3. The utility of serum neutrophil gelatinase‐associated lipocalin level on predicting autosomal dominant polycystic kidney disease progression.

Abstract

Introduction: We focused on neutrophil gelatinase‐associated lipocalin (NGAL) and autosomal dominant polycystic kidney disease (ADPKD) progression. Methods: ADPKD patients with an estimated glomerular filtration rate (eGFR) ≥ 30 mL/min/1.73 m2 were included. Serum NGAL level and NGAL to eGFR ratio (NGR), height‐adjusted total kidney volume (hTKV) were assessed initially. Patients were followed‐up for 5 years. Results: Sixty one patients were enrolled and initial eGFR was 73.6 (48.9–101.5) ml/min/1.73m2. EGFR declined by 3.7 mL/min/1.73m2 per year. Thirty four patients (55.7%) exhibited rapid progression. Rapid progression group had lower serum NGAL levels (p < 0.001) and higher hTKV (p < 0.001). Lower serum NGAL level was a risk factor for rapid progression (p < 0.001). NGR was not associated with rapid progression. Serum NGAL level was predictive in for rapid progression ROC analysis (cut‐off <10.62 ng/mL). Conclusion: Relatively lower serum NGAL levels can predict worse outcomes in ADPKD and can provide risk stratification in patients with ADPKD. [ABSTRACT FROM AUTHOR]

5. The Utility of Bone Morphogenetic Protein 7 Levels on Predicting the Alteration of the Peritoneal Membrane Permeability.

Abstract

Background: This study aimed to investigate the potential relationship between bone morphogenetic protein 7 (BMP-7) and peritoneal permeability, as well as clinical outcomes. Methods: We conducted a cross-sectional study involving 67 peritoneal dialysis (PD) patients who had been on PD for at least 1 year. We measured BMP-7 levels in both plasma and PD effluent fluid. Two peritoneal equilibration tests, the first and last ones, were compared to assess alterations in peritoneal membrane permeability. Dialysate to plasma ratio for creatinine (DPRC) was used to define membrane permeability. BMP-7 level was measured concurrently in both plasma and PD effluent. Results: Among the patients, 81% (n = 51) were on ambulatory PD, while 19% (n = 12) were on automated PD. The average PD duration was 58.9 months. Increased DPRC was determined in 61.9% of the patients. There was a positive correlation between plasma BMP-7 levels and the change in DPRC (r = 0.323, P = .004). No correlation was determined between effluent BMP-7 levels and changes in DPRC. A statistically significant negative correlation was determined between plasma BMP-7 levels and serum albumin levels (r = -0.274, P = .02). Plasma BMP-7 levels independently predicted changes in DPRC (P = .02), while effluent BMP-7 levels did not (P = .212). Conclusion: We identified a correlation between plasma BMP-7 level and changes in peritoneal membrane permeability. However, we did not find any relationship with the PD clinical outcomes. Abstract

Aim: We aim to examine the role of HIF-2α on chronic allograft nephropathy (CAN), which is a consequence of chronic inflammation and fibrosis in patients with renal transplantation. Materials and Methods: The study was conducted between November 2019 and January 2021, patients who had a renal biopsy in our university in the last two years were enrolled in this study. Fifteen recipients with a diagnosis of chronic allograft nephropathy proven by biopsy, 15 recipients with an eGFR below 60 ml/dk for other reasons as non-CAN group. Also, fifteen patients with an eGFR above 60 ml/dk were enrolled in the study as a control group. Serum HIF-2α levels were analyzed by the immunoassay technique. Results: HIF-2α was found higher in the CAN group compared to other groups. HIF-2α was significantly predictive in ROC analysis in identifying renal transplantation patients with CAN. The sensitivity and specificity of HIF-2α were 100% and 76% (cut off >0.2) with an area of under the ROC curve of 0.941 (95% CI 0. 850-1.000), p < 0.001). Conclusion: Serum HIF-2α may be useful both as potential biomarkers for diagnosing CAN after kidney transplantation and may be a guide in determining treatment strategies. [ABSTRACT FROM AUTHOR]

Abstract

BackgroundAcute kidney injury (AKI) is common in coronavirus disease-2019 (COVID-19) and the severity of AKI is linked to adverse outcomes. In this study, we investigated the factors associated with in-hospital outcomes among hospitalized patients with COVID-19 and AKI.MethodsIn this multicenter retrospective observational study, we evaluated the characteristics and in-hospital renal and patient outcomes of 578 patients with confirmed COVID-19 and AKI. Data were collected from 34 hospitals in Turkey from March 11 to June 30, 2020. AKI definition and staging were based on the Kidney Disease Improving Global Outcomes criteria. Patients with end-stage kidney disease or with a kidney transplant were excluded. Renal outcomes were identified only in discharged patients.ResultsThe median age of the patients was 69 years, and 60.9% were males. The most frequent comorbid conditions were hypertension (70.5%), diabetes mellitus (43.8%), and chronic kidney disease (CKD) (37.6%). The proportions of AKI stages 1, 2, and 3 were 54.0%, 24.7%, and 21.3%, respectively. 291 patients (50.3%) were admitted to the intensive care unit. Renal improvement was complete in 81.7% and partial in 17.2% of the patients who were discharged. Renal outcomes were worse in patients with AKI stage 3 or baseline CKD. The overall in-hospital mortality in patients with AKI was 38.9%. In-hospital mortality rate was not different in patients with preexisting non-dialysis CKD compared to patients without CKD (34.4 versus 34.0%, p = 0.924). By multivariate Cox regression analysis, age (hazard ratio [HR] [95% confidence interval (95%CI)]: 1.01 [1.0–1.03], p = 0.035], male gender (HR [95%CI]: 1.47 [1.04–2.09], p = 0.029), diabetes mellitus (HR [95%CI]: 1.51 [1.06–2.17], p = 0.022) and cerebrovascular disease (HR [95%CI]: 1.82 [1.08–3.07], p = 0.023), serum lactate dehydrogenase (greater than two-fold increase) (HR [95%CI]: 1.55 [1.05–2.30], p = 0.027) and AKI stage 2 (HR [95%CI]: 1.98 [1.25–3.14], p = 0.003) and stage 3 (HR [95%CI]: 2.25 [1.44–3.51], p = 0.0001) were independent predictors of in-hospital mortality.ConclusionsAdvanced-stage AKI is associated with extremely high mortality among hospitalized COVID-19 patients. Age, male gender, comorbidities, which are risk factors for mortality in patients with COVID-19 in the general population, are also related to in-hospital mortality in patients with AKI. However, preexisting non-dialysis CKD did not increase in-hospital mortality rate among AKI patients. Renal problems continue in a significant portion of the patients who were discharged.

Prolonged exposure to high glucose concentrations, degradation products, uremic toxins, and episodes of peritonitis can cause some changes in the peritoneal membrane, resulting in intraperitoneal inflammation and PF, leading to failure of ultrafiltration and dialysis. CA-125 can be used as a biomarker of peritoneal mesothelial cell count in the peritoneal dialysate and for monitoring cell count in PD patients. Hypoxia-inducible factor 1-alpha (HIF-1α) has been reported to cause PF, but has not been reported to be associated with changes in peritoneal structure. We hypothesized that peritoneal adequacy can be followed using HIF-1α and CA-125 values. In the present study, therefore, we investigated the relationship between HIF-1α and CA-125 levels and parietal membrane permeability changes in PD patients., Methods: Forty-five patients were included in the study. Peritoneal permeability was constant in 20 of these, while peritoneal permeability increased in 11 and decreased in 14. The HIF-1α value from the blood samples of the patients and the CA-125 measurement from the peritoneal fluids were measured. The relationship between peritoneal variability and CA-125 and HIF levels after follow-up was investigated., Results: We compared serum HIF-1α and peritoneal fluid CA-125 levels in the three groups receiving peritoneal dialysis treatment. HIF-1α levels increased with peritoneal permeability changes, while CA-125 levels decreased. In patients with high to low permeability changes, HIF-1α levels were higher compared to those with stable or low to high changes, which was statistically significant. Conversely, CA-125 levels significantly decreased in patients whose peritoneal permeability changed from high to low, compared to the other two groups., Conclusion: Changes in peritoneal structure can be followed with biomarkers. It has been shown that CA-125 and HIF-1α levels can guide the changes in the peritoneal membrane.
