Pediatric nephrology has a history rooted in pediatric advocacy and has made numerous contributions to child health policy affecting pediatric kidney diseases. Despite this progress, profound social disparities remain for marginalized and socially vulnerable children with kidney disease. Different risk factors, such as genetic predisposition, environmental factors, social risk factors, or health care access influence the emergence and progression of pediatric kidney disease, as well as access to life-saving interventions, leading to disparate outcomes. This review will summarize the breadth of literature on social determinants of health in children with kidney disease worldwide and highlight policy-based initiatives that mitigate the adverse social factors to generate greater equity in pediatric kidney disease., Author(s): Jill R. This article, titled "Kidney paired donation," discusses the current state of Kidney Paired Donation (KPD) in pediatrics. The authors highlight that while KPD rates have increased in recent years, they are still relatively low and limited to a few centers across the nation. The study also emphasizes that the outcomes of KPD are similar to non-KPD Living Donor Kidney Transplants (LDKT) in the short term. The authors argue for increased utilization of KPD in pediatric patients to expand transplant access. The article also addresses the barriers to KPD utilization, such as lack of necessary personnel and infrastructure, financial costs, and the need for large donor pools for size matching in pediatric patients. The authors suggest that KPD organizations, such as the National Kidney Registry (NKR), can help offset some of the financial barriers and disparities in living donation. However, the article acknowledges that there are challenges in expanding KPD, including the lack of a roadmap for increasing accessibility and the need for increased experience, education, and funding.
