Introduction: In this study, it was aimed to obtain information about the perspectives of infectious diseases and clinical microbiology (IDCM) specialists on online education applications and web-based seminars, and their usage and utilization rates. Materials and Methods: In this descriptive, cross-sectional study, online survey responses of the IDCM specialists in who were members of Infectious Diseases Clinical Microbiology Specialty Society of Turkey (EKMUD) were evaluated. Results: A total of 184 IDCM specialists answered the survey questions. Of the participants, 77.2% thought that the pandemic disrupted postgraduate medical education and 84.8% thought that the pandemic disrupted pre-graduate medical education. Medical educators involved in education during the Coronavirus disease-2019 (COVID-19) pandemic reported that they mostly continued online education. Of the participants, 84.9% reported that they were able to use the internet in an effective way to gain access to information during the COVID-19 pandemic. Of the responders, 58.2% reported that they followed online training programs, such as web-based seminars and lectures, and 51.6% reported that these online programs contributed to their learning. Participants found online training programs such as web-based seminars and lectures useful in terms of learning competence, that online training programs provided people with gain in space and time, and that they found them more instructive because they gave them the chance to receive training in subjects of their interest. Participants reported that these programs provided learning competency in that these programs were more beneficial to the participants in view of place, the subject that they need/they are interested in and saving time for the trainer and the participants. Due to the COVID-19 pandemic, the participants mostly wanted the 2020 EKMUD congress to be held online. A total 92.9% of the participants reported that online congresses and seminars would decrease travel, participation and sponsorship costs. Only 24.5% of the participants considered that online congresses and seminars would be more effective in terms of social interaction and gaining information than face-to-face congresses. Conclusion: The IDCM specialists reported that online educational practices such as webinars were benefical. A 33-year-old man was admitted to our emergency department for severe frontal headache followed by a state of consciousness disturbance and right-sided hemiparesis. No previous febrile disease, head trauma, vascular risk factor, and medication for any systemic disease were defined in his history. He had experienced a similar disorder three years ago and had recovered completely. Cerebrospinal fluid (CSF) analysis revealed pleocytosis and electroencephalography (EEG) showed diffuse slow wave activity. Hyperintense foci on T2 and FLAIR sequences representing bilateral cortical ischemia, prominent on the right hemisphere, were seen on MRI. Contrast-enhanced T1 images showed marked leptomeningeal thickening with enhancement. The patient was considered as having CSF lymphocytosis (HaNDL syndrome) due to temporary headache with neurologic deficit and CSF pleocytosis. Diagnostic criteria have been identified for this syndrome according to the International Classification of Headache Disorders, 2nd edition (ICHD-II). According to these criteria, neuroimaging should be normal. Positive neuroimaging findings and impairment of consciousness have been reported in a limited number of HaNDL cases so far. Diversity of neurological signs, duration and distinctness from migraine headache have been described in comment section under the diagnostic criteria. Objective The aim of this study is to evaluate the treatment and its outcomes of patients having vulvar necrotizing fasciitis. Material and methods Parameters that evaluated from patients treated in our department between 1st of January, 2006 and 31st of December, 2009 were age, conditions causing immune deficiency, habits of cigarette or alcohol, the way of perineal hair removal, glucose and HbA1c levels at admission, number of debridement under general anesthesia, results of microbiological studies and antibiotics used. Diagnosis was made with physical examination and the finger test proposed by Andresen. All patients underwent to surgical debridement immediately. Wide spectrum antibiotics and attempt to eliminate the underlying causes were started simultaneously with surgical debridement under general anesthesia. Results Six patients were available regarding the study criteria. The mean age was 46,5 ± 10,4. All patients except one had uncontrolled diabetes. The remaining had hematological malignancy and immune suppression caused by the chemotherapeutics. The hygienic condition of perinea was poor in all. Numbers of debridement under general anesthesia were three, two and one in one, two and three of the patients consecutively. The mean hospital staying was 35 ± 14,9 days. Death due to necrotizing fasciitis was not observed in any cases. Two patients died long after their discharge because of the underlying disease. Necrotizing fasciitis did not relapse in anyone. Conclusion Results derived from the small number of patients show that synchronous and intensive surgical and medical treatments are promising. However, they need to be reevaluated in studies having larger number of cases.

No data on whether brucellar meningitis or meningoencephalitis can be treated with oral antibiotics or whether an intravenous extended-spectrum cephalosporin, namely, ceftriaxone, which does not accumulate in phagocytes, should be added to the regimen exist in the literature. The aim of a study conducted in Istanbul, Turkey, was to compare the efficacy and tolerability of ceftriaxone-based antibiotic treatment regimens with those of an oral treatment protocol in patients with these conditions. This retrospective study enrolled 215 adult patients in 28 health care institutions from four different countries. The first protocol (P1) comprised ceftriaxone, rifampin, and doxycycline. The second protocol (P2) consisted of trimethoprim-sulfamethoxazole, rifampin, and doxycycline. In the third protocol (P3), the patients started with P1 and transferred to P2 when ceftriaxone was stopped. The treatment period was shorter with the regimens which included ceftriaxone (4.40 +/- 2.47 months in P1, 6.52 +/- 4.15 months in P2, and 5.18 +/- 2.27 months in P3) (P = 0.002). In seven patients, therapy was modified due to antibiotic side effects. When these cases were excluded, therapeutic failure did not differ significantly between ceftriaxone-based regimens (n = 5/166, 3.0%) and the oral therapy (n = 4/42, 9.5%) (P = 0.084). The efficacy of the ceftriaxone-based regimens was found to be better (n = 6/166 [3.6%] versus n = 6/42 [14.3%]; P = 0.017) when a composite negative outcome (CNO; relapse plus therapeutic failure) was considered. Accordingly, CNO was greatest in P2 (14.3%, n = 6/42) compared to P1 (2.6%, n = 3/ 117) and P3 (6.1%, n = 3/ 49) (P = 0.020). Seemingly, ceftriaxone-based regimens are more successful and require shorter therapy than the oral treatment protocol.

Aim: To indicate the clinical course and results of brucellosis in our region during the recent years, and to compare these findings to the literature. Materials and methods: This study was based on a review of the medical records of adult patients older than 14 years who were followed with the diagnosis of brucellosis from March 1997 to October 2010. Results: Included in this analysis were 317 patients, including 136 males (43%), with an average age of 40 ± 17 years. In 66 patients (21%), reproduction of Brucella was identified in the blood. Of the patients, 61% were identified as having the acute form, 35% the subacute form, and 4% the chronic form. Arthralgia, fever, weight loss, sacroiliitis, and spondylitis were the most frequent symptoms and findings accompanying the disease. There was a significant relationship between advanced age and the development of both spondylitis and arthritis (P = 0.000 and P = 0.028, respectively). Furthermore, there was a significant relationship between a high erythrocyte sedimentation rate and the presence of spondylitis, sacroiliitis, and visceral abscesses (P = 0.001, P = 0.013, and P = 0.049, respectively). Conclusion: This study provides a review of the disease and its complications. Osteoarticular involvement, and particularly the presence of spondylitis in patients and the complications in elderly patients, should be studied. Laboratory parameters, the patient's age, and the duration of symptoms may help to identify complicated cases. © TÜBİTAK.

Background: Training of infectious disease (ID) specialists is structured on classical clinical microbiology training in Turkey and ID specialists work as clinical microbiologists at the same time. Hence, this study aimed to determine the clinical skills and knowledge required by clinical microbiologists. Methods: A cross-sectional study was carried out between June 1, 2010 and September 15, 2010 in 32 ID departments in Turkey. Only patients hospitalized and followed up in the ID departments between January-June 2010 who required consultation with other disciplines were included. Results: A total of 605 patients undergoing 1343 consultations were included, with pulmonology, neurology, cardiology, gastroenterology, nephrology, dermatology, haematology, and endocrinology being the most frequent consultation specialties. The consultation patterns were quite similar and were not affected by either the nature of infections or the critical clinical status of ID patients. Conclusions: The results of our study show that certain internal medicine subdisciplines such as pulmonology, neurology and dermatology appear to be the principal clinical requisites in the training of ID specialists, rather than internal medicine as a whole.

Aim: To determine our setting's IDAI rates, infecting microorganisms, and their resistance patterns to achieve standardization and make comparisons among other Turkish and developed country hospitals all over the world. Materials and methods: The numbers of total patient days, ventilator days, central catheter days and, urinary catheter days in the ICUs were recorded and IDAI rates were calculated. Clinical specimens were obtained from patients, cultivated at appropriate culture media, and infecting microorganisms and resistance patterns were determined. Results: Totally 1450 invasive device-associated infection episodes were determined (16.4% of patients) with a rate of 21.12/1000 days. Ventilator associated pneumonia rate was 22.05/1000 ventilator days and most common microorganism was Acinetobacter baumannii. Central catheter associated blood stream infection rate was 9.14/1000 central catheter days and the most common infecting organism was A. baumannii. Catheter associated urinary infection rate was 10.12/1000 urinary catheter days and the most common pathogen was Candida species. MRSA rate decreased from 89.6% in 2006 to 61.8% in 2009 (P < 0.001). ESBL production rates were between 70.7% and 45.6% in Escherichia coli and 66.7% and 55.9% in Klebsiella pneumoniae isolates. Vancomycin resistance among Enterococci was between 34.3% and 21.7% in these years. Conclusion: Our hospital infection rates were found to be similar to those of country data but higher than those in developed nations. Considering the high infection and resistance rates to most of the available antibiotics, it is highly urgent that infection control measures be taken and more effective antibiotic control policies be adopted. © TÜBİTAK.

Objective: This study is aimed to determine the local profile of blood culture isolates and changes in the susceptibility patterns to guide the antibiotic therapy in oncology and haematology units. Methods: Microbiologically documented hospital-acquired bloodstream infections were reviewed between 2005 - 2007 as a part of infection control surveillance in haematologyoncology department of a university hospital. Results: 194 microorganisms were isolated in 170 bloodstream infections episodes. Among these episodes, 79,1% (n=31), 80,9% (n=51) and 70,5% (n=48) were monomicrobial in years 2005, 2006 and 2007, respectively. Among the isolated 194 microorganisms, the ratio of the gram-negative bacteria were slightly increasing throughout these three years; 68,9%, 70,4% and 77%, whereas the gram-positive bacteria were decreasing 31,1%, 23,9% and 19,2%, respectively and Candida species were the cause of 4 episodes (5,6%) in 2006 and 3 episodes in 2007 (3,8%) (p>0.05). The majority of the cases were primary bloodstream infections. The most prevalent secondary cause of bacteremia was urinary tract infections in year 2005 and 2007 and pneumonia in 2006. Extended spectrum beta lactamases (ESBL) rate among E.coli and Klebsiella spp. isolates were 69,6%, 40% and 79,2% in years 2005, 2006 and 2007 respectively. Vancomycin resistance was high; 15/20 among Enterococcus species in the three years. The most effective agents against gram negative bacteria were aminoglycosides and carbapenems. Conclusion: As a conclusion, gram negative microorganisms especially the Enterobacteriaecea are the major cause of bacteremia in haematology and oncology patients. PubMedID: 21248460 Myocarditis due to H1N1 influenza infection has not been previously described. We report on a case of acute fulminant myocarditis caused by H1N1 influenza A virus infection that mimicked acute coronary syndrome. A 50-year-old man was admitted with dyspnea, fever, cough, vomiting, and atypical chest pain of three-day history. His body temperature, pulse rate, and blood pressure were 39.2 °C, 115 beats/min, and 80/40 mmHg, respectively. Electrocardiography showed sinus tachycardia, 1-mm ST-segment elevation, and absence of R wave progression in anterior leads, and ST depression in anterolateral leads. The chest radiogram revealed diffuse bilateral alveolar infiltrates. Cardiac enzymes were elevated. Despite treatment with aspirin, clopidogrel, low-molecular weight heparin, metoprolol, and an ACE inhibitor, he developed hemodynamic instability on the first day of admission. Echocardiographic examination showed anteroseptal, apical, and lateral wall hypokinesia, left ventricular diastolic dysfunction, and dilatation of all the chambers. There was no abnormal finding on coronary angiography. The diagnosis was considered to be myocarditis; thus, anticoagulant and antiaggregant therapies were discontinued, and empirical broad-spectrum antimicrobial treatment was initiated together with antiviral oseltamivir (2x75 mg/day). The patient's clinical condition significantly improved. Nasopharyngeal samples were positive for H1N1 influenza A virus. He was discharged on the 15th in good medical condition.

Chronic lymphocytic leukemia (CLL) is the most common leukemia in many countries. Infections are the most common causes of morbidity and mortality; lymphoid cell dysfunction and neutropenia associated with chemotherapy are main predisposing conditions for infection. For this reason infectious conditions must be excluded in a case with CLL and fever. Richter's transformation (RT) is a kind of lymphoma that is a rare condition in CLL cases Fever of unknown origin is a rare finding in Richter's transformation (RT) but it has been reported as anecdotal reports. Here a case with RT as the cause of fever in a case with CLL was reported and literature was reviewed.

The most active antibiotic against Gram-positive bacteria was imipenem (87.2%) and this was followed by piperacillin/tazobactam (81.7%) and meropenem (77.8%). The rates of production of ESBL by Escherichia coli were 20.9%, Klebsiella pneumoniae 50% and Serratia marcescens were 46.7%. Extended-spectrum beta-lactamase production increased each year (21.7%, 22.1% and 45.5%). All of the ESBL producing isolates were sensitive to meropenem and 98.5% sensitive to imipenem. AmpC beta-lactamase was produced by 20.9% of the Enterobacter species spp, Citrobacter spp. and Serratia marcescens. All of these were sensitive to meropenem and 77.8% to imipenem and ciprofloxacin. Multi-drug resistance rates in Acinetobacter spp were 45.4% and 37.7% in Pseudomonas aeruginosa isolates. Conclusion: As in the entire world, resistance to antibiotics is a serious problem in our country. Solving of this problem depends primarily on prevention of the development of resistance.

PubMedID: 20424552 Background: This study aimed to identify the agents causing community-acquired urinary tract infections (CAUTIs) and their resistance patterns and to investigate risk factors for ESBL production. Material/Methods: Patients diagnosed at the Department of Infectious Diseases in the Cukurova University School of Medicine Hospital with CAUTI between January 2006 and April 2007 were included prospectively. Patient data were recorded and the microorganisms and their sensitivity patterns were evaluated by the university's central microbiology laboratory. Results: A total 146 patients with CAUTIs, 109 women and 37 men (mean age: 50.9±18.44 years), were included in the study. The most common infectious agents were Escherichia coli (76.9%), Klebsiella pneumoniae (9.2%), Proteus mirabilis (4.1%), and Enterococcus spp. (1.6%). The ciprofloxacin resistance rate for E. coli was 35% and resistance to TMP-SMZ 43%, whereas amikacin resistance was substantially low (3%). Four of 12 K. pneumoniae strains were resistant to ciprofloxacin and 2 to TMP-SMZ. Resistance to amikacin was not found in the K. pneumoniae strains. ESBL production was identified in 25 of the 112 E. coli and K. pneumoniae strains. A history of a UTI within the last 6 months (p=0.029) and a history of frequent UTI (p=0.028) were found to be significant risk factors for ESBL production by univariate analysis. The only independent risk factor was a history of urinary system infection in the past 6 months (p=0.025) according to multivariate regression analysis. Conclusions: These high resistance rates to antimicrobials and particularly the extremely high rate of ESBL production in CAUTI should be carefully considered. © Med Sci Monit, 2010.

International audience; BACKGROUND: Mucormycosis is an emerging aggressive mold infection. This study aimed to assess the outcome of hospitalized adults with rhino-orbito-cerebral mucormycosis (ROCM). The secondary objective was to identify prognostic factors in this setting. METHODS: This study was an international, retrospective, multicenter study. Patients’ data were collected from 29 referral centers in 6 countries. All qualified as "proven cases" according to the EORTC/MSGERC criteria. RESULTS: We included 74 consecutive adult patients hospitalized with ROCM. Rhino-orbito-cerebral type infection was the most common presentation (n = 43; 58.1%) followed by rhino-orbital type (n = 31; 41.9%). Twenty (27%) had acquired nosocomial bacterial infections. A total of 59 (79.7%) patients (16 in combination) received appropriate antifungal treatment with high-doses of liposomal amphotericin B. Fifty-six patients (75.7%) underwent curative surgery. Thirty-five (47.3%) required intensive care unit admission (27; 36.5% under mechanical ventilation). Hospital survival was 56.8%, being reduced to 7.4% in patients with invasive mechanical ventilation. A multivariate binary backward logistic regression model identified confusion at admission (OR 11.48), overlapping hospital-acquired infection (OR 10.27), use of antifungal treatment before diagnosis (OR 10.20), no surgical debridement (OR 5.92), and the absence of prior sinusitis (OR 6.32) were independently associated with increased risk for death. CONCLUSION: Today, ROCM still has high mortality rate. Improving source control, rational therpy, and preventing nosocomial infections may improve survival in this severe infection.

The consultation patterns were quite similar and were not affected by either the nature of infections or the critical clinical status of ID patients.Conclusions: The results of our study show that certain internal medicine subdisciplines such as pulmonology, neurology and dermatology appear to be the principal clinical requisites in the training of ID specialists, rather than internal medicine as a whole. © 2011 Erdem et al; licensee BioMed Central Ltd.
