Einleitung: Das Multiple Myelom (MM) nimmt mit 10% einen bedeutenden Anteil der malignen hämatologischen Systemerkrankungen ein. Während sowohl histologische als auch zytologische Untersuchungen routinemäßig zur Bestimmung der Plasmazellquantität bei der Diagnosestellung eines MM eingesetzt werden, hat die Durchflusszytometrie (DFZ) in der MM-Diagnostik noch keinen weitreichenden Einzug in die klinische Routine gehalten. Dennoch wurden bereits zahlreiche Studien durchgeführt, die das Potential der DFZ zur Diagnosestellung eines MM untersuchten. Eine klare Abgrenzung eines MM von seiner Vorstufe konnte lediglich in einer Studie gezeigt werden. Methodik: Sämtliche Patienten, bei denen in den Jahren 2008 und 2009 an der Medizinischen Klinik mit Schwerpunkt Hämatologie und Onkologie des Campus Charité Mitte unter der Verdachtsdiagnose eines MM eine Knochenmarkpunktion durchgeführt wurde, wurden retrospektiv hinsichtlich ihrer histologischen, zytologischen und durchflusszytometrischen Befunde evaluiert und deren Aussagekraft gegenübergestellt und kritisch diskutiert. Außerdem erfolgte die Einteilung des Kollektivs in MM- und MGUS-Patienten sowie gesunde Personen (gP) anhand aller zu Verfügung stehender Befunde („abschließende Diagnose“). Ergebnisse: Es wurden insgesamt 104 Personen in die Analyse eingeschlossen. Unter Berücksichtigung der abschließenden Diagnose wurden somit 42 MM- und 39 MGUS-Patienten sowie 23 gesunde Personen betrachtet. Die histologischen und zytologischen Befunde zeigten eine Übereinstimmungsrate von 60%, während die histologischen und durchflusszytometrischen Befunde in 80% der Fälle übereinstimmten. Der durchflusszytometrisch ermittelte mediane Gesamtplasmazellanteil an allen gemessenen Knochenmarkzellen betrug 5% (MM) und lag somit signifikant über dem von je 0,8% (MGUS und gP). Der Anteil phänotypisch normaler Plasmazellen (nPZ) an allen Plasmazellen zeigte sich mit im Median 2% (MM), 34% (MGUS) und 57% (gP) in den verschiedenen Gruppen signifikant divergent. Es zeigten 14% der MM- und 82% der MGUS-Patienten einen nPZ-Anteil ≥20%. Schlussfolgerungen: Im Kontext der Diagnosestellung stellt sich die zytologische Untersuchung des Knochenmarkes als weniger verlässliche Methode im V ergleich zu histologischen Untersuchung dar. Die durchflusszytometrische Analyse des Anteils normaler Plasmazellen zeigt dahingegen deutlich verlässlichere Befunde als die Zytologie. Allerdings ist eine klare Differenzierung zwischen MGUS und MM anhand eines absoluten Grenzwertes von 20% normalen Plasmazellen nicht immer möglich., Background: Multiple Myeloma (MM) accounts for 10% of all malignant hematological disorders. While the estimation of plasma cell infiltration in bone marrow biopsy as well as in bone marrow aspirates each represents a standard method in diagnosis of MM, flow cytometric immunophenotyping (FC) has not been implemented in clinical routine diagnostics yet. Nevertheless, numerous studies have been conducted which analyze the value of FC in regard to diagnosing MM. However, only one study was able to differentiate precisely between MM and its precursor disease using FC. Methods: Patients that had undergone bone marrow biopsie in the Department of Hematology, Oncology and Tumor Immunology at Charité – University Medicine Berlin, Campus Mitte in 2008 and 2009 were retrospectively analyzed in terms of their histological, cytological and flow cytometric results. A “final diagnosis” was made by contemplating all diagnostic results available dividing the collective into three different groups. Results: A total of 104 people were included in this study. Thus, 42 MM- and 39 MGUS-patients as well as 23 healthy subject (HS) were analyzed. In 60% of observed cases the histological und cytological results matched, whereas this could be seen in 80% of the cases when comparing the histological and flow cytometrical results. The median percentage of total bone marrow plasma cells (BMPC) identified by flow cytometry in MM patients differed significantly from that of MGUS patients and HS (5% vs. 0.8%). The median amount of phenotypically normal plasma cells (NPC) composed of 2% (MM), 34% (MGUS) and 57% (HS) showed significant differences between the three groups. 14% of MM-patients together with 82% of MGUS-patients showed 20% or more NPC. Conclusion: Regarding the diagnosis of Multiple Myeloma the cytological results appear to be less reliable compared to the histological analysis of bone marrow plasma cells. Flow cytometric evaluation of the bone marrow, however, shows more solid results than those obtained by cytological testing. Nevertheless, a consistent differentiation between MGUS and MM by using a cutoff ratio of 20% normal to all bone marrow plasma cells could not be reached.
