Abstract

Celiac disease, a chronic autoimmune disorder caused by genetic factors and exposure to gluten, is increasingly being recognized and diagnosed in both children and adults. Scientists have been searching for a cure for this disease for many years, but despite the impressive development of knowledge in this field, a gluten-free diet remains the only recommended therapy for all patients. At the same time, the increasing diagnosis of celiac disease in adults, which was considered a childhood disease in the 20th century, has opened a discussion on the etiopathology of the disease, which is proven to be very complex and involves genetic, immunological, nutritional, environmental and gut microbiotarelated factors. In this review, we extensively discuss these factors and summarize the knowledge of the proposed state-of-the-art treatments for celiac disease to address the question of whether a better understanding of the etiopathogenesis of celiac disease has opened new directions for therapy. Abstract

Lower bone mineral density (BMD) constitutes a common issue in inflammatory bowel disease (IBD). Studies often explore the association between BMD and folic acid level. The presented study aimed to evaluate the impact of MTHFRgene polymorphism and folic acid levels on BMD in patients with IBDs: Crohn’s disease (CD) and ulcerative colitis (UC). The study group comprised IBD patients and a healthy control group. BMD, T-score, and Z-score of the lumbar spine (L1-L4) and femoral neck (FN) were assessed using dual-energy X-ray absorptiometry. Folic acid level was determined using direct chemiluminescence, and the MTHFR677C > T (rs1801133) and 1298A > C (rs1801131) genotyping were performed by HRMA. Our study found no significant differences in the folic acid levels between the groups. Patients with CD and UC presented a lower BMD, T-score, and Z-score of the FN and L1-L4 than the CG. UC patients who were homozygotes AA in loci c.1298A>C presented lower than controls lumbar spine L1-L4 BMD and T-score values. Regarding MTHFR677 polymorphism, we found that IBD patients carrying CC genotype demonstrated lower than controls femoral neck Z-score, lumbar spine L1-L4 BMD, T-score and Z-score. MTHFRpolymorphisms were found to have no impact on folic acid concentrations. IBD patients presented a higher risk of low BMD than the healthy controls, regardless of MTHFR677 and 1298 genotypes. However, MTHFRpolymorphism may influence on bone in IBD patients. Nevertheless, it appears essential to conduct further studies.

19. Impact of Folate Intake on Bone Mineral Density in Patients with Inflammatory Bowel Disease.

Abstract

Background: Decreased bone mineral density (BMD) is a common problem among patients with inflammatory bowel disease (IBD). We hypothesised that an insufficient intake of folate might affect BMD. Methods: The study subjects included 26 with Crohn's disease—CD, 30 with ulcerative colitis—UC, and 31 healthy adults (control group—CG) aged 18–50 years. Participants were asked to follow their usual diet, and dietary intake was assessed by a 4-day, 24 h dietary recall. All the participants filled in a questionnaire referring to folic acid supplementation. The BMD, T-score, and Z-score of the lumbar spine (L1–L4) and femoral neck (FN) were assessed. Results: We found significant differences in the body mass, BMI (body mass index), CRP (C-reactive protein), BMD, Z-score, and T-score of the L1–L4 and FN between groups. There were no differences in energy and folate intake or the percentage coverage of recommended dietary allowances (RDA) of folate in all groups. Moreover, 70% of patients with UC, 92% of patients with CD, and 77% of CG patients showed insufficient folate intake. Folic acid was supplemented with a similar frequency in patients covering and not covering the RDA of folate. The intake of folate per 1000 kcal correlated positively with the CD group's BMD and T-score of L1–L4. Conclusions: Insufficient folate intake is common in patients with IBD and healthy individuals. The impact of folate on BMD in IBD is not clear. We need more studies on the association between folate intake, folic acid concentration, and BMD in IBD. [ABSTRACT FROM AUTHOR]

Abstract

Background: The aim of this study was to find an association between moderate, vigorous and total physical activity (PA); diet quality; and bone mineral density (BMD) among patients suffering from inflammatory bowel disease (IBD). Methods: We enrolled 54 IBD patients, including those with Crohn's disease (CD) and ulcerative colitis (UC), and 24 healthy adults. All subjects completed the Questionnaire of Eating Behaviour based on which prohealthy and nonhealthy diet indexes were calculated, and the questionnaire included questions from the International Physical Activity Questionnaire. Prohealthy and nonhealthy diet indexes were divided into low‐, medium‐ and high scores. BMD and T‐ and Z‐scores of the lumbar spine (L1–L4) and femoral neck (FN) were assessed using dual‐energy X‐ray absorptiometry method. Results: BMD, T‐ and Z‐scores of the FN and the Z‐score of L1–L4 were significantly lower among patients with CD and UC than healthy controls. We did not find any differences in the time of PA among CD, UC and control groups (CG). The prohealthy diet index was higher among healthy subjects than the CD and UC groups. The nonhealthy diet index was lower among UC patients compared with the CG or CD patients. Prohealthy diet index positively correlated with BMD and T‐ and Z‐scores of L1–L4 and FN in IBD. The prohealthy diet index correlated negatively with C‐reactive protein and positively with body mass index. The prohealthy diet index correlated only with total PA in the CD group. Conclusion: A well‐balanced diet and proper PA may decrease the risk of osteoporosis in IBD, so education of patients referring to nutrition and PA is needed. Abstract

Introduction. Selected dietary components are crucial for a properly functioning thyroid gland. Therefore a healthy and balanced diet is crucial for treating patients with Hashimoto's disease. Aim. This study aims to evaluate the dietary habits of women suffering from autoimmune thyroiditis. Material and methods. A questionnaire study was conducted in a group of 58 women with Hashimoto's disease and among 42 women without autoimmune thyroiditis using the Food Frequency Questionnaire (FFQ-6). To assess compliance with the normal distribution, we used the Shapiro-Wilk test. The comparative assessment was made using the Student's t-test and Mann-Whitney test. The Chi-square test was used to assess the relationship between the response and the group. A p-value of less than 0.05 was considered significant. Results. The study showed no significant differences in dietary habits between the two groups. Only 29% (n = 15) of women with HT declared daily consumption of fruits vs 31% (n = 12) in the control group (p = 0.3800), and only 2% (n = 1) declared consuming berries daily vs. 5% (n = 2), (p = 0.5270). The consumption of the recommended several portions of vegetables per day was declared by only 22% (n = 11) of women with HT vs 28% (n = 11) in the control group (p = 0.3983). 42% (n = 24) of women with HT vs 38.5% (n = 16) of controls reported eating cruciferous vegetables several times a week (p = 0.8241). 14% (n = 8) of women with HT vs 19% (n = 8) of women in the control group declared daily consumption of green leafy vegetables (p = 0.3653). Most people in the study group consume nuts and seeds several times a month, and those in the control group several times a week. Most HT respondents also declare not to consume the recommended amount of fish. Conclusions. The diet of patients with Hashimoto's disease does not differ significantly from that of non-Hashimoto's disease patients. Considering the study results and current scientific knowledge, it seems necessary to undertake educational activities for patients with Hashimoto's disease to increase patient awareness of the impact of diet on the course of the disease.
