BACKGROUND: Antiphospholipid syndrome (APS) is an autoimmune thrombophilia leading to life-threatening cardiovascular events. Cross-sectional data support that APS is associated with accelerated atherosclerosis, but this has not been confirmed in prospective studies. We aimed to compare the rate of atherosclerosis progression over a 3-year period between patients with APS, diabetes mellitus (DM) and healthy controls (HC). METHODS: Eighty-six patients with APS (43 with primary [PAPS], 43 with systemic lupus erythematosus-related APS [SLE/APS]) and an equal number of age- and sex-matched patients with DM and HC, who underwent a baseline ultrasound of the carotid and femoral arteries, were invited for a 3-year follow-up evaluation for atherosclerotic plaque progression. Multivariate analysis was performed for the assessment of determinants of plaque progression after adjustment for disease-related and traditional cardiovascular risk factors. RESULTS: Seventy-four APS patients (74.3\% female, 38 with PAPS), 58 DM patients and 73 HC were included. APS patients exhibited a 3.3-fold higher risk of new atherosclerotic plaque formation compared with HC (p= 0.031), similar to that in DM (odds ratio [OR]=3.45, p= 0.028). In APS patients, plaque development risk was higher in SLE/APS vs PAPS (OR = 7.75, p= 0.038) and was independently associated with the presence of traditional cardiovascular risk factors, as expressed by the Systematic Coronary Risk Evaluation (SCORE) risk (OR = 2.31, p= 0.008). CONCLUSION: APS is characterized by accelerated rates of subclinical atherosclerosis to a degree comparable to DM, which is more pronounced in SLE/APS patients. Traditional cardiovascular risk factors are major determinants of this risk, warranting aggressive management as in other high cardiovascular-risk disorders.

Background/Aims: Rheumatoid arthritis (RA) is associated with the emergence of cardiovascular disease, while chronic inflammation is considered a common denominator for their parallel progression. The Proprotein convertase subtilisin/kexin type 9 (PCSK9)/LDL-Receptor (LDLR) system is of high importance during atherogenesis, via regulating the clearance of LDL from the circulation; nevertheless the role of this molecular mechanism during RA-related atheromatosis is not known. Methods: Herein, high-resolution ultrasound measurements for arterial hypertrophy, atheromatosis and arterial stiffness as well as comprehensive biochemical profiling were performed in 85 RA patients. The circulating levels of PCSK9 and LDLR were measured and their potential associations as well as of the PCSK9/LDLR ratio with patients' characteristics and the degree of atherosclerosis were investigated. Results: Increased LDLR levels and decreased PCSK9/LDLR ratio were found in RA patients with at least 2 atheromatic plaques as compared to the ones without any plaques. In addition the levels of both PCSK9 and LDLR were positively correlated with the presence of atheromatic plaques as an age- and gender- adjusted multivariate analysis revealed. Conclusions: Our data imply that the PCSK9/LDLR system plays a significant role during RA-related atherosclerosis and may therefore be used as a screening tool for disease progression in the future.

Geriatric Medicine (GM) holds a crucial role in promoting health and managing the complex medical, cognitive, social, and psychological issues of older people. However, basic principles of GM, essential for optimizing the care of older people, are commonly unknown or undermined, especially in countries where GM is still under development. This narrative review aims at providing insights into the role of GM to non-geriatrician readers and summarizing the main aspects of the added value of a geriatric approach across the spectrum of healthcare. Health practitioners of all specialties are frequently encountered with clinical conditions, common in older patients (such as cancer, hypertension, delirium, major neurocognitive and mental health disorders, malnutrition, and peri-operative complications), which could be more appropriately managed under the light of the approach of GM. The role of allied health professionals with specialized knowledge and skills in dealing with older people's issues is essential, and a multidisciplinary team is required for the delivery of optimal care in response to the needs and aspirations of older people. Thus, countries should assure the educational background of all health care providers and the specialized health and social care services required to meet the demands of a rapidly aging society.

Σκοπός-Αντικείμενο: Η παρούσα διδακτορική διατριβή αφορά στη διερεύνηση των επιδημιολογικών παραμέτρων και των παραγόντων κινδύνου υποκλινικής αθηρωμάτωσης σε ασθενείς με συστηματικό ερυθηματώδη λύκο (ΣΕΛ) και αντιφωσφολιπιδικό σύνδρομο (ΑΦΣ). Μεθοδολογία-Διαδικασία: Υποψήφιοι για συμμετοχή στη μελέτη ήταν όλοι οι ασθενείς με ΣΕΛ ή/και ΑΦΣ που παρακολουθούνταν στο εξωτερικό ρευματολογικό ιατρείο της 1ης Προπαιδευτικής Παθολογικής Κλινικής της Ιατρικής Σχολής του ΕΚΠΑ (Γενικό Νοσοκομείο «Λαϊκό») μεταξύ 1/2016 και 1/2017. Κριτήρια αποκλεισμού ήταν η συνύπαρξη κλινικά εμφανούς αθηροσκληρωτικής καρδιαγγειακής νόσου, ενεργού κακοήθειας, ή σακχαρώδους διαβήτη (ΣΔ). Οι συμμετέχοντες αντιστοιχίστηκαν για ηλικία και φύλο με αναλογία 1:1 με ασθενείς που παρακολουθούνταν στο ίδιο ιατρείο για ρευματοειδή αρθρίτιδα (ΡΑ), με πάσχοντες από ΣΔ που παρακολουθούνταν στο αντίστοιχο εξωτερικό ιατρείο της ίδιας κλινικής κατά το χρονικό διάστημα επιλογής των συμμετεχόντων, και με υγιείς μάρτυρες από την τοπική κοινότητα. Κατόπιν ενυπόγραφης πληροφορημένης συγκατάθεσης, όλοι οι συμμετέχοντες (N=460) υποβλήθηκαν σε υπερηχογραφικό έλεγχο για μέτρηση του πάχους του μέσου-έσω χιτώνα (IMT) και ανίχνευση αθηρωματικών πλακών σε τρεις θέσεις κάθε καρωτίδας αρτηρίας, και σε μία θέση κάθε μηριαίας αρτηρίας. Πολυπαραγοντικά υποδείγματα γραμμικής και λογαριθμιστικής παλινδρόμησης εφαρμόστηκαν για την ανίχνευση στατιστικά σημαντικής διαφοράς στους βιοδείκτες αθηροσκλήρωσης (IMT και αθηρωματικών πλακών) μεταξύ ομάδων ασθενών και μαρτύρων, και για τη συσχέτιση των δεικτών αυτών με κλασικούς καρδιομεταβολικούς και νοσοεξαρτώμενους παράγοντες κινδύνου στους ασθενείς με ΣΕΛ ή/και ΑΦΣ. Τρία χρόνια μετά την αρχική εξέταση διενεργήθηκε επαναληπτικός υπερηχογραφικός έλεγχος των καρωτίδων και μηριαίων αρτηριών από τον ίδιο χειριστή, με την ίδια μεθοδολογία, λογισμικό, και μηχάνημα, με σκοπό τη μελέτη του ρυθμού επιδείνωσης των δεικτών υποκλινικής αθηρωμάτωσης, και τη συσχέτισή του με την έκθεση των ασθενών σε κλασικούς καρδιομεταβολικούς και νοσοεξαρτώμενους παράγοντες κινδύνου, συμπεριλαμβανομένης της ενεργότητας των αυτοάνοσων νοσημάτων τους. Αποτελέσματα-Συμπεράσματα: Στην αρχική εξέταση, ο αριθμός αθηρωματικών πλακών στις καρωτίδες και μηριαίες αρτηρίες των ασθενών με ΣΕΛ, ΡΑ, και ΣΔ ήταν σχεδόν διπλάσιος από τον αντίστοιχο που ανιχνεύτηκε στους υγιείς μάρτυρες, λαμβάνοντας υπ’ όψη την επίδραση των κλασικών καρδιομεταβολικών παραγόντων κινδύνου (RR=1.80, 95% CI 1.05-3.08, p= 0.033, RR=1.90 (1.11-3.26), p= 0.019, RR=1.93 (1.14-3.28), p=0.015, αντίστοιχα). H αθηροσκληρωτική νόσος σε ασθενείς με ΣΕΛ φάνηκε να χαρακτηρίζεται πρωτίστως από ανάπτυξη αθηρωματικών πλακών χωρίς σημαντική διάχυτη αύξηση του ΙΜΤ, σε αντίθεση με τις αθηροσκληρωτικές βλάβες που παρατηρήθηκαν σε ασθενείς με ΣΔ στις οποίες οι αθηρωματικές πλάκες συνήθως συνοδεύονται από γενικευμένη πάχυνση του έσω-μέσου χιτώνα. Το προοπτικό τμήμα της παρούσας μελέτης αποκάλυψε ότι η εξέλιξη της υποκλινικής αθηροσκλήρωσης είναι επιταχυμένη σε ασθενείς με ΣΕΛ, εν συγκρίσει με τους υγιείς μάρτυρες: Οι πιθανότητες εξέλιξης της αθηρωματικής βλάβης ήταν σε στατιστικά σημαντικό βαθμό υψηλότερες στον ΣΕΛ έναντι των μαρτύρων (OR=2.81, p=0.043), αλλά όχι στη ΡΑ (OR=2.22, p=0.109). Τα αποτελέσματα ήταν παρόμοια στους ασθενείς με χαμηλή ενεργότητα νόσου (88% των ΣΕΛ, 74% των ΡΑ). Συμπερασματικά, η επιδείνωση των αθηροσκληρωτικών δεικτών στον ΣΕΛ καθορίζεται κυρίως από την παρουσία των κλασικών καρδιομεταβολικών παραγόντων κινδύνου και από τη χρόνια χρήση κορτικοστεροειδών, και δεν αναχαιτίζεται στην ομάδα των ασθενών που επιτυγχάνουν χαμηλή ενεργότητα νόσου (Lupus Low Disease Activitystatus), ιδίως αν αυτή επιτυγχάνεται με το κόστος των υψηλών αθροιστικών δόσεων κορτικοστεροειδών. Μεταξύ ασθενών με ΑΦΣ, πλάκες στις καρωτίδες βρέθηκαν σε 28% των πασχόντων από πρωτοπαθές ΑΦΣ (ΠΑΦΣ), 23% των πασχόντων από ΣΕΛ/ΑΦΣ, και 30% των πασχόντων από ΣΔ, έναντι 9% που ανιχνεύτηκαν στους μάρτυρες (p=0.006). Πλάκες στις μηριαίες αρτηρίες βρέθηκαν σε 33% των ασθενών με ΠΑΦΣ, 19% των ασθενών με ΣΕΛ/ΑΦΣ, 20% των ασθενών με ΣΔ, και 9% των μαρτύρων (p=0.032). Οι εκτιμήσεις σχετικού κινδύνου για αθηρωματικές πλάκες οποιασδήποτε εντόπισης, που προέκυψαν από πολυπαραγοντικό υπόδειγμα παλινδρόμησης λαμβάνοντας υπ’ όψη την επίδραση της ηλικίας, του φύλου, του δείκτη μάζας σώματος, του οικογενειακού ιστορικού πρώιμης στεφανιαίας νόσου, και της παρουσίας αρτηριακής υπέρτασης, δυσλιπιδαιμίας, και καπνίσματος, ήταν 2.72 για τους ασθενείς με ΠΑΦΣ, 2.63 για τους ασθενείς με ΣΕΛ/ΑΦΣ, και 1.98 για τους ασθενείς με ΣΔ (p=0.004, 0.009, 0.032 αντίστοιχα). In conclusion, atherosclerosis progression in SLE is driven by the presence of TCRF, antiphospholipid antibodies and chronic corticosteroid use, and is not slowed down when patients achieve “lupus low disease activity status”, particularly if this is achieved at the cost of high cumulative corticosteroid doses. Among patients with APS, carotid plaques were found in 28% of those with primary APS (PAPS), 23% of SLE/APS, and 30% of DM patients versus 9% of controls (p=0.006). Femoral plaques were found in 33% of PAPS, 19% of SLE/APS, 20% of DM, and 9% of controls (p=0.032). Multivariate regression-derived relative risk estimates for atherosclerotic plaques in any location were 2.72 for PAPS, 2.63 for SLE/APS, and 1.98 for DM (p=0.004, 0.009, 0.032 respectively), after adjusting for age, gender, hypertension, dyslipidemia, smoking, BMI, and family history of coronary disease.
