Trigemino cardiac reflex (TCR) which was originally called as OCCULOCARDIAC REFLEX is a physiological response due to the pressure effect on the largest cranial nerve, the trigeminal nerve. Oral and maxillofacial procedures can induce the development of this reflex. TCR is a triad of bradycardia , bradypnea and gastric motility changes due to the efferent activation of the vagal nerve in response to the pressure distribution in Trigeminal nerve. TCR may be generated as a result of procedures or conditions that increase intraocular pressure, strabismus surgery, nasal packing after rhinoplasty, the reduction of zygoma and zygomatic arch fractures, elevation of bone flap or osteotomies, reflection of a palatal flap for removal of a mesiodens, during Le Fort I downfractures, sagittal split ramus retraction, midface disimpaction, cutting maxillary tuberosity, and temporomandibular joint arthroscopy. The purpose of this paper is to discuss the pathophysiology and to review the main risk factors , treatment, prevention and management with emphasis on the role of maxillofacial surgeons and attending anesthetist. Maxillofacial surgeons should be familiar with presentations, preventive measures for the effective management of this complication.

Mandibular fractures represent approximately two-thirds of all maxillofacial fractures (nearly 70%), out of which fractures of mandibular ramus represent 3.09% which is considered to be a rare fracture site. They occur alone or in combination with other facial bone fractures resulting in loss of function and disfigurement. The pattern of mandibular fractures varies with geographic location, physical activity, social, cultural, and environmental factors. The main causes of mandibular fracture are road traffic accidents, interpersonal violence, falls, sports injuries, industrial trauma, and pathological fractures. In this clinical case, the authors describe a vertical ramal fracture extending from subsigmoid region to inferior border of angle of the mandible combined with symphysis fracture which was surgically operated through open reduction and internal fixation by extraoral approach.

Nanocrystalline bismuth telluride, Bi2Te3 has been synthesized by mechanical alloying (MA) the powder mixture composed of Bi and Te in the inert gas atmosphere within a short duration of 15 min. Microstructure characterization of the prepared powder has been investigated adopting the Rietveld’s method using X-Ray diffraction (XRD) data and analyzing high-resolution transmission electron microscopy (HRTEM) images. The XRD pattern of 15 min milled powder is composed of reflections of major Bi2Te3 phase with other two minor phases. Detailed structural information of these minor phases has been reported here which was not explored in any previous works. XRD analysis reveals that the stoichiometric Bi2Te3 phase has been formed after 1h of milling and there is no phase transition up to 10h of milling. HRTEM study also reveals the formation of single phase nanostructured Bi2Te3 phase. The DC (direct current) conductivities of 15min, 30min and 10h ball milled powders are measured within the temperature range of 359 to 633K. The electrical conductivity (σ) of Bi2Te3 indicates the semiconducting nature of the sample. There is no significant change of electrical conductivity of Bi2Te3 phase since its formation after 15min to 10h of milling, as there is no noticeable change in crystallite size.
