Introduction: Podcasts are audio broadcasts distributed over the internet that can be consumed through various platforms. From students to independent clinicians, the profile of medical podcast users is varied and a richer understanding of this user is essential for effective podcast development. Objective: To review the current literature on the use of podcasts as a health education tool and to understand the stages involved in preparing a medical podcast. Methods: The study is a narrative review developed through research in databases carried out at Unichristus University, in the city of Fortaleza-CE. An analysis of articles on podcasts or data on the prevalence of use, production or results of the use of podcasts for health education purposes was performed. A screening was performed by reading the abstracts and titles of 165 articles found in the Medline database, using the keywords ‘Podcasts’ and ‘medical education’, which resulted in 23 articles that met the inclusion criteria. Results: Based on the content found in the literature, the review was divided into seven categories: formats, episode duration, activities carried out while listening to a podcast, learning assessment, suggestions, developing a medical podcast and the podcast as a complementary teaching tool. Conclusion: the podcast is considered a promising medium of communication as a complementary tool to the traditional teaching methods, and educators should focus on developing evaluation processes for this technology, refining evidence-based guidelines for creating new podcasts. Resumo: Introdução: Podcasts são transmissões de áudio distribuídas através de internet e podem ser consumidas por meio de plataformas variadas. De estudantes a clínicos independentes, o perfil de quem consome podcast médico é variado e uma compreensão mais rica deste usuário é essencial para desenvolvimento eficaz do podcast. Objetivo: Revisar a literatura atual sobre uso do podcast como ferramenta de educação médica e entender sobre as etapas de elaboração de um podcast médico. Métodos: O estudo trata-se de uma revisão narrativa desenvolvida através de pesquisa em bases de dados realizada na Universidade Unichristus, na cidade de Fortaleza-CE. Foi realizada análise de artigos sobre podcast ou com dados sobre prevalência de uso, produção ou resultados do uso de podcasts para fins de ensino médico. Foi feita triagem por resumo e título de 165 artigos encontrados na base de dados no Medline com as palavras-chave Podcasts e educação médica, que resultou em 23 artigos que preencheram os critérios de inclusão. Resultados: A partir do conteúdo encontrado na literatura, a revisão foi dividida em sete categorias: formatos, duração do episódio, atividades realizadas enquanto se escuta um podcast, avaliação do aprendizado, sugestões, desenvolvendo um podcast médico e o podcast como ferramenta de ensino complementar. Conclusão: O meio é considerado promissor como ferramenta complementar aos métodos tradicionais de ensino, devendo os educadores se concentrarem no desenvolvimento de processos de avaliação desta tecnologia, refinando diretrizes baseadas em evidências para criação de novos podcasts.

RESUMO Introdução Na residência médica, os residentes exercem o duplo papel de professor e aprendiz. Treinamentos de como ensinar residentes a exercer a função de docência com qualidade estão sendo implementados em todo o mundo e são denominados programas de Residente como Professor (Resident as Teacher – RaT). De acordo com as Diretrizes Curriculares Nacionais do Curso de Graduação em Medicina, pode-se aprender a estabelecer objetivos educacionais e matriz de competência, além de habilidades como comunicação, empatia, mediação de conflitos, entre outras. A implementação desses programas deve ser baseada na compreensão do contexto de cada residência médica na qual se deseja inseri-los, e os residentes também devem conhecer as potencialidades de cada cenário de atuação da sua especialidade para promover uma docência eficaz. Objetivo O objetivo do estudo foi o desenvolvimento de uma estrutura de programa de treinamento de docência (RaT) dividido em módulos e detalhado com suas respectivas referências bibliográficas. Metodologia Realizou-se uma revisão de literatura sobre ensino médico e testaram-se alguns métodos em cenário de prática durante uma dissertação de mestrado, utilizando a técnica SNAPPS, e durante uma tese de doutorado, utilizando a técnica OMP. Resultado Foi construído um programa de treinamento de docência nas residências médicas baseado nos resultados positivos encontrados na literatura sobre ensino médico. Propõe-se que cada módulo tenha a duração de um mês aproximadamente, com atividades de um turno por semana. O formato da discussão pode variar de acordo com cada especialidade e com o número de residentes de cada programa. Sugere-se a realização de flipped classroom (sala de aula invertida) com o envio do referencial bibliográfico de suporte sobre os temas aos residentes por e-mail uma semana antes da realização de cada módulo. Diversas atividades podem ser postas em prática em cada módulo para sedimentação do material estudado. Conclusão Após a estruturação de um roteiro a ser implementado pelos programas de residência médica no Brasil, espera-se que os residentes desempenhem com maior eficácia o papel que muitos já exercem na prática e que consequentemente o nível de aprendizagem dos alunos de graduação e internos melhore. ABSTRACT Introduction In medical residency, residents play the dual role of teachers and learners. Training programs aiming at teaching residents how to become teachers are being implemented around the world and are referred to as ‘Resident as teacher’ (RaT) programs. According to the National Curricular Guidelines of the Medical Undergraduate Course, one can learn to establish educational goals and competence matrix, as well as skills such as communication, empathy, mediation of conflicts, among others. The implementation of these programs should be based on the understanding of the context of each medical residency where they are to be included and the residents should also know the potential of each scenario of their specialty to promote an effective teaching. Objective The objective of the study was the development of the structure of a Teaching Training Program (Resident as Teacher) divided into modules and detailed with their respective bibliographical references. Method a literature review was performed on medical education and some methods were tested in a practice scenario during a master’s degree dissertation, using the SNAPPS technique, and during a doctoral thesis presentation using the OMP technique. Outcome A Teaching Training Program was built in medical residencies based on the positive results found in the medical education literature. It is proposed that each module has a duration of approximately one month, with activities lasting one shift per week. The format of the discussion may vary with each specialty and with the number of residents in each program. It is suggested that a flipped classroom situation be carried out, sending the bibliographic reference of support on the subjects to the residents by email one week before the completion of each module. Several activities can be implemented in each module to consolidate the studied material. Conclusion After structuring a script to be implemented by the residency programs in Brazil, residents are expected to more effectively play the role that many already exercise in practice and that the level of learning of undergraduate and internship students consequently improves.

Introdução: Os inibidores da bomba de prótons (IBPs) foram clinicamente introduzidos em 1989 e mostraram-se eficazes para o tratamento de ampla gama de patologias atribuíveis ao ácido gástrico. Com o tempo tornaram-se vítimas do seu próprio sucesso, sendo prescritas em excesso, em altas doses e períodos maiores que o indicado. Por outro lado, a tecnologia da informação (TI) passou a ser utilizada para auxiliar o processo de aprendizagem, desempenhando um papel cada vez mais importante na educação médica. Objetivo: Sendo a TI um método de uso tão versátil dentro do campo da medicina e educação em saúde, seria esse recurso capaz de reduzir, através de informação e educação de médicos e pacientes, o uso indiscriminado dos IBPs? Método: Realizamos uma revisão narrativa, usando MEDLINE, Bases de dados PubMed, SciELO, LILACS, SCOPUS e Google acadêmico para extrair artigos. Pesquisamos a literatura dos últimos 10 anos, sendo termos DeCS/MeSH usados ​​na busca “proton pump inhibitor”, “medical education”, “ gastroenterology” e “internato e residência”, além de termos como “prescription”, “ulcer”, “free open acces medical education”, “FOAM”, FOAMed”, “information technology”, “medical apps”, “aplicativos”, “inibidores da bomba de prótons”. Resultado: Após pesquisa realizada, foram encontrados artigos com diferentes ferramentas educacionais, mas apenas dois (PINCER e FORTA-EPI) utilizaram TI para prescrição mais racional de medicações. Conclusão: Observou-se escassez de publicações relacionando tecnologia da informação e gastroenterologia, bem como uso de inibidores da bomba de prótons, indicando a necessidade de estudos que de fato comprovam que haveria redução de prescrições sem indicação dos IBPS e validação de aplicativos para esse fim. Diante do apresentado, pôde-se concluir que intervenções de feedback e orientações educacionais têm uma possibilidade de orientar de forma positiva a conduta médica na prática clínica. Dessa forma, propomos o desenvolvimento de aplicativo para facilitar essa difícil área de intervenção.

The need to change from the traditional teaching model to the virtual one due to the social isolation leads to discussions about maximizing the use of Information and Communication Technologies (ICT) in education. This study aimed to evaluate the profile of the use of technologies in education and the acceptance and resistance to the use of ICT by faculty members in higher education in health during the COVID-19 pandemic. This study is a quantitative observational cross-sectional study, carried out from March to December 2020, with the universe of investigation being all the teachers linked to the undergraduate level courses in the health area of a higher education institution. The results show that of all the professionals surveyed (n = 248), 46.4% (n = 115) had already had access to knowledge or some training in ICT. When comparing data from before and during the pandemic, we observed a significant increase in the indicators of teachers' experience in remote classes (from 5.2% to 72.2%), knowledge about virtual learning resources (from 8,1% to 60.9%), consideration of the importance of knowledge about ICT (from 46.4% to 98.4%) and training in the area (from 19.4% to 55.7%), besides the reduction in the level of difficulty of teachers regarding the change from the traditional model to the technological one (from 26.6% to 3.6%). We conclude that most teachers improved their acceptance of ICT use during the pandemic. The transition from the traditional to the virtual model positively impacted this process.

Introduction: Medical residency represents a challenging moment, with a high level of demand and the need to develop several personal and professional skills. Therefore, in April 2019, the unprecedented mentoring program for thirty internal medicine residents was implemented, seeking to offer personal support and encourage professional development. Experience Report: A mentoring program was structured in April 2019, lasting 11 months, consisting of 30 internal medicine residents and 6 mentors with experience in clinical preceptorship, covering topics that are not usually discussed in the training of these specialists, such as professionalism, empathy, burnout and quality of life, time management, conflict management and role-model. The meetings took place monthly, in a location defined by the group. After the end of the activity, an opinion poll was conducted using a standardized questionnaire with answers arranged on a Likert scale to assess the relevance of the discussed topics and the impact on the medical residents’ practice. A 90% response rate was obtained, highlighting the relevance of the topic Burnout and quality of life and empathy. A total of 96.3% of residents stated that the program changed their mind about their professional behavior. They declared that mentoring “opened their mind about the diversity of opinions, allowing personal and professional growth” and that “to manage time, conflicts, burnout are present in our daily lives and are also part of the medicine we practice”. However, there were complaints about some difficulties among the group members to define the time and day when the meetings should be held. In summary, the activity had a positive evaluation from the mentees. Discussion: There is a positive impact of the mentoring program on the training of a resident doctor, improving self-confidence and having a positive impact on the professional career despite the faced difficulties. Mentoring also serves as a psychosocial support, creating free access between the resident and the preceptor, culminating, based on our evaluation, in an excellent relationship between mentors and mentees. Conclusion: In Ceará, the mentoring program for internal medicine residents was unprecedented. There is a positive effect, in which important topics were discussed in more depth, with greater personal and professional growth. We suggest that mentoring be maintained and included as an essential activity in the curriculum of Medical Residency Programs.

Introduction: The pandemic caused by the SARS-CoV-2 virus has accelerated an educational revolution, with implications for health care and medical education, generating some insecurities and uncertainties. The article reports the experience of Centro Universitário Christus (Unichristus) about the changes that occurred in the practical scenarios of clinical experiences during the first semester of 2020, marked by social distancing. Experience Report: The services at Clínica Escola de Saúde (CES) were suspended and the associated hospitals stopped receiving students, making the experience of real-life scenarios unfeasible, resulting in the beginning of a project characterized by care of patients with coronavirus infection through telemedicine. The face-to-face nursing team’s participation occurred concomitantly, while students attending the eighth semester of the medical course were present at the consultation through screen sharing using the Google Meet ® program. After the consultation was concluded, there was a discussion about the case and other relevant aspects, similarly to what would happen with a face-to-face experience. In parallel with the project, students also attended lectures that addressed aspects of the disease from primary to tertiary level of health care. Discussion: Patients received care and students were taught through a flexible, innovative, accessible and safe media, following a worldwide trend, generating opportunities for professional development and innovations in medical education. The experience with telemedicine can be complemented by e-learning, allowing the development of a new hybrid teaching model. Conclusion: The current circumstances may result in some educational loss, such as the impossibility to perform physical examinations and to interact better with the health care team and patients; however the technological resources can result in opportunities for changes, improvement and development of teaching methodologies, in line with the current generation of digital natives.

Introdução: A Clínica Médica comprova sua importância na medicina no momento em que engloba conteúdo das diversas especialidades médicas, sendo pré-requisito para grande parte destas. Objetivo: Este trabalho visa analisar e comparar a percepção dos estudantes de medicina sobre a clínica médica através da aplicação de 2 questionários. Método: Trata-se de um estudo quantitativo, observacional, descritivo e transversal, com a aplicação de dois questionários, sendo um destes o questionário The Dundee Ready Education Environment Measure (DREEM) e outro elaborado pelos autores através de escala Likert, comparando a percepção dos estudantes acerca das diversas subespecialidades da clínica médica no pré-internato e no pós-internato, quanto ao grau de importância e à dificuldade para aprendizagem. Ambos foram aplicados em alunos do oitavo semestre e do décimo segundo semestre do Centro Universitário Christus (Unichristus). Resultados: Foram aplicados questionários para 147 estudantes, 79 (53,7%) do 8° semestre e 68 (46,2%) do 12° semestre. Houve predomínio do sexo feminino (61,2%), resultando em 90 alunas. Ademais, foi observado que grande parte dos estudantes anos de idade. Ao observar o questionário DREEM, obtido pela pontuação média de todos os participantes, houve identificação de pontos fortes, como a percepção positiva dos alunos sobre os preceptores, e de pontos fracos, tal qual a percepção sobre a aprendizagem. A média total da pontuação foi de 132.76, significando “More Positive than Negative”. Conclusão: Com os resultados obtidos, foi viável analisar o ambiente educativo no Centro Universitário Christus, além de comparar a percepção dos estudantes pré-internato e pós-internato sobre algumas especialidades médicas, constatando que o ambiente educacional em clínica médica da instituição é considerado bom pelo corpo discente.

Introduction: Mini-CEX is an evaluation method that covers the domains: anamnesis, physical examination, counseling, clinical judgment, organization, and professionalism. It has been tested and validated for use in any practice scenario. With its characteristic of providing feedback after a clinical assessment, the Mini-CEX can also be used as a training method to guide the professional development of students and teachers, promoting greater knowledge retention in undergraduate students and continuously providing information for students to realize how far they are from the desired objectives. The aim of this study was to assess the perception of interns, residents, and preceptors of Internal Medicine (IM) regarding the Mini-CEX instrument. Methods: Qualitative study, using the focus group technique, carried out from February to July 2017. Twenty interns, thirteen residents, and five IM preceptors participated. It consisted of six focus groups, two with interns, two with residents, and two with preceptors, using semi-structured questions that identified perceptions, through the methodology used, on the quality of the evaluation and possible repercussions for the teaching-learning process. Results: In the focus group of interns, the feedback moment of the assessment was considered essential for the learning process, although the bedside assessment was tense due to the preceptor’s presence. The residents reported that the evaluation was a valid one, as it led them to review some points in the medical literature, in addition to stimulating clinical reasoning in the face of a real situation. The preceptors validated the importance of the feedback for those who were evaluated and identified the bedside assessment as a moment for the best analysis of the individualities. Conclusion: Through the perceptions of the groups in focus, the mini-CEX was identified as a fundamental instrument for the teaching and learning process of all those involved and the need to structure the moment of feedback aiming to attain a more effective result. During the bedside assessment, the stimulus to clinical reasoning was identified as a positive point and the strangeness, anxiety, and tension as negative points. Resumo: Introdução: O Mini-CEX é um método de avaliação que abrange os seguintes domínios: anamnese, exame físico, aconselhamento, julgamento clínico, organização e profissionalismo. Foi testado e validado para utilização em qualquer cenário de prática. Com sua característica de fornecer feedback após uma avaliação clínica, o Mini-CEX também serve como um método de formação para guiar o desenvolvimento profissional de formandos e formadores, promovendo maior retenção de conhecimento no corpo discente e fornecendo, continuamente, informações para que o estudante perceba o quão distante está dos objetivos almejados. O objetivo deste estudo foi verificar a percepção de internos, residentes e preceptores da clínica médica (CM) sobre instrumento Mini-CEX. Método: Trata-se de um estudo qualitativo realizado pela técnica de grupo focal no período de fevereiro a julho de 2017. Participaram 20 internos, 13 residentes e cinco preceptores de CM. Constituíram-se seis grupos focais, dois com os internos, dois com residentes e dois com docentes, com perguntas semiestruturadas que identificaram as percepções, por meio da metodologia empregada, na qualidade da avaliação e eventuais repercussões para o processo de ensino-aprendizagem. Resultados: No grupo focal dos internos, o momento de feedback da avaliação foi considerado essencial para o processo de aprendizado embora a avaliação a beira-leito tenha se mostrado tensa pela presença do tutor. Os residentes relataram que a avaliação foi válida, pois os levou a revisar alguns pontos da literatura médica, além de estimular o raciocínio clinico diante de uma situação real. Os preceptores validaram a importância do feedback para os avaliados e identificaram a avaliação a beira-leito como momento de melhor análise das individualidades. Conclusão: Por meio das percepções dos grupos em foco, identificamos o Mini-CEX como ferramenta fundamental para o processo de ensino-aprendizagem de todos os envolvidos e a necessidade de estruturação do momento de feedback para um resultado mais eficaz. Durante a avaliação a beira-leito, foi identificado o estímulo ao raciocínio clínico como ponto positivo e o estranhamento, a ansiedade e a tensão como pontos negativos.

Introdução: O início do Lúpus Eritematoso Sistêmico (LES), após 50 anos de idade, ocorre em 3 a 18% dos pacientes e é denominado Lúpus Tardio ou Lúpus Senil (LESS). Objetivo: descrever o caso clínico de um paciente diagnosticado com Lúpus Eritematoso Sistêmico Senil que esteve internado no Hospital Geral Doutor Waldemar de Alcântara, além de fazer uma revisão bibliográfica sobre o tema a fim de compartilhar com a comunidade médica a experiência e o conhecimento obtidos. Métodos: Estudo descritivo do tipo relato de caso, utilizando-se informações obtidas através de interação com o paciente durante a internação, bem como dados obtidos no prontuário dos resultados de exames laboratoriais e de imagens. Discussão: A gravidade do lúpus diminui com a idade. Alguns estudos evidenciaram que pessoas acometidos por LES de início tardio apresentam menor envolvimento de órgãos específicos em comparação aos pacientes de início precoce. Da mesma forma, eles apresentam mais frequentemente serosite, síndrome de Sjögren e manifestações neurológicas. Conclusão: Apesar de rara, a hipótese diagnóstica de LESS deve ser lembrada como diagnóstico diferencial. Para isso, é necessária a solicitação precoce do FAN (fator antinuclear), que é critério mandatório do lúpus. Dado o diagnóstico precoce do LESS, o paciente pode beneficiar-se com um menor tempo de internação, além de diminuir o uso de medicamentos desnecessários.

O uso das Tecnologias de Informação e Comunicação repercute no ensino médico, favorecido pelo perfil atual dos alunos, em sua grande maioria, nativos digitais. Considerando a reumatologia como uma especialidade complexa e de extrema importância na formação médica, o objetivo do presente estudo consiste em desenvolver e avaliar uma plataforma móvel, inédita em reumatologia, denominada ReumaChristus, visando facilitar o conhecimento e difundir a especialidade no meio acadêmico. Trata-se de um estudo transversal com delineamento descritivo e abordagem quantitativa do tipo analítica, havendo a participação de 71 acadêmicos de medicina, sendo a usabilidade avaliada por meio da Escala de Usabilidade de Sistema (SUS). Introdução: As Neoplasias de apêndice são raras e identificadas em cerca de 1% de todas as apendicectomias, sendo as etiologias mais comuns em primeiro de origem epitelial e em segundo, os neuroendócrinos. Os tumores neuroendócrinos são derivados das células enterocromafins, produzindo os grânulos Cromogranina A e Sinaptofisina e são o segundo tipo histológico mais comum de neoplasia em apêndice, muitas vezes, encontrados incidentalmente após apendicectomia devido sua apresentação indolente, mas, por vezes, são os causadores da apendicite aguda. Objetivo: Relatar a experiência sobre o caso de um paciente diagnosticado com tumor neuroendócrino de apêndice após apendicite aguda perfurada que havia apresentado, anteriormente, tromboses de vasos abdominais. Relato de caso: Paciente do sexo masculino, 28 anos, há 2 meses com dor abdominal em mesogástrio e epigástrio, sendo diagnosticado inicialmente com trombose intra-abdominal. Neste internamento, apresentou dor abdominal difusa mais proeminente em fossa ilíaca direita com descompressão dolorosa, submetido à videolaparoscopia exploradora e evidenciado apendicite grau 4. À análise histopatológica evidenciado tumor neuroendócrino em apêndice bem diferenciado G1 com tamanho de 2mm em maior eixo, índice mitótico com menos de 2 mitoses, estadiamento patológico pT1 pNx, margens livres, invasão linfovascular e perineural. Imuno-histoquímico com índice de proliferação celular (Ki-67) de 1% e identificação dos componentes Cromogranina A clone policlonal e Sinaptofisina clone DARKSYNAP positivo. Conclusão: Relatamos um caso de um paciente com Tumor Neuroendócrino de apêndice com apresentação inicial como apendicite aguda e, à patologia, limitada ao apêndice sem invasão linfovascular e teve a apendicectomia total como procedimento curativo.

Relata-se o caso de um homem de 51 anos, que evoluiu insidiosamente com tetraparesia após ferimento cervical por arma de fogo há 19 anos, apresentando-se com poliartralgia, cefaleia, dor abdominal, constipação, polineuropatia e anemia. Diante da hipótese diagnóstica de saturnismo pelos elevados níveis séricos e urinários de chumbo, optou-se pela retirada cirúrgica do projétil em região cervical, obtendo-se melhora clínica. Após 5 meses foi readmitido com recidiva do quadro abdominal e anêmico. A tomografia de abdome evidenciou possível fragmento de projétil em músculo reto abdominal direito. Na internação, apresentou febre, poliartralgia com monoartrite em punho, e sinais flogísticos em abaulamento em cotovelo esquerdo, o qual foi puncionado. Devido relato na literatura de que a intoxicação por chumbo aumenta a reabsorção de ácido úrico no túbulo contorcido proximal e o antecedente de Gota relatado, tratamos o paciente com anti-inflamatórios e prednisona, com melhora clínica. O estudo do líquido revelou cristais de pirofosfato de cálcio, contrapondo o diagnóstico de artrite gotosa. O paciente foi submetido a cirurgia para retirada do projétil em músculo reto abdominal, sendo optado por não iniciar quelante devido ao nível sérico de chumbo inferior a 50mcg/dl e a melhora do paciente, que recebeu alta poucos dias após. Relatamos esse caso devido a raridade dessa associação, e assim, esperamos suscitar possível associação destas patologias, com mecanismo fisiopatológico desconhecido, ainda não descrito em livros de clínica médica, com apenas um caso publicado na literatura mundial.

O grande desafio das escolas médicas durante a pandemia da COVID-19 foi fazer a transição, em curto período, do ensino presencial da Aprendizagem Baseada em Problemas (ABP) para o ensino remoto de emergência (ERE). Este trabalho teve como objetivo relatar os resultados da implementação da ABP no ERE no curso de Medicina de um Centro Universitário em Fortaleza. Trata-se de um estudo de corte transversal, descritivo e quantitativo. Um formulário eletrônico no formato Google Docs, enviado a todos os docentes após conclusão das atividades de tutoria do primeiro semestre letivo de 2020, avaliou sua percepção quanto à eficácia da ABP no ERE, às dificuldades de adaptação à modalidade de ERE, à sobrecarga cognitiva das sessões tutoriais e aos fatores interferentes na interação tutor-aluno online. Os tutores assinaram um termo de consentimento e o anonimato foi garantido. Responderam ao formulário 89 tutores, 60,7% do sexo feminino, idade média de 40,9 anos e mediana do tempo de experiência como tutor de dois anos. Quanto à eficácia do ABP no ERE, 49,4% dos tutores julgaram ser menor em relação ao ensino presencial; 70,8% não tinha nenhuma/mínima experiência com tecnologias educacionais antes da pandemia; 46,1% tiveram dificuldades de adaptação na modalidade de ERE; 69,6% evidenciaram a sobrecarga cognitiva dos discentes e 74,1% concordaram com a necessidade de diminuição da carga horária da tutoria. Os principais fatores que interferiram na interação tutor-aluno foram interrupção da rede de internet, câmera desligada e comunicação não verbal deficiente. Portanto, a utilização de ABP no ERE deve ser realizada de forma modificada, com diminuição das horas de atividades síncronas, com melhor capacitação dos tutores e alunos quanto às TICs, melhor conhecimento das avaliações interativas online e das medidas profiláticas da fadiga zoom.

A chikungunya é uma doença tropical infecciosa causada pelo vírus Chikungunya (CHIKV), um vírus de RNA da família Togaviridae, gênero Alphavirus. É transmitida por mosquitos do gênero Aedes, infectados com o CHIKV. A doença evolui em três fases: aguda, subaguda e crônica, esta última tendo início após 3 meses de sintomatologia. A mialgia é um dos sintomas mais prevalentes na doença podendo ser intensa. Na literatura, alguns casos de Miopatia secundária à Chikungunya em fase aguda foram descritos, porém poucos foram relatados na fase crônica da doença. Apresentamos o caso de uma paciente feminina, que em 2016 desenvolveu fraqueza muscular progressiva às custas de dermatomiosite após dois anos da infecção pelo CHIKV.

The purposes of this study were to determine the prevalence of metabolic syndrome (MetS) in patients with ankylosing spondylitis (AS) receiving anti-TNFα therapy and evaluate the association of the two conditions with clinical and laboratory findings and predictors of cardiovascular risk. In this cross-sectional study, 63 patients diagnosed with AS according to the modified New York criteria and treated with TNFα blockers and 33 healthy controls were submitted to clinical examination and anthropometric measurements. Glucose levels, lipid profile, and inflammatory markers were registered. The Framingham score (FS), atherogenic index of plasma (AIP), and waist-to-height ratio (WHtR) were calculated. MetS was diagnosed according to the revised National Cholesterol Education Program - Adult Treatment Panel III guidelines. The prevalence of MetS was higher among AS patients than controls (27 vs. 9.1%, p = 0.04). Os linfomas cutâneos de células T (LCCT) compreendem um grupo heterogêneo de Linfomas Não Hodgkin que se apresentam comumente como micose fungóide, de curso indolente, e uma forma leucêmica, com evolução mais agressiva, conhecida como síndrome Sézary. Essas neoplasias tornam o indivíduo mais susceptível a infecções decorrentes da imunossupressão. Em março de 2020, deflagrou-se a pandemia pelo novo coronavírus (COVID-19), que rapidamente se disseminou e causou numerosas mortes. O presente estudo descreve um caso de difícil diagnóstico representado lesões eritêmato-descamativas em tronco e membros, associada à prurido intenso em curso por oito meses, conduzida até então como farmacodermia, que evoluiu durante a internação, após infecção viral por COVID-19 intra-hospitalar, com infiltrado na face, rarefação de sobrancelhas e cabelos, desfigurando a paciente, semelhante à face leonina. Paciente cursou com síndrome gripal pelo novo coronavírus, permanecendo quatorze dias em isolamento respiratório, evoluindo sem complicações graves e conduzida, em seguida, à serviço oncológico especializado.

OBJECTIVES To evaluate volumetric BMD (vBMD), microarchitecture and strength and vertebral fractures (VFs) in primary SS (pSS). METHODS We evaluated 71 female pSS patients and 71 gender-, age-, and race-matched controls. Clinical data including risk factors for osteoporosis (OP) and fractures were collected through a standardized protocol. Areal BMD and VFs were analysed by DXA. Bone microarchitecture, vBMD and bone strength were assessed by high-resolution peripheral quantitative CT (HR-pQCT), a non-invasive method. RESULTS pSS patients and controls were comparable for age, BMI, calcium intake, smoking, menopause, sedentary lifestyle and family history of fractures (P > 0.05). OP or low BMD for the patient's age (33.8 vs 5.6%; P < 0.0001) and VFs (19.7 vs 5.6%; P = 0.043) were more frequent in patients than controls. HR-pQCT showed deterioration of cortical and trabecular components and strength at the radius, and of cortical components and strength at the tibia (P < 0.05) in patients compared with controls. pSS patients and controls were also analysed by multivariate analysis adjusted for age, ethnicity, prednisone use, weight and height, which showed that the pSS group had lower values of cortical vBMD, cortical thickness and apparent modulus (P < 0.05) at the radius and cortical vBMD and apparent modulus (P < 0.05) at the tibia. Patients with VFs had more cortical bone deterioration (cortical vBMD/cortical thickness) at the tibia compared with patients without VFs (P < 0.05). CONCLUSIONS This study was the first to assess bone microarchitecture in pSS and demonstrated that cortical deterioration is the most important abnormality observed in pSS patients with VFs. This novel finding shows that this compartment contributes to vertebral fragility, suggesting that this non-invasive evaluation may be useful in the clinical practice.

BACKGROUND: There is extensive evidence, mainly from the United States and Canada, that points towards the need to train medical residents in teaching skills. Much of the “informal curriculum”, including professional values, is taught by residents when consultants are not around. Furthermore, data from the 1960s show the importance of acquiring these skills, not only for residents but also for all doctors. ­Teaching moments can be identified in simple daily situations, like discussing a clinical situation with patients and their families, planning patients’ care with the healthcare team or teaching peers and medical students. The aim here was to examine the significance of resident teaching courses and estimate the effectiveness of these courses and the state of the art in Brazil. METHODS: We conducted a review of the literature, using the MEDLINE, PubMed, SciELO and LILACS databases to extract relevant articles describing residents-as-teachers (RaT) programs and the importance of teaching skills for medical residents. This review formed part of the development of a doctoral project on medical education. RESULTS: Original articles, reviews and systematic reviews were used to produce this paper as part of a doctoral project. CONCLUSIONS: RaT programs are important in clinical practice and as role models for junior learners. ­Moreover, these educational programs improve residents’ self-assessed teaching behaviors and teaching confidence. On the other hand, RaT program curricula are limited by both the number of studies and their methodologies. In Brazil, there is no such experience, according to the data gathered here, except for one master’s thesis.

ObjectiveThe objectives of this paper are to objectively measure habitual physical activity levels in patients with primary Sjögren’s syndrome (pSS) with mild disease activity and to determine to which extent it may be associated with physical capacity and function and clinical features.MethodsIn this cross-sectional study, 29 women with pSS were objectively assessed for habitual physical activity levels (using accelerometry) and compared with 20 healthy women (CTRL) frequency-matched for physical activity levels, age, body mass index, and body fat percentage with regard to physical capacity and function, fatigue, depression, pain, and health-related quality of life.ResultspSS showed 8.5 min/day of moderate-to-vigorous physical activity (MVPA) when only MVPA accumulated in bouts ≥ 10 min was considered; when considering total MVPA (including bouts 2peak, lower muscle strength and function, higher fatigue, and poorer health-related quality of life when compared with CTRL ( p ConclusionWhen compared to physical activity-matched healthy controls, pSS patients showed reduced physical capacity and function, increased fatigue and pain, and reduced health-related quality of life. Except for aerobic conditioning, these differences were sustained when only more physically active participants were compared, indicating that minimum recommended levels of physical activity for the general population may not be sufficient to counteract pSS comorbidities.

A Síndrome Metabólica (SM) tem sido descrita nas doenças autoimunes. No entanto, existem poucos dados na literatura sobre a síndrome metabólica e o perfil de adipocitocinas na síndrome de Sjögren primária (SSp). Setenta e um pacientes do sexo feminino com SSp com idades entre 18-65 anos (Critérios do Consenso Euramericano, 2002) e 71 mulheres saudáveis pareadas por idade e raça foram incluídas neste estudo caso-controle. Os dados clínicos foram coletados por meio de um protocolo padronizado. Os níveis sanguíneos de glicose, colesterol total, LDL-colesterol (LDL-C), HDL-colesterol (HDL-C), triglicérides, interleucina-1 beta (IL-1 beta)/ IL-6, fator de ativação das células B (BAFF), insulina e leptina/ adiponectina/ visfatina/ resistina foram determinados. Os pacientes e os controles foram comparáveis com relação ao índice de massa corporal (IMC), tabagismo, sedentarismo e menopausa (p > 0,05). A síndrome metabólica (39,4 vs. 16,9%, p= 0,005), hipertensão (p= 0,004) e a dislipidemia (p= 0,002) foram mais frequentes nos pacientes do que nos controles. Os níveis de IL-1 beta, IL-6, BAFF, resistina e adiponectina foram mais elevados nos pacientes do que nos controles (p < 0,05). Os pacientes com SSp com SM (n= 28) apresentaram maiores valores de IMC, circunferência abdominal, colesterol total, LDL-C, triglicérides, insulina, leptina e HOMA-IR, além de maiores taxas de hipertensão e diabetes do que os pacientes com SSp sem SM (n= 43) (p < 0,05). O uso atual e/ou prévio de prednisona (75,0 vs. 62,8%, p = 0,313), a dose atual (3,0 ± 4,5 vs. 1,6 ± 3,2 mg/dia, p= 0,299) e a dose cumulativa de prednisona (p= 0,495) foram semelhantes em ambos os grupos. Entretanto, os níveis de IL-1 beta foram maiores em pacientes com SM do que nos pacientes sem SM (p= 0,012). Este achado foi confirmado por análise multivariada (p= 0,048) com ajustes para idade, etnia, uso de prednisona, doses cumulativas e atuais de prednisona e ainda duração do uso da mesma. Nós identificamos elevada frequência de síndrome metabólica e um perfil anormal de adipocitocinas em pacientes com SSp. Candida albicans and other species are usually involved in opportunistic infections in patients with acquired immunological deficiency syndrome (AIDS). The virulence mechanisms by which this yeast expresses its pathogenicity include adherence patterns, ability to form pseudomycelia and production of extracellular enzymes, among others. The objective of this research was to verify the frequency of Candida and the production of proteinase and phospholipase in 52 strains of Candida albicans from the oral cavity of patients infected by HIV treated at Hospital São José, AIDS reference and training center in Fortaleza, Ceará. Samples were collected of patients, with or without oral lesions characteristic of candidosis. From 100 patients, 80% presented positivity for Candida: 65% (52) were identified as C. albicans, 27.5% (22) as C. tropicalis, 2.5% (2) as C. glabrata, 2.5% (2) as C. krusei and 2.5% (2) as C. guilliermondii. Among the strains of C. albicans isolated from the oral cavity, proteinase and phospholipase were detected in 69.2% and 73%, respectively. The results suggested that C. albicans was the most frequent species observed, with intermediate expression of proteinase and phospholipase. Candida albicans e outras espécies são usualmente envolvidas em infecções de pacientes com a síndrome da imunodeficiência adquirida (AIDS). Os mecanismos de virulência pelos quais a levedura expressa sua patogenicidade incluem padrões de aderência, habilidade por formar pseudomicélio, produção de enzimas extracelulares e outros. O objetivo deste trabalho foi verificar a freqüência de Candida e a produção de proteinase e fosfolipase em 52 cepas de Candida albicans da cavidade oral de pacientes infectados pelo HIV atendidos no Hospital São José, hospital de referência e centro de treinamento em AIDS em Fortaleza, Ceará. Neste trabalho foram coletadas amostras de pacientes com ou sem lesões características de candidose. Dos cem pacientes 80% apresentaram positividade para Candida, sendo 65% (52) identificados como C. albicans, 27,5% (22) como C. tropicalis, 2,5% (2) como C. glabrata, 2,5% (2) como C. krusei e 2,5% (2) como C. guilliermondii. Entre as cepas de C. albicans isoladas da cavidade oral foram detectadas proteinase e fosfolipase em 69,2% e 73%, respectivamente. Os resultados sugerem que a C. albicans foi a espécie mais freqüentemente observada, com intermediária expressão de proteinase e fosfolipase.

Introduction Impostor Phenomenon (IP) is the inability to internalize success and a tendency to attribute success to external causes. Social and institutional support are strategies to avoid the problem, but only partially successful. Resilience has been considered protective against mental health disorders in medical students. This study aims to investigate the association between IP and resilience in undergraduate medical students. Methods Using Clance Impostor Phenomenon Scale (CIPS), Connor-Davidson Resilience Scale (CD-RISC), and a socio-demographic questionnaire, we conducted a cross-sectional, quantitative study with undergraduate medical students. Results The study included 425 pre-clerkship medical students of average age of 23.03 years. The prevalence of IP was 47.76% (CIPS ≥ 62 score, as recommended in prior literature.) and there were no differences among age, gender, current semester, religion. The majority student's CD-RISC score was 68.57 ± 14.66 and there was no statistical difference in resilience scale among age, gender, religion and training semester. “Other religions” group was associated with a higher resilience score when compared to “Catholic” or “Non-religious” students. We found an inverse correlation between the level of resilience by CD-RISC and the level of symptoms related to IP by CIPS (ρ = −0.409; p-value Conclusions Findings from this study suggest that higher levels of resilience are likely protective against IP among undergraduate medical students. Furthermore, the non-inclusion of medical internship students and the cross-sectional design of this study are restraining aspects noticed within the project, suggesting the need for further studies to adjust those limitations.

RESUMO INTRODUÇÃO Feedback é uma informação provida pelo professor para melhorar o desempenho do estudante. Embora seja uma importante ferramenta de ensino e seja desejada pelos discentes, ainda é utilizada de maneira insuficiente. OBJETIVO Este estudo criou um manual para uso de feedback estruturado com base nos resultados de grupos focais que verificaram a percepção dos alunos do internato de Medicina sobre o tema. MÉTODOS Trinta e um alunos do internato de Medicina de uma instituição de ensino superior participaram de entrevistas de grupo focal sobre suas impressões relativas ao feedback. Os dados foram analisados pela Análise de Conteúdo de Bardin. RESULTADOS Os alunos compreendem o que é feedback e o vivenciam por meio das metodologias ativas de ensino. Reconhecem sua importância para o aprendizado, mas se queixam da baixa frequência desse instrumento. Sobre como gostariam de receber o feedback e as virtudes inerentes aos docentes e discentes para o sucesso do mesmo, notou-se o alinhamento de suas ideias com o que diz a literatura. CONCLUSÃO Os alunos conhecem feedback e o vivenciam na prática, reconhecendo sua importância para o aprendizado e sendo receptivos a ele. Desenvolveu-se um manual com estratégia de feedback estruturado voltado ao uso em ambulatórios didáticos na graduação em Medicina. ABSTRACT INTRODUCTION Feedback is information provided by the teacher to improve student performance. Despite being an important teaching tool, and desired by the students, it is still not sufficiently used. OBJECTIVE This study created a manual for the use of feedback, structured based on the results of focus groups that explored the views of medical school interns on the subject. METHODS 31 medical interns of a Higher Education Institution took part in focus group interviews to gather their impressions on feedback. The data were analyzed using Bardin Content Analysis. RESULTS The students understand what feedback is, and experience it through active teaching methodologies. They recognize its importance for learning; however they complain that they do not receive feedback often enough. As to how they would like to receive feedback, and the virtues inherent to teachers and students for it success, it was found that the students’ ideas are aligned with the literature. CONCLUSION Students know what feedback is, and experience it in practice; they recognize its importance for learning and are receptive to it. A manual was developed with a structured feedback strategy for use in university undergraduated students

RESUMO A residência médica, instituída legalmente no Brasil em 1977 pelo Decreto nº 80.281, é considerada a melhor estratégia de ensino em cenário de prática, sendo o padrão ouro da especialização médica. A característica mais marcante da residência é o treinamento em serviço, que articula ensino no cenário de prática, além de construir um perfil profissional. Os residentes desenvolvem também a função docente. Pesquisas americanas estimam que exerçam tal função em boa parte de suas atividades, chegando a um quarto do período total de tempo de seus programas de residência. Esse processo de ensino-aprendizagem durante a residência ainda é pouco estudado, principalmente no Brasil. O objetivo deste estudo foi realizar uma ampla revisão narrativa sobre RaT (Resident as Teacher), tema pouco explorado na literatura brasileira, avaliando historicamente o processo de ensino-aprendizagem dos programas de residência médica. Foi realizada uma revisão na literatura acerca do processo de ensino-aprendizagem da residência médica no Brasil e no mundo. Diversos países estão implementando treinamentos formais de ensino denominados programas de Resident as Teacher (RaT). Somente nos EUA, mais de 50% dos programas de residência têm alguma forma de treinamento RaT. Vários programas foram desenvolvidos e se diferenciam no conteúdo, na duração e no formato, porém são baseados em atributos de ensino considerados essenciais ao ensino. No que diz respeito ao conteúdo, os programas RaT enfatizam predominantemente o modelo preceptor minuto (One Minute Preceptor – OMP), a estrutura de ensino clínico do Programa de Desenvolvimento da Faculdade de Stanford ou os domínios mostrados por Irby como essenciais à excelência em ensino clínico. Em conclusão, sugere-se que os programas brasileiros de residência médica invistam em estudos e, consequentemente, em estratégias efetivas para aprimorar as técnicas de ensino para médicos residentes.

Introduction: The pandemic caused by the SARS-CoV-2 virus has accelerated an educational revolution, with implications for health care and medical education, generating some insecurities and uncertainties. The article reports the experience of Centro Universitário Christus (Unichristus) about the changes that occurred in the practical scenarios of clinical experiences during the first semester of 2020, marked by social distancing. Experience Report: The services at Clínica Escola de Saúde (CES) were suspended and the associated hospitals stopped receiving students, making the experience of real-life scenarios unfeasible, resulting in the beginning of a project characterized by care of patients with coronavirus infection through telemedicine. The face-to-face nursing team’s participation occurred concomitantly, while students attending the eighth semester of the medical course were present at the consultation through screen sharing using the Google Meet ® program. After the consultation was concluded, there was a discussion about the case and other relevant aspects, similarly to what would happen with a face-to-face experience. In parallel with the project, students also attended lectures that addressed aspects of the disease from primary to tertiary level of health care. Discussion: Patients received care and students were taught through a flexible, innovative, accessible and safe media, following a worldwide trend, generating opportunities for professional development and innovations in medical education. The experience with telemedicine can be complemented by e-learning, allowing the development of a new hybrid teaching model. Conclusion: The current circumstances may result in some educational loss, such as the impossibility to perform physical examinations and to interact better with the health care team and patients; however the technological resources can result in opportunities for changes, improvement and development of teaching methodologies, in line with the current generation of digital natives.
