Abstract

In this paper we propose to extend the definition of fuzzy transform in order to consider an interpolation of models that are richer than the standard fuzzy transform. We focus on polynomial models, linear in particular, although the approach can be easily applied to other classes of models. As an example of application, we consider the smoothing of time series in finance. A comparison with moving averages is performed using NIFTY 50 stock market index. Experimental results show that a regression driven fuzzy transform (RDFT) provides a smoothing approximation of time series, similar to moving average, but with a smaller delay. Abstract

Realization of uncertainty of prices is captured by volatility, that is the tendency of prices to vary along a period of time. This is generally measured as standard deviation of daily returns. In this paper we propose and investigate the application of fuzzy transform and its inverse as an alternative measure of volatility. The measure obtained is compatible with the definition of risk measure given by Luce. Abstract

One of the major advantages in using Deep Learning for Finance is to embed a large collection of information into investment decisions. A way to do that is by means of compression, that lead us to consider a smaller feature space. Several studies are proving that non-linear feature reduction performed by Deep Learning tools is effective in price trend prediction. The focus has been put mainly on Restricted Boltzmann Machines (RBM) and on output obtained by them. Few attention has been payed to Auto-Encoders (AE) as an alternative means to perform a feature reduction. Abstract

Developing a cultural formulation during a diagnostic assessment with a young child is the foundation for work with diverse families. It requires critical reflection on the part of the clinician about the socio-cultural, historical, and political influences and consequently aspects of privilege and oppression in their own lives and professional practice, as well as on the impact of structural inequities in the lives of the families with whom they work. This article explores the origins and importance of the DC:0-5 Cultural Formulation for Use with Infants and Toddlers Table and its potential to be used as a tool to support the clinician's ongoing commitment and accountability to engage in critical self-exploration and diversity-informed practice. The authors consider implications for practice and policy, and provide reflective questions to deepen one's use of the Cultural Formulation in "DC:0-5[TM]: Diagnostic Classification of Mental Health and Developmental Disorders in Infancy and Early Childhood."

31. A Rare Case of Ovarian Abscess Secondary to Salmonella typhiInfection in Women with MRKH: A Case Report and Review of Literature

Abstract

Most common clinical presentation of a woman with MRKH is primary amenorrhea. It is rare to have pelvic masses in women with MRKH. Only few cases have been reported in literature. Although many case reports of ovarian abscesses have been published, none has been published of an ovarian abscess secondary to Salmonella typhiinfection in a woman with MRKH. We report a case of a 32 yr old woman with fever of unknown origin unresponsive to medical management. During her fever work up, her blood tests suggested typhoid infection while her MRI suggested right sided ovarian mass with high suspicion of mucinous cystadenoma. On staging laparotomy, huge right sided ovarian abscess was found. Right salpingo-oopherectomy was performed. Pus culture suggested Salmonella typhiinfection and histopathology suggested infected ovarian endometrioma.

Abstract

To increase deceased donation in an organ‑depleted country like India, improved understanding of the mechanisms influencing donor yield is important. Certain technical and emotional modifications in approach and methods can help in increasing donations. “Kidney first” retrieval in situations such as delay in heart/liver team’s arrival, diagnostic dilemma for quality of organ, or single surgeon for both kidney transplants can prove useful in optimizing donor maintenance quality and cost and minimizing cold ischemia time of the other kidney, without altering the structural viability of other organs. Early handover of the deceased body for cremation adds a positive perspective to the grieving family’s decision for consent. This approach can be safely suggested for emerging transplant centers yearning to increase their scope of organ transplantation amidst the constraints of time, expertise, logistics, and infrastructure. 39. Laparoscopic Management of Partial Molar Caesarean Scar Ectopic Pregnancy.

Abstract

Study Objective: To demonstrate laparoscopic management of a molar scar ectopic pregnancy.Design: Stepwise demonstration of the technique with narrated video footage.Setting: Cesarean scar ectopic pregnancy and molar pregnancy are 2 separate extremely rare pathologies with an incidence range from 1/1800 to 1/2500 of all pregnancies for the former [1,2]. The concurrence of both cesarean scar ectopic and molar pregnancy is furthermore exceptionally rare, and there are only 8 reported cases of cesarean scar molar pregnancy in literature till date [3]. There is a high risk of uterine rupture, uncontrolled hemorrhage, hysterectomy, and significant maternal morbidity owing to thin myometrium and fibrous scar after cesarean section [4,5]. Knowledge and awareness about this clinical condition aid in early diagnosis and reduced morbidity. Here, we present a rare case of cesarean scar ectopic pregnancy that was operated for failed medical management and diagnosed to be molar scar ectopic pregnancy intraoperatively.Interventions: Total laparoscopic approach to molar scar ectopic pregnancy excision involved the following steps, strategies to minimize blood loss, and complete enucleation of tissue: (1) Hysteroscopy to localize the scar ectopic and its type and size (2) Bladder dissection to expose scar (3) Intramyometrial injection of vasopressin (4) Use of harmonic scalpel to delineate the gestational sac (5) Complete evacuation of products of conception (6) Excision of scar tissue (7) Uterine repair in 2 layers CONCLUSION: There are only 8 reported cases of cesarean scar molar pregnancy in literature till date, and all patients had at least 2 previous uterine curettages with abnormally increased β-hCG levels. The clinical manifestations were varied, the most common symptom being vaginal bleeding for a period >1 month, including our case [3]. Considering the limitations of ultrasound, magnetic resonance imaging, and serum hCG levels in the differential diagnosis of molar cesarean scar pregnancy from normal cesarean scar pregnancy, postoperative specimen should be sent for histologic examination [6]. As seen in our case, the possibility of molar pregnancy at cesarean scar ectopic site should be kept in mind in cases with rising β-hCG levels despite continuous medical interventions, which was being medically managed for 3 months. Our case is the first to be successfully managed with laparoscopic surgery as the previously reported cases were managed with suction evacuation, chemotherapy, laparotomy, or hysterectomy [3].
