Seis años después de firmado el Acuerdo de Paz entre las FARC-EP y el gobierno colombiano, las principales transformaciones rurales terminaron desdibujadas y las cifras de violencia demostrarían que la paz no se materializa. Desde una metodología activista, desplegada en el departamento del Cauca con comunidades rurales, este artículo analiza tres fases del modelo de ‘paz territorial’: el giro local, que consistió en la implementación participativa y densamente burocrática del Acuerdo de Paz; el giro militarista, que sustituyó el desarrollo territorial integral; y el nuevo ciclo de conflicto interno armado, en cuya explicación causal enfatizamos la dimensión étnica y racial de las violencias. Concluimos que haber desviado la construcción de paz de la hoja de ruta trazada para abordar de manera integral las causas estructurales de la guerra, en cambio optando por la seguridad militar y estabilización, genera la revictimización y desesperanza de los habitantes rurales empobrecidos, quienes vuelven a quedar implicados entre conflictos nuevos y reciclados.

This article discusses a study conducted at Dr. Mohammad Hoesin (RSMH) Palembang to determine the accuracy of vaginal probe transperineal ultrasound as a screening tool for anal sphincter injury in women after vaginal delivery. The study found that ultrasound examination using vaginal probe transperineal had a sensitivity of 100% and a specificity of 80.5% in detecting obstetric anal sphincter injuries (OASIS). The ultrasound examination was able to detect OASIS in 8 subjects who were not detected on clinical examination. The study concludes that vaginal probe transperineal ultrasound can be used as both a screening and diagnostic tool for OASIS. [Extracted from the article]

During the pandemic, the learning process in schools was considered less effective. The lack of involvement of students in the learning process makes learning activities only focus on the teacher. This happens because students need to adapt to the online learning process at home. Therefore, the government issued a policy for schools to implement a blended learning learning system as an effort to maximize learning, including Islamic Religious Education (PAI) lessons during the ongoing pandemic. The purpose of carrying out this research was to explore information related to the implementation of blended learning in improving the character of students through PAI subjects and direct it with library research. This research process was also carried out by exploring various kinds of data sourced from material in relevant journals in various media on the internet. This study describes the results that blended learning has been successfully used as an effective solution to optimize the PAI learning process and can overcome the deficiencies that exist in captivating and courageous learning to develop students' character. The conclusions of this study are 1) there is an effect of blended learning on optimal PAI learning objectives, 2) the need for various types of media designed in the implementation of blended learning as a form of effort to shape the character of students and learning can be achieved as well as possible. Hal tersebut terjadi karena peserta didik perlu beradaptasi dengan pelaksanaan proses pembelajaran secara online di rumah. Oleh karena itu, pemerintah mengeluarkan kebijakan kepada sekolah untuk menerapkan sistem pembelajaran blended learning sebagai upaya untuk memaksimalkan pembelajaran termasuk pada pelajaran Pendidikan Agama Islam (PAI) saat masa pandemi berlangsung. Tujuan dari dilaksanakannya penelitian ini ialah untuk menggali informasi yang berkaitan dengan implementasi blended learning dalam meningkatkan karakter peserta didik melalui mata pelajaran PAI dan dilangsungkan dengan studi pustaka (library research). Proses penelitian ini dilakukan juga dengan menggali berbagai macam data yang bersumber dari materi pada jurnal-jurnal yang relevan di dalam berbagai media dalam internet. Penelitian ini memaparkan hasil bahwasannya blanded learning berhasil digunakan sebagai solusi efektif untuk mengoptimalkan proses pembelajaran PAI serta dapat mengatasi kekurangan yang ada dalam pembelajaran luring dan daring untuk mengembangkan karakter peserta didik. Simpulan penelitian ini adalah 1) adanya pengaruh blended learning terhadap pencapaian pembelajaran PAI yang optimal, 2) perlunya berbagai macam media yang dirancang dalam pelaksanaan blended learning sebagai bentuk upaya untuk membentuk karakter peserta didik dan pembelajaran dapat tercapai dengan sebaik mungkin. The New Order period experienced problems in the political, economic, social and educational fields so that in 1998 reforms were made to improve the systems of various fields. Execution the new order Islamic religious education could not develop. So that with the reform of education Islamic education is expected to be able to develop in the community.Islamic education is able to change the mindset and behavior of people for the better. In the current era of globalization, religious education is still needed to be able to organize people's behavior.

In recent time, various techniques have been applied for the hernia repair surgery. Considering the possibility of the recurrence rates up to 40%, umbilical and umbilical port hernias have been repaired without mesh for a long time. The repair of these hernias by the Ventralex hernia patch may decrease recurrent hernia rates. The aim of this study was to evaluate the outcome and complications of mesh repair in umbilical hernia by the Ventralex hernia patch. A total of 36 patients were studied retrospectively who underwent mesh repair surgery by the Ventralex hernia patch between April 2017 and October 2019 at Anower Khan Modern Medical College & Hospital, Care Medical College & Hospital and LABAID Specialized Hospital. They have also been evaluated for instant post-operative complications, recurrence rate & pain. Short-term clinical outcomes were assessed during post-operative follow up at first and sixth week. One year long-term outcomes were assessed over telephone. In this study, there were 28 female and 8 male patients with the range of age from 24-68 years (Mean 41.5 ±8.36) having 27 umbilical hernias and 9 umbilical port hernias. No major short-term complications were found but early minor complications were found in 5 patients (13.89%). No pain or mild pain was reported in 31 patients after one week’s post-operative follow up. 34 patients reported being very satisfied with their repairs. In this study we found that the Ventralex hernia patch is an effective and easier technique for mesh repair in umbilical and umbilical port hernias. This technique can also save the operative time with less post-operative complications and better outcomes. BSMMU J 2021; 14(3): 79-84

Surgical site infections (SSIs) are the second most common nosocomial (hospital-acquired) infections after urinary tract infections. Despite recent advances in aseptic techniques, till now SSI has been considered as a major source of morbidity and mortality in developing countries and continued to represent about one fifth of all healthcare-associated infections. This cross-sectional study was conducted to analyze the pattern & aetiological factors of SSI among patients undergoing surgery at Sir Salimullah Medical College Mitford Hospital, Dhaka, Bangladesh involving 160 admitted patients during the study period. Of all SSIs, 73.0% were superficial and 27.0% were deep infections. Molecular factors play an important role in the management and treatment outcome of breast cancer. Molecular subtyping has been developed depending upon estrogen and progesterone receptors, human epidermal growth factor receptor-2 and ki76 level. This cross-sectional study was done to assess the molecular subtypes of locally advanced breast cancer and its associated risk factors. Total 94 patients with locally advanced breast cancer were included in the study. The mean age was 42.6 years with a standard deviation of 9.56. In total, 91.5 percent of respondents had menarche at or after the age of 12, and 26.6 % had used hormonal contra- ception in the past. Tobacco users and positive family history were found in 21.2% and 5.35% of the cases. Among the patients, 3.4 % of cases had their first childbirth after the age of 30 and 95.5% of patients feed their babies from their both breasts. Among 94 cases 5 did not have any child. Estrogen receptor was found positive in 35% of cases, progesterone receptor-positive patient was 33% and HER-2 was found positive in 39.4% of cases. Ki-67 level was found high in 66% of cases. Among the 94 cases, the Luminal A subtype was found in 18% and the Lumi- nal B subtype was found in 27.7% cases. The human epidermal growth factor receptor-2 subtype was found relatively less frequent than Luminal type B (24.5% vs. 27.7%). Triple-neg- ative breast cancer was most commonly diagnosed among the patients (almost 30%). The increased number of triple-negative variants signifies poor prognostic outcomes. Cancer patients are the vulnerable group of population and have more chances of contracting Corona virus disease 2019 (COVID-19). They are at great risk of passing through very stressful events during this COVID-19 pandemic that may lead to different psychological problems. Different psychological symptoms of breast cancer patients are evaluated in this study during the COVID- 19 pandemic. Consecutive fifty women selected with a non-metastatic breast cancer, scheduled to receive cancer treatment in the upcoming days/weeks not. Received treatment in the past, completed the Impact of Event Scale-Revised(IES-R), the Hospital Anxiety and Depression Scale (HADS-A and HADS-D), the Fear of Cancer Recurrence Inventory (FCRI). and the Insomnia Severity Index (ISI) and questionnaires. The study was conducted at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from the 1st July 2020 to the 30th June 2021.. Patients were investigated about their worries regarding the impact of COVID-19 on their lives as carcinoma breast patients. Since the 1st July 2020 to the 30th June 2021, Consecutive 50 patients were prospectively evaluated. They were diagnosed with non-metastatic breast cancer. The mean age was 42.3 (range 30–70). Most of the patients (90%, n. 45) are married, 8% (n.4) patients are single and 2% (n. 1) patients are widowed. Among the patients 24 (48%) patients have high school or less education level, 19 (38%) have college experience and 7 (14%) have university degrees. Ten patients (20%) lived alone, 8 (16%) with one or two family members, and 32 (64%) with three or more family members. Concerning their working life, 36 (72%) were not working (namely housewives, retired, unemployed, or students) and 14 (28%) was occupied. In consideration of economic status 52% (n. 26) patients have annual family income in BDT 250001- 350000. On the other hand 22% (n. 11) within BDT 350001- 450000, 14% (n. 7) within BDT 150000- 250000, 6% (n. 3) within BDT 450001-550000 and 6% (n. 3) within BDT >550000 respectively. We found that 62% of patients had anxiety (HADS-A), 66% depression (HADS-D), and 68% fulfilled the diagnostic criteria for mild post-traumatic stress disorder (PTSD), 2% patients for moderate and 2% patients for severe PTSD. Results revealed that 50% (n.25) patients were suffering from subthreshold insomnia. On the other hand 46% (n.23) patients had no clinically significant Insomnia but 2% (n.1) patients had moderate and severe insomnia in each category. The Fear of Cancer Recurrence Inventory (FCRI) score was 14.7(SD±6.2) which was more than the cut-off value..Breast cancer patients pass through many stressful events that may develop significant psychological symptoms during this COVID-19 pandemic. The outcome of this study definitely contributes to understand the psychological distress of cancer patients and a to formulate a better plan for their management and psychological support. The neural computational model GrowthEstimate is introduced with focusing on new perspectives for the practical estimation of weight specific growth rate (SGR, % day-1). It is developed using recurrent neural networks of reservoir computing type, for estimating SGR based on the known data of three key biological factors relating to growth. These factors are: (1) weight (g) for specifying the age of the growth stage; (2) digestive efficiency through the pyloric caecal activity ratio of trypsin to chymotrypsin (T/C ratio) for specifying genetic differences in food utilization and growth potential, basically resulting from food consumption under variations in food quality and environmental conditions; and (3) protein growth efficiency through the condition factor (CF, 100 × g cm-3), as higher dietary protein level affecting higher skeletal growth (length) and resulting in lower CF. The computational model was trained using four datasets of different salmonids with size variations. It was evaluated with 15% of each dataset, resulting in an acceptable range of SGR outputs. Additional tests with different species indicated similarity between the estimated SGR outputs and the real SGR values, and the same ranking of wild population growth. The developed model GrowthEstimate is exceptionally useful for the precise and comparable growth estimation of living resources at individual levels, especially in natural ecosystems where the studied individuals, environmental conditions, food availability and consumption rates cannot be controlled. It is a revelation and will help to minimize uncertainty in wild stock assessment process. This will improve our knowledge in nutritional ecology, through the biochemical effects of climate change and environmental impact on the growth performance quality of aquatic living resources in the wild, as well as in aquaculture. It will be improved for generality for future use, and required co-operations of the biodata collections of different species from different climate zones. Azaspiracid-1 is an algal toxin that accumulates in edible mussels, and ingestion may result in human illness as manifested by vomiting and diarrhoea. When injected into mice, it causes neurotoxicological symptoms and death. Although it is well known that azaspiracid-1 is toxic to most cells and cell lines, little is known about its biological target(s). A rat PC12 cell line, commonly used as a model for the peripheral nervous system, was used to study the neurotoxicological effects of azaspiracid-1. Azaspiracid-1 induced differentiation-related morphological changes followed by a latter cell death. The differentiated phenotype showed peripherin-labelled neurite-like processes simultaneously as a specific isoform of peripherin was down-regulated. The precise mechanism behind this down-regulation remains uncertain. However, this study provides new insights into the neurological effects of azaspiracid-1 and into the biological significance of specific isoforms of peripherin.

The work focuses on development of digestive enzymes (amylase, total protease, trypsin, and chymotrypsin) and activity ratio of trypsin to chymotrypsin (T/C ratio) for digestive efficiency and growth, in blue swimming crab (Portunus pelagicus) during metamorphosis and molting. Specific activities of all enzyme parameters studied were associated with growth during metamorphosis, while only those of trypsin and T/C ratio were associated during molting cycle where trypsin and chymotrypsin specific activities associated with consumption rate with especially high levels during late intermolt and early premolt stages. About 50% increased weight gain was observed with at least double increased T/C ratio at the end of molting period, compared to the stages prior to molting. Growth of carapace would be more significant after finishing molting. Carapace width gain and T/C ratio were highest at the first crab stage. Studies of in vitro protein digestibility of different feed raw materials indicated that Artemia, Rotifer, and Moina are the best for larval stages. Otherwise, the use of shrimp feed and Artemia flake could be the alternatives. Incorporating of cassava meal into the feed formula for early adult stage (juvenile) could be an advantage. The proteins from animals are more beneficial for adult crab culture than the proteins from plants and bacteria. The digestible quality of dietary protein is very important during larval stages, while the protein level of diet is more important during adult stages with fully developed digestive enzymes.

BACKGROUND: Because of the development of resistance in trypanosomes to trypanocidal drugs, the livelihood of millions of livestock keepers in sub-Saharan Africa is threatened now more than ever. The existing compounds have become virtually useless and pharmaceutical companies are not keen on investing in the development of new trypanocides. We may have found a breakthrough in the treatment of resistant trypanosomal infections, through the combination of the trypanocide isometamidium chloride (ISM) with two affordable veterinary antibiotics. METHODOLOGY/PRINCIPAL FINDINGS: In a first experiment, groups of mice were inoculated with Trypanosoma congolense strains resistant to ISM and either left untreated or treated with (i) tetracycline, (ii) ISM or (iii) the combination of the antibiotic and the trypanocide. Survival analysis showed that there was a significant effect of treatment and resistance to treatment on the survival time. Leadership has always been an unresolved problem across time or space. Balancing destructive leadership practices is essential especially in the context of religion and the state. Changes in models occur to answer the need for effective and relevant leaders. Various models of leadership are attested including servant leadership, transactional leadership, supportive leadership, laissez-faire leadership, transformational leadership and other positive leadership. The importance of a positive model in bringing full awareness to leaders in carrying out the leadership mandate is emphasised in this essay. The study focuses on the definition of destructive leadership and destructive leadership models practised by Israelite leaders during the ministry of Prophet Hosea in the eighth century B.C.E based on the text of Hosea 7:1-16. A destructive leader is described as one who negatively influences his followers. The negative influence may lead to the destruction of the organisation he/she leads. The destructive leadership model found in Hosea 7:1-16 is not integrity-oriented but power-oriented and individual-oriented. BACKGROUND Assessment of labor progress via digital vaginal examination is subjective, while intrapartum transperineal ultrasonography is deemed more objective and effective. The angle of descent (AoD) is a reliable intrapartum transperineal ultrasound parameter with minimal inter-observer and intra-observer errors. This study compared intrapartum transperineal ultrasonography with digital vaginal examination in evaluating the angle of descent for vaginal delivery selection in 70 women. MATERIAL AND METHODS This cross-sectional study was conducted at Dr. Mohammad Hoesin Hospital Palembang in Indonesia, using a consecutive sampling method to measure AoD of 70 patients at admission and every 2 h during labor progression. Patients were monitored until childbirth, and outcomes of successful vaginal or cesarean delivery were reported. A diagnostic test determined the optimal AoD for predicting successful vaginal delivery and its predictive value. RESULTS The study included 70 patients with a mean age of 28.29±6.09 years and a body mass index of 30.59±3.49 kg/m². Among them, 29 (41%) had successful vaginal deliveries. Adequate contraction frequency at admission was strongly associated with successful vaginal delivery (P<0.001). The optimal AoD for predicting successful vaginal delivery was 94.5°, with an area under the curve of 0.567 (95% CI 0.425-0.710), sensitivity of 84.2%, specificity of 45.7%, and accuracy of 54.3%. CONCLUSIONS Intrapartum transperineal ultrasonography for measuring AoD can be a valuable adjunct in labor management. Further investigations are needed to validate the optimal AoD in Indonesian pregnant women.

Objectives: To evaluate subjective and objective outcomes, complication, recurrence, and reoperation rates after transvaginal mesh surgery for the management of pelvic organ prolapse., Methods: This was a retrospective analysis of transvaginal mesh surgery carried out using self-cut mesh measuring subjective outcomes using validated questionnaires, and objective outcomes using Pelvic Organ Prolapse Quantification. Patients diagnosed with stage ≥2 pelvic organ prolapse were counseled about all possible surgical options. After thorough explanation about the benefits and risks during transvaginal mesh surgery, patients who gave signed consent were scheduled for surgery and evaluated at 1 and 3 years postoperatively., Results: We included 101 patients who completed a minimum of 3-year follow up. One year and 3-year follow up showed significant improvement both on subjective and objective outcomes. Recurrences were observed in three patients (3%), with one (1%) patient undergoing reoperation. One case (1%) of intraoperative complication (bladder injury) and four cases (4%) of postoperative complications (two mesh exposure, one hematoma and one significant increase in post-voiding residual) were recorded. Overall patients' satisfaction was positive., Conclusions: Transvaginal mesh surgery using self-cut mesh is associated with significant improvement in both subjective and objective outcomes, offering low recurrence and complication rates, and high patient satisfaction rates. It can be a safe, effective and cost-efficient option not only for recurrence cases, but also as primary management of pelvic organ prolapse using a standardized technique and proper patient selection., (© 2019 The Japanese Urological Association.)
