Abstract

INTRODUCTION: This study was created to analyze dynamic alterations in coagulation, hematological and biochemical parameters and their association with mortality of COVID-19 patients. To identify the most sensitive biomarkers as predictors of mortality more research is required. METHODS: The present study was a prospective, one-year-long observational study conducted on all critically ill, COVID–19 patients with respiratory failure. The following data were collected: demographic and clinical characteristics of the study population, comorbidities, coagulation, biochemical and hematological parameters. The primary outcome was the proportion of patients who died. RESULTS: 91 patients with median age 60 (50–67), 76.9% male, met the acute respiratory distress syndrome criteria. It was tested whether dynamic change (delta-Δ) of parameters that were found to be predictors of mortality is independently associated with poor outcome. Adjusted (multivariate) analysis was used, where tested parameters were corrected for basic and clinical patients characteristics. The only inflammatory parameter which dynamic change had statistically significant odds ratio was ΔCRP (p < 0.005), while among coagulation parameters statistically significant OR was found for Δ fibrinogen (p < 0.005) in predicting mortality. CONCLUSION: Monitoring of coagulation, hematological and biochemical parameters abnormalities and their dynamical changes can potentially improve management and predict mortality in critically ill COVID –19 patients. Abstract

RESUMO Apresentamos o caso de uma paciente de 71 anos de idade admitida à unidade de terapia intensiva clínica em condição de falência de múltiplos órgãos. Após o quarto dia, com aplicação de todas as modalidades de suporte à vida (estímulo com vasopressores, ventilação mecânica, terapia com diálise contínua, terapia com antibióticos de amplo espectro e outras medidas de suporte), dados inespecíficos fornecidos por acompanhante revelaram que a paciente vinha apresentando dificuldade persistente para deglutir líquidos e alimentos sólidos por alguns dias antes da admissão ao hospital. Após a realização de procedimentos adicionais de diagnóstico radiológico e endoscópico, detectou-se a presença de um corpo estranho no esôfago: um fragmento de fio de aço com cerca de 6 cm de comprimento e dobrado ao meio, que havia penetrado a parede do esôfago e se projetava ao nível da sétima vértebra cervical. Conseguimos remover o corpo estranho por via endoscópica sem maiores complicações e, a seguir, estabilizar nossa paciente, com utilização de medidas terapêuticas adicionais, conforme necessário. ABSTRACT We present the case of a 71-year-old patient who was admitted to the medical intensive care unit in a state of multiple organ dysfunction. After the fourth day of applying all needed life-saving measures (vasopressor stimulation, mechanical ventilation, continuous dialysis treatment, broad spectrum antibiotic therapy, and other supportive measures), nonspecific heteroanamnestic data revealed that the patient had been having a persistent difficulty in swallowing liquids and food for a few days prior to hospital admission. After performing additional radiological and endoscopic diagnostic procedures, a foreign body was detected; a steel wire that had a length of approximately 6cm and was bent in a half had penetrated the esophagus and was projected into the seventh neckline. We managed to evacuate the foreign body endoscopically without further complications, and we stabilized our patient using additional therapeutic measures as needed.

Abstract

Background: This study aimed to demonstrate that having clinical pharmacist as a member of oncology team in low and middle income countries might lead to significant reduction in the number of erlotinib interactions in the treatment of non-small cell lung cancer patients. Methods: A group of 44 patients was labeled as intervention group and they were analyzed prospectively in the period from 1 January 2017 to 1 May 2018 during clinical pharmacist's participation in regular weekly multidisciplinary oncology team meetings. The control group consisted of 44 out of 110 patients treated with erlotinib before the involvement of a clinical pharmacist in oncology team, match paired with 44 patients in intervention group. Results: Clinically significant interactions were identified in two-thirds of studied patients (57 out of 88). Most drug interactions, 38%, potentially result in decrease of serum concentration of erlotinib. Clinical pharmacist provided therapy modification suggestions for 32 out of 44 (72.72%) patients in the intervention group, most of which were accepted by doctors. In the intervention group, there were significantly less clinically significant interactions compared to the control group (10 versus 24, p = 0.002). Progression-free survival was significantly longer in the pharmacist's intervention group (p = 0.001). Conclusions: Clinical pharmacist's intervention led to significant decrease in erlotinib interactions which may result in treatment optimization of lung cancer patients. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: There is a bulk of literature data on the physiological roles of nitric oxide (NO) and endothelin-1 (ET-1), but many facts remain unknown, especially in certain diseases such as peripheral arterial disease. Material and methods: This is an observational cross-sectional study. The subjects were patients diagnosed with PAD. Serum levels of NO and ET-1 were determined for all patients, and statistical data processing was performed according to the set goals. Results: The study included 64 patients with mean age 60.2 ± 12.7 years, mostly in stage II PAD according to Fontain (46.9%). Statistical analysis failed to determine a significant difference in serum NO or ET-1 values with respect to disease stage, sex, and body mass index (BMI). Certain oscillations were found in the mean values of NO related to smoking and diabetes but without statistical significance. There were also oscillations in the values of ET-1, with higher levels found in women, smokers and non-diabetics in whom this difference reached statistical significance (p = 0.041). Conclusion: Serum levels of NO and ET-1 in this study show some causal relationship with certain risk factors for PAD such as diabetes and smoking, but additional research is needed to fully understand their effects and interactions. Abstract

We investigated activities of glutathione peroxidase (GSH-Px), glutathione reductase (GR), and the phase II biotransfomation enzyme glutathione-S-transferase (GST) in the foot of three freshwater mussel species: Unio pictorum (Up), Unio tumidus (Ut), and Sinanodonta woodiana (Sw) from the Sava River. Specific and total GSH-Px activity was lower in Sw than in Up and Ut. Total GR activity was higher in Up than in Sw. Specific GST activity was higher in Up than in Ut. Total GST activity was higher in Up than in Ut and Sw. Electrophoretic analysis of proteins shows species specifities between the investigated mussel species. Our study represents the first comprehensive report of the investigated glutathione-dependent enzyme activities in the foot of three freshwater mussel species from the Sava River, Serbia.

Abstract

Critically ill COVID-19 patients are usually subjected to clinical, laboratory, and radiological diagnostic procedures resulting in numerous findings. Utilizing these findings as indicators for disease progression or outcome prediction is particularly intriguing. Exploring the significance of dynamic changes in haematological and biochemical parameters in predicting the mortality of critically ill COVID-19 patients. The present study was a prospective and observational study involving mechanically ventilated 75 critically ill adult COVID-19 patients with hypoxemic respiratory failure. The collected data included baseline patient characteristics, treatment options, outcome, and laboratory findings at admission and 7 days after. The dynamics of the obtained findings were compared between survivors and non-survivors. The 28-day survival rate was 61.3%. In the group of non-survivors significant dynamic changes were found for C-reactive protein (p= 0.001), interleukin-6 (p< 0.001), lymphocyte (p= 0.003), neutrophil-lymphocyte ratio (p= 0.003), platelets (p< 0.001), haemoglobin (p< 0.001), iron (p= 0.012), and total iron-binding capacity (p< 0.001). Statistically significant changes over time were found for ferritin (p= 0.010), D-dimer (p< 0.001), hs-troponin T (p< 0.002), lactate dehydrogenase (p= 0.001), glucose (p= 0.023), unsaturated iron-binding capacity (p= 0.008), and vitamin D (p< 0.001). Abstract

We examined the concentrations of vitamin E (vit E), sulphydryl groups (-SH), total protein and protein electrophoretic profiles in the hepatopancreas, the gills and muscle of three freshwater crayfish species: Noble crayfish (Astacus astacus), stone crayfish (Austropotamobius torrentium) and spiny cheek crayfish (Orconectes limosus). Vit E concentration in the hepatopancreas of O. limosuswas lower compared to A. astacus, while in the gills of O. limosusit was lower compared to both A. astacus and A. torrentium. The concentration of -SH groups was lower in the hepatopancreas of A. astacuscompared to A. torrentiumand O. limosus. In the gills of A. astacusand A. torrentiumthe concentration of -SH groups was higher compared to O. limosus. Protein concentration was higher in the hepatopancreas of A. torrentiumcompared to A. astacusand O. limosus. A lower protein concentration in muscle of A. torrentiumwas found compared to O. limosusand A. astacus. Electrophoretic analysis of proteins indicated species and tissue specifities between investigated crayfish species. Our results represent the first study of its kind and provide the basis for future studies that will consider our reported parameters as potential biomarkers for biomonitoring of basic environmental conditions and some anthropogenic impacts.

Abstract

Background and Purpose: In order to assess exposure to effects of environmental pollutants on aquatic oiganisms, several different biochemical and physiological parameters were studied. The activity of antioxidant defense enzymes was measured in the Mediterranean Sea shrimp (Parapenaeus longirostris) and compared with the presence of polychlorinated biphenyls (PCBs) and polycyclic aromatic hydrocarbons (PAHs) in their sea water environment. Materials and Methods: The activity of the antioxidant defence enzymes superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and glutathione reductase (GR), as well as glutathione-S transferase (GST) as a phase II detoxification enzyme, was measured in the abdominal muscle of the shrimp in winter and spring at four different locations in the Montenegrine costal region of the Adriatic Sea. The results were correlated to the presence of PCBs and PAHs in the sea water at the time of sampling. Results and Conclusions: There was no obvious direct correlation between antioxidant defense activity and the presence and concentration of PCBs and PAHs in the sea water: The activities of SOD, GSH-Px and GR showed seasonal changes and therefore seasonality should be incorporated into the interpretation of antioxidant changes in future biomonitoring studies. Generally, SOD and GR activities were lower and GSH-Px was higher in winter when compared to spring. However these effects were not uniform and statistical trends were influenced by local environment. 50. Antioxidant defense enzyme activities in the liver of red mullet (Mullus barbatus L.) from the Adriatic Sea: The effects of locality and season

Abstract

The effects of locality and season on the activity of antioxidant defense enzymes (ADE), namely, total superoxide dismutase (Tot SOD), copper zinc containing superoxide dismutase (CuZn SOD), manganese containing superoxide dismutase (Mn SOD), catalase (CAT), glutathione peroxidase (GSH-Px), glutathione reductase (GR) and biotransformation phase II enzyme glutathione-S-transferase (GST) in the liver of red mullet (Mullus barbatus L.) from the Platamuni and Valdanos localities (Adriatic Sea) in winter and spring season were studied. All enzyme activities were expressed as specific (units/mg proteins) and total (units/g wet tissue) ones. Our results show increased specific and total activities of almost all investigated ADE in winter, with respect to the spring, both in Platamuni and Valdanos. The opposite effect was obtained for the specific and total GSH-Px activity at both localities. The presented results show that the activity of investigated ADE depends mainly on the analyzed season. Also, it was confirmed that biotransformation phase II enzyme GST is the most sensitive indicator of environmental status in the liver of red mullet. The results of Principal Component Analysis (PCA) based on correlations show similar trends in the positive and negative correlations between specific and total ADE in both seasons. At the same time, examination of seasonal patterns of ADE revealed clear differences between the investigated localities in winter and spring for specific and total activities. From the presented results, it can be concluded that seasonal variations of ADE should be incorporated into interpretation of further biomonitoring studies of red mullet. null
