1. CORRELATION OF HEAVY METAL LEVELS IN THE BLOOD OF PATIENTS WITH COLORECTAL CANCER WITH BIOLOGICAL FACTORS AND TREATMENT METHODS

Abstract: The aim of the study. To evaluate the effect of radiation and chemotherapy treatment on the content of Cr, Cd, Cu, Pb, Zn in the blood of patients with colorectal cancer and to study the correlation between metal content and stage of CRC, sex, primary tumour location, blood type and rhesus factor. Object and methods. The study is based on the results of observation of 157 patients with CRC. The control group consisted of 15 individuals without malignant neoplasms. The studied metals in the blood of patients were determined using the method of atomic absorption determination of micro- and macroelements in ash solutions of biological materials. Results. The change in the content of Cr and Cd after radiation therapy was detected and no statistically significant change in the content of metals after chemotherapy was found. There was no significant correlation of metal content depending on sex and location of the primary tumour. There was a correlation of Cu, Zn and Pb content among patients with blood groups II and III, as well as Cu with blood groups I and II. The results of studying of the influence of patients’ rhesus status on the metal content showed a higher content of Cu and Zn which was statistically significant in the group of rhesus negative patients; for Cd, Pb and Cr the results were statistically insignificant. When studying the content of HM depending on the stage of CRC, the highest correlation was found in Pb, the lowest – in Cu. Conclusions. Cr content was 1.3 and 1.2 times lower in patients after radiotherapy and surgery + radiotherapy, respectively, compared with patients who received surgical treatment. The level of Cd was 1.3 times higher in patients after radiation treatment compared with patients after surgery. There is a statistically significant higher content of Cu (1.14 times), Zn (1.1 times) and Pb (1.29 times) in patients with blood group III compared with blood group II, as well as higher Cu content (1.13 times) in patients with blood group I compared with group II; there is also a correlation between Cu and Zn content with rhesus status: in both cases higher concentrations of metals are registered in the group with negative rhesus status of patients (1.15 and 1.11 times, respectively), for all other comparison groups the error was statistically insignificant.

Abstract: Here, we present research results on developing a compact multifunctional mobile robotic platform based on tracks. Nowadays, choosing the optimal control system for such equipment remains relevant. We have proposed a system that allows measuring thermal indicators of objects or other environmental parameters in hazardous areas and can be controlled remotely via video channels and a specially developed free-programmable console. We also suggest using a steerable two-axis camera gimbal on a moving platform to implement enhanced VR/AR/FPV technology for efficient robot control. It provides a new quality of operator immersion in control compared to common land-based mobile platforms. With this in mind, we have presented the main points related to the set of components and algorithms of the control system, which is installed directly on the moving platform, and the control system that ensures the operation of the operator panel.

Abstract: This article presents a practical implementation of an approach for the automated attestation of the indicators of manipulation for systems of stationary industrial robots with one arm and one clamping device. A developed mathematical model of systems for the manipulation of the metric attestation process for industrial robots is presented. Attestation is performed by properly performing the relevant calculation procedures using the CoppeliaSim and MATLAB software products. A certification example of a real robotic handling system model ABB IRB 140 and clamping device model RG2 is shown, illustrating the effectiveness of the proposed approach to improvement.

10. Challenges for non-English speakers: inter- and intralingual factors shaping the writing of Ukrainian authors in biologial sciences

Abstract: Background. Nowadays, almost all indexed journals expect submissions in English, which is a great challenge for exophonic authors. Code-switching context, where cross-language effects, especially native language interference, are well distinct, is critical for approaching the dilemma. Navigating the complicated issues of language-related challenges will be impossible without referring to three crucial levels of written production: lexical, syntactic, and textual. In our investigation, we address the nature of potential errors and their inter- and intralingual origins. In particular, we identify and interpret the deviations from Standard English in scholarly research writing of Ukrainian authors in the field of life sciences, exemplify and classify errors into categories based on the type of language misuse. Materials and Methods. Language material for the study comprised 50 manuscripts submitted by authors from Ukraine to the journal “Studia Biologica”. This research is a mixed-method study encompassing descriptive qualitative and descriptive quantitative methods. Content analysis was employed as the data gathering technique. The analysis of texts was focused on tracing deviations from consistent principles and rules of Standard English and linguistic features of English research discourse and encompassed such steps as highlighting the error, cross-checking and stating the deviation, listing and classifying the errors, and tracing a possible connection of the error to authors’ first language interference. Results. The study identified language areas where Ukrainian authors fail to effectively communicate their ideas to the global academic community. At the textual level, the problem areas encompass defective paragraph structure and excessive verbosity. At the syntactic level, the most critical deviations from the language and stylistic norm comprised misuses of word order and clauses, wordy and confusing sentences with multiple issues that hinder the readability of text. The most widespread grammatical mistakes include missing predicates, faulty subject-verb agreement, incorrect forms of the verb, and inappropriate use of articles, pronouns, demonstratives and quantifiers. At the lexical level, the prevalent errors relate to various types of loan translation, but also include improper word choices and poor vocabulary. Orthographic mistakes, though in minority, refer to the spelling of toponyms, capitalisation, switching from American to British orthographic standards and other random spelling errors. Conclusions. An insight into the nature of the analysed deviations suggests the presence of both intra- and interlingual factors that cause mistakes in papers submitted for publication in the field of life sciences. The error analysis can be beneficial in the educational process for both educators and practitioners. Proper understanding of the functional mechanism of the mistakes might increase the awareness of the potential pitfalls and consequently help avoid them. The classification of errors can be adopted in the educational process and contribute to the development of error pedagogy.

Abstract: Background Tumor samples from the phase III IMpower010 study were used to compare two programmed death-ligand 1 (PD-L1) immunohistochemistry assays (VENTANA SP263 and Dako 22C3) for identification of PD-L1 patient subgroups (negative, positive, low, and high expression) and their predictive value for adjuvant atezolizumab compared with best supportive care (BSC) in resectable early-stage non-small cell lung cancer (NSCLC).Methods PD-L1 expression was assessed by the SP263 assay, which measured the percentage of tumor cells with any membranous PD-L1 staining, and the 22C3 assay, which scored the percentage of viable tumor cells showing partial or complete membranous PD-L1 staining.Results When examining the concordance at the PD-L1-positive threshold (SP263: tumor cell (TC)≥1%; 22C3: tumor proportion score (TPS)≥1%), the results were concordant between assays for 83% of the samples. Similarly, at the PD-L1–high cut-off (SP263: TC≥50%; 22C3: TPS≥50%), the results were concordant between assays for 92% of samples. The disease-free survival benefit of atezolizumab over BSC was comparable between assays for PD-L1-positive (TC≥1% by SP263: HR, 0.58 (95% CI: 0.40 to 0.85) vs TPS≥1% by 22C3: HR, 0.65 (95% CI: 0.45 to 0.95)) and PD-L1-high (TC≥50% by SP263: HR, 0.27 (95% CI: 0.14 to 0.53) vs TPS≥50% by 22C3: HR, 0.31 (95% CI: 0.16 to 0.60)) subgroups.Conclusions The SP263 and 22C3 assays showed high concordance and a comparable clinical predictive value of atezolizumab at validated PD-L1 thresholds, suggesting that both assays can identify patients with early-stage NSCLC most likely to experience benefit from adjuvant atezolizumab.Trial registration number NCT02486718.

Abstract: BACKGROUND Among patients with resectable early-stage non-small-cell lung cancer (NSCLC), a perioperative approach that includes both neoadjuvant and adjuvant immune checkpoint inhibition may provide benefit beyond either approach alone. METHODS We conducted a randomized, double-blind, phase 3 trial to evaluate perioperative pembrolizumab in patients with early-stage NSCLC. Participants with resectable stage II, IIIA, or IIIB (N2 stage) NSCLC were assigned in a 1:1 ratio to receive neoadjuvant pembrolizumab (200 mg) or placebo once every 3 weeks, each of which was given with cisplatin-based chemotherapy for 4 cycles, followed by surgery and adjuvant pembrolizumab (200 mg) or placebo once every 3 weeks for up to 13 cycles. The dual primary end points were event-free survival (the time from randomization to the first occurrence of local progression that precluded the planned surgery, unresectable tumor, progression or recurrence, or death) and overall survival. Secondary end points included major pathological response, pathological complete response, and safety. RESULTS A total of 397 participants were assigned to the pembrolizumab group, and 400 to the placebo group. At the prespecified first interim analysis, the median follow-up was 25.2 months. Event-free survival at 24 months was 62.4% in the pembrolizumab group and 40.6% in the placebo group (hazard ratio for progression, recurrence, or death, 0.58; 95% confidence interval [CI], 0.46 to 0.72; P<0.001). The estimated 24-month overall survival was 80.9% in the pembrolizumab group and 77.6% in the placebo group (P = 0.02, which did not meet the significance criterion). A major pathological response occurred in 30.2% of the participants in the pembrolizumab group and in 11.0% of those in the placebo group (difference, 19.2 percentage points; 95% CI, 13.9 to 24.7; P<0.0001; threshold, P = 0.0001), and a pathological complete response occurred in 18.1% and 4.0%, respectively (difference, 14.2 percentage points

Abstract: This article presents a practical implementation of an approach for the automated attestation of the indicators of manipulation for systems of stationary industrial robots with one arm and one clamping device. A developed mathematical model of systems for the manipulation of the metric attestation process for industrial robots is presented. Attestation is performed by properly performing the relevant calculation procedures using the CoppeliaSim and MATLAB software products. A certification example of a real robotic handling system model ABB IRB 140 and clamping device model RG2 is shown, illustrating the effectiveness of the proposed approach to improvement. 34. MANIFESTATIONS OF URINARY SYSTEM PATHOLOGY AT YOUNG AGE PATIENTS WITH TEMPORОMANDIBULAR JOINT DISEASES

Abstract: Relevance. In recent years, many have been devoted to the problem of the temporomandibular joint (TMJ) diseases, in which the attention is paid to the widespread pathology of TMJ at young people, which develops against the background of genetically-determined weakness of connective tissue (CT), which is also present occurrence of various concomitant diseases of polygenic-multifactorial nature, including the organs of the urinary system (US). Objective. To investigate the manifestations of US pathology in young patients with TMJ diseases. Materials and methods. The study involved 32 patients (m-4, w-28) with diseases of the TMJ, whose average age was 31.6 ± 7.7 years, who were treated at the Dental Medical Center of the Bogomolets NMU. Manifestations of US pathology in patients of this group have been investigated. Examination of patients was performed according to the classical method of examination of patients with diseases of the TMJ. The orthopantomography with examination of the mandibular heads, open mouth X-ray by Parma, computed tomography or MRI scan, iridobiomicroscopy were obligatory. The presence of pathology of US was founding out from the anamnesis vitae, the patient was referred for consultation to a nephrologist. Ultrasound of the kidneys, urinary tract, urine tests were performed for all examined patients. The obtained laboratory data were referenced in the International System of Units and processed by variational statistics using MedStat and EZR v.1.35 (Saitama Medical Center, Jichi Medical University, Saitama, Japan, 2017), which is a graphical interface to RFSC (The R Foundation for Statistical Computing, Vienna, Austria). Results. The following manifestations of TMJ pathology were found in young patients with US diseases: chronic cystitis – 37.5% of cases, oxalate or urinary crystalluria – 37.5 %, nephroptosis – 9.4 %, urolithiasis – 6.3 %, chronic pyelonephritis – 6.3 %, double kidney – 3.1 %. Structural local or chromatic changes of the iris stroma in the projection region of the kidney (75 %) and bladder (43.6 %) in young patients with TMJ diseases showed a congenital weakness of the CT of these organs and a tendency to develop pathology of the US. Changes in urine tests were mainly in the form of oxalate crystalluria (34.4 %), increase specific gravity (15.6 %), proteinuria (9.4 %), which further confirmed the present pathology of the urinary system. Conclusions. The manifestations of developmental abnormalities and other kidney and bladder diseases found in young patients with TMJ disease suggest that there is a correlation between flow of pathological processes in the TMJ and US, which may be based on changes in the CT system. When planning the treatment of young patients with TMJ diseases and pathology of US, medicines that do not affect the urinary system and do not provoke exacerbation of the disease should be prescribed. In the presence of pathology of the kidneys and/or urinary tract, the ultrasound of this system should be prescribed in consultation with a nephrologist.

35. INCIDENCE OF UTERINE SARCOMA IN IVANO-FRANKIVSK REGION

Abstract: Uterine sarcoma (US) is a rare disease characterized by an aggressive course and poor prognosis, accounting for 3–7% of malignant neoplasms of the uterus. The goal. Study the incidence of uterine sarcoma in Ivano-Frankivsk region and to analyze the clinical and morphological characteristics of the tumor. Materials and methods. Our research is based on clinical observations at the Municipal Non-Profit Institution “Precarpathian Clinical Oncology Center of the Ivano-Frankivsk Regional Council” for the period from 2010 to 2021. The results. The analyzed data showed that the number of patients with uterine sarcoma was: stage I – 51.61%, stage II – 25.8%, stage III – 6.45%, and stage IV – 16.15%. Special attention should be paid to the observation of 48 patients with stages I, II uterine sarcoma (pT1-2NOMO). The peak incidence of uterine sarcoma in women is observed in the age group of 40-59 years (60.4%). The most common histological type is leiomyosarcoma, which accounts for 66.67%. Due to the different pathogenesis of the development of mesenchymal tumors, their clinical characteristics also differ. Leiomyosarcoma of the uterus is most often detected in the age group of 40-59 years (43.75%), but for endometrial stromal sarcomas, the peak incidence falls on the age group of 50-81 years, and it is 22.9%. In patients with leiomyosarcoma stage I, the distribution of histological grading is almost the same for G1, G2 – 28.1% and 25%. In stage II leiomyosarcoma, G2 prevails – 18.75%. For endometrial stromal sarcoma, in contrast to leiomyosarcoma, G3 prevails for stage I and II, which is 38% and 25%, respectively. In most patients with uterine sarcoma, an unencumbered oncology history was observed. Indicators of tumor markers (CA125, REA) for most patients with stage I-II uterine leiomyosarcoma were normal, for 66.67%. Conclusions. Most often, uterine sarcoma occurs in women of perimenopausal and menopausal age; the peak incidence of uterine leiomyosarcoma is in the age group of 40-59 years. The peak incidence in women with endometrial stromal sarcoma was observed in the age group of 50-81 years. Most cases are diagnosed at stage I of the disease, and the most common histological type of all uterine sarcomas is leiomyosarcoma. Endometrial stromal sarcoma is the second histological type of spreading, which in world practice demonstrates the advantage of detecting a low degree of malignancy G1-2. In our clinical study, in stages I-II, the frequency of detection, corresponding to the degree of tumor differentiation, G3 prevails. Among patients with stage I leiomyosarcoma, women with normal BMI prevailed (20.83%); with stage II, women with excess body weight and obesity (Class III) prevailed, which amounted to 25%. The majority of patients with stage I endometrial stromal sarcoma had excessive body weight, which amounted to 36.4%. Women with obesity of Class I predominated in stage II endometrial stromal sarcoma – 27.3%. Non-aggravated oncology history was characteristic for majority of patients with uterine sarcoma. 87.5% of women with stages I, II leiomyosarcoma and 73% of patients with stages I, II endometrial stromal sarcoma had non-aggravated oncology history. In most patients with uterine sarcoma and those who had more than three pregnancies, uterine leiomyoma in the anamnesis was found. Indicators CA125, CEA for most patients with leiomyosarcoma were normal.

Abstract: The process of choosing the robotic machine - assembling technologies (RMAT) is implemented as the fuzzy multi - criteria selection of alternatives by the suggested previousely method of quasi - best case. The basic concept feature of the given method is the developed specific correlations which are based on the corresponding comparisons to better alternative Р◦nd to the most important criterion. All the mentioned above determines the practical and scientific value of this paper. The results of strict ranking of the elements of local criteria discrete set (LCDS) are input dР◦ta. It is performed by the method of expert survey and at the same time demonstrates RMAT phenomenon. The idea of selections the process of ordering constituents of initially unordered LCDS elements which finally form ordered set. The selection is performed within the set of these elements. The obtained ordering of RMAT phenomenon as a result of selection is recommended to be analyzed in the process of choosing. The base of solving the task of RMAT selection is its formalized description and on its base the generalized content formalisms of quasi - best case method are determined. The performance of the presented the theoretical issues is demonstrated step - by - step with the real example of the automated RMAT selection.

37. Political Participation and Motivation: The Heuristic Potential of Sociological Concepts of Donatella Della Porta and Drew Halfmann (comparative analysis)

Abstract: The aim of the article is to demonstrate the potential of sociological concepts by Italian political scientist and sociologist Donatella Della Porta and American sociologist Drew Halfmann, to analyze political participation and motivation for it, to determine a new perspective for understanding the essence of political participation, the factors by which political involvement can be transformed. Within the framework of the analysis of D. Della Porta’s concept, the features of the organization of social movements and the mobilization of their participants in the information society are outlined, based on the emergence of horizontal ties between the participants of social movements based on the formation of identity, solidarity, collective actions and democratic innovations. The features of citizens’ political participation and the process of opinion formation in connection with the functioning of communicative spaces with high discursive quality are described. Attention is drawn to the D. Della Porta’s concept of political consumerism, which defines heuristic possibilities for studying political participation through the lens of political consumption and demonstrative behavior, as well as identifying factors influencing political participation (belonging to a group, hedonism, demonstration of a certain way of life, etc.). Within the framework of the analysis of the concept of D. Halfmann, the specificity of the implementation of social policy and its influence on citizens, the success of social movements depending on the political context are considered. D. Halfmann’s argument regarding the conceptual advantages of using the term “political context” to identify institutional conditions and factors conducive to political participation is outlined, and the author’s position on the expediency of using the term “political opportunity” is indicated, taking into account the institutional, structural and dynamic factors of the political system. The tools that can facilitate the mobilization of social movement participants, in particular the grotesque proposed by D. Halfmann and meme as its counterpart in modern society, are considered.

38. Predictive value and changes of inducible nitric oxide synthase expression in patients with rectal cancer under the influence of antitumor therapy

Abstract: The aim of investigation – to establish the features of iNOS expression in the rectum wall under the influence of various neoadjuvant therapy options in patients with rectal cancer (RC) and to find out its predictive significance. Materials and methods. In 88 patients with stage II–III rectal adenocarcinoma we established iNOS expression in the tumor before and after neoadjuvant radiotherapy (NRT), therapeutic pathomorphosis grade and density of immune cell infiltrates. Patients were divided into 3 groups: 1 – patients after the course of NRT on the tumor site; 2 – patients who underwent chemoradiomodification of the NRT with drug tegafur; 3 – patients who underwent chemoradiomodification of the NRT with tegafur and L-arginine. Results. In the tumors of rectal cancer the level of iNOS expression by 4.5 times exceeded that in the intact rectum wall (P < 0.001), correlated with metastatic lesion of the regional lymph nodes (r = 0.55; P = 0.026) and depended on the type of growth of RC. The level of iNOS expression in the tumor prior to treatment in Group 3 patients, in which the high efficacy of NRT was reported, was 54.61 ± 3.55 versus 35.10 ± 2.28 СU in the rest of the patients (P = 0.01). After NRT, most Groups 1–2 patients reduced the iNOS expression in the RC tissue – on the average to 37.30 ± 4.13 СU, more in the residual parenchyma as compared to the tumor stroma, while Group 1 patients showed the lowest total immunosensitivity intensity of iNOS – 24.34 ± 3.10 CU, and Group 3 patients – the highest – 45.35 ± 5.84 CU. Conclusions. After the use of NRT, iNOS expression in the residual parenchyma of the RC decreased, but in the stroma the marker expression remained unchanged or there was an increase in the marker expression, which was most inherent to therapeutic pathomorphosis grades III–IV. iNOS expression in the RC stroma was highest in patients after NRT with radiomodification using tegafur and L-arginine and positively correlated with the density of immune cell infiltrates in the tumor. Evaluation of the preoperative level of iNOS in patients with RC can serve as a predictive test of advisability of NRT used on the background of radiomodification with L-arginine.

39. RISK FACTORS OF POSTOPERATIVE COMPLICATIONS DEVELOPMENT IN PATIENTS WITH RECTAL CANCER

Abstract: Rectal cancer remains one of the most pressing problems today. Despite the development of technologies that can stage the disease and the use of modern treatment protocols, there is no continuous linear improvement in patient survival. The various concepts proposed for the treatment of colorectal cancer do not lead to a statistically significant improvement in overall survival. This may be due to the variability of the concepts of rectal cancer treatment, including the choice of a short or long course of neoadjuvant therapy, a standard regimen or total neoadjuvant therapy, surgery or "watch and wait" tactics. The likelihood of developing surgical complications, especially in cases of neoadjuvant treatment, also remains high, despite the development of minimally invasive accesses and the use of suturing devices, which in turn can lead to poor survival, local recurrence, and disability in patients. Understanding different concepts and strategies for treating patients with rectal cancer, systematically assessing various risk factors for postoperative complications, and modifying them individually can improve not only local outcomes but also overall survival of rectal cancer patients. Several studies have shown that the use of neoadjuvant chemotherapy in patients with rectal cancer significantly increases the risk of postoperative complications, although it reduces the likelihood of local and long-term recurrence. Therefore, given that neoadjuvant therapy is usually a mandatory step in treatment, further research should be conducted on the effects of different doses and regimens, and there is an urgent need to study other modified risk factors for postoperative complications, including BMI, stapler use, size stitching circular apparatus and others. Only in-depth study and accumulation of more data will allow you to more effectively choose the right treatment tactics that will increase life expectancy and reduce the disability of patients with rectal cancer. Thus, despite the great advances in understanding the treatment of rectal cancer, the application of the concept of combination therapy and the development of technical equipment, there are still several controversial issues that require detailed study. In particular, this applies to neoadjuvant chemotherapy, and a detailed study of the timing and different courses of radiation therapy, to ensure maximum effectiveness and reduce the risk of complications. This can be achieved only with the use of an individualized approach to the treatment of patients. The aim of the study is to assess the impact of various risk factors for postoperative complications, including anastomotic failure in patients receiving neoadjuvant chemotherapy. A research and analysis of PubMED publications for 2015-2022 has been made, which provided data on the use of neoadjuvant therapy in patients with rectal cancer and assessed risk factors for postoperative complications. The search queries included "rectal cancer", "anastomosis failure", "neoadjuvant therapy", "risk factors", and "survival of rectal cancer patients". Analysis of treatment and postoperative surgical complications in patients with adenocarcinoma of the rectum stage I-III in the Precarpatian clinical oncological centre during 2019-2022 was performed.

Abstract: Background: Homologous recombination repair (HRR) enables fault-free repair of double-stranded DNA breaks. HRR deficiency is predicted to occur in around half of high-grade serous ovarian carcinomas. Ovarian cancers harbouring HRR deficiency typically exhibit sensitivity to poly-ADP ribose polymerase inhibitors (PARPi). Current guidelines recommend a range of approaches for genetic testing to identify predictors of sensitivity to PARPi in ovarian cancer and to identify genetic predisposition. Design: To establish a European-wide consensus for genetic testing (including the genetic care pathway), decision making and clinical management of patients with recently diagnosed advanced ovarian cancer, and the validity of biomarkers to predict the effectiveness of PARPi in the first-line setting. The collaborative European experts’ consensus group consisted of a steering committee (n = 14) and contributors (n = 84). A (modified) Delphi process was used to establish consensus statements based on a systematic literature search, conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines. Results: A consensus was reached on 34 statements amongst 98 caregivers (including oncologists, pathologists, clinical geneticists, genetic researchers, and patient advocates). The statements concentrated on (i) the value of testing for BRCA1/2 mutations and HRR deficiency testing, including when and whom to test; (ii) the importance of developing new and better HRR deficiency tests; (iii) the importance of germline non-BRCA HRR and mismatch repair gene mutations for predicting familial risk, but not for predicting sensitivity to PARPi, in the first-line setting; (iv) who should be able to inform patients about genetic testing, and what training and education should these caregivers receive. Abstract: The object of research: Masters of Dentistry professional training in Bogomolets National Medical University (Kyiv, Ukraine) with the distance learning technology Investigated problem: Educational process in extreme condition as pandemics and wars, possible ways to solve the problem how students and teachers cope with both challenges in Ukraine on the way of preparing competent doctor from the point of view of foreign students. The main scientific results: Preliminary data of medical university students from foreign countries - future masters in dentistry of the Bogomolets Medical University (Kyiv, Ukraine) survey, conducted in term April-November 2022 about their perceptions of online learning and teaching in extreme conditions as they experienced during lockdown in terms of COVID -19 outbreak and continuing russian`s aggressive war in Ukraine. Our findings from survey concern questions of student`s general view on online educational technology as well peculiarities of approaches in teaching we can use for acceptable in such extreme conditions educational outcome. We start discussion about some useful methodological approaches for distance education technology in extreme conditions. The area of practical use of the research results: Sharing with educational community our experience and features about distant learning in extreme conditions of dental students.

Abstract: This article explores the use of tutor-mediated peer reviews in ESOL classes. It discusses a collaboration between the ESOL Program and writing consultants from the University Writing Center to provide peer review sessions for ESOL writing classes. The benefits of tutor-mediated peer reviews are highlighted, including increased student engagement, improved feedback, and positive relationships with the writing center. The article also addresses the challenges faced in implementing this practice and emphasizes the importance of tutor training in working with multilingual students. Strategies for tutors during in-class visits and peer review facilitation are discussed, emphasizing the importance of dialogue, open-ended questions, and nonverbal communication. 50. МОЖЛИВОСТІ ЗАСТОСУВАННЯ ЕНЗИМОТЕРАПІЇ В КОМПЛЕКСНОМУ ЛІКУВАННІ ЗАХВОРЮВАНЬ СКРОНЕВО-НИЖНЬОЩЕЛЕПНОГО СУГЛОБА

Abstract: Мета дослідження. Оцінити ефективність застосування серратіопептидази у пацієнтів із захворюваннями скронево-нижньощелепного суглоба (СНЩС). Матеріали і методи. У дослідженні приймали участь 36 пацієнтів (ч-11, ж-25) із захворюваннями СНЩС, середній вік яких становив 45,2 ± 7,3 років, що знаходилися на лікуванні в стоматологічному медичному центрі НМУ імені О.О. Богомольця. Пацієнти були розділені на 2 групи: основна – 18 осіб, порівняння – 18 осіб. Пацієнти приймали НПЗП, хондропротектори, комбіновані препарати кальцію та вітаміну D3. Пацієнти основної групи додатково приймали серратіопептидазу 10 мг по 1 таблетці 3 рази на добу протягом 30 днів. Обстеження хворих проводили за класичною методикою обстеження пацієнтів із захворюваннями СНЩС. Оцінювали динаміку клінічних проявів у пацієнтів на 7, 21, 30 добу лікування. При об’єктивному обстеженні враховували ступінь відкривання рота, об’єм рухів нижньої щелепи, відмічали наявність звуків (шумів) у суглобі. З’ясовували наявність, вираженість больових відчуттів у СНЩС за вербальною шкалою оцінки інтенсивності болю, Verbal Descriptor Scale (VDS). Отримані дані лабораторних досліджень наводили в Міжнародній системі одиниць та обробляли методами варіаційної статистики з використанням пакету MedStat та статистичного пакету EZR v.1.35 (Saitama Medical Center, Jichi Medical University, Saitama, Japan, 2017), що являє собою графічний інтерфейс до RFSC (The R Foundation for Statistical Computing, Vienna, Austria). Результати. Відмічено зменшення болю як у стані спокою так і під час жування у всіх (100%) пацієнтів обох груп через 7 днів лікування, відповідно 2,1 ± 1,1 та 4,3 ± 0,9 бали. У хворих обох груп під час повторних відвідувань було відмічено збільшення відкривання рота до 3,7 ± 0,7 см, покращився об’єм рухів щелепи, зменшився хрускіт і крепітація в суглобі. Пацієнти основної групи під час повторних відвідувань через 1 місяць спостереження мали достовірно кращі показники зменшення болю як в стані спокою так і під час жування: відповідно 0,6 ± 0,4 та 2,1 ± 1,1 бали (p < 0,05) – стан спокою; 1,6 ± 1,1 та 3,3 ± 1,0 бали (p < 0,05) – під час активних рухів щелепи. Така сама закономірність прослідковувалася і в покращенні об’єму рухів щелепи та відкриванні рота в пацієнтів основної групи, порівняно з групою порівняння, відповідно 4,4 ± 1,0 см та 3,9 ± 0,8 см (p < 0,05). Висновки. Застосування серратіопептидази в комплексному лікуванні пацієнтів із захворюваннями СНЩС зменшує біль у суглобі як в стані спокою, так і при активних рухах нижньої щелепи, дозволяє покращити об’єм рухів у СНЩС, ступінь відкривання рота до 4,4 ± 1,0 см, порівняно з групою порівняння (p < 0,05). Виявлено достовірне поступове зниження болю, покращення відкривання рота, зменшення хрускоту та крепітації в СНЩС у обох групах протягом усього часу дослідження (p < 0,05).
