Abstract

Mechanical ventilation is a life-saving intervention, which despite its use on a routine basis, poses the risk of inflicting further damage to the lung tissue if ventilator settings are chosen inappropriately. Medical decision support systems may help to prevent such injuries while providing the optimal settings to reach a defined clinical goal. In order to develop and verify decision support algorithms, a test bench simulating a patient’s behaviour is needed. We propose a Java based system that allows simulation of respiratory mechanics, gas exchange and cardiovascular dynamics of a mechanically ventilated patient. The implemented models are allowed to interact and are interchangeable enabling the simulation of various clinical scenarios. Model simulations are running in real-time and show physiologically plausible results.

Abstract

Background and Purpose: Evidence supports aerobic exercise for positive health and wellness. Unfortunately, physical and mental impairments can limit aerobic exercise potential by elders and those with neurodegenerative disease. Two Quality Assurance (QA) studies evaluated if rehabilitation technology could enable individuals with mild-moderate PD to exercise aerobically to gain mobility and balance skills without injury or exacerbation of self reported PD signs and symptoms. Methods: Participants volunteered to train aerobically on two body weight supported treadmill systems (AlterGR and GlideTrakTM) (QA I) and/or a recumbent elliptical trainer (NuStep TR5x) with compression and cooling (VasperTM) (OA II). Pre and post training, the 10 Meter Walk, Six Minute Walk, Timed Up and Go (TUG) and Five Times Sit to Stand (FTSTS) were administered while signs/symptoms of PD were self reported before, during and after training. Abstract

In der vorliegenden Arbeit sollte die Häufigkeit und Verteilung von vier Serinproteasen- Autotransporter der Enterobacteriaceae (SPATE)- Vertreter im Genom von 152 Shiga Toxin- produzierenden- Escherichia coli (STEC) ermittelt werden. Keines der Isolate beherbergte das tsh- Gen. Das Gen für Pic konnte in 3,9% der STEC nachgewiesen werden. espP zeigte sich in 32,2%. espI konnte in 13 der 40 eae- negativen E.coli- Isolate identifiziert werden, aber zusätzlich auch in sechs der eae- positiven STEC. Überblickend lässt sich sagen, dass 48,7% der untersuchte STEC wenigstens ein Protease- Gen beherbergten, sechs Stämme zeigten eine Kombination. Bei den aufgefundenen pic und espI wurde eine fast vollständige DNA- Homologie zu den Referenz- Proteasen nachgewiesen. Die Gene von Serinprotease-Autotransportern in STEC kommen folglich in einer hohen Frequenz und in verschiedenen Kombinationen vor, eventuell ist dies ein Hinweis für einen Gentransfer.
