- Abstract
Review articles summarize state-of-the-art work and provide a means to organize the growing number of scholarly publications. However, the current review method and publication mechanisms hinder the impact review articles can potentially have. Among other limitations, reviews only provide a snapshot of the current literature and are generally not readable by machines. In this work, we identify the weaknesses of the current review method. Afterwards, we present the SmartReview approach addressing those weaknesses. The approach pushes towards semantic community-maintained review articles. At the core of our approach, knowledge graphs are employed to make articles more machine-actionable and maintainable.

- Abstract
Background: Maternal early pregnancy overweight (body mass index [BMI] 25.0–29.9 kg/m²) and obesity (BMI ≥ 30 kg/m²) are associated with mental and physical health adversities in the offspring. Prenatal programming of the hypothalamic-pituitary-adrenal (HPA) axis has been put forward as one of the mechanisms that may play pathophysiological role. However, evidence linking maternal overweight and obesity with offspring HPA-axis activity is scarce. We studied if maternal early pregnancy BMI is associated with diurnal salivary cortisol, a marker of HPA-axis activity, in young adult offspring. Methods: At a mean age of 25.3 (standard deviation [SD) = 0.6) years, 653 Arvo Ylppö Longitudinal Study participants collected saliva samples for cortisol analyses, at awakening, 15 and 30 min thereafter, 10:30AM, 12:00PM, 5:30PM and at bedtime. Maternal BMI was calculated from weight and height verified by a measurement in the first antenatal clinic visit before 12 weeks of gestation derived from healthcare records. Results: Per each one kg/m² higher maternal early pregnancy BMI offspring diurnal average salivary cortisol was −1.4% (95% CI:−2.6, −0.2, pFDR = 0.033) lower, at awakening it was −2.4% (95% CI:−4.0, −0.7, pFDR = 0.025) lower and the morning average salivary cortisol was −2.0% (95% CI:−3.4, −0.5, pFDR =0.017) lower. These associations were independent of the offspring’s own young adulthood BMI, and other important covariates. Conclusion: Our findings show that young adult offspring born to mothers with higher early pregnancy BMI show lower average levels of diurnal cortisol, especially in the morning. Whether these findings reflect prenatal programming of the offspring HPA-axis activity warrants further investigation.

7. Personalization Needs Extension towards Task Stages in Collaborative Research Work Tasks

- Abstract
People have various goals and intentions while interacting with information items. The intentions are derived from larger, motivating work tasks, and the intentions during the stages of task performance may vary. Further, the information activities are often conducted in collaborative work settings. We present preliminary findings on a collaborative research task process indicating that task-stages are also observable in collaborative tasks. We show that trust building and common understanding of the task emerge and discuss the implications to evaluation of information interaction. Based on our understanding, we claim that the personalization based on individuals’ past actions is not necessarily enough, but needs extension towards task stages in collaborative research tasks.

- Abstract
Background: Previous studies have linked maternal obesity with depressive symptoms during and after pregnancy. It remains unknown whether obesity associates with consistently elevated depressive symptoms throughout pregnancy, predicts symptoms postpartum when accounting for antenatal symptoms, and if co-morbid hypertensive and diabetic disorders add to these associations. We addressed these questions in a sample of Finnish women whom we followed during and after pregnancy. Methods: Early pregnancy body mass index, derived from the Finnish Medical Birth Register and hospital records in 3234 PREDO study participants, was categorized into underweight (<18.5 kg/m²), normal weight (18.5–24.99 kg/m²), overweight (25–29.99 kg/m²), and obese (⩾30 kg/m²) groups. The women completed the Center for Epidemiological Studies Depression Scale biweekly during pregnancy, and at 2.4 (S.D. = 1.2) and/or 28.2 (S.D. = 4.2) weeks after pregnancy. Results: In comparison to normal weight women, overweight, and obese women reported higher levels of depressive symptoms and had higher odds of clinically significant depressive symptoms during (23% and 43%, respectively) and after pregnancy (22% and 36%, respectively). Underweight women had 68% higher odds of clinically significant depressive symptoms after pregnancy. Overweight and obesity also predicted higher depressive symptoms after pregnancy in women not reporting clinically relevant symptomatology during pregnancy. Hypertensive and diabetic disorders did not explain or add to these associations. Conclusions: Maternal early pregnancy overweight and obesity and depressive symptoms during and after pregnancy are associated. Mental health promotion should be included as an integral part of lifestyle interventions in early pregnancy obesity and extended to benefit also overweight and underweight women.

9. Utelämningar och konventioner i amatörundertexter:en jämförelse av amatörundertexter med professionella undertexter

Syftet med avhandlingen är att undersöka hur mycket amatörundertextare gör utelämningar vid undertextningen och hurdana undertextningskonventioner är i bruk i amatörundertexter. Syftet är ytterligare att jämföra amatörundertexter med professionella undertexter och därigenom visa om utelämningsfrekvensen och konventionerna i amatörundertexterna skiljer sig från de som man möter i professionella undertexter. Som material står amatörundertexter och professionella undertexter till två filmer. Undertexterna av den ena är på finska och av den andra på svenska. De finska amatörundertexterna är gjorda av medlemmarna av en gemenskap för amatörundertextare DivX Finland och de svenska undertexterna av medlemmarna av gemenskapen divXsweden. Undersökningen av utelämningarna är kvantitativ och metoden är jämförelsen mellan käll- och måltexten samt jämförelsen av amatörundertexterna med de professionella undertexterna. Analysen av konventionerna är för sin del beskrivande och baserar sig starkt på litteraturen om ämnet. Jämförelsen av amatörundertexterna med de professionella är en del också i undersökningen av konventionerna. Resultaten visar att amatörundertextarna har beaktat behovet att förkorta den ursprungliga dialogen i undertexter. Amatörundertextarna har egentligen gjort mer utelämningar än den professionella undertextaren. Orsaken till det större antalet utelämningar i amatörundertexterna kan vara att det är lättare för amatörundertextarna att göra rena utelämningar i stället för att förkorta källtexten genom omformuleringar så att man inte orsakar informationsförlust. När det gäller konventionerna visar resultaten att amatörundertextarna inte har introducerat nya undertextningskonventioner. Ett undantag till detta gör dock användning av kursiveringar och teckenmängden per sekund i amatörundertexterna. På DivX Finland har amatörundertextarna bestämt sig att avstå från konventionen att göra kursiveringar på grund av att dessa inte syns på rätt sätt med alla mediaspelare. Amatörundertextarna på divXsweden har för sin del gjort kursiveringar i sina undertexter men inte konsekvent — möjligen på grund av olika sätt att tolka reglerna för hur kursiveringar bör användas eller att de ännu inte har lärt sig konventionen. I amatörundertexterna finns inga väsentliga avvikelser från professionella konventioner när det gäller antalet tecken och liggtider. Den lästid som ges till tittarna, det vill säga antalen tecken per sekund, är dock kortare än den som rekommenderas för professionella undertexter i flera undertextblock. Allt som allt verkar amatörundertextarna ändå vara medvetna om professionella konventioner och ha viljan att följa dessa. Avhandlingen är en inblick i amatörundertexter, med andra ord en fallstudie gjort på basis av två amatörundertexter. Avhandlingen ger exempel på de egenskaper som man möjligen kan vänta sig att hitta i andra amatörgjorda undertexter. Undersökning av amatörundertexter av olika gemenskaper skulle i fortsatt forskning utvidga resultaten. Tutkielman tarkoituksena on selvittää, kuinka paljon harrastelijatekstittäjät tekevät poistoja kääntäessään alkuperäistä dialogia tekstityksiksi, ja minkälaisia ruututekstikonventioita harrastelijat käyttävät. Tutkielman tarkoituksena on lisäksi verrata harrastelijatekstityksiä ammattilaisten tekemiin tekstityksiin, ja siten selvittää onko näiden välillä eroja poistojen määrässä, ja poikkeavatko harrastelijoiden noudattamat konventiot ammattilaisten ruututekstikonventioista. Tutkielman aineistona ovat harrastelijatekstitykset ja ammattilaisten tekemät tekstitykset kahteen elokuvaan. Toisen elokuvan tekstitykset ovat suomenkieliset ja toisen ruotsinkieliset. Suomalaiset tekstitykset on tehnyt harrastelijatekstittäjäyhteisö DivX Finland ja ruotsalaiset tekstitykset yhteisö divXsweden. Poistojen tutkimus on kvantitatiivinen, tutkimusmenetelminä lähde- ja kohdetekstin vertailu sekä harrastelijatekstitysten ja ammattilaisten tekemien tekstitysten vertailu. Konventioiden analyysi on puolestaan kuvaileva, ja se perustuu vahvasti aiheesta kertovaan kirjallisuuteen. Vertailu tekstitysten välillä on osa myös konventioiden tutkimista. Tutkimustulokset osoittavat, että harrastelijatekstittäjät ovat ottaneet huomioon tarpeen lyhentää alkuperäistä dialogia tekstityksissä. Harrastelijat ovat tehneet enemmän poistoja kuin ammattitekstittäjät. Harrastelijatekstittäjille saattaa olla helpompaa lyhentää lähdetekstiä tekemällä poistoja sen sijaan, että he tiivistäisivät lähdetekstiä muuntelemalla sitä muulla tavoin kadottamatta samalla informaatiosisältöä. Tämä voi olla syynä siihen, että harrastelijat ovat tehneet enemmän poistoja kuin ammattilaiset. Konventioiden osalta tulokset osoittavat, että harrastelijatekstittäjät eivät ole käyttäneet uusia ruututekstikonventioita. Poikkeus tähän ovat kuitenkin kursivointien käyttö sekä sekuntikohtaiset merkkimäärät tekstityksissä. DivX Finland -yhteisö on päättänyt olla käyttämättä kursivointeja, koska nämä eivät näy oikealla tavalla kaikissa mediasoittimissa. DivXsweden-yhteisön tekstittäjät ovat puolestaan käyttäneet kursivointeja tekstityksissään, mutta eivät johdonmukaisesti — ehkä siksi, että he ovat tulkinneet kursivointisääntöjä omalla tavallaan tai eivät ole vielä oppineet kursivointia koskevaa konventiota. Merkkimäärien ja ajastuksien osalta harrastelijatekstitykset eivät poikkea olennaisesti ammattilaisten tekemistä tekstityksistä. Lukuajat, eli toisin sanoen sekuntikohtaiset merkkimäärät, ovat kuitenkin useassa tekstitysruudussa lyhemmät kuin ajat, joita suositellaan ammattilaisten tekemille tekstityksille. Harrastelijatekstittäjät vaikuttavat kuitenkin olevan tietoisia ammattilaisten noudattamista konventioista ja haluavan noudattaa niitä. Tutkielma on silmäys harrastelijatekstityksiin — toisin sanoen tapaustutkimus, joka perustuu kahteen harrastelijatekstitykseen. Tutkielma antaa viitteitä ominaisuuksista, joita voidaan mahdollisesti löytää muistakin harrastelijatekstityksistä. Useampien erilaisten harrastelijatekstittäjäyhteisöjen tekemien tekstitysten tutkiminen laajentaisi tulevissa tutkimuksissa tämän tutkimuksen tuloksia.

Precipitate and water samples were collected over different seasons from the surroundings of the closed Hammaslahti Zn-Cu-Au, Otravaara pyrite and Paroistenjarvi Cu-W-As sulphide mines. The sampling was carried out from the ditches and drainage ponds of the tailings and waste rock piles. Schwertmannite was the most typical Fe hydroxide mineral found. Goethite was almost as common, was often poorly ordered, and contained up to 10 wt% of SO4. Goethite and schwertmannite were commonly found as mixtures, and they occurred in similar pH and SO4 concentrations. Ferrihydrite was typically found in areas not affected by acid mine drainage, and in acid mine waters with high organic matter or As content. Jarosite was found only in one site. Some gypsum and aluminous sulphate precipitates were also identified. In the spring, after the snowmelt, the acid mine drainage precipitates were predominantly schwertmannite, and were partly transformed during warm summer months to goethite. The phase transformation of precipitates was followed by a decrease in pH values and increase in the SO4 concentrations of waters. Four iron oxides and related minerals were identified in ochreous precipitates collected from the areas around 12 sulphide mines, ore deposits and talc quarries in Finland. Jarosite was found to be characteristic of low-pH (less than 3) environments high in dissolved sulphate, schwertmannite was typically formed where pH was between 3 and 4 and ferrihydrite was found only in neutral environments. Goethite was formed either through transformation via solution of metastable minerals or by precipitation from solution at pH 3 or higher in the presence of sulphate or bicarbonate. Surface sorption of sulphate was typical of low pH goethite. 20. Surgical treatment of ovarian cancer in different hospital categories--a prospective nation-wide study in Finland.

- Abstract
This prospective nation-wide study was performed to evaluate the effect of hospital category and subspeciality training on surgical treatment of ovarian cancer. Data were obtained from a questionnaire filled in by the operating unit, and from the surgical and histopathology reports. The survey included 307 patients. Half of them were operated in the university hospitals where gynaecologic oncologists performed 72% of the operations. This was the case in only 4% and 19% in the central and district hospitals, respectively. In university hospitals, pelvic lymphadenectomy was performed in 88%, and para-aortic lymphadenectomy in 73%, of the patients with stage I disease. The corresponding figures ranged from 11% to 21% in central and district hospitals. For stage III patients operated by gynaecologic oncologists, the estimated odds ratio for no macroscopic tumour was 3.0 times higher (95% CI 1.2-7.5) than for those operated by general gynaecologists. These results favour centralisation of surgical treatment of ovarian cancer. [ABSTRACT FROM AUTHOR]

An important area of improving access to health information is the study of task-based information access in the health domain. This is a significant challenge towards developing focused information retrieval (IR) systems. Due to the complexities of this context, its study requires multiple and often tedious means of data collection, which yields a lot of data for analysis, but also allows triangulation so as to increase the reliability of the findings. In addition to traditional means of data collection, such as questionnaires, interviews and observation, there are novel opportunities provided by lifelogging technologies such as the SenseCam. Together they yield an understanding of information needs, the sources used, and their access strategies. The present paper examines the strengths and weaknesses of the traditional and the more novel means of data collection and addresses the challenges in their application in molecular medicine, which intensively uses digital information sources.

- Abstract
Background: Despite functional and cognitive benefits, few adults and older adults do strength training twice per week with sufficient intensity. Exercise-based active video games (exergaming) may amplify the cognitive benefits of exercise and increase adherence and motivation toward training. However, the benefits of a well-defined and monitored dose of strength training, executed simultaneously or sequentially with a cognitive element, has received little attention. In this study we have two aims: First, to systematically gather the available evidence; second, to suggest possible ways to promote strength exergaming innovations., Methods: We systematically reviewed randomized controlled trials using simultaneous or sequent combined strength and cognitive training or strength exergaming to improve cognitive or functional outcomes in adults and older adults., Results: After screening 1,785 studies (Google Scholar, ACM Digital Library, IEEE Xplore Library, PsycARTICLES, Scopus, Cochrane Library and PubMed) we found three eligible studies. Of the two studies using sequent strength and cognitive training, one showed improved functionality, but the other showed negative effects on cognition. The third study using simultaneous intervention, reported a positive influence on both cognition and function, when compared with either strength training alone or a control group. Moderate level of evidence was showed on GRADE analysis., Conclusion: The existing little evidence suggests that strength and cognitive training improves cognition and function in adults and older adults. The following suggestions may help to promote further innovation: (1) ensure minimal dosage of strength training (30-60 min, 2 × /week), (2) use machine-based strength training devices to control volume and intensity (to prevent cognitive components from interfering with strength training), (3) include power training by using cognitive tasks requiring rapid reactions, and (4) add cognitive memory tasks (to extend the cognitive benefits of strength training per se ), and (5) include motivational exergame elements to increase adherence., Competing Interests: The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest., (Copyright © 2022 Esmaeilzadeh, Kumpulainen and Pesola.)

- Abstract
Objective: Correct assessment of patient urgency is critical to ensuring patient safety in emergency departments (EDs). Although significant time and effort have been devoted to developing triage systems, less attention has been paid to the development of quality control. The aim of this study is to introduce and test observation technique, which enables identifying of patient groups at risk of erroneous assessment in triage. The introduced technique is aimed to be less laborious to use than existing triage quality control methods., Design: The study developed an observation technique for identifying patients with possible erroneous assessments in triage. Data sample for the observation technique is carried out with survey form filled in by nurse., Setting: Hospital ED with ~74 000 patient visits annually., Participants: Consecutive adult patients in an ED for baseline study period of 14 days (1774 patients) in 2010 and control study period of 4 days (541 patients) in 2012., Intervention: Triage observation technique for continuous improvement of triage performance., Main Outcome Measures: Primary measures of triage improvement were triage accuracy and nurses' ability to predict patient admissions., Results: With the observation technique the ED staff was able to identify patient groups at risk for erroneous triage. Under-triage related mostly to patients with chest pain, shortness of breath, collapse, stomach pain and infections. Instead injures and muscular skeletal symptoms were seldom undertriaged even though they are common., Conclusions: EDs can control triage quality with simple observation technique. The usability of observation technique and triage quality improvement process were good., (© The Author 2017. - Abstract
Unlabelled: Paired associative stimulation (PAS) is a non-invasive stimulation method developed to induce bidirectional changes in the excitability of the cortical projections to the target muscles. However, very few studies have shown an association between changes in motor evoked potentials (MEP) after PAS and behavioral changes in healthy subjects. In the present study we hypothesized that the functional relevance of PAS can be seen during fatiguing exercise, since there is always a central contribution to the development of fatigue. Transcranial magnetic stimulation was applied over the motor cortex to measure changes in the MEPs of the soleus muscle before and after PAS. Furthermore, fatigue resistance was tested during 15s sustained maximal isometric contractions before and after PAS. On average, fatigue resistance did not change after PAS, however the change in excitability correlated significantly with the change in fatigue resistance., Discussion: Functionality of PAS intervention was not demonstrated in this study. - Abstract
Purpose: Extensive evidence exists that regular physical exercise offers neuroplastic benefits to the brain. In this study, exercise-specific effects on motor cortex plasticity were compared between 15 skilled and 15 endurance trained athletes and 8 controls., Methods: Plasticity was tested with a paired associative stimulation (PAS) protocol. PAS is a non-invasive stimulation method developed to induce bidirectional changes in the excitability of the cortical projections to the target muscles. Motor cortex excitability was assessed by motor-evoked potentials (MEPs) in the task-relevant soleus muscle, elicited with transcranial magnetic stimulation, before and following PAS. To test for changes at the spinal level, soleus short latency stretch reflexes (SLSR) were elicited before and after PAS., Results: PAS induced a significant (76 ± 83 %) increase in MEP amplitude in the skill group, without significant changes in the endurance (-7 ± 35 %) or control groups (21 ± 30 %). Baseline MEP/post MEP ratio was significantly different between the skill and endurance groups. SLSR remained unchanged after the PAS intervention., Conclusion: The possible reason for differential motor cortex plasticity in skill and endurance groups is likely related to the different training-induced adaptations. The findings of the current study suggest that long-term skill training by skill group induced preferable adaptations in the task-related areas of the motor cortex because increased plasticity is known to enhance motor learning.

- Abstract
Changes in the excitability of the cortical projections to muscles in the upper and lower limbs can be induced in the intact human by paired associative stimulation (PAS). An interstimulus interval (ISI) of 25 ms between peripheral nerve and transcranial magnetic stimuli has been found to be effective when targeting hand muscles. The optimal ISI to induce plasticity changes in the cortical projections to lower limbs is still not well established. The purpose of this study was twofold: first, to investigate the effect of PAS with four different ISIs based on the individual latency of the sensory evoked potential (SEP plus 6, 12, 18 and 24 ms) and second, to evaluate the repeatability of the established optimal ISI. Transcranial magnetic stimulation was used to measure changes in the motor evoked potentials (MEPs) of the soleus (SOL) muscle before and after the PAS interventions. Significant increases in the amplitude of SOL MEPs (88 %) were attained with an ISI of SEP latency plus 18 ms (P32 + 18 ms). The PAS effect was long-lasting, input-specific and supraspinal in origin. The intraclass correlation coefficient to test the repeatability of the PAS intervention with the optimal ISI was 0.85. The results show that the excitability of cortical projections to the soleus muscle can be repeatedly increased after PAS with an optimal ISI of SEP plus 18 ms.

- Abstract
Objective: Our recent prospective, nation-wide study indicated better surgical outcome in ovarian cancer patients operated at university hospitals compared to other hospitals. Here we report how this is reflected in 5-year cancer-specific survival (CSS)., Methods: Detailed 5-year follow-up data were obtained on 275 patients by using a special questionnaire, and the data were verified from the Finnish Cancer Registry data. The hospitals were categorized to university and other hospitals and by the number of operations performed in 1999 (<10, 10-20, or >20 operations). Data were analyzed using the Cox's proportional hazards regression analysis., Results: The study population covered 90% of the epithelial ovarian cancer patients operated in 1999, in Finland. Eighty-two percent of the patients received platinum-based chemotherapy. The percentage of patients treated with a platinum-taxane combination was higher in university hospitals (63% vs. 49%, P=0.037). The 5-year CSS was 56% and the median disease-free survival (DFS) was 33 months. In multivariate analysis prognostic factors for CSS were stage (P=0.0027), residual tumor (P=0.0001), and primary chemotherapy (P<0.0001). Hospital operative volume was associated with residual tumor (P=0.027). When hospital operative volume increased with ten patients per year, the odds ratio for no residual disease was 1.203 (95% CI 1.022-1.417)., Conclusion: FIGO stage, residual tumor, and primary chemotherapy are significant prognostic factors for ovarian cancer. Hospital volume is associated with residual tumor. The results favor performance of ovarian cancer surgery in hospitals with higher operative volumes.

- Abstract
In acid conditions, as in acid mine drainage waters, iron oxide particles are positively charged, attracting negatively charged organic particles present in surrounding natural waters. Schwertmannite (Fe8O8(OH)6SO4) and goethite (alpha-FeOOH) are the most typical iron oxide minerals found in mine effluents. We studied schwertmannite formation in the presence of humic acid. Further, surface charge and adsorption of humic acid on synthetic schwertmannite and goethite surfaces in pH 2-9 and in humic acid concentrations of 0.1-100 mg/L C were examined. Schwertmannite did precipitate despite the presence of humic acid, although it contained more sulphate and had higher specific surface area than ordinary schwertmannite. Specific surface area weighted results showed that schwertmannite and goethite had similar humic acid adsorption capacities. Sulphate was released from schwertmannite surfaces with increasing pH, resulting in an increase in specific surface area. Presence of sulphate in solution decreased the surface charge of schwertmannite and goethite similarly, causing coagulation. In acid conditions (pH 2-3.5), according to the zeta potential, schwertmannite is expected to coagulate even in the presence of high concentrations of humic acid (< or = 100 mg/L C). However, at high humic acid concentrations (10-100 mg/L C) with moderate acid conditions (pH>3.5), both schwertmannite and goethite surfaces are strongly negatively charged (zeta potential < -30 mV) thus posing a risk for colloid stabilization and colloidal transport.

- Abstract
Background: Surgical treatment and staging of ovarian borderline tumors have been reported to be often suboptimal and differ considerably. We evaluated the extent of surgical treatment of these tumors in different hospital categories., Material and Methods: A prospective survey performed in 1999 included 65 patients operated on for borderline ovarian tumors and covered 78% of such patients reported to the Finnish Cancer Registry. Detailed information of demographic data and surgical treatment was reported by the responsible physicians using a special questionnaire after confirmation of histopathology., Results: Fifty-eight patients (89%) had stage I tumor, only two patients (3%) had stage II disease and five patients (8%) had stage III disease with peritoneal implants. The majority of the patients underwent bilateral salpingo-oophorectomy (66%) and hysterectomy (58%). Unilateral salpingo-oophorectomy was performed for 21 (32%) and omentectomy for 22 (34%) patients. Ten out of the 16 women under 40 years of age had fertility-sparing surgery. Peritoneal biopsies were taken in 16 (25%) women and lymphadenectomy was performed for 9 (14%) patients with clinical suspicion of invasive ovarian carcinoma. Frozen section was taken in half of the patients and the histology remained the same in 72% of the final pathology reports. No clear differences of the extent of surgical treatment were detected between different hospital categories. Overall cumulative 5-year relative survival rate was 96%., Conclusions: Bilateral salpingo-oophorectomy and hysterectomy was performed for the majority of patients with borderline ovarian tumor. More attention should be paid to adequate staging of borderline tumors in all hospital categories.

- Abstract
To assess the effect of different hospital types or surgical volume on the survival of ovarian cancer patients, a nationwide and population-based analysis was carried out in Finland. The study included all 3,851 ovarian cancer patients operated from 1983-94. The patients were classified according to the hospital of the first surgery. The hospitals were categorized by type (university, central or other hospital) and, separately, into quartiles by the number of operated patients (surgical volume). The patients operated at university hospitals had better survival than those operated in central hospitals, the 5-year relative survival rates (RSR) being 45% (95% CI = 42-48%) and 37% (34-40%), respectively. RSR in the 'other hospital' category was 45% (42-48%). The RSR for the patients operated in the highest volume hospitals was 47% (43-50%), and by decreasing volume (quartile) the RSR was 40% (36-43%), 40% (36-43%) and 42% (38-45%), respectively. After controlling for potential confounding by stage and age using regression models, the results remained practically the same. The results indicate that further centralizing of operative treatment of ovarian cancer may still improve survival rates on a population level in Finland., (Copyright 2002 Wiley-Liss, Inc.)
