Abstract

Stroke is a leading cause of disability and mortality worldwide, and it disproportionately affects low- and middle-income countries (LMICs), which account for 88% of stroke fatalities. Prehospital stroke care delays are a crucial obstacle to successful treatment in these settings, especially given the limited therapeutic window for thrombolytic treatments, which may greatly improve recovery chances when initiated early after stroke onset. These delays are caused by a lack of public understanding of stroke symptoms, sociodemographic and cultural variables, and insufficient healthcare infrastructure. This review discusses these issues in detail, emphasizing the disparities in stroke awareness and reaction times between locations and socioeconomic classes. Innovative options for reducing these delays include the deployment of mobile stroke units and community-based educational campaigns. This review also discusses how technology improvements and personalized educational initiatives might improve stroke awareness and response in LMICs. The primary goal is to give a thorough assessment of the challenges and potential remedies that might serve as the foundation for policy reforms and healthcare improvements in LMICs, eventually improving stroke care and lowering disease-related mortality and disability. Abstract

Background: Studies have shown that stem cells have promising effect in ischemic stroke management. As an alternative therapy, the effectiveness and safety of mesenchymal (MSC) and mononuclear (MNC) stem cells in acute stroke are still unclear. This review evaluated the efficacy and safety of the use of MSC and MNC in acute ischemic stroke in terms of clinical and structural improvement. Methods: This is a systematic review which is conducted based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guideline. Our review focused on RCT, with acute ischemic stroke population using MSC or MNC, and evaluated the clinical and structural improvements, and safety outcome after administration of the stem-cells. Results: Eight studies were included, consisted of 155 patients in intervention and 408 in control group. In the MNC group, there was significant improvement of NIHSS within one month and achieved mRS = 1 by six months. Slightly different from MNC, studies using MSC showed mRS improvement occurred after 3 months and NIHSS-motor improvement achieved after 2 years of infusion. One month after cell infusion, MRI showed structural improvement, with infarct expansion ratio of 0.9 ± 0.2 (p < 0.05). There was no differences in adverse events between intervention and control group in all studies. Conclusions: This review show that MSC and MNC stem-cells are effective and safe to use as alternative treatment in acute ischemic stroke if other definitive measures could not be done or not available. Abstract

The issue of environmental contamination remains unsolved. The problem continues to have a substantial detrimental impact. This research aimed to identify provinces in Indonesia with high or low levels of environmental pollution so that the government may offer treatment to provinces with high levels of pollution and seek a significant reduction in the incidence of environmental pollution in Indonesia. Clustering is required to identify provinces with high and low pollution levels using the complete linkage method because this method can provide tight clusters and is less impacted by outliers. The analysis of the complete linkage method with Elbow optimization revealed two optimal clusters, namely high and low clusters. The high cluster consists of three provinces: Central Java, West Java, and East Java. The low cluster consists of 31 provinces. This research used a Silhouette Coefficient validity test. The value of the Silhouette Coefficient is 0.75. The value indicates that the data object is in the correct cluster and that the cluster structure is relatively strong.

Abstract

The coronavirus (SARS-CoV-2) pandemic is currently spreading around the world. The crisis that followed was multidimensional in nature, affecting all levels of society. The measures adopted by domestic authorities cover a wide spectrum of restrictions: from general warnings to mandatory quarantines and individual isolation, to travel bans and city closures, and in some cases, country closures. Many governments have declared states of emergency, thereby assuming extraordinary powers. This dire crisis raises several important questions: What are the relevant obligations, powers and procedures under public international law? Have they been complied with? What role, if any, has international law, through its institutions, played so far? The World Health Organization (WHO), a specialized agency of the United Nations, is an international agency with a core mandate in global health issues. In addition, the International Health Regulations (IHR) are the primary legally binding instrument that establishes rules for the transboundary spread of disease. Against this background, to answer the questions above, this journal provides an overview of IHR in light of current issues and disputes. The journal then evaluates these issues and disputes under other international law regimes, such as human rights and peace and security law.
