Podstawy: Stwardnienie rozsiane (SM) jest przewlekłą, zapalną, demielinizacyjną i autoimmunologiczną chorobą OUN, w której dochodzi do wieloogniskowego uszkodzenia tkanki nerwowej. Na SM chorują najczęściej osoby młode między 20. a 40. rokiem życia. U chorych występują różne objawy (ruchowe, czuciowe, móżdżkowe, zaburzenia widzenia, zaburzenia autonomiczne, zespoły bólowe oraz objawy psychiatryczne), a w związku z tym jest to jedna z najczęstszych przyczyn niepełnosprawności u osób młodych. Cel: Ocena egzogennych potencjałów wywołanych (PW, wzrokowych, słuchowych i somatosensorycznych) u hospitalizowanych w klinice pacjentów, u których na podstawie badań klinicznych, laboratoryjnych i neuroobrazowych rozpoznano SM. Materiał i metody: Badaniami objęto 22 pacjentów w wieku 16,11 ± 1,3 roku z rozpoznaną zgodnie z kryteriami McDonalda postacią rzutową SM. Badania neurofizjologiczne egzogennych PW zostały wykonane zgodnie z zaleceniami Międzynarodowego Towarzystwa Neurofizjologii Klinicznej. Wyniki: Zmiany w zakresie wzrokowych PW stwierdzono u 20 z 22 pacjentów i najczęściej było to wydłużenie latencji końcowej i/lub zmniejszenie amplitudy załamka P100. Największe nieprawidłowości w tym zakresie wystąpiły u pacjentów po przebytym pozagałkowym zapaleniu nerwu wzrokowego, które rozpoznano u 11 z 22 chorych. U żadnego pacjenta nie stwierdzono wydłużenia latencji załamków potencjałów słuchowych, a u 4 z 21 stwierdzono nieznaczne wydłużenie interlatencji tych załamków. W zakresie somatosensorycznych PW stwierdzono wydłużenie uśrednionych latencji wszystkich załamków i wydłużenie czasu ośrodkowego przewodzenia wraz ze zmniejszeniem uśrednionej amplitudy większości z nich. Wnioski: Wzrokowe PW są przydatne w diagnostyce subklinicznych ognisk demielinizacyjnych w OUN. Droga słuchowa cechuje się wysoką odpornością na ośrodkowe procesy demielinizacyjne. Zmiany w somatosensorycznych potencjałach wywołanych wskazują na konieczność wnikliwej pozamózgowej diagnostyki SM. Backgroud: Multiple sclerosis (MS) is a chronic inflammatory, demyelinating and autoimmune disease of the CNS, in which a multifocal damage of the nervous system tissue occurs. MS usually affects young persons between 20. and 40. years of age. There are various symptoms in affected patients (movement, sensory, cerebellar, visual, autonomic, pain syndromes and psychiatric symptoms) and so it is one of the most common causes of disability in the young. Aim: To evaluate the exogenous evoked potentials (EP), visual, auditory and somatosensory in hospitalized patients, in whom MS was diagnosed based on clinical, laboratory and neuroimaging examination. Material and methods: 22 patients were included, aged 16,11 ± 1,3 years, with the relapsing remitting MS diagnosed according to the McDonald criteria. The EP neurophysiological testing was conducted in accordance with the International Federation of Clinical Neurophysiology. Results: Changes in the visual EP were observed in 20/22 patients and the most often was a prolongation of the distal latency and/ or decreased P100 amplitute. The most severe disturbances in this regard were observed in patients after retrobulbar neuritis, which was diagnosed in 11/22 patients. Prolongation of the auditory wave latency was not observed in any of the patients, and a mild prolongation of the interlatency was observed in 4/21 patients. Regarding to somatosensory EP a prolongation of averaged latency of all components and a prolongation of the central conduction time with a decreased averaged amplitude of most of them. Conclusions: Visual EP are useful in the diagnostics of the subclinical focal demyelination of the CNS. Changes in the somatosensory evoked potentials indicate a necessity for thorough extracerebral diagnostics of SM.

Nowoczesne programy leczenia ostrej białaczki limfoblastycznej (ALL) umożliwiły wyleczenie wysokiego odsetka dzieci. Długoletnie przeżycia pacjentów chorujących w wieku dziecięcym na ALL zachęcają do poznania jakości ich życia, w tym również łagodnych, często ukrytych, ograniczeń ich aktywności psychofizycznej. Farmakoterapia groźnych dla życia chorób wymusza niejednokrotnie trudne merytorycznie i emocjonalnie balansowanie między niepożądanymi objawami toksycznymi stosowanych leków a korzyściami terapeutycznymi wynikającymi z ich zastosowania. Nowoczesne chemioterapeutyki używane w leczeniu ALL cechuj ą się wysoką skutecznością, jednak ich stosowanie obarczone jest niejednokrotnie wieloma powikłaniami ogólnoustrojowymi, w tym również ze strony układu nerwowego. Polineuropatia obwodowa jest jednym z częściej spotykanych objawów niepożądanych związanych ze stosowaniem wielu chemioterapeutyków. Wśród objawów ośrodkowych największy niepokój wzbudzają objawy udaropodobne, encefalopatia oraz uszkodzenie nerwów czaszkowych. Z kolei zajęcie układu wegetatywnego może prowadzić do niedrożności porażennej jelit, zaparć czy też hipotonii ortostatycznej, a także zaburzeń rytmu serca. Aby uniknąć trwałych następstw leczenia, powikłania w zakresie ośrodkowego i obwodowego układu nerwowego w czasie leczenia ALL wymagają szybkiej diagnozy i leczenia. Pomimo szeroko zakrojonych badań mechanizmy powstawania większości niepożądanych następstw terapii lekami przeciwnowotworowymi pozostają niejasne. Niekiedy niezbyt nasilone następstwa kliniczne i neurofizjologiczne w obwodowym układzie nerwowym oraz mięśniach są obserwowane nawet wiele lat po zakończeniu leczenia. Badania neurofizjologiczne cechują się dużą przydatnością w diagnostyce tych ośrodkowych i obwodowych następstw. Najlepiej poznano wpływ leczenia ALL na obraz badania elektroencefalograficznego. Mniej znane są natomiast wpływ ALL i jej leczenia na parametry badania elektroneurograficznego, elektromiograficznego, a także potencjałów wywołanych egzogennych, a szczególnie endogennych. Modem treatment programs of acute lymphoblastic leukemia (ALL) resulted in high cure rate of children. Long-term survival of children who had ALL prompt to examine their quality of life, including mild restrictions of their psychophysical activity. Pharmacotherapy of life-threatening diseases needs to be adjusted to strike a balance between effectiveness and side effects of used medications. Modern chemotherapeutics used in ALL treatment are highly effective however associated with many side effects, including central nervous system. Peripheral polyneuropathy is one of the most common side effect of many chemotherapeutics. Stroke-like symptoms, encephalopathy and cranial nerves impairment are the most worrying of central nervous system symptoms. However, vegetative nervous system impairment may lead to bowel paralysis, constipation, orthostatic hypotonia or even arrhythmia. In order to avoid permanent sequelae of ALL treatment, side effects concerning central and peripheral nervous system need to be diagnosed and treated as soon as its possibile. In spite of active research, mechanisms of majority advers events of anticancer drugs remain unclear. Mild clinical and neurophysiological impairment of peripehral nervous system and muscles is sometimes observed several years after treatment termination. Neurophysiological examinations are very useful in diagnostics of these central and peripheral sequelae. Influence of ALL treatment on electroencephalography examination is best known. Influence of ALL treatment on electroneurography, electromyography parameters, egzogenous and particularly endogenous evoked potentials is less known.

Podstawy: Migrenie towarzyszą objawy dysfunkcji autonomicznego układu nerwowego, dlatego podejrzenie dysautonomii budzi duże zainteresowanie. Cel: Celem tej prospektywnej pracy jest ocena równowagi autonomicznego układu nerwowego podczas testów stymulacyjnych, a szczególnie podczas biernej pionizacji u dzieci z migreną. Materiał i metody: Do obecnego badania zakwalifikowano 10 dzieci z migreną w wieku 12-17 lat oraz 10 dzieci z grupy kontrolnej. Kryterium wykluczającym była padaczka oraz choroby autonomicznego układu nerwowego. Badanie wykonano za pomocą Task Force Monitor w spoczynku, podczas 10 minut biernej pionizacji, a następnie w pozycji leżącej (przeprowadzono test izometrycznego skurczu przedramienia, stymulacji zimnem oraz pogłębionych oddechów). Wyniki: Za pomocą t testu wykazano, że wartości RRI (RR-interwał) w teście HG są istotnie różne od wartości RRI spoczynkowego w grupie dzieci z migreną. W pozostałych testach nie stwierdzono istotnych różnic pomiędzy dziećmi z migreną i grupą kontrolną. Wnioski: Wstępne wyniki badania nie wskazują na zaburzenia równowagi autonomicznego układu nerwowego podczas biernej pionizacji u dzieci z migreną. Różnice stwierdzone podczas izometrycznego skurczu przedramienia wymagają potwierdzenia w większej grupie dzieci. Background: Migraine is associated with symptoms of autonomic dysfunction, therefore dysautonomia become the issue of interest. Aim: The aim of this prospective research was the analysis of autonomic balance during tests stimulating autonomic nervous system, especially during head up tilt test. Material and methods: The examined group consisted of 10 children with migraine aged 12-17 and the control group of 10 children without headaches. The criteria of exclusion were epilepsy and autonomic nervous system disorders. The examination was performed using Task Force Monitor, as a basal measurements and recordings during 10 minutes of passive tilting, hand grip (HG), cold pressor test and hyperventilation in the lying position. Results: Using a t test it was established that there were significant differences of RRI (heart rate interval) during HG between migraine patients and controls. The results of other tests were similar in both groups. Conclusions: Preliminary data in children with migraine does not suggest disordered autonomic balance during passive tilting. Differences detected during hand grip should be confirmed in a larger population.

At neurodevelopment age arachnoid cysts (ac) are commonly found by accident in neuroimaging examinations performed in the process of differential diagnostics of different diseases, including epilepsy. They are neurodevelopmental disorders which usually develop in the fetal life. Ac are usually supratentorial, less often subtentorial and sporadically interstellar. They are often the only structural lesions of the brain found in the child with epilepsy. There is still lack of univocal statement whether presence of ac determines type of seizures and focal paroxysmal changes in EEG recording.Estimation the association between cyst localization and changes in bioelectric activity in inter seizure EEG recording and type of epileptic seizures observed in children with ac.Analysis included 14 children out of 35 with found pineal cyst hospitalized in the Department of Pediatric Neurology Chair of Pediatric and Adolescent Neurology in Krakow, in the years 2007-2009. There were 4 girls and 10 boys at the age range from 1 month to 14 years. On the basis of clinical symptoms and EEG results diagnosis of epilepsy was established.Pc were usually localized in the temporal region and posterior cranial fossa and had characteristics of single lesions. In minority of children they were co-occurring with other brain development disorders. In children with pc generalized and polymorphic seizures were dominating and in EEG recording generalized paroxysmal changes.No association between presence and localization of pc and type of epileptic seizures and characteristics of changes in inter seizure EEG recording was found.
