finally, one more diet was prepared by adding 0.5% Tau to the BD. The experimental diets were fed to hens on an ad libitum basis for 8 wk. Average room temperature and relative humidity were maintained at 30.7±1.41°C and 72.5±11.61%, respectively. Results indicated that laying hens in Arg and Trp treatments tended (p= 0.06) to have a higher egg yolk color (Roche color fan) than those in the Tau treatment. Likewise, there was a tendency (p= 0.05) for a lower liver color score in the Tau treatment than Arg and Trp treatments. In conclusion, dietary supplementation of Arg, Trp, and Tau at the current levels (0.37% SID Arg, 0.075% SID Trp, and 0.5% Tau) in diets has no positive effects on productive performance, egg quality, liver visual characteristics, antioxidant status, immune response, and stress indicators of laying hens raised under the current heat stress conditions. Objective: The purpose of this study was to evaluate the incidence of gonadal vein refluxes associated with lower-extremity varicose veins with Doppler ultrasonography (DUS). Method: A total of 6279 patients with venous disease-related symptoms of the lower extremity were evaluated with DUS in the vascular lab. Gonadal vein reflux using abdominal ultrasound was further evaluated in patients with unusual varices, defined as varices in the inguinal, inner or upper thigh and the vulvar area without refluxes in the saphenofemoral junction (SPJ). Those patients who showed gonadal vein reflux were diagnosed as having pelvic-origin varicosity. Results: Unusual varices were found in a total of 237 patients (3.8%), and of these patients, pelvic-origin varicosity was discovered with transabdominal ultrasound in 156 (65.8%). A total of 66.7% (n = 38/57) of unusual varix patients with pelvic pain had gonadal vein reflux. The measurement of gonadal vein diameter was larger in ultrasonography than CT scans (8.835 vs. 8.81, p < 0.001). Two patients with severe symptoms but no obstructive venous diseases were treated with gonadal vein embolization. Conclusion: The incidence of pelvic-origin varicosities was 2.5% (n = 156/6279). However, more than half of the patients with unusual varices had gonadal vein reflux and 24.4% of these patients also presented with pelvic pain. The evaluation of pelvic-origin varicosities should be performed in patients who present with unusual forms of varices of the lower extremity.

In May-Thurner syndrome (MTS), the left iliac vein is compressed by the right iliac artery, leading to restricted blood flow from the leg to the heart. MTS commonly manifests in young females; however, its occurrence in older adults necessitates verifying the possibility of malignancy. A 77-year-old female experienced leg swelling and discomfort for 1 week. Computed tomography angiography suggested extensive thrombosis in the left iliac vein. Despite heparin treatment, the thrombus persisted and required mechanical thrombectomy, balloon venoplasty, and stent placement for tight iliac vein stenosis. The procedure was successful; however a biopsy of thrombus revealed malignant cells. An underlying malignancy and a hypercoagulable state were identified as the contributing factors after further evaluations. This case underscores the importance for vigilant diagnosis of hypercoagulable conditions and malignancies in MTS, emphasizing the role of malignancy in the development of DVT and MTS.
