7. Enhancing Force Absorption, Stress-Strain and Thermal Properties of Weft-knitted Inlay Spacer Fabric Structures for Apparel Applications

Abstract

The pursuit of materials that offer both wear comfort and protection for functional and protective clothing has led to the exploration of weft-knitted spacer structures. Traditional cushioning materials such as spacer fabrics and laminated foam often suffer from deformation under compression stresses, thus compromising their protective properties. This study investigates the enhancement of the force absorption, stress-strain and thermal properties of weft-knitted spacer fabrics with inlays. Surface yarns with optimal stretchability and thermal properties are used and combined with elastic yarns in various patterns to fabricate nine different inlay samples. The mechanical and thermal properties of these samples are systematically analyzed, including their compression, stretchability, thermal comfort and surface properties. The results show that the inlay spacer fabric exhibits superior compression properties and thermal conductivity compared to traditional laminated foam and spacer fabrics, while maintaining stretchability, thus providing a better performance than traditional fabrics for protective clothing and wearable cushioning products. This study further confirms that the type of inlay yarn and inlay structure are crucial factors that significantly influence the thermal, tensile and compressive properties of fabric. This research provides valuable insights into the design and development of advanced textile structures to improve wear comfort and protection in functional garment applications.

Abstract

Background: In patients receiving venovenous (VV) extracorporeal membrane oxygenation (ECMO) packed red blood cell (PRBC) transfusion thresholds are usually higher than in other patients who are critically ill. Available guidelines suggest a restrictive approach, but do not provide specific recommendations on the topic. The main aim of this study was, in a short timeframe, to describe the actual values of haemoglobin and the rate and the thresholds for transfusion of PRBC during VV ECMO. Methods: PROTECMO was a multicentre, prospective, cohort study done in 41 ECMO centres in Europe, North America, Asia, and Australia. Consecutive adult patients with acute respiratory distress syndrome (ARDS) who were receiving VV ECMO were eligible for inclusion. Patients younger than 18 years, those who were not able to provide informed consent when required, and patients with an ECMO stay of less than 24 h were excluded. Our main aim was to monitor the daily haemoglobin concentration and the value at the point of PRBC transfusion, as well as the rate of transfusions. The practice in different centres was stratified by continent location and case volume per year. Adjusted estimates were calculated using marginal structural models with inverse probability weighting, accounting for baseline and time varying confounding. Findings: Between Dec 1, 2018, and Feb 22, 2021, 604 patients were enrolled (431 [71%] men, 173 [29%] women; mean age 50 years [SD 13·6]; and mean haemoglobin concentration at cannulation 10·9 g/dL [2·4]). Abstract

Individuals with minor ischaemic stroke and intracranial occlusion are at increased risk of poor outcomes. Intravenous thrombolysis with tenecteplase might improve outcomes in this population. We aimed to test the superiority of intravenous tenecteplase over non-thrombolytic standard of care in patients with minor ischaemic stroke and intracranial occlusion or focal perfusion abnormality.

Abstract

In recent decades, we have witnessed great advances on the Internet of Things, mobile devices, sensor-based systems, and resulting big data infrastructures, which have gradually, yet fundamentally influenced the way people interact with and in the digital and physical world. Many human activities now not only operate in geographical (physical) space but also in cyberspace. Such changes have triggered a paradigm shift in geographic information science (GIScience), as cyberspace brings new perspectives for the roles played by spatial and temporal dimensions, e.g., the dilemma of placelessness and possible timelessness. As a discipline at the brink of even bigger changes made possible by machine learning and artificial intelligence, this paper highlights the challenges and opportunities associated with geographical space in relation to cyberspace, with a particular focus on data analytics and visualization, including extended AI capabilities and virtual reality representations. Consequently, we encourage the creation of synergies between the processing and analysis of geographical and cyber data to improve sustainability and solve complex problems with geospatial applications and other digital advancements in urban and environmental sciences.

Abstract

Background: Few mobility-based studies have investigated the associations between multiple environmental exposures, including social exposures, and mental health. Objective: To assess how exposure to green space, blue space, noise, air pollution, and crowdedness along people's daily mobility paths are associated with anxiety symptoms. Methods: 358 participants were cross-sectionally tracked with Global Positioning System (GPS)-enabled mobile phones. Anxiety symptoms were measured at baseline using the Generalized Anxiety Disorder-7 (GAD-7) questionnaire. Green space, blue space, noise, and air pollution were assessed based on concentric buffers of 50 m and 100 m around each GPS point. Crowdedness was measured by the number of nearby Bluetooth-enabled devices detected along the mobility paths. Multiple linear regressions with full covariate adjustment were fitted to examine anxiety-environmental exposures associations. Random forest models were applied to explore possible nonlinear associations and exposure interactions. Results: Regression results showed null linear associations between GAD-7 scores and environmental exposures. Random forest models indicated that GAD-7-environment associations varied nonlinearly with exposure levels. We found a negative association between green space and GAD-7 scores only for participants with moderate green space exposure. We observed a positive association between GAD-7 scores and noise levels above 60 dB and air pollution concentrations above 17.2 μg m−3. Crowdedness was positively associated with GAD-7 scores, but exposure-response functions flattened out with pronounced crowdedness of >7.5. Blue space tended to be positively associated with GAD-7 scores. Random forest models ranked environmental exposures as more important to explain GAD-7 scores than linear models. Conclusions: Our findings indicate possible nonlinear associations between mobility-based environmental exposures and anxiety symptoms. More studies are needed to obtain an in-depth understanding of underlying anxiety-environment mechanisms during daily life.

Abstract

BackgroundEvidence is urgently needed to support treatment decisions for children with multisystem inflammatory syndrome (MIS-C) associated with severe acute respiratory syndrome coronavirus 2.MethodsWe performed an international observational cohort study of clinical and outcome data regarding suspected MIS-C that had been uploaded by physicians onto a Web-based database. We used inverse-probability weighting and generalized linear models to evaluate intravenous immune globulin (IVIG) as a reference, as compared with IVIG plus glucocorticoids and glucocorticoids alone. There were two primary outcomes: the first was a composite of inotropic support or mechanical ventilation by day 2 or later or death; the second was a reduction in disease severity on an ordinal scale by day 2. Secondary outcomes included treatment escalation and the time until a reduction in organ failure and inflammation.ResultsData were available regarding the course of treatment for 614 children from 32 countries from June 2020 through February 2021; 490 met the World Health Organization criteria for MIS-C. Of the 614 children with suspected MIS-C, 246 received primary treatment with IVIG alone, 208 with IVIG plus glucocorticoids, and 99 with glucocorticoids alone; 22 children received other treatment combinations, including biologic agents, and 39 received no immunomodulatory therapy. Receipt of inotropic or ventilatory support or death occurred in 56 patients who received IVIG plus glucocorticoids (adjusted odds ratio for the comparison with IVIG alone, 0.77; 95% confidence interval [CI], 0.33 to 1.82) and in 17 patients who received glucocorticoids alone (adjusted odds ratio, 0.54; 95% CI, 0.22 to 1.33). The adjusted odds ratios for a reduction in disease severity were similar in the two groups, as compared with IVIG alone (0.90 for IVIG plus glucocorticoids and 0.93 for glucocorticoids alone). The time until a reduction in disease severity was similar in the three groups.ConclusionsWe found no evidence that recovery from MIS-C differed after primary treatment with IVIG alone, IVIG plus glucocorticoids, or glucocorticoids alone, although significant differences may emerge as more data accrue. Abstract

In this correspondence, we emphasize methodological caveats of ecological studies assessing associations between COVID-19 and its physical and social environmental determinants. First, we stress that inference is error-prone due to the modifiable areal unit problem and the modifiable temporal unit problem. The possibility of confounding from using aggregated data is substantial due to the neglect of person-level factors. Second, studying the viral transmission of COVID-19 solely on people's residential neighborhoods is problematic because people are also exposed to nonhome locations and environments en-route along their daily mobility path. We caution against an uncritical application of aggregated data and reiterate the importance of stronger research designs (e.g., case-control studies) on an individual level. To address environmental contextual uncertainties due to people's day-to-day mobility, we call for people-centered studies with mobile phone data., Highlights • Number of ecological studies of environmental determinants of COVID-19 is increasing. • Their results may be biased and confounded by spatiotemporal aggregation of data. • People's day-to-day mobility away from the home can further bias their results. • Stronger research designs on a person-level with day-to-day mobility are needed.

Abstract

Background Exposure to transportation noise is hypothesized to contribute to anxiety, but consistent associations have not been established. Objective To provide a comprehensive synthesis of the literature by examining associations between traffic-related noise (i.e., road traffic noise, railway noise, aircraft noise and mixed traffic noise) and anxiety. Methods We systematically searched Web of Science, Scopus, Embase, PubMed, and PsycINFO for English-language observational studies published up to February 2020 reporting on the traffic noise-anxiety association. We appraised the risk of bias using an assessment tool and the quality of evidence following established guidelines. A random-effects meta-analysis was performed for pooled and separated traffic-related noise sources. Results Of the 3575 studies identified, 11 fulfilled the inclusion criteria and 9 studies were appropriate for meta-analysis. For the pooled overall effect size between transport noise and anxiety, we found 9% higher odds of anxiety associated with a 10 dB(A) increase in day-evening-night noise level (Lden), with moderate heterogeneity (OR = 1.09, 95% CI: [0.97; 1.23], I2 = 70%). The association was more likely to be significant with more severe anxiety (OR = 1.08, 95% CI: [1.01; 1.15], I2 = 48%). Sub-group analysis revealed that the effects of different noise sources on anxiety were inconsistent and insignificant. The quality of evidence was rated as very low to low. Conclusions Our findings support the hypothesis of an association between traffic noise and more severe anxiety. More high-quality studies are needed to confirm associations between different noise types and anxiety, as well as to better understand underlying mechanisms.
