6. Transversus Abdominis Plane Block Following Cesarean Section: A Prospective Randomized Controlled Study Comparing the Effects on Pain Levels of Bupivacaine, Bupivacaine + Dexmedetomidine, and Bupivacaine + Dexamethasone.

Abstract

Background: The transversus abdominis plane (TAP) block is providing effective postoperative analgesia in patients undergoing cesarean section (CS). This study aims to evaluate and compare the effects on pain levels of bupivacaine alone versus bupivacaine combined with dexmedetomidine and bupivacaine combined with dexamethasone in ultrasound-guided TAP block for postoperative pain after CS. Material and Method: In this randomized controlled trial, 120 patients with American Society of Anesthesiologists (ASA) physical status I and II scheduled for elective cesarean section under spinal anesthesia were randomly divided into three groups. At the end of the surgery, an ultrasound-guided TAP block was performed on all patients: bupivacaine 0.5% (Group B), bupivacaine 0.5% + dexmedetomidine (1 µg/kg) (Group BD), and bupivacaine 0.5% + dexamethasone (4 mg) (Group BDx). Postoperatively, all patients were evaluated at 0, 1, 4, 8, 16, and 24 h for visual analog scores VASs, tramadol consumption, complications, and patient satisfaction. A p value of < 0.05 is statistically significant. Results: At 0 h, VASs in the sitting and supine positions were significantly higher in the BDx group (0.85 ± 1.61 and 0.85 ± 1.36, respectively) compared to the B group (0.05 ± 0.32 in both positions) and the BD group (0.15 ± 0.48 in both positions) (p = 0.005 and p = 0.001, respectively). At the 24th hour, VASs in the sitting and supine positions were significantly lower in the BDx group (1.7 ± 1.2 and 1.43 ± 1.05) compared to the B group (2.3 ± 0.68 and 2.2 ± 0.72) and the BD group (2.57 ± 1.01 and 2.28 ± 0.78) (p = 0.005 and p = 0.001, respectively). At 0 h, the tramadol requirement was highest in the BDx group at 12.5%, while it was not required in the B and BD groups (p = 0.005). At 0 h, the rate of nausea and vomiting was highest in the BDx group at 17.5%, compared to 2.5% in the BD group and 0% in the B group (p = 0.003). Patient satisfaction scores were higher in the dexamethasone group compared to the other groups. This was significant between Group B and Group BDx (p = 0.009 < 0.05). Conclusions: Adding dexmedetomidine or dexamethasone to bupivacaine in ultrasound-guided TAP blocks reduces postoperative pain and increases patient satisfaction after cesarean sections. Dexamethasone, due to its delayed onset but extended duration, achieves lower pain scores and higher satisfaction. Further research is necessary to confirm these findings. [ABSTRACT FROM AUTHOR]

12. THE EFFECT OF LOW-FLOW VERSUS HIGHFLOW ANESTHESIA ON POSTOPERATIVE COGNITIVE FUNCTIONS IN GERIATRIC PATIENTS UNDERGOING TUR-P SURGERY.

Abstract

Introduction: This paper investigates the effect of low-flow anesthesia applications on postoperative cognitive function in geriatric age group (≥65 years old) patients who underwent elective transurethral resection of the prostate surgery. Materials and Method: A total of 98 patients aged 65 and over who underwent elective transurethral resection of the prostate surgery under general anesthesia between December 2021 and November 2022 in Hatay Mustafa Kemal University Research Hospital’s Department of Anesthesiology and Reanimation were included in the study. The patients were subjected to a mini mental test the day before the operation and postoperatively at six hours, one day, three days, and seven days. Visual analogue scale scores were evaluated at 3, 6, 12, 24, 48, and 72 hours. The data obtained were compared between the patient groups who underwent low-flow (1 L/min, n: 49) and high flow (4 L/min, n: 49) anesthesia. P< 0.05 was considered statistically significant. Results: A comparison between the postoperative 6thhour, 1st day, 2nd day, 3rd day, and 7th day mini mental testing scores of the low-flow anesthesia and high flow anesthesia groups did not exhibit any notable variations (p: 0.668, 0.785, 0.745, 0.705, respectively). The visual analogue scale scores of the cases at 3, 6, 12, 24, 48, and 72 hours did not differ statistically according to the type of flow applied (p: 0.316, 0.925, 0.651, 0.548, 0.624, 0.466, respectively). Conclusion: It is thought that low-flow anesthesia can be applied safely, but it does not have a significant effect on cognitive functions compared to high flow anesthesia. [ABSTRACT FROM AUTHOR]

Abstract

In this study, nutrient contents, nutritive values and rumen ingestibility of Krascheninnikovia ceratoides (L.) Gueldenst, Atriplex canescens (Pursh.) Nutt, Quercus infectoria G. Olivier and Q. robur L. were investigated by laboratory studies and so studies conducted with cannulated rams. The plants within the scope of the study were examined in terms of their raw protein, dry matter, raw ash, organic matter, raw oil, ADF, NDF, ADL, ingestibility in 48 hours, digestible protein, metabolizable energy, relative feed value and their P, K, Ca, Mg, Cu, Mn, Fe and Zn contents. In terms of nutrient content and feed values, a ranking as A. canescens, K. ceratoides, Q. infectoria and Q. robur was determined and it was revealed that these plants could be used in animal feeding., Bu çalışmada; hayvancılıkta, yetersiz mera idaresi sonucu dejenere olmuş veya marjinal alanlarda ilave yem katkısı ile yem desteği sağlayacak Krascheninnikovia ceratoides (L.) Gueldenst, Atriplex canescens (Pursh.) Nutt, Quercus infectoria G. Olivier ve Q. robur L.’un besin madde içerikleri, besin değerleri ve rumen hazmolunabilirlikleri laboratuvar çalışmaları ve kanüllü koçlarda gerçekleştirilen çalışmalar aracılığı ile incelenmiştir. Çalışma kapsamındaki bitkiler ham protein, kuru madde, ham kül, organik madde, ham yağ, ADF, NDF, ADL, selüloz, 48 saatte hazmolunabilirlik, sindirilebilir enerji, metebolik enerji, nispi yem değeri ile fosfor, potasyum, kalsiyum, magnezyum, bakır, mangan, demir ve çinko içerikleri yönünden incelenmiştir. Besin madde içerikler ve yem değerleri açısından A. canescens, K. ceratoides, Q. infectoria ve Q. robur şeklinde bir sıralama belirlenerek bu bitkilerin hayvan beslemede kullanılabileceği ortaya konmuştur.

36. Safety and Complications of Sedation Anesthesia during Pediatric Auditory Brainstem Response Testing.

Abstract

Objective: The auditory brainstem response (ABR) test has been widely used in childhood. Although it is a painless procedure, sedation can be needed in pediatric patients. Thus, this study aimed to evaluate safety and complications of sedation anesthesia applied in pediatric patients during ABR testing. Methods: Medical records of 75 children who underwent ABR testing between 2018 and 2020 were evaluated retrospectively in terms of applicability, safety, and complications of sedation anesthesia. Results: The ages ranged from 3 to 9 (mean 6.2) years. Comorbidity was detected in 20% (n = 15); 3 had multiple comorbidities, and the most common comorbidity was Down syndrome (4%). The drugs used in sedation anesthesia were midazolam in 81.3% (n = 61), a combination of propofol and ketamine in 14.7% (n = 11), and only propofol in 4% (n = 3) of the patients. An additional drug use was needed in 44% (n = 33). The mean procedure time was 40 (range 30–55) min. The mean anesthesia duration was 45 (range 35–60) min. The mean recovery time was 10 (range 5–15) min. Complications related to anesthesia developed in 4 (5.33%) of the patients; respiratory distress, agitation, cough, and nausea-vomiting were seen in one of the patients, respectively. Complications like bradycardia and respiratory or cardiac arrest were not seen at all. Conclusions: The complication rate of sedation anesthesia performed during ABR testing of pediatric patients is quite low. It may be more beneficial to use combinations of sedation drugs instead of using a single sedation drug. Although sedation anesthesia appears to be safe in general, the potentially life-threatening complications of sedative agents should be remembered, especially in children who have comorbidities. Abstract

Inthis study, the chemical contents of different wheat varieties and theirsimilarities/differences were determined by Magnetic Resonance (NMR)spectroscopy method and Cluster Analysis. Bread wheat genotypes, Gerek 79,Carisma, Bayraktar, Artico and Advanced Line were used. It was determined thatthe chemical contents of bread wheat varieties including amino acids and sugarscan be determined safely by NMR spectroscopy method. Carisma and Bayraktargenotypes, closer to the biscuit quality feature, formed the same group.Besides, having bread quality features, Gerek 79, Artico and Advanced Linecreated another group. This reveals that NMR spectroscopy method can be safelyused in bread wheat studies., Buçalışmada Manyetik Rezonans (NMR) spektroskopisi metodu ile faklı buğdayçeşitlerinin kimyasal içerikleri ve bunların benzerlik ve farklılıkları ClusterAnalizi belirlenmeye çalışılmıştır. Abstract

The use of combined peripheral nerve blocks significantly reduces morbidity and mortality in high-risk patients. The widespread use of ultrasound in peripheral nerve blocks provides success with less local anesthetic amounts in patients with multiple blocks. In this case report, we aimed to present the use of a combination of sciatic nerve block, femoral nerve block and fascia iliaca block in a patient with implanted cardioverter defibrillator (ICD) and vascular pathologies who underwent an arthrodesis operation for bimalleolar fracture.

Düzensiz talep altında öngörüde bulunma oldukça zahmetli bir süreç olup, klasik öngörü yöntemlerinden verim almak oldukça zordur. Bu çalışmada incelenen firmanın üretim talep yapısı da düzensiz seyretmektedir ve firma vazgeçilmez süreç geliştirme faaliyetlerini planlayabilmek için iş yükü öngörüsüne ihtiyaç duymaktadır. Öngörülecek iki parametre toplam üretim hacmi ve fason üretim miktarının toplam üretime oranı şeklinde belirlenmiştir. Göreceli daha düzenli bir yapı sergileyen fason üretim oranı öngörüleri durağan öngörü yöntemiyle gerçekleştirilmiştir. Daha kritik olan düzensiz toplam üretim miktarı ise Hızlı Fourier Dönüşüm algoritmasıyla hesaplanmıştır. Toplam üretim parametresi ayrıca tahmin güvenilirliği ölçümü amacıyla bir yıl boyunca gözlenmiştir. Gözlenen Ortalama Mutlak Hata Yüzdeleri %11 (fason üretim oranı) ve %14-%22 (toplam üretim miktarı) olarak hesaplanmış ve düzensiz talep göz önüne alındığında kabul edilebilir olarak değerlendirilmiştir., Forecasting process may become cumbersome under erratic demand structures where no conventional methodology seems to provide plausible outcomes. The company investigated in this paper is subject to this condition and needs to forecast the workload in order to plan the indispensable process enhancement activities. Two parameters to be forecasted are the total production volume and the ratio of in-house production ratio with respect to the total production. The required forecasts for relatively regular structured in-house production to total production ratio were computed via static forecasting. The more important erratic structured total production volume forecasts were computed using FFT (Fast Fourier Transform) algorithm. Total production was further observed during the next year to check the reliability of the forecasts. The resulting respective MAPEs (Mean Absolute Percentage Errors) of approximately 11% (in-house ratio) and 14-22% (total production) were considered acceptable under the erratic demand structure.

Abstract

Abstract

Myastenia Gravis is an autoimmune neuromuscular transmission disorder. The main symptoms of the illness muscle weakness, progressive muscle tiredness, ptosis, disorders. In these patients group, the main problems are determination of appropiate anesthesia medicine doses and shortness of breath in the postoperative period. There were a few reports of spinal anesthesia prefered for the patients with myastenia gravis in the literature. In this article, we present a case of spinal anesthesia was applied for inguinal hernia operation, Myastenia Gravis otoimmün birnöromüsküler iletim bozukluğudur.Hastalığın başlıca semptomları kasgüçsüzlüğü, ilerleyici kas yorgunluğu,pitozis, oftalmopleji ve bulbarbozukluklardır. Bu hasta grubunda anesteziaçısından başlıca problemler uygunanestezik ilaç dozlarının belirlenmesi vepostoperatif süreçte solunum yetmezliğidir.Literatürde myastenia gravisli hastalardaspinal anestezi tercih edildiğine dairyayınlar mevcuttur. The purpose of this study is to determine changes of organic acid levels oxalic acid, propionic acid, tartaric acid, butyric acid, malonic acid, malic acid, lactic acid, citric acid, fumaric acid, maleic acid succinic acid, gibberellic acid, salicylic acid, indol acetic acid, abscisic acid in thirteen bread wheat genotypes BW1: Es-26, BW2: Bezostaja-1, BW3: Müfitbey, BW4: Altay-2000, BW5: Sönmez-01, BW6: Soyer-02, BW7: Çetinel-2000, BW8: Harmankaya-99, BW9: Sultan-95, BW10: Alpu-01, BW11: Atay-85, BW12: Özdemir and BW13: Gerek-79 during growth stages of wheat tillering, flowering, maturity stages and seeds in growing periods of 2012-2013.The highest and the lowest levels of organic acids belonged to BW5 Sönmez-01 and BW7 Çetinel-2000 in all amino acids. Bread wheat genotypes and developmental stages have significant changes in organic acids. Great different levels on organic acids in bread wheat genotypes means that they are acted in different intensity and level under genetic capacity and differences. Differences of organic acid levels in different growth stages shows that genotype x environment interaction monitors organic acid levels and metabolic processes in different development stages, Bu çalışmanın amacı, 13 ekmeklik buğday çeşidinde 2012-2013 yılında BW1: Es-26, BW2: Bezostaja-1, BW3: Müfitbey, BW4: Altay-2000, BW5: Sönmez-01, BW6: Soyer-02, BW7: Çetinel-2000, BW8: Harmankaya-99, BW9: Sultan-95, BW10: Alpu-01, BW11: Atay-85, BW12: Özdemir ve BW13: Gerek-79 değişik gelişme dönemlerinde sapa kalkma, çiçeklenme, olgunluk denemleri ve tohumda amino asit düzeylerinin oksalik asit, propiyodik asit, tartarik asit, butirik asit, malonik asit, malik asit, laktik asit, sitrik asit, fumarik asit, maleik asit süksinik asit, giberellik asit, salisilik asit, indol asetik asit, absisik asit değişimi incelenmiştir. Buğdayın bütün dönemlerinde ve tohumda bütün çeşitlerde organik asit seviyelerinde benzer değişim gözlenmiştir. Organik asitlerde en yüksek ve en düşük seviyeler BW5 Sönmez-01 ve BW7'ye Çetinel-2000 ait olmuştur. Ekmek buğday genotipleri ve gelişim aşamaları, organik asitler açısından önemli değişiklikler göstermiş olup, farklı yoğunluk ve seviyelerdeki organik asitlerin farklı genetik yapı ve kapasiteye sahip genotiplerin etkisi altında olduğu belirlenmiştir. Abstract

OBJECTIVE: Acetaminophen is effective for acute surgical pain, but whether it reduces persistent incision pain remains unknown. We tested the primary hypothesis that patients given perioperative acetaminophen have less incisional pain three months after surgery. Our secondary hypotheses were that patients randomized to acetaminophen have less postoperative pain and analgesic consumption, and better functional recovery at three months.

Abstract

Hassaslık, doğruluk ve çözünürlük, yoğun dalga boyu bölmeli çoğullamalı (YDBÇ) optik iletişim sistemleri ve Fiber Bragg Izgara (FBI) tabanlı optik algılama sistemlerinde yer alan dalga boyu ölçümleri için son derece önemlidir. Bu gibi uygulamalar için bir pasif dalga boyu ölçüm sisteminin temel bileşeni, doğrusal bir kenar filtredir. Pasif doğrusal kenar filtre sistemlerinin avantajı, aktif dalga boyu ölçüm sistemlerine kıyasla düşük maliyetli, imal kolaylığı ve yüksek ölçüm hızlarıdır. Bu tez çalışmasında, FiberCore firmasına ait I-6, I-25, M-5, M-12 Erbiyum katkılı fiberler (EKF) kullanılarak pasif kenar filtre tasarımı gerçekleştirilmiştir. Bu amaçla, öncelikli olarak çeşitli EKF türleri için, sistemin giriş gücü ve EKF uzunlukları değiştirilerek sistem çıkışına etkileri analiz edilmiştir. Bu analizler için sistem, OptiAmplifier 4.0 yazılımında benzetimi yapılarak çalışılmıştır. Benzetim çalışmasında bulunan sonuçlar değerlendirilmiş ve uygun uzunluklarda EKF modelleri kullanılarak deneysel olarak çıktıları karşılaştırılmıştır. Sonuç olarak pasif kenar filtre için hangi model EKF kullanıldığında en uygun boyun hangisi olduğu belirlenmiş, Erbiyum Katkılı Fiber Yükselteç (EKFY) sisteminde yükseltilmiş kendiliğinden yayılma (YKY) da kenar filtre olarak kullanılacak en uygun bölgenin yeri saptanmıştır. Sensitivity, accuracy and resolution are crucial for wavelength measurements in Dense Wavelength Division Multiplexing (DWDM) optical communication systems and optical detection systems based on Fiber Bragg Gratings (FBG). The basic component of a passive wavelength measurement system for such applications is a linear edge filter. The advantage of passive linear edge filter systems is their low cost, ease of manufacture and high measurement speeds compared to active wavelength measurement systems. In this thesis, a passive edge filter design was performed by using a I-6, I-25, M-5, M-12 Erbium doped fibers (EDFs) which are produced by Fibercore Company. For this purpose, the effects on the system output were analyzed by changing the input power of the system and the EDF length in various EDF types. For this analysis, the system was simulated by OptiAmplifier 4.0 software. The simulated values were analyzed and the experimental results were compared using the EDF models in the appropriate lengths. As a result, it is determined the best lenght of EDF type and the most suitable region for amplified spontaneous emission (ASE) in the Erbium Doped Fiber Amplifier (EDFA) system as the edge filter. 85
