Toch zijn er daarentegen bepaalde recepten waarin staat dat je de bloemkool lekker krokant kan blakeren in de oven. En dat is misschien niet helemaal fout. In dat zwarte stukje worst is namelijk iets anders gebeurd dan in het zwarte stukje bloemkool. Ofwel: het ene zwarte randje is slechter voor je, dan het andere. The 3Rs concept, calling for replacement, reduction and refinement of animal experimentation, is receiving increasing attention around the world, and has found its way to legislation, in particular in the European Union. This is aligned by continuing high-level efforts of the European Commission to support development and implementation of 3Rs methods. In this respect, the European project called "ONTOX: ontology-driven and artificial intelligence-based repeated dose toxicity testing of chemicals for next generation risk assessment" was recently initiated with the goal to provide a functional and sustainable solution for advancing human risk assessment of chemicals without the use of animals in line with the principles of 21st century toxicity testing and next generation risk assessment. ONTOX will deliver a generic strategy to create new approach methodologies (NAMs) in order to predict systemic repeated dose toxicity effects that, upon combination with tailored exposure assessment, will enable human risk assessment. For proof-of-concept purposes, focus is put on NAMs addressing adversities in the liver, kidneys and developing brain induced by a variety of chemicals. The NAMs each consist of a computational system based on artificial intelligence and are fed by biological, toxicological, chemical and kinetic data. Data are consecutively integrated in physiological maps, quantitative adverse outcome pathway networks and ontology frameworks. Supported by artificial intelligence, data gaps are identified and are filled by targeted in vitro and in silico testing. ONTOX is anticipated to have a deep and long-lasting impact at many levels, in particular by consolidating Europe's world-leading position regarding the development, exploitation, regulation and application of animal-free methods for human risk assessment of chemicals.

De rivier de Maas dient vele gebruikers; zoals scheepvaart, industrie, energievoorziening, natuur, landbouw en recreatie. Ook voor drinkwatervoorziening is de Maas belangrijk: 7 miljoen mensen in Nederland en België krijgen drinkwater geleverd dat bereid is met water uit de Maas. Om de levering van drinkwater veilig te kunnen stellen is voldoende water van goede kwaliteit nodig. Dit uitgangspunt komt onder druk te staan bij langdurige droogte. Naar verwachting zullen door klimaatverandering laagwaterperiodes in de toekomst vaker en heviger gaan voorkomen. Het is daarom van belang om goed inzicht te hebben in de huidige en toekomstige waterhoeveelheden gecombineerd met het huidige en toekomstige gebruik. Binnen deze studie wordt de oorsprong van het Maaswater tijdens lage afvoerperioden onderzocht. De Maasafvoer is opgebouwd uit de afvoer van verschillende zijrivieren. Om beter inzicht te krijgen in de vraag waar het water van de Maas vandaan komt bij lage rivierafvoeren, is voor deze studie een 23-jarige (1998-2020) dataset opgezet met daarin 18 belangrijke zijrivieren, 2 onttrekkingen (Albertkanaal en Zuid-Willemsvaart) en 5 meetstations in de hoofdstroom van de Maas. De dataset is gevalideerd en waar nodig opgevuld om een goede werkbare dataset te krijgen. Op basis van literatuur en de verkregen dataset kan het stroomgebied van de Maas worden opgedeeld in vier gebieden: Boven: het bovengebied van de Maas bevat het stroomgebied bovenstrooms van Stenay, de Chiers en de Bar. Vanwege het bufferend vermogen van de ondergrond, leveren deze zijrivieren tijdens laagwater een relatief grote bijdrage aan de afvoer van de Maas. Tijdens laagwater is de bijdrage circa 25%, wat overeenkomt met het oppervlakte circa 6500 km2 (26,5% van het bemeten stroomgebied) bedraagt. Ardennen: In de Ardennen liggen de zijrivieren Semois, Viroin, Houile, Hermeton, Lesse, Sambre, Ourthe, Ambleve en Vesdre. Het gebied beslaat een oppervlakte van circa 10.000 km2 (40%). Gezien de ontwerplevensduur van een groot deel van de natte kunstwerken verwachten waterbeheerders dat zij in de komende decennia veel investeringsbeslissingen moeten gaan nemen over vervanging en renovatie. Deze beslissingen zijn een belangrijk keuzemoment; ze bieden de kans om, met het oog op de toekomst, wenselijke veranderingen in de (multifunctionele) infrastructuur in de afweging mee te nemen. Binnen het Kennisprogramma Natte Kunstwerken 2017-2020 werkten Rijkswaterstaat en Deltares een methodiek uit die de afname van (functionele) prestaties in de tijd meeneemt en zo een inschatting maakt van de functionele levensduur. Dit artikel beschrijft de methode met het systeem ‘Maas’ als voorbeeld.

Vismigratie vormt een belangrijk aspect van het functioneren van het ecosysteem in het hoofdwatersysteem van Rijkswaterstaat. Slechte passeerbaarheid voor vis van barrières zoals stuwcomplexen is een risico voor het in stand houden van vispopulaties. Het passeerbaar maken van deze barrières, en daarmee het herstel van de longitudinale connectiviteit in de rivieren, speelt een sleutelrol in het herstel van de populaties van migrerende vissoorten. In de literatuur is allerlei informatie beschikbaar over de bijdrage van de verschillende verliesposten bij de stuwcomplexen - zowel in stroomopwaartse als in stroomafwaartse richting - voor verschillende vissoorten/gildes. Veel van deze informatie is gebaseerd op meetgegevens bij een specifiek object, voor een specifieke soort, op een bepaald moment, onder bepaalde omstandigheden. De bijlages van dit rapport geven een overzicht van deze informatie. Om deze informatie op een gestructureerde wijze te verwerken, en op een generieke wijze toe te passen voor een watersysteem met diverse stuwcomplexen, zoals bijvoorbeeld de Maas en de Nederrijn, is een flexibel analysetool ontwikkeld. Dit analysetool biedt de mogelijkheid om de gekozen waardes voor de cruciale parameters te variëren, daarmee de verliezen te berekenen van een soort, voor de afzonderlijke objecten binnen de stuwcomplexen, alsmede het totale verlies bij een stuwcomplex, en het verlies in het volledige watersysteem. Het analysetool is in deze studie toegepast voor de Maas en Nederrijn.

UDP-glucuronosyltransferases (UGTs) are involved in phase II conjugation reactions of xenobiotics and differences in their isoform activities result in interindividual kinetic differences of UGT probe substrates. Here, extensive literature searches were performed to identify probe substrates (14) for various UGT isoforms (UGT1A1, UGT1A3, UGT1A4, UGT1A6, UGT1A9, UGT2B7 and UGT2B15) and frequencies of human polymorphisms. Chemical-specific pharmacokinetic data were collected in a database to quantify interindividual differences in markers of acute (Cmax) and chronic (area under the curve, clearance) exposure. Using this database, UGT-related uncertainty factors were derived and compared to the default factor (i.e. 3.16) allowing for interindividual differences in kinetics. Overall, results show that pharmacokinetic data are predominantly available for Caucasian populations and scarce for other populations of different geographical ancestry. Furthermore, the relationships between UGT polymorphisms and pharmacokinetic parameters are rarely addressed in the included studies. The data show that UGT-related uncertainty factors were mostly below the default toxicokinetic uncertainty factor of 3.16, with the exception of five probe substrates (1-OH-midazolam, ezetimibe, raltegravir, SN38 and trifluoperazine), with three of these substrates being metabolised by the polymorphic isoform 1A1. Data gaps and future work to integrate UGT-related variability distributions with in vitro data to develop quantitative in vitro–in vivo extrapolations in chemical risk assessment are discussed.

In het aquatisch milieu worden steeds meer stoffen gevonden die nadelige effecten kunnen hebben op het zenuwstelsel. Toch worden er nauwelijks testen toegepast voor effectmetingen van de chemische waterkwaliteit op het gebied van neurotoxiciteit. Deze studie laat op grond van verkennend onderzoek zien dat in principe voor de watersector neurotoxiciteitstesten beschikbaar zijn. Deze testmethoden maken gebruik van menselijke of dierlijke zenuwcellen en modelorganismen (bioassays). Welke modellen en technieken het meest geschikt zijn voor toepassing, moet verder worden onderzocht. Aanbevolen wordt om de ontwikkelingen op het gebied van bioassays voor neuroactieve stoffen nauwlettend te volgen. Deze kennis draagt bij aan toekomstige implementatie van neurotoxiciteitstesten voor waterkwaliteitsmonitoring.

Quality performance indicators (QPIs) are used to monitor the delivery of cancer care. Neuroendocrine tumours (NETs) are a family of individually uncommon cancers that derive from neuroendocrine cells or their precursors, and can occur in most organs. There are currently no QPIs available for NETs and their heterogeneity makes QPI development difficult. CommNETs is a collaboration between NET clinicians, researchers and advocates in Canada, Australia and New Zealand. We created QPIs for NETs using a three-step consensus process. First, a multidisciplinary team used the nominal group technique to create candidates (n = 133) which were then curated into appropriateness statements (62 statements, 44 sub-statements). A two-stage modified RAND/UCLA Delphi consensus process was conducted: an online survey rated the statement appropriateness then the top-ranked statements (n = 20) were assessed in a face-to-face meeting. Finally, 10 QPIs met consensus criteria; documentation of primary site, proliferative index, differentiation, tumour board review, use of a structured pathology report, presence of distant metastasis, 5-and 10-year diseasefree and overall survival. These NET QPIs will be trialed as a method to monitor and improve care for people with NETs and to facilitate international comparison.Copyright © 2019 by the authors. Contains fulltext : 205519.pdf (publisher's version ) (Closed access), PURPOSE: Breast cancer is the most frequently diagnosed cancer and leading cause of cancer death among women, representing a considerable public health burden in South Africa and other low-middle income countries. Short- and long-term complications of these treatments include shoulder morbidities such as pain, decreased range of motion, tightness, weakness, pain, numbness and lymphoedema and may be present for up to 6 years post-treatment. An understanding of baseline demographic and clinical risk factors can guide rehabilitation and management strategies for high-risk patients. The aims of this study were to quantify the burden of shoulder pain and disability in a tertiary academic hospital in Cape Town, South Africa, and identify potential risk factors for the development of shoulder morbidity. METHODS: This study was a cross-sectional analysis of the prevalence of shoulder pain and dysfunction in women attending their post-treatment annual follow-up visit for unilateral breast carcinoma. RESULTS: Three in four patients reported a presence of any pain or disability while only 9% experienced severe pain and disability. Multivariable ordinal logistic regression analysis identified race, side, axillary surgery, chemotherapy and age as significant predictors of pain and chemotherapy a significant predictor of disability. CONCLUSION: The substantial burden of shoulder morbidity in this population represents a significant public health burden. The use of identified clinical and demographic characteristics may guide in the development of survivorship programmes incorporating surveillance and management of these high-risk patients.

Bees are exposed to a wide range of multiple chemicals “chemical mixtures” from anthropogenic (e.g. plant protection products or veterinary products) or natural origin (e.g. mycotoxins, plant toxins). Quantifying the relative impact of multiple chemicals on bee health compared with other environmental stressors (e.g. varroa, viruses, and nutrition) has been identified as a priority to support the development of holistic risk assessment methods. Here, extensive literature searches and data collection of available laboratory studies on combined toxicity data for binary mixtures of pesticides and non-chemical stressors has been performed for honey bees (Apis mellifera), wild bees (Bombus spp.) and solitary bee species (Osmia spp.). From 957 screened publications, 14 publications provided 218 binary mixture toxicity data mostly for acute mortality (lethal dose: LD50) after contact exposure (61%), with fewer studies reporting chronic oral toxicity (20%) and acute oral LC50 values (19%). From the data collection, available dose response data for 92 binary mixtures were modelled using a Toxic Unit (TU) approach and the MIXTOX modelling tool to test assumptions of combined toxicity i.e. concentration addition (CA), and interactions (i.e. synergism, antagonism). The magnitude of interactions was quantified as the Model Deviation Ratio (MDR). The CA model applied to 17% of cases while synergism and antagonism were observed for 72% (MDR > 1.25) and 11% (MDR < 0.83) respectively. Most synergistic effects (55%) were observed as interactions between sterol-biosynthesis-inhibiting (SBI) fungicides and insecticide/acaricide. The mechanisms behind such synergistic effects of binary mixtures in bees are known to involve direct cytochrome P450 (CYP) inhibition, resulting in an increase in internal dose and toxicity of the binary mixture. Met de nieuwe langjarige berekeningen van het Nationaal Water Model is voor het eerst de variabiliteit van droogte en het effect op het water-bodemsysteem goed op landelijk niveau in beeld gebracht. In dit artikel wordt deze langjarige hydrologische reeks gebruikt om te kwantificeren hoe extreem de droogte van 2018 was voor verschillende gebieden in Nederland. Dit laat de meerwaarde zien van een langjarige reeks als onderdeel van de droogterisicobenadering voor zoetwatervoorziening op de lange termijn.
