Abstract

This chapter presents a critical appraisal of mental health nursing’s interest in and relationship to advocacy with a view to highlighting the role of advanced practice mental health nurses (APMHNs), amongst other team members, in both performing advocacy and respecting and acting upon the voice of advocates. In this regard, a distinction will be made between nurses taking up an advocacy role and the important role and function of independent advocates, the latter requiring understanding and support from nurses and health-care teams to be maximally effective. Nurses, thus, are charged with a responsibility to be advocates and support other advocates, especially in the case of recognition of the limitations of nurse advocacy. Within the nursing advocacy role, a further distinction will be made between advocating at the level of patient care for meeting the rights and needs of individuals and advocating on a broader stage for social justice ends salient to nursing and health care.

15. Adolescent alcohol and cannabis use in Iceland 1995-2015.

Abstract

Introduction: Over the past two decades, alcohol consumption of Icelandic adolescents has decreased dramatically. The aim of this study was to quantify the extent of this reduction and compare it with the trend in cannabis use over a 20 year period and to identify possible explanations.Methods: We used data from the Icelandic participants to the European School Survey Project on Alcohol and Other Drugs study (collected via paper-and-pencil questionnaires in classrooms). The sample included all students in the 10th grade (54-89% response rate).Results: The percentage of participants who had never used alcohol during their lifetime rose from 20.8% in 1995 to 65.5% in 2015. Similarly, there was a decline in the proportion of students who had consumed alcohol 40 times or more, from 13.7% to 2.8%. During the same period, the number of students who had never used cannabis rose from 90.2% to 92.0%. In contrast, we found a small, but statistically significant, increase in the prevalence of those who had used cannabis 40 times or more, from 0.7% in 1995 to 2.3% in 2015. Parental monitoring increased markedly between 1995 and 2015, but availability of alcohol decreased. Perceived access to cannabis and youth attitudes towards substance use remained unchanged.Discussion: Although Iceland has enjoyed success in lowering alcohol use among adolescents over the past decades, and somewhat fewer claim to have ever tried cannabis, there has been a threefold increase among heavy users of cannabis. 16. The effects of a mindfulness based intervention on impulsivity, symptoms of depression, anxiety, experiences and quality of life of persons suffering from substance use disorders and traumatic brain injury.

Abstract

Background: Studies have shown that of the 1.4 million Americans who experience a traumatic brain injury (TBI) each year, many have significant cognitive disabilities as a result of their injuries and up to half suffer from substance use disorders (SUDs). This often leads to significant issues such as increased rates of mental health problems and delay or lack of return to full employment. Aims: The purpose of this pilot study was to examine the effectiveness and impact of an adapted MBSR program on different psychosocial dimensions of persons suffering from a dual diagnosis of SUDs and TBI. Method: Anxiety, depression, quality of life, impulsivity and participants’ and staff persons’ experiences were explored using a mixed methods design to gain a comprehensive picture of the impact of the adapted eight week mindfulness intervention on the participants of the study. The qualitative evaluation focused on the impact that mindfulness meditation practice had on clients’ and staff persons experiences using an adapted grounded theory approach. Descriptive and analytical statistics were used to examine outcomes of quantitative measures Results: The general health perceptions scale of the SF-36 declined over the course of the study in a statistically significant manner. Changes in other measures were mostly in the predicted direction but were not statistically significant. The qualitative interviews revealed numerous perceived benefits reported by participants gleaned from their involvement in the intervention. These included subjective reports of positive effects of the intervention on mood, impulsivity, anxiety and personal relationships. Conclusions: The adapted mindfulness based intervention applied in this study is an inexpensive and safe method that appeared to fit the needs of the participants in this study well according to qualitative interviews. However, the exploratory nature of this study and the paucity of statistically significant results, indicates that future studies with a larger sample size are needed in this field.
