Abstract

Item does not contain fulltext, ALK-positive histiocytosis is a rare subtype of histiocytic neoplasm first described in 2008 in 3 infants with multisystemic disease involving the liver and hematopoietic system. This entity has subsequently been documented in case reports and series to occupy a wider clinicopathologic spectrum with recurrent KIF5B-ALK fusions. The full clinicopathologic and molecular spectra of ALK-positive histiocytosis remain, however, poorly characterized. Here, we describe the largest study of ALK-positive histiocytosis to date, with detailed clinicopathologic data of 39 cases, including 37 cases with confirmed ALK rearrangements. The clinical spectrum comprised distinct clinical phenotypic groups: infants with multisystemic disease with liver and hematopoietic involvement, as originally described (Group 1A: 6/39), other patients with multisystemic disease (Group 1B: 10/39), and patients with single-system disease (Group 2: 23/39). Nineteen patients of the entire cohort (49%) had neurologic involvement (7 and 12 from Groups 1B and 2, respectively). Histology included classic xanthogranuloma features in almost one-third of cases, whereas the majority displayed a more densely cellular, monomorphic appearance without lipidized histiocytes but sometimes more spindled or epithelioid morphology. Neoplastic histiocytes were positive for macrophage markers and often conferred strong expression of phosphorylated extracellular signal-regulated kinase, confirming MAPK pathway activation. KIF5B-ALK fusions were detected in 27 patients, whereas CLTC-ALK, TPM3-ALK, TFG-ALK, EML4-ALK, and DCTN1-ALK fusions were identified in single cases. Robust and durable responses were observed in 11/11 patients treated with ALK inhibition, 10 with neurologic involvement. This study presents the existing clinicopathologic and molecular landscape of ALK-positive histiocytosis and provides guidance for the clinical management of this emerging histiocytic entity.

The growing interest over the mobile based communication has attracted the Long-Term Evolution (LTE) technique which facilitates higher data transmission rates to its dynamic users. The LTE-based network involves with scheduling which governs the better communication at different traffic (large, small, medium) condition. The LTE caters up network demands and encounters some potential and unreported problems in presence of large traffic with uncertainty of its associated load. Abstract

Vertical root fracture (VRF) is one of the important causes of root canal treatment failure. The diagnosis of VRF can be difficult and it may occur many years after root canal treatment. In this thesis, the clinical and radiographic aspects of VRF, the factors associated with VRF and the time from root canal treatment to a diagnosis of VRF was investigated. This thesis also investigated the prevalence of microcracks in teeth without root canal treatment. The effect of root filling length on VRF and the effect of in vivo root canal instrumentation on the formation of radicular microcracks was also evaluated. The first part of this thesis concluded that the mean time period from root filling to the clinical presentation of VRFs in root canal treated teeth restored with crowns and without posts was 4.35 (± 1.96) years. Posterior teeth, older patients (>40 years), female patients, and overfilled canals were found to be potential risk factors for VRF. The second part of this thesis has reported a 7.1% prevalence of radicular dentinal microcracks in teeth without root canal treatment, which were found more commonly in mandibular teeth and in older patients (>40 years). The third part of this thesis has indicated that root canal filling to or beyond the radiographic apex can be associated with VRF. The final part of this thesis concluded that the in vivo preparation of root canals with ProTaper hand or ProTaper rotary instruments did not result in the formation of root dentinal microcracks.

Abstract

A case-control study involving 54 cases and 108 matched controls was conducted to find out the association of risk factors like smoking, chewing paan and tobacco and the occurrence of cancers of the oral cavity. More than 80% of the cases were over 40 years of age, with a male: female ratio of 2:1, paan and tobacco chewing were significantly related to the oral cancers (Odds Ratio of 9.3 and 7.8 respectively). Smoking showed a statistically significant relationship with oral cancers among male patients. In addition, the study also established dose-response and time-response relationship these risk factors and oral cancer.

Abstract

Far-infrared transmission and reflection of YBa(sub.2)Cu(sub.3)O(sub.7-delta) (YBCO) films on silicon substrates are discussed. Measurements show the feasibility of using thin high-temperature superconducting (HTSC) films deposited on transparent substrates as radiation modulators. Frequency and temperature-dependent refractive indexes for the HTSC films found can be helpful for design of potential far-infrared devices based on YBCO films on Si substrates.

Abstract

Human fetal lung explants and cultures of a human lung adenocarcinoma cell line were used in the investigation of the regulation of two surfactant protein genes, SP-A1 and SP-A2. These cell lines were cultured in a variety of media to test the effect of a cAMP analog, dexamethasone and insulin on the expression of the surfactant protein. Results indicate that the expression of the two genes undergoes different regulatory mechanisms in both cell lines.
