10. ALEX2 multiplex examination – results of specific IgE to fish and shrimps in patients suffering from atopic dermatitis.

Abstract

The sensitivity of ALEX2 Allergy Explorer test in patients suffering from atopic dermatitis and clinical reactions to fish/shrimps was evaluated. We compared the results of specific IgE to allergen reagents (molecular components) and clinical reactions in the open exposure test (patient's history); the sensitivity was calculated with Fisher's Exact test (Rates and Confidence Intervals 95% C. I., value, lower, upper). Altogether 100 atopic dermatitis patients were examined. We confirmed significant relationship between the results of specific IgE to allergen reagents (molecular components) of fish and the results open exposure test/history; the sensitivity is in the range of 33.3% (7.49%–70.07%)–44.4% (13.7%–78.80%). We did not confirm the significant relationship between the results of specific IgE to allergen reagents (molecular components) of shrimps and the results open exposure test/history; the sensitivity is in the range of 0%–40% (5.27%–85.34%). 39. Roztroušená skleróza mozkomíšní, úloha střevní mikrobio-ty v poškozujícím zánětu.

Abstract

Majority of body compartments, especial-ly the gut, skin, respiratory and genitourinary tracts, is normal-ly inhabited by highly complex microbial populations designated as microbio-ta. Gut microbio-ta is the best studied so far. Colonisation patterns of gut microbio-ta which are acquired early dur--ing ontogeny, especially during the infant's age, are normal-ly maintained for life. Both mucosal and systemic im-munity, which are highly individualized, are determined by physiological gut microbio-ta. Dysbio-sis which is disturbed gut microbio-ta is fol-lowed by extensive negative impacts on human physiology. Recently, substantial links between gut microbio-ta and CNS were found. Whereas physiological colonisation patterns are support--ing optimal development of brain structures by provid--ing them with some energy and homeostatic regulations on inflammatory processes, dysbio-tic microbio-ta results in abnormal functional polarisation of T cell subsets. The result is the initiation and the progres-sion of a harmful inflam-matory response which might be a part of the pathophysiological proces-ses leading to the development of MS. Gut microbio-ta could be positively modulated via optimal nutrition rich in prebio-tic oligosaccharides, probio-tics, and postbio-tics. The optimalisation of nutrition can influence harmful inflam-mation in MS patients as evidenced in some animal models. Clinical trials to evaluate the ef-ficacy of fecal microbio-ta transplantation in MS patients can be expected in the near future. 40. Efektivní léčebná modulace poškozujícího zánětu u nemocných s psoriázou guselkumabem cílícím na specifickou podjednotku p19 IL-23 regulační osy IL-23/Th17.

Abstract

The special role in the pathogenesis of psoriasis is given to keratinocytes which are releasing damage associated molecular patterns (DAMP) as a consequence of the exposure to the various noxious stimuli. The identification of DAMP by PRR receptor is followed by the both local and systemic release of proinflammatory cytokines. Some DAMP pattern, so called alarmins, are processed in dendritic cells and are presented as antigenic fragments in optimal cytokine and costimulatory microenvironment to T cells. The abnormal functional polarisation of proliferating T cells in to Th17, Th1, and Th22 subsets is the consequence of this presentation. The critical role into the polarisation to the Th1 and Th17 subsets is given to IL-12 and IL-23 cytokines which share in their unique dimeric structures the common subunit p40. IL-23 is irreplaceable for the whole activity of Th17 subset. This concept is evidenced by the excellent results of several clinical studies in which the neutralisation of p19 subunit specific for IL-23 by guselkumab is substantially alleviating clinical activity of psoriasis. The clinical efficacy and excellent safety profil of guselkumab is superior to other biologicals used in the treatment of psoriasis. [ABSTRACT FROM AUTHOR]

Abstract

The main aim of the present study was a search for a characteristic serum marker of inflammatory activity in the airways of asthmatics with difficult-to-control disease. Therefore, serum levels of interleukin-4 (IL-4), serum low-affinity Fc Epsilon Receptor II (sFcER II), Interferon-gamma (INF-gamma) Immunoglobulin-E (IgE), Interleukin-2 (IL-2), serum Interleukin Receptor 2 (sIL-2R) and Intercellular Adhesion Molecule-1 (sICAM-1) were measured in 2 groups of asthmatics: 1-26 patients with difficult-to-control asthma (DTCA), 2-22 asthmatics, minimally symptomatic (MSA). Results: No significant difference in either measured parameters between the DTCA and MSA group in peripheral blood samples was found. Conclusion: The above mentioned serum markers of T- and B-cell activation as well as the serum ICAM-1 level are not sensitive enough to determine the type, activity and severity of the inflammatory process in the asthmatic airways.

43. Pentraxin 3(PTX 3): An Endogenous Modulator of the Inflammatory Response

Abstract

Inflammatory or anti-inflammatory? That is the question as far as the acute-phase response and its mediators, the pentraxins, are concerned. Only some ten years ago, the classical or short pentraxin C-reactive protein and the newly discovered long pentraxin PTX3 were considered to exert most of the detrimental effects of acute inflammation, whether microbial or sterile in origin. However, accumulating evidence suggests an at least dichotomous, context-dependent outcome attributable to the pentraxins, if not a straightforward anti-inflammatory nature of the acute-phase response. This paper is focused on the inherent effects of pentraxin 3 in inflammatory responses, mainly in coronary artery disease and in Aspergillus fumigatus infection. Both are examples of inflammatory reactions in which PTX3 is substantially involved; the former sterile, the latter infectious in origin. Apart from different inducing noxae, similarities in the pathogenesis of the two are striking. All the same, the introductory question still persists: is the ultimate impact of PTX3 in these conditions inflammatory or anti-inflammatory, paradoxical as the latter might appear? We try to provide an answer such as it emerges in the light of recent findings.

Abstract

Goeckerman's therapy (GT), which combines exposure to coal tar (polycyclic aromatic hydrocarbons - PAHs) and UV radiation (UV) is often used as the first option for treatment of psoriasis. However, PAHs and UV represent mutagenic, carcinogenic and immunotoxic agents. Therefore GT can represent a health risk for the patients. The group under observation consisted of thirty patients undergoing GT. Before and after the treatment, blood samples were collected and chromosomal aberrations and selected immunological markers were determined. The relationships between chromosomal aberrations and immunological markers and the extent (duration) of exposure to GT were evaluated. The Psoriasis Area and Severity Index (PASI) score confirmed the high efficacy of GT.
