症例は59歳，男性．2年前に直腸癌（Ra）で低位前方切除，膀胱部分切除術を施行した．病理組織学的にはtub2，pSI（膀胱），ly1，v1，pN0，P0，H0，M0，pStageIIであった．退院後UFTを内服し経過観察としていたが，術後2年目のCTで膵尾部に腫瘤を認め精査となった．腫瘍は5cm大で造影効果を認めなかった．腫瘍により脾静脈は閉塞しており，周囲への浸潤を認め，典型的な膵癌の像を呈していた．HbA1c 12.4％と耐糖能の悪化を認め，CEAは6.5ng/mlと上昇していた．膵尾部癌cT4（PV(+)），cN0，cM0，cStage IVaの術前診断で膵体尾部切除術を施行した．病理組織学的には，免疫染色でCK7(-)，CK20(+)，cdx2(+)であり直腸癌膵転移と診断された．術後は化学療法を導入し，外来経過観察中である．大腸癌の膵転移例は少なく，その中でも外科的切除が可能であったのは稀であり，本邦での報告例は自験例を含めて34例であった．

術前化学療法後に胃切除を施行し，乳糜腹水を発症した2例を経験した．症例1:66歳，男性．大動脈周囲リンパ節転移，右鎖骨上リンパ節転移を認めS-1+CDDPを2クール施行し画像上リンパ節転移は消失．幽門側胃切除，D2+化療効果判定のため16b2リンパ節を切除．術後1カ月から乳糜腹水が顕在化し，その後3カ月の保存的治療を行うも治癒せず手術を施行．術中，明らかな乳糜の漏出点は認めず，有効な外科治療は不能．その後2週間の保存的治療で軽快．症例2:64歳，男性．大動脈周囲リンパ節転移を認め術前化学療法としてS-1+CDDPを3クール施行し画像上リンパ節転移は消失．症例1と同様の手術を施行．術後1カ月から乳糜腹水を発症し2カ月の保存的治療で軽快．これまで大動脈周囲リンパ節転移陽性胃癌7例に術前化学療法を行ったが，その内2例に乳糜腹水が発症した．大動脈周囲リンパ節転移陽性胃癌に対する術前化学療法は術後乳糜腹水のリスクを高める可能性がある．

目的:腹腔鏡下胆嚢摘出術（LC）後に診断された胆嚢癌症例の病態を明らかにする．対象と方法:1992年から2004年の術前診断良性のLC 1665例のうち術後判明した胆嚢癌症例12例（0.72％）を2463例の開腹胆嚢摘出（OC）後判明の15例（0.61％）と比較検討した．結果:LC後12例の癌深達度はm 6例，mp 1例，ss 5例と早期癌が58.3％であった．OC後判明例に比較して癌の組織学的所見に差はなかった．LC後の症例ではm癌の全例は経過観察し，胆嚢管断端が癌陽性であった1例のmp癌と，ss癌5例中4例に胆嚢床切除にD2郭清を基本とし追加手術を行った．LC後のss癌1例が他病死，OC後のss癌2例が原病死したが，残る24例は無再発生存中である．結論:m癌ではLC後に追加切除なしで再発はみられずLCが適応となる可能性が示唆された．LC後のss癌判明例では追加切除により良好な成績が得られた．

大腸癌において腸間膜静脈内に腫瘍塞栓を形成する進展様式は稀である．今回，上腸間膜静脈(SMV）に腫瘍塞栓を形成した上行結腸癌の1例を経験した.症例は66歳の女性.嘔吐と腹部膨満感を主訴に来院した．検査の結果上行結腸癌と診断された．CT portographyではSMV本幹に陰影欠損を認め，腫瘍塞栓の存在が強く示唆された．開腹すると原発巣と腫大リンパ節が大きな腫瘤を形成し，SMV本幹は約3cmにわたる腫瘍塞栓によって閉塞していた．結腸右半切除，D3，十二指腸部分切除，SMV合併切除を施行して完全切除した．病理組織学的診断は原発巣，腫瘍塞栓ともに中分化腺癌(tub2）で，原発巣に高度脈管侵襲(ly2，v3）を認めた．最終進行度診断はpSI(大網·十二指腸），pN1，cP0，cH0，cM0，fStage IIIaであった．術後はFOLFIRI療法を施行，1年10カ月の現在まで無再発で経過している．

Holographic data storage capacity is proportional to the thickness of a photopolymer recording medium. To maximize the performance of a photopolymer recording medium, it is necessary to develop a multiplexing technique that has sensitivity in the vertical direction of the medium. As a method to achieve this, we suggest the focus-shift multiplexing method by which holograms are recorded in the vertical direction of the medium. By combining angle multiplexing and this focus-shift multiplexing, we confirm that the multiplexed holograms that are recorded by angle multiplexing are well spatially separated in the vertical direction of the medium, and these holograms can be reconstructed without crosstalk noise.

腹腔鏡下胃切除術は多くのディスポーザブル製品を使用するため，請求可能な材料費を上回る分の使用する材料費が開腹胃切除に比べて大きいといわれている。そこで腹腔鏡下胃切除におけるコスト面での問題を調査するために開腹胃切除と腹腔鏡下胃切除において使用した材料費の合計，手術料および入院費を比較した。 2007年8月から11月に当院で施行した幽門側胃切除 (開腹: ODG5例，腹腔鏡下: LADG5例)，胃全摘 (開腹: OTG5例，腹腔鏡下: LATG5例) において手術材料費，手術料，入院費用 (診療収入＋入院料) を調査した。胃切除術による利益＝胃切除の手術料－(使用したディスポーザブル製品の材料費－請求可能なディスポーザブル製品の材料費) とした。材料費は定価を用いた。 手術利益はODGが278,756.2円，LADGが190,292.8円，OTGが395,922.6円，LATGが330,653.6円となった。平均入院費用 (診療収入＋入院料) はODGが1,390,464円，平均入院期間は21.4日であり，1日あたりの平均入院費用は65,140.0円であった。それぞれLADGでは1,484,254.0円，18.8日，80,805.4円，OTGでは1,956,664.0円，24.4日，82,397.1円，LATGでは1,686,936.0円，18.4日，91,894.8円であった。 腹腔鏡下胃切除は開腹胃切除に比較して手術費は高いが手術による利益は少ない。現状では十分なコスト削減対策が必須である。全入院中の入院費は必ずしも高くならないが在院日数が短いため1日あたりの入院費は高い。

症例は51歳，男性．直腸癌で，1996年8月低位前方切除，D3郭清を行った．病理組織学的検査では，RaRb, 3型，43×42mm，中分化腺癌，SE, ly2, v1, N1, P0, H0, M0, Stage IIIaであった．術後は5'-DFURを内服していたが，1998年9月胸部CTで多発肺転移を認め，FP, UFTとPSKの内服，低用量CPT-11, 5'DFURとPSKの内服，IFL療法などを続けた．肺転移は著変なく経過した．2002年8月，脳MRIで小脳転移を認め，ガンマナイフ治療を行った．その後，小脳，大脳に新たな転移が出現し，2006年1月まで合計11回，20カ所(小脳5カ所，大脳15カ所）のガンマナイフ治療を施行したが，脳転移出現後も，明らかな神経学的異常を認めず，通常通り就労していた．しかし2006年5月，術後9年9カ月(肺転移後7年8カ月，脳転移後3年9カ月）で死亡した．多発脳転移，肺転移をともなった直腸癌に対して，ガンマナイフと化学療法により長期生存をえることができた．

To maximize the use of the performance of a photopolymer recording medium, a multiplexing technique that can record holograms in the thickness direction of such a medium is demanded. To this end, we propose a new multiplexing method that combines shift, peristrophic, and focus-shift multiplexings, and successfully records holograms in the thickness direction of the medium by controlling the incidence angle of the reference beams. We confirm that the holograms recorded are well spatially separated in the thickness direction of the medium, and that up to 50 holograms can be recorded.

症例は60歳代男性．当科初診の4年前に，健診時胸部X線にて左肺門部異常陰影を指摘され前医を受診した．CTにて径2cmの類円形腫瘤を認めたが臨床所見から悪性疾患を積極的に疑わず経過観察となった．2年後，腫瘤径2.5cmとなり末梢閉塞性肺炎が度々みられるようになった．発見から4年後，腫瘤径は3.5cmとなり閉塞性肺炎の増悪を来たしたため手術を施行したが，肺動脈への広範な腫瘍浸潤により肺全摘を余儀なくされた．病理検査にてリンパ上皮腫様癌・T2N1M0，stage IIBと診断された．EBER in-situ hybridizationはシグナル陰性であり，EBウイルスとの関連は証明されなかった．治療方針，特に手術施行時期に関して反省すべき症例であるが，リンパ上皮腫様癌の長期臨床経過を詳細に観察し得た貴重な経験であった．

合併症の早期発見, 予測を可能にするツールとしてクリティカルパスを発展させるためにバリアンスを利用した.バリアンスに明確な基準を設けるために2003年4月から2003年12月に施行した胃切除例で合併症のない66例の日々のバイタルサインを集積し, 各指標の90パーセンタイル値をバリアンス基準値とした. 2005年2月から2005年12月に胃切除を行った120例で実際にクリティカルパスを使用した.バリアンスは70/120例 (58.3%) に発生し, その内, 合併症が発生したものは31/70例 (44.3%) であった. 合併症発生例とバリアンスの関係をみると15/31例 (48.4%) は合併症発生日以前にバリアンスが発生し, 他は合併症発生日と同日に発生していた. 逆にバリアンスの発生しなかった症例では合併症が発生しなかった.この結果からわれわれのクリティカルパスにより合併症の早期発見, 予測が可能になると考えられた.

Objective: The benefits of combining cancer screenings with clinical surveys have become increasingly obvious as cancer morbidity and mortality have steadily increased. This paper discusses a study on the current status of and issues surrounding breast cancer screening in a clinical survey. The study also investigated the patients' awareness of the benefits of breast cancer screening. A secondary aim of the study was to promote mammographic screening. Subjects and Methods: During the 72 months between April 1999 and March 2005, a total of 36,505 women underwent clinical surveys in our hospital. In October 2002, mammographic examination was included as an optional part of the routine physical examination. We evaluated the results of breast cancer screening with or without mammographic examination and used a questionnaire to investigate the patients' awareness of the benefits of breast cancer screening. Results: Compared with the pre-2001 results, the detection rate of breast cancer significantly increased after 2003 when physical examination was combined with mammographic examination. Our study also found that both elderly patients and those residing in the suburbs of Sapporo City tended to choose physical examination alone rather than combining it with mammographic examination. An analysis of the questionnaires collected from these patients indicates they had a poor understanding of and lacked awareness of the benefits of mammographic examinations during breast cancer screenings. Conclusions: The inclusion of mammographic screenings with clinical surveys was found to be significantly useful in the detection of breast cancer. Further continued education is needed for women, particularly the elderly and residents in the suburbs, so they understand the benefits of breast cancer screening by mammographic examination for the early detection of breast cancer and, consequently, decreased mortality of the disease.

症例は77歳, 男性. 検診で貧血を指摘された. 腹部CT, 腹部MRIにて膵頭部から十二指腸下行脚にかけて存在する鉄亜鈴状の腫瘍を指摘された. 血管造影では膵背動脈の分枝に栄養されるhypervascularな腫瘍であった. 膵内分泌腫瘍・十二指腸GIST・膵腺房細胞癌等を考え手術を施行した. 術中所見で腫瘍と膵実質の境界は明瞭ではなく, また腫瘍は十二指腸乳頭近傍に及んでおり, 幽門輪温存膵頭十二指腸切除術を施行した. 病理組織学的には腫瘍は紡錘形細胞が主に索状あるいは渦巻状に配列し, 一部に類上皮細胞も認められた. 免疫染色でc-kit, s-100陽性, CD34, α-SMA, cytokeratin, HHF-35陰性で十二指腸GISTと診断した. 術後1年経過し, 再発なく経過している. その血行支配から膵腫瘍との鑑別が困難な症例であった.

症例は63歳の女性. 左鎖骨上窩の腫瘤を自覚して来院した. 生検で腺癌のリンパ節転移であった. さらに全身を検索するとS状結腸に2型の癌を認め, Virchowリンパ節転移と判明した. さらに肝転移も認めた. 手術はS状結腸切除, D3郭清を行った. 病理組織学的所見は, 中分化腺癌, SS, N2, P0, H1, M1 (Virchowリンパ節No. 216), Stage IVであった. 術後5'-DFUR 600mg/dayの経口投与と週1回のCPT-11 60mgの点滴静注を開始した. 2カ月後にVirchowリンパ節転移とともにCTで肝転移の消失も確認した. しかし, その後, 術後3年11カ月目に左腋窩リンパ節を触知した. 全身精査で他に転移を認めなかったため, 左腋窩腫瘍摘出術を行った. 病理組織学的所見はS状結腸癌のリンパ節転移であった. 現在, 初回手術後5年4カ月経過し再発なく健存中である. 遠隔転移, 肝転移をともなった大腸癌に対して, 積極的な手術と化学療法により長期生存を得ている稀な症例を経験した.

2003年7月から2005年6月までの2年間に当院で外科的切除を行った原発性肺癌62例に対し，重複癌の有無，臨床的背景，病理所見等を検討した．重複癌症例は24例（38.7％）存在し，平均年齢は66.6歳であった．内訳は異時性重複癌16例，同時性重複癌8例．二重癌19例，三重癌3例，四重癌2例．重複癌の臓器は，胃10病巣，大腸8病巣，乳房5病巣，肺3病巣，食道，腎臓，肝臓，舌，前立腺各1病巣であった．肺癌の発見方法では重複癌症例の22例（91.7％），非重複癌症例の31例（81.6％）が無症状での画像検査による発見であった．肺癌腫瘍径に差は認めなかったが，重複癌症例では全例リンパ節転移陰性であったのに対し，非重複癌症例にはリンパ節転移陽性が9例（23.7％）存在した（p＜0.05）．また1994～2003年の10年間に当院で胃癌切除を受けた1640例の術後調査において，2003年7月からの2年間で9例の肺癌罹患が確認された．これは全国肺癌罹患集計値をもとに算出した推測数6.53人よりも多く，胃癌切除既往患者は一般人よりも肺癌に罹患しやすい可能性が示唆されたが，今後全国規模での疫学調査よる検討が必要である．

はじめに: 99mTc-GSAシンチグラフィーによる分肝機能測定の試みが報告されている. これを応用し, 肝右葉切除の術後経過を術前の99mTc-GSAシンチグラフィーから予測できるかを検討した. 方法: 肝右葉切除症例16例を対象として術前術後に99mTc-GSAシンチグラフィーを行った. 血中消失率HH15に対する肝集積率LHL15の比である血中消失補正肝摂取率LHL15/HH15 (GSA index) を算出し, 術後肝不全との関係をみた. さらに, 術前のSPECTから全肝に対する左葉のカウント比を乗じたものを左葉GSA indexと定義して, 実際に術後の99mTc-GSAシンチグラフィーから得られた各パラメーターとの比較検討を行った. 結果: 術後2週間以上続く腹水貯留, 高アンモニア血症などの肝不全傾向をきたした症例の術後2週GSA indexは1.15以下であった. また, 術前左葉GSA indexと術後GSA indexとは相関し, 肝不全傾向を示した症例の術前左葉GSA indexは0.38以下であった. 考察: 術前左葉GSA indexが0.38以下の低値を示す症例では肝右葉切除術後肝不全のリスクの可能性が高いと考えられる. これまで, 99mTc-GSAシンチグラフィーは多くの施設において肝機能の指標として予備的に用いられてきたに過ぎない. 今回は肝右葉切除例のみの検討であるが, 最近報告されているGSA indexによる簡素化と, SPECTからの分肝機能評価法という二つを組み合わせることにより, 術後肝不全予測のための新しい残肝機能測定法となる可能性がある.

症例は48歳女性. C型肝硬変,肝細胞癌(HCC),脾腫による汎血球減少の診断で腫瘍切除および血小板増加を目的とした脾摘を企図した.肝S3の部分切除および脾摘を施行したが,術後6日目のCT検査で門脈内に血栓を認めた.同日よりダナパロイドナトリウムを投与したところ投与開始後12日目で血栓は消失した.ダナパロイドナトリウムはヘパリンナトリウムに比べ,出血傾向などの副作用を生じにくい新しい抗凝固剤である.本邦では播種性血管内凝固症候群のみが適応となっているが,欧米では肺血栓塞栓症など深部静脈血栓症に対しても広く適応とされ本症例においてもその有効性が示された.

症例は57歳,女性.右季肋部と背部の重苦感を主訴に当院受診.造影CTで膵頭部に拡張した血管を内部に多数認める腫瘍が存在した. ERCPで下部胆管は圧排伸展,主膵管も滑らかに伸展され狭小化していた.血管造影では腫瘍血管の増生が著明で, AV shuntが存在し動脈相早期より流出静脈と門脈が描出された.上腸間膜動脈より分岐する栄養血管を塞栓した後に,幽門輪温存膵頭十二指腸切除術(PPPD)を施行した.病理組織検査で腫瘍は紡錘形細胞からなり,免疫染色ではc-kit, vimentinが陽性, CD-34, S-100, desmin, α-SMA, HHF-35は陰性で十二指腸GISTと診断した.膵頭十二指腸領域の血流の豊富な病変に対するPPPD術前の動脈塞栓術は術中出血量を減らす有効な前処置である.

胸壁浸潤を伴う高齢者肺癌症例に対しe-PTFE patchと広背筋弁との併用による胸壁再建術を行った.症例は81歳男性.胸壁浸潤を伴う左S3原発の扁平上皮癌と診断し手術を行った.第3, 4, 5肋骨および前鋸筋と小胸筋の一部を左肺上葉と共にen-blockに切除した.胸壁欠損は約12×10cmで,同部にe-PTFE patchを補填し固定した.さらに強度を安定させるためその上層に有茎広背筋弁をのせ,周辺を大胸筋,前鋸筋,大円筋に固定した.術後奇異呼吸はみられず術後のQOLは良好であった.現在術後4年を経過し無再発生存中である.側胸部のように胸壁が薄く人工材料あるいは筋肉弁補填単独では強度が不十分となりやすい部位に適した胸壁再建方法といえる可能性が示唆された.

転移をともなう腫瘍径10mm以下の直腸カルチノイドの2例を経験した.症例1は64歳,男性.占拠部位はRb,肉眼型はIsで腫瘍径は10mmであったが中心陥凹をみとめた,肝転移をともなっていた.症例2は65歳,女性.占拠部位はRb,肉眼型はIIaで腫瘍径は9mmであったが中心陥凹をみとめた.No.251リンパ節に転移をみとめた.2例ともカルチノイド徴候はみとめず,病理組織検査では深達度sm,lyO,vOで,核異型,mitosisは軽度でありclassical typeのカルチノイドの診断であった.転移をみとめた10mm以下の微小直腸カルチノイドの2例を経験した.2例とも中心陥凹をみとめた.

大腸癌の肝転移切除後の肺転移に対する外科的治療法の意義を確かめるため,1977年から2002年12月までの大腸癌肝転移巣の切除例89例のうち肺に重複転移をきたし切除しえた12例の臨床病理学的所見を検討した.原発巣と同時性の肝転移例は7例で,3例は肝肺ともに同時性であった.残る5例のうち3例は異時性に肝肺同時転移を認め,他の2例は異時性に肝転移をきたしたのち,さらに遅発性に肺転移を認めた.12例とも術後在院死はなく退院した.12例の最終手術後の5年生存率は40.0%,5年無再発生存率は12.5%であった.1例が12年間無再発生存中である.最終手術後1年以内に再発した9例の各転移巣の出現間隔の平均は7.1カ月で,1年以上無再発であった3例の平均37.0カ月に比較し,出現間隔が短かった(p

症例は48歳,女性.検診の上部消化管造影検査で胃癌(胃体部小彎, 0-IIc)と診断され当院を受診.画像診断で膵頭体部に,約5cmの境界明瞭な充実性腫瘍を認めた.軽度の貧血以外は血液,生化学,内分泌学的検査および腫瘍マーカーに異常を認めなかった.胃癌および膵のsolid-pseudopapillary tumorと診断した.手術所見は, 6×3cmの腫瘍が総肝動脈周囲と高度な癒着をしていたが,膵頭体部とは軽度の癒着を認めるのみで,摘出術が可能であった.術中迅速病理で神経鞘腫と診断されたため,腫瘍摘出のみとした.胃癌には幽門側胃切除術を施行した.術前,膵腫瘍との鑑別が困難であったが,術中の肉眼所見および迅速病理にて,後腹膜に発生した神経鞘腫と診断された.膵腫瘍の診療にあたり,膵外より発生した後腹膜腫瘍の可能性も念頭に置くことが必要である.

症例は47歳,男性.検診にて十二指腸の異常所見を指摘され当院受診.上部消化管内視鏡検査にて十二指腸副乳頭部に径15mmの中央に浅い陥凹を伴う隆起性病変を認め,生検でカルチノイドと診断された.超音波内視鏡検査では深達度smと考えられた.腹部CTでは,肝転移,十二指腸周囲のリンパ節腫脹は認めなかった.十二指腸副乳頭部カルチノイドの診断で幽門輪温存膵頭十二指腸切除術を施行した.病理組織学的所見は,十二指腸副乳頭部カルチノイド, 14×13mm,深達度sm,リンパ節14dに3個の転移を認めた.十二指腸副乳頭部カルチノイドは腫瘍径が小さくても本例のようにリンパ節転移陽性例があり,膵頭十二指腸切除のようにリンパ節郭清を伴った根治術が望ましいと考えられた.

胃上部進行癌の脾門リンパ節転移に関連する臨床病理学的因子を明らかにし,脾門リンパ節転移high risk群の選定と脾摘の適応について検討した.対象は1990年から1998年までに胃全摘術,脾合併切除により治癒切除が行われた胃上部進行癌99例である.脾門リンパ節転移例は16例(16.2%)に認め,リンパ節転移陰性例との間で占居部位,肉眼型,腫瘍径,深達度,総リンパ節転移個数,脈管侵襲,進行度において有意差を認めた.多変量解析における脾門リンパ節転移の危険因子は占居部位(大彎,前壁,後壁,全周),肉眼型(3型, 4型)であった.脾門リンパ節転移陽性例の5年生存率は20.8%で,陰性例と比較し有意に低率であった. 1) 占居部位が大彎,前壁,後壁,全周, 2) 肉眼型が3型, 4型の症例は脾門リンパ節転移の可能性が高く脾摘の適応であり,これ以外の症例では脾門リンパ節転移の可能性が低く,脾臓を温存できる可能性があると考えられた.

症例は67歳男性.1985年3月,早期胃癌で幽門側胃切除術を受けた(L,Gre,IIc,tub1,sm,ly0,v0,n0,stage IA).1995年7月,貧血の精査で盲腸癌と診断し結腸右半切除術(D3)を行った.腫瘍は2型,35×25mmで病理組織所見は高分化腺癌,se,Iy3,v1,nl(+),stage IIIaであった.1997年1月,CEAが17.1ng/mlと上昇し,CTで膵尾部,脾の腫瘍と大動脈周囲リンパ節の腫大を認めた.1997年3月,膵体尾部切除,脾摘,残胃全摘,リンパ節郭清を施行した.病理組織学的に盲腸癌の膵,脾転移と診断された.術後CEAは3.1ng,/mlと正常化したが,1998年3月に多発肝転移を認め,1999年1月より癌性腹膜炎となり転移巣切除術後23カ月目に死亡した.大腸癌術後の切除可能な膵,脾転移は極めて稀であるが,切除後長期生存の報告もあり積極的に切除することが望ましい.

過去6年間に3例の嚢胞形成性悪性肝腫瘍の切除例を経験した.症例1は78歳,女性で,肝外側区に嚢胞性腫瘤を認め,周囲に拡張した末梢胆管とB3の間に交通を認め,粘液産生性肝内胆管癌と診断した.切除組織標本で腫瘍の被膜および周囲肝実質への浸潤を認めた.症例2は68歳,男性.嚢胞内に壁在性の乳頭状隆起を伴う多房性病変をS4-8に認め,乳頭腺癌以外に乳頭腺腫,管状腺腫の像も認め,腺腫からの癌化例と考えた.症例3は48歳,女性で,肝S4に球状の多房性の腫瘍と内腔に突出する結節が存在していた.切除標本では腫瘍と胆管との交通を認めたが,腫瘍周囲の細い胆管枝の存在や表層拡大進展がないため,単純嚢胞からの癌化と考えた.症例1は初回手術後25カ月で腫瘍死したが,症例2, 3はそれぞれ56カ月, 16カ月生存中である.

十二指腸乳頭部癌は比較的予後良好とされるが再発をおこすことも稀ではない.今回,十二指腸乳頭部癌に対する膵頭十二指腸切除後5年で肝再発を生じ肝拡大右葉切除術を施行,肝切除後さらに5年以上経過した現在まで再発なく生存している稀な症例を経験した.症例は41歳,女性. 1989年10月13日,十二指腸乳頭部癌の診断で膵頭十二指腸切除, D2を施行した.病理組織検査は papillary adenocarcinoma, pat Acbpd, panc0, dul, n0, stage IIであった. 5年後の1994年11月,発熱のため来院しCTで肝右葉S8を中心とする約10cmの内部に壊死を伴う単発の肝転移を認め肝拡大右葉切除術を施行した,十二指腸乳頭部癌の肝再発は予後不良であるが,切除可能な場合には積極的切除が長期生存につながることがある.自験例は肝再発に対する肝切除が奏効した極めて稀な症例と考えられる.

症例は60歳, 女性. 34歳時, 子宮癌で子宮摘出術と放射線照射を受け, 1年後に照射による直腸狭窄のためS状結腸双孔式ストーマを造設された. 58歳時, ストーマに隆起性病変を認め生検で高分化型腺癌であった. 手術を拒否して以後来院せず, 2年後の受診時にはストーマはほぼ全体が癌であり, MRIではストーマ周囲の腹壁への直接浸潤を認めた. 手術時の皮切は下方に凸の弧状切開とし, ストーマ周囲は紡錘状に腹壁を合併切除した. ハルトマン手術を施行し, 下行結腸単孔式ストーマを左上腹部に再造設した.結腸ストーマから発癌した症例の報告は稀であり, 双孔式ストーマからの発癌例は検索した限り皆無であった. 本症例ではストーマ部の結腸は放射線照射領域にあり, 照射による二次発癌の可能性も考えられる. また皮切に弧状切開を用いたことにより旧ストーマを含む腹壁切除と開腹操作が可能であり, ストーマ再造設部位の選定も容易であった.

Goblet cell carcinoid (GCC) tumor of the appendix is rare, and the prognosis is considered poor compared to classical carcinoid tumor. We present a case of a 60-year old woman who underwent appendectomy for acute appendicitis. Histopathological studies revealed a GCC tumor with submucosal invasion. Based on the malignancy of this tumor and the possible risk of regional lymph node metastases, we performed laparoscopic ileocecectomy with lymph node dissection. Histologically, no residual tumor or lymph node metastases were found. The patient remains well without any sign of recurrence.

Malignant melanoma of the anorectal region is rare, and the prognosis is considered to be poor. We present a case of long-term survival in a 56-year-old patient with primary malignant melanoma in the anorectal area, who complained of anal bleeding. Barium enema showed an elevated lesion in the anorectal region. Colonoscopy revealed a 3 cm sessile tumor with focal pigmentation, and a satellite nodule, 1 cm in diameter. Based on diagnosis of malignant melanoma by biopsy, abdominoperineal resection with lateral node dissection was performed. Pathologically the tumor remained in the mucosa, and no lymph node metastasis was found. This patient refused any adjuvant chemotherapy after the operation, and remains well without any sign of recurrence for seven years.

Recent studies have shown that the high affinity of mesothelin-CA125 interaction might cause intracavitary tumor metastasis. We examined the clinicopathologic significance and prognostic implication of mesothelin and CA125 expression in pancreatic ductal adenocarcinoma.Tissue samples from 66 pancreatic ductal adenocarcinomas were immunohistochemically examined. Proportion and intensity of constituent tumor cells with mesothelin and CA125 expression were analyzed and classified as high-level expression, defined as expression by more than 50% of tumor cells and/or moderate to strong staining, or low-level expression otherwise.A high level of mesothelin was correlated with a higher histological grade (P = 0.049) and the level of blood vessel permeation (P = 0.0006), whereas a high level of CA125 expression was correlated with a higher recurrence rate (P = 0.015). The expression of mesothelin was strongly correlated with that of CA125 (P = 0.0041). Co-expression of mesothelin and CA125 were associated with an unfavorable patient outcome (P = 0.0062).This is the first report showing that co-expression of mesothelin and CA125 were in pancreatic ductal adenocarcinoma, and such co-expression is associated with a poor prognosis. Our finding suggests that co-expression of these two factors plays a significant role in the acquisition of aggressive clinical behavior.

This is a case of a 72-year-old male whose chest computed tomography (CT) revealed a 2.0 x 1.6 cm anterior mediastinal solid tumor during follow-up of an abnormal shadow of the lung. The tumor increased its size during preoperative follow-up, and multilocular cyst was also observed. Radical thymectomy was performed, and histopathologically the tumor was diagnosed as thymic basaloid carcinoma. Thymic basaloid carcinoma is a rare tumor and is often associated with multilocular thymic cyst. There are only 32 cases reported both locally and internationally. Surgical resection is the general treatment for this disease. Adjuvant radiotherapy can be considered in cases of incomplete resection and invasive tumor. In our case, no recurrence of the tumor was noted 12 months post-operative. Generally, the malignancy of thymic basaloid carcinomas are regarded as low-grade compared with other thymic carcinomas, however, since mortality and recurrence have been reported, careful follow-up is required.

There have been few case reports of 3rd-line chemotherapy for gastric cancer. So we reported the results of CPT-11 therapy as the 3rd-line chemotherapy for gastric cancer.549 cases underwent gastrectomy from Jan. 2004 to Aug. 2007 in our hospital. In 76 of these cases, which underwent non-curative resection or evidenced a recurrence until July 2009, were analyzed in this study. CPT -11 3rd-line chemotherapy was administered to 11 cases.The mean survival time of non-curative or recurrent cases was 16.9 months. Mean survival times of the non-chemotherapy group, the group administered only 1st-line chemotherapy, the group administered until 3rd-line chemotherapy, the group administered 3rd-line chemotherapy were 7.9 , 11.3 , 21.4 and 28.9 months, respectively(p=0.000 ). Adverse effects occurred in 90.9% of 3rd-line CPT-11, however, all cases were categorized in GradeI.The group administered 3rd-line chemotherapy survived the longest. It is probably correct to administer 3rd-line chemotherapy, if the patient maintains a good performance status.

A 70-year-old man was admitted to our hospital for further examination of pneumobilia and atrophy in the gallbladder. Abdominal CT scan and EUS revealed that the atrophic gallbladder was occupied by a tumor lesion. In addition, ERCP showed choledochocolonic fistula. Colonoscopy revealed an elevated lesion in the colonic side of fistula, and biopsy of the elevated lesion revealed adenocarcinoma. Cholecystectomy and right hemicolectomy was performed under a preoperative diagnosis of gallbladder carcinoma with choledochocolonic fistula. Pathologically, most of the tumor was localized in the gallbladder, and grew along the mucosa of choledchocolonic fistula. This case was of interest with regard to the relationship between the choledochocolonic fistula and gallbladder carcinoma.

It is predictable that since distal gastrectomy (DG) with Billroth I anastomosis involves no procedures caudal to transverse colon, the effects of the surgical wound are the main cause of adhesive obstruction. Thus, it is an appropriate operation to test the efficiency of a synthetic absorbable adhesion barrier (Seprafilm).The subjects were 282 patients diagnosed with gastric cancer who underwent open DG with Billroth I anastomosis between 2001 and August, 2005. Seprafilm was not used in any patients operated on before April, 2003 (n = 169), but it was used in all patients operated on from May 2003 onward (n = 113). We retrospectively compared the incidences of adhesive obstruction in the Seprafilm group and the non-Seprafilm group.The cumulative incidence of adhesive obstruction was significantly lower in the Seprafilm group than in the non-Seprafilm group (P = 0.021). The respective incidences of adhesive obstruction 2 years after surgery were 0.9% and 6.5%. Multivariate analysis of the occurrence of adhesive obstruction revealed no significant differences in sex, age, body mass index, operation time, blood loss, or degree of lymph-node dissection; however, it revealed a significant difference in relation to the use of Seprafilm (P = 0.049).In this series, Seprafilm reduced the incidence of adhesive obstruction after DG significantly; however, a prospective randomized study will be necessary to confirm this result.

We developed the simulator for volume holograms with the focus-shift multiplexing methods. The Bragg selectivity of the focus-shift multiplexing has been investigated by both of the experiment and the simulation.

Since only a few extensive reports are available on the less invasive nature of laparoscopic gastrectomy, we compared postoperative changes over time in vital signs and hematological parameters between this surgery and laparotomic gastrectomy. Of 188 patients who underwent distal gastrectomy for preoperatively diagnosed early gastric cancer between January 2004 and September 2006, 87 underwent laparoscopy-assisted distal gastrectomy (LADG) and 101 underwent laparotomic distal gastrectomy (DG). The invasiveness of the two procedures was evaluated in 164 patients with no postoperative complications (82 cases of LADG and 82 cases of DG by measuing vital signs daily and performing hematological examination on postoperative days (POD) 1, 4, 7, and 10. For body temperature, heart rate, and blood pressure, significantly lower values were obtained with LADG on 3 and 4 POD, 4 POD, and 3 and 4 POD, respectively. For white blood cell counts (WBC) and C-reactive protein (CRP), significantly lower values were obtained with LADG on 7 and 10 POD, and 10 POD, respectively. For serum protein levels and lymphocyte counts, significantly higher values were obtained with LADG on 1, 4, 7, and 10 POD, and 4 and 10 POD, respectively. Body temperature, WBC, and CRP showed no significant difference immediately after surgery but earlier recovery occurred with LADG. For protein levels and lymphocyte counts, higher values were obtained immediately after surgery. There seemed to be two patterns of less invasiveness in the parameters: the early recovery found for body temperature, WBC and CRP, and the smaller shift immediately after surgery in protein level and lymphocyte count, and probably, heart rate and blood pressure. The complication rate was 18.8% for DG and 5.7% for LADG. LADG is a less-invasive surgical procedure as it produces early normalization or smaller shifts in various parameters and exhibits a low prevalence of complications.

Magnetic resonance cholangiopancreatography (MRCP) was performed in 35 patients to evaluate the feasibility of its use as a postsurgical imaging technique after resection of the pancreas. The surgical procedures performed were: pancreatoduodenectomy in 22 patients, segmental pancreatectomy in 1, distal pancreatectomy in 7, and pyrolus-preserving pancreatoduodenectomy in 5. The pancreatic duct was shown in its entirety in 24 of the 35 patients (68.6%) and was partially visualized in 8 patients (22.9%), but the intrahepatic and extrahepatic bile ducts were visualized completely in all patients. Furthermore, MRCP was able to demonstrate lesions in 3 of 6 patients who had shown clinical evidence of recurrence. The visualization of the pancreatic and bile duct system was satisfactory despite anatomical changes brought about by resection of the pancreas. Thus, we conclude that MRCP is an appropriate follow-up screening test for patients with suspected abnormalities of the biliary and pancreatic duct system.

Background and Objectives Superficial spreading type early gastric cancer is characterized by its atypical growth pattern and occasionally indistinct tumor margin. Because it is a rare form of early gastric cancer, the clinicopathological details are not apparent. The aim of this study was to clarify the clinicopathological features of the superficial spreading type of early gastric cancer. Methods A retrospective study was conducted in 1,062 surgically resected patients with early gastric cancer. Hospital records were compared between patients with superficial spreading type early gastric cancer and those with more common types of early gastric cancer. Results Sixty-nine patients (6.9%) had superficial spreading lesions. The male to female ratio was 1.2:1. The most frequent histological type was signet-ring cell carcinoma (32%). The distinguishing histopathological features were submucosal invasion (67%), lymphatic invasion (32%), and lymph node metastasis (30%). There were discrepancies in tumor area between surgical findings and pathological diagnosis in 24 patients (35%) with superficial spreading type. More extensive lymph node dissection was performed and all patients survived in the group with superficial spreading lesions. Conclusions The most appropriate treatment for the superficial spreading type of early gastric cancer is wide surgical resection with extensive lymph node dissection. J. Surg. A case of distant metastasis to mesh-plug prosthesis in gastrointestinal cancer is presented herein. An 88-year-old man had received mesh-plug repair with high ligation for a recurrence of a right inguinal hernia. Six months later, advanced gastric cancer and advanced transverse colon cancer were detected, and therefore a distal gastrectomy and partial colectomy were performed. Two weeks after the operation, the patient complained of right groin tenderness, and the mesh-plug prosthesis was removed to control any infection. A histopathological investigation demonstrated adenocarcinoma in the plug prosthesis. The patient died of carcinomatosis peritonei 45 days after the last operation.

Background. Mucinous adenocarcinoma of the stomach is a rare histologic type of gastric adenocarcinoma and its features are still controversial. We attempted to clarify the clinicopathologic characteristics of this histologic type. Methods. We reviewed the records of 112 patients with mucinous adenocarcinoma of the stomach (MUC) and 4160 patients with nonmucinous gastric adenocarcinoma (NMUC) for factors including age; sex; tumor location, size, and depth; lymph node metastasis; lymphatic or venous permeation; peritoneal dissemination; liver metastasis; and survival rate. We also investigated the relationship between cancer depth and lymph node metastasis. Statistical analysis included χ 2 and Student's t -tests. Survival rates were calculated by the Kaplan-Meier method and analyzed by the log rank test. Results. MUC was found mostly in younger patients, in the lower part of the stomach and was of larger size, invading to or beyond the muscularis propria, positive for lymph node involvement, and associated with peritoneal dissemination. The overall 5-year survival rate in patients with MUC was lower than that in NMUC patients, because the MUC patients more frequently had advanced stage disease; however, the survival rates in the two groups did not differ significantly. As for the relationship between cancer depth and lymph node involvement, the frequency of lymph node metastasis increased when invasion reached the submucosa in patients with MUC compared with those with NMUC while the frequency of lymph node metastasis increased in the muscularis propria in patients with NMUC compared with those with NMUC. Conclusion. MUC was characteristically found in younger patients, at lower sites, at an advanced stage, and with peritoneal dissemination; MUC had a poor prognosis. Lymph node metastasis should be suspected when MUC invades to the submucosa or deeper.
