Abstract

Since the beginning of the 1990s, an increase in dissolved organic carbon (DOC) has been observed in rivers and lakes in various parts of Europe and North America. The processes responsible for the increased DOC concentrations are complex and not entirely understood. The aim of this review is to provide an overview of the recent debate about increases in the DOC concentrations in surface water and their possible drivers.

5. Impact of maternal body mass index on neonatal outcome

Abstract

Abstract Introduction Maternal body mass index has an impact on maternal and fetal pregnancy outcome. An increased maternal BMI is known to be associated with admission of the newborn to a neonatal care unit. The reasons and impact of this admission on fetal outcome, however, are unknown so far. Objective The aim of our study was to investigate the impact of maternal BMI on maternal and fetal pregnancy outcome with special focus on the children admitted to a neonatal care unit. Methods A cohort of 2049 non-diabetic mothers giving birth in the Charite university hospital was prospectively studied. The impact of maternal BMI on maternal and fetal outcome parameters was tested using multivariate regression analysis. Outcome of children admitted to a neonatal ward (n = 505) was analysed. Results Increased maternal BMI was associated with an increased risk for hypertensive complications, peripheral edema, caesarean section, fetal macrosomia and admission of the newborn to a neonatal care unit, whereas decreased BMI was associated with preterm birth and lower birthweight. In the neonatal ward children from obese mothers are characterized by hypoglycaemia. They need less oxygen, and exhibit a shorter stay on the neonatal ward compared to children from normal weight mothers, whereas children from underweight mothers are characterized by lower umbilical blood pH and increased incidence of death corresponding to increased prevalence of preterm birth. Conclusion Pregnancy outcome is worst in babies from mothers with low body mass index as compared to healthy weight mothers with respect to increased incidence of preterm birth, lower birth weight and increased neonate mortality on the neonatal ward. We demonstrate that the increased risk for neonatal admission in children from obese mothers does not necessarily indicate severe fetal impairment.

Abstract

Zusammenfassung: Die chronische Urtikaria (CU) gehört zu den häufigsten Krankheitsbildern in der Dermatologie. Aktuelle Studien zeigen eine Punktprävalenz zwischen 0,5 und 1% in der Gesamtbevölkerung mit einem Übergewicht an Frauen. Prinzipiell können alle Altersgruppen und Bevölkerungsschichten betroffen sein. Allerdings gibt es eine Häufung in der 3. und 4. Lebensdekade. Nach Definition der aktuellen Leitlinien besteht eine chronische Urtikaria dann, wenn Quaddelbildung und/oderAngioödeme spontan und für länger als 6 Wochen auftreten. Epidemiologische Studien belegen, dass die Mehrheit der Betroffenen über Monate, oft Jahre, unter den Beschwerden leidet. Negativ auf die Dauer der Erkrankung scheinen sich das Vorkommen von Angioödemen, das zeitgleiche Bestehen einer physikalischen Urtikaria, Positivität im autologen Serumtest und eine hohe Krankheitsschwere auszuwirken. Studien zur Lebensqualität zeigen, dass CU-Patienten ähnlich stark unter der Erkrankung leiden wie Menschen mit einer ausgeprägten koronaren Herzkrankheit. Neben den rein körperlichen Symptomen erleben die Patienten auch Einschränkungen bei alltäglichen Tätigkeiten und im Sozialleben. Zudem ist der Schlaf oft stark beeinträchtigt, und die Patienten leiden überdurchschnittlich häufig an psychiatrischen Begleiterkrankungen. Um Missverständnisse und Frustration bei der Betreuung zu vermeiden, ist es wichtig, die Patienten mit ihrer Erkrankung in all ihren Dimensionen wahr- und ernst zu nehmen. Das Ziel der Therapie von CU-Patienten sollte es sein, komplette Beschwerdefreiheit zu erreichen.

Abstract

Objective: The present paper describes the design, development, and implementation of the AutoInflammatory Disease Alliance (AIDA) International Registry specifically dedicated to patients with Schnitzler syndrome. Methods: This is a clinical physician-driven, population- and electronic-based registry implemented for the retrospective and prospective collection of real-life data from patients with Schnitzler’s syndrome; the registry is based on the Research Electronic Data Capture (REDCap) tool, which is thought to collect standardised information for clinical research, and has been realised to change over time according to future scientific acquisitions and potentially communicate with other existing or future similar registries. Results: Since its launch, 112 centres from 23 countries in 4 continents have been involved. Fifty-four have already obtained the approval from their local Ethics Committees. The platform counts 303 users (113 Principal Investigators, 186 Site Investigators, 2 Lead Investigators, and 2 data managers) at current (April 12th 2022). The registry collects baseline and follow-up data using 3922 fields organised into 25 instruments, including patient’s demographics, history, clinical manifestations and symptoms, trigger/risk factors, laboratory, instrumental exams, therapies, socioeconomic information, and healthcare access. Conclusions: This International Registry for patients with Schnitzler syndrome facilitates standardised data collection, enabling international collaborative projects through data sharing and dissemination of knowledge; in turn, it will shed light into many blind spots characterizing this complex autoinflammatory disorder
