Introduction: Heart failure (HF) is a prothrombotic state that is also associated with the progression of renal dysfunction. However, it is unknown whether coagulation abnormalities are associated with progressive cardiorenal syndrome. Aim of the research: To evaluate activators and inhibitors of coagulation and fibrinolysis and their relationship with renal failure in HF patients. Material and methods: Coagulation biomarkers such as thrombin-antithrombin III, human tissue-type plasminogen activator, human plasminogen activator inhibitor, von Willebrand factor (vWF), soluble thrombomodulin (sTM), human prothrombin fragments (F1+F2), and protein C were evaluated in 36 consecutive HF patients without anticoagulation and in 19 controls matched in age and gender. Results: HF patients, compared to controls, had lower levels of C protein (p = 0.04) and F1 + F2 (p < 0.001) but higher levels of vWF (p < 0.001) and borderline sTM (p = 0.07). Similarly, haemoglobin (p < 0.001) and glomerular filtration rate (GFR) (p = 0.004) were lower in HF, while INR (p < 0.001), NT-proBNP (p < 0.001), and asymmetric dimethylarginine (ADMA) (p < 0.001) were higher. Most of the echocardiographic parameters differed between the 2 groups. From coagulation biomarkers, sTM (r = -0.66; p < 0.001) and vWF (r = -0.41; p = 0.002) were associated with eGFR. Most of the echocardiographic and laboratory parameters were also related to eGFR. After classifying all variables into 5 categories; laboratory tests, echocardiographic parameters, vascular reactivity, haemodynamics, and coagulation parameters, multivariable linear regression showed that coagulation parameters were the most strongly associated with eGFR (r2 = 0.48, p < 0.001). Niewydolność serca jest główną przyczyną zgonów na świecie i najczęstszym powodem hospitalizacji pacjentów po 65. roku życia. Współczesna terapia niewydolności serca opóźnia postęp choroby, ale nie zmniejsza całkowitej śmiertelności, która wynosi nawet 20% rocznie. Bez względu na etiologię niewydolności serca: ostry zawał serca, przewlekły stres związany z chorobą czy starzenie, u podłoża problemu leży postępująca utrata kardiomiocytów i zmniejszony rzut serca. Od ponad 10 lat podejmowane są próby regeneracji uszkodzonego mięśnia serca za pomocą terapii komórkowej. W doświadczeniach laboratoryjnych na zwierzętach oraz badaniach klinicznych I generacji wykorzystano w tym celu wiele różnych typów komórek, m.in.: mioblasty szkieletowe, komórki jednojądrzaste szpiku kostnego i krwi obwodowej, progenitory endotelialne, kardiomiocyty płodowe, komórki mezenchymalne, zarodkowe komórki macierzyste, indukowane pluripotencjalne komórki macierzyste oraz komórki macierzyste serca. Wyniki obserwacji wskazują, że zastosowanie komórek macierzystych w leczeniu schyłkowych postaci niewydolności serca może poprawić mechaniczną funkcję serca. Ostatnio wykazano, że serce nie jest narządem ostatecznie zróżnicowanym, a obecne w tkance komórki macierzyste mogą być wykorzystane w celu regeneracji kardiomiocytów, komórek mięśni gładkich i komórek śródbłonka naczyniowego. W niniejszym artykule podsumowano pokrótce wyniki badań klinicznych I generacji oraz omówiono perspektywy rozwoju terapii regeneracyjnej w najbliższej przyszłości, ze szczególnym uwzględnieniem nowych populacji komórek macierzystych. Introduction: Endothelial progenitor cells (EPCs) in nontransplant settings have reparative properties. However, their role in heart transplantation (HT) is not well defined., Objectives: The aim of this study was to prospectively evaluate changes in EPC levels in relation to post‑HT rejection., Patients and Methods: EPC levels were measured in 27 HT recipients for 6 months after HT. Acute cellular rejection (ACR) or antibody‑mediated rejection (AMR) were assessed by right ventricular endomyocardial biopsy., Results: ACR and AMR were observed in 7 (25.9%) and 6 (22.2%) patients, respectively. The ACR status at 1 month post‑HT did not differ with respect to EPC immediately post‑HT. At 1 month post‑HT in patients without ACR or AMR, EPC levels were significantly reduced compared with the measurements immediately post‑HT (P <0.001). On further follow‑up, EPC levels were similar regardless of the rejection events. Nonetheless, greater changes (coefficient of variation) in EPClog (logarithmic transformation) were associated with the risk of AMR or ACR compared with those without any rejection event (median [lower-upper quartile], 15 [13-18] vs 8 [5-13]; P = 0.02 and 22 [14-26] vs 8 [5-13]; P = 0.01, respectively). The receiver operating characteristic curve showed that the coefficient of variation of EPClog of 12 was the optimal cutoff value for the prediction of rejection (area under the curve = 0.85). Higher levels were associated with greater risk of ACR or AMR (P <0.005)., Conclusions: Early reduction of EPC levels was related to a lower risk of ACR or AMR. Greater changes of EPC‑levels during follow‑up were associated with a significantly higher risk of rejection.
