This study was designed to determine the toxicities, maximum-tolerated dose (MTD), and pharmacokinetic profile of FMdC. FMdC was given orally for 5 consecutive days every 3 or 4 weeks in patients with advanced solid tumors. The starting dose was 8 mg/m2/day. Pharmacokinetic studies were carried out on days 1 through 5 of the first cycle. Ten patients with non-small cell lung cancer received 15 courses of FMdC at doses which were de-escalated from 8 mg/m2/day to 2 mg/m2/day because of unexpected severe toxicities at the starting dose level. Neutropenia was the dose-limiting toxicity. Thrombocytopenia and anemia were mild. Flu-like symptoms and fever were the common non-hematologic toxicities. The MTD was 4 mg/m2/day, since four of six patients developed grade 3--4 neutropenia. At the 4 mg/m2/day dose level, the mean terminal half-life, maximum plasma concentration (Cmax), plasma clearance, and mean residence time on day 1 were 3.20 h, 15.8 ng/ml, 2.91 l/h/kg, and 4.03 h, respectively. The recommended dose for phase II studies with this schedule is also 4 mg/m2/day for 5 days. Further investigations are necessary to establish optimal dosing schedules and routes for the administration of FMdC. Abstract

The mechanisms that mediate the bronchoprotective effects of the proadrenomedullin NH2-terminal 20 peptide (PAMP) on capsaicin-induced bronchoconstriction were analyzed in vivo in anesthetized guinea pigs. The activation of C-fibers by capsaicin induced the release of neuropeptides which resulted in bronchoconstriction. However, capsaicin-induced bronchoconstriction was inhibited by PAMP by preventing the release of tachykinins at the airway C-fiber endings.

Background : Although sex hormones are thought to play an important role in the carcinogenesis of non-small cell lung cancer (NSCLC) in never-smokers, the causative mechanism remains unknown. Passive smoking (PS) is common among East Asian women and has been suggested to be a potential cause of the disease. / Methods : We systematically evaluated the expression of estrogen receptor (ER), the prevalence of PS, and genetic mutations using tumor samples from a prospectively registered cohort of never-smokers with lung cancer. Abstract

Abstract Background Rash, liver dysfunction, and diarrhea are known major adverse events associated with erlotinib and gefitinib. However, clinical trials with gefitinib have reported different proportions of adverse events compared to trials with erlotinib. In an in vitro study, cytochrome P450 (CYP) 2D6 was shown to be involved in the metabolism of gefitinib but not erlotinib. It has been hypothesized that CYP2D6 phenotypes may be implicated in different adverse events associated with gefitinib and erlotinib therapies. Methods The frequency of each adverse event was evaluated during the period in which the patients received gefitinib or erlotinib therapy. CYP2D6 phenotypes were determined by analysis of CYP2D6 genotypes using real-time polymerase chain reaction techniques, which can detect single-nucleotide polymorphisms. The CYP2D6 phenotypes were categorized into 2 groups according to functional or reduced metabolic levels. In addition, we evaluated the odds ratio (OR) of the adverse events associated with each factor, including CYP2D6 activities and treatment types. Results A total of 232 patients received gefitinib therapy, and 86 received erlotinib therapy. Reduced function of CYP2D6 was associated with an increased risk of rash of grade 2 or more (OR, 0.44; 95% confidence interval [CI], 0.21–0.94; *p = 0.03), but not diarrhea ≥ grade 2 (OR, 0.49; 95% CI, 0.17–1.51; *p = 0.20) or liver dysfunction ≥ grade 2 (OR, 1.08; 95% CI, 0.52–2.34; *p = 0.84) in the gefitinib cohort. No associations were observed between any adverse events in the erlotinib cohort and CYP2D6 phenotypes (rash: OR, 1.77; 95% CI, 0.54–6.41; *p = 0.35/diarrhea: OR, 1.08; 95% CI, 0.21–7.43; *p = 0.93/liver dysfunction: OR, 0.93; 95% CI, 0.20–5.07; *p = 0.93). Conclusions The frequency of rash was significantly higher in patients with reduced CYP2D6 activity who treated with gefitinib compared to patients with functional CYP2D6. CYP2D6 phenotypes are a risk factor for the development of rash in response to gefitinib therapy.

Background : Computed tomography (CT)-guided needle biopsy is a well-established and dependable procedure for the diagnosis of pulmonary lesions. Abstract

Etoposide is effective against a variety of human cancers. Unlike like some chemotherapeutic agents which directly act on the genetic material inside the cell, etoposide seems to damage DNA indirectly by affecting an enzyme within the nucleus, DNA topoisomerase II. The interaction between the drug and the enzyme is reversible, and when the drug administration is stopped the topoisomerase II activity begins to return to normal. The mechanism of action of etoposide makes the effectiveness of the drug strongly dependent upon the schedule of administration. Usually, without prolonged exposure the cancer cells treated with etoposide will recover from the toxic effects rather than die. Since etoposide appears to act synergistically with the anticancer drug cisplatin, a study was undertaken to evaluate a schedule of treatment with these two drugs for 22 patients with advanced lung cancer. Eighteen patients had primary lung cancer and four had metastatic cancer in the lungs which had spread from cancer originating elsewhere in the body. The treatment consisted of intravenous cisplatin and oral etoposide on the first day of a course of treatment; the course then continued with oral etoposide every day for 21 days. After 21 days, the etoposide was discontinued for one week prior before the start of the next course. Five different dose levels were evaluated, ranging from 80 to 100 milligrams of cisplatin per square meter of body area and from 20 to 50 milligrams per day etoposide per square meter of body area. Hair loss was a common side effect, and most patients experienced nausea and vomiting that was not so severe that it could not be controlled with antiemetic drugs. Four patients achieved partial responses, which ranged from 1.3 months to over 5.8 months. The study found that an 80 milligram dose of cisplatin on day one and a 40 milligram dose of etoposide daily for 21 days, per square meter of body area, would be a reasonable starting point for further evaluation of the treatment. Abstract

Small cell lung cancer is more responsive to chemotherapy and radiotherapy than are other forms of lung cancer. Nevertheless, most patients still relapse, and cures are rare. After failure of initial therapy, death comes rapidly, occurring within seven to eight weeks following relapse. An attempt was made to evaluate the use of high-dose etoposide with cisplatin for the treatment of small cell lung cancer in 20 patients who had relapsed after initial treatment. Two patients died from infections following treatment. Of the remaining 18 patients, half achieved responses. The average survival time was 20.3 weeks, indicating a synergistic effect between the high-dose etoposide and cisplatin. The study also experimentally treated five patients with recombinant granulocyte colony-stimulating factor in an effort to reduce the period of white blood cell suppression which follows the chemotherapy. (It is during this period that potentially fatal infections are most likely to occur.) Abstract

Small cell lung cancer (SCLC) grows rapidly and has a high propensity for spreading quickly and widely. Although SCLC is sensitive to both radiation and chemotherapy, most patients suffer relapses within a year or two, probably due to the emergence of subpopulations of cancer cells resistant to chemotherapy. Since some anecdotal evidence suggested that doxorubicin played a role in the health of the few patients who survived SCLC more than five years, a retrospective study was completed of 159 patients with SCLC. Ten patients survived for five years or more; this indicates a five-year survival rate of 6.3 percent, and a cure rate comparable to other published reports. The nine patients who could be evaluated had a complete response to initial chemotherapy. Statistical analysis of the data did not reveal a significant role for radiotherapy. In the patients studied, age and sex did not appreciably affect survival. These results are not in agreement with some other published data. Chemotherapeutic regimens containing doxorubicin were, however, significantly correlated with improved chances of survival. Abstract

Background : Epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) are routinely used to treat advanced non-small cell lung cancer (NSCLC) patients with activated EGFR mutations, and are associated with excellent response and improvement of performance status. Adipose tissue produces and releases substances called adipokines, which include adiponectin, leptin, resistin, and hepatocyte growth factor (HGF), etc. Previously, we reported that high levels of plasma HGF at diagnosis indicated intrinsic resistance to EGFR-TKIs. EGFR-TKIs have been hypothesized to affect these adipokines. / Methods : This prospective study, to evaluate the correlation between plasma adiponectin and insulin levels and non-hematological adverse effects in advanced NSCLC following EGFR-TKIs administration, was conducted at the Osaka City University Hospital.
