- Abstract
In this study, a cost-effective sandwich ELISA test, based on polyclonal antibodies, for routine quantification SARS-CoV-2 nucleocapsid (N) protein was developed. The recombinant N protein was produced and used for the production of mice and rabbit antisera. Polyclonal N protein-specific antibodies served as capture and detection antibodies. The prototype ELISA has LOD 0.93 ng/mL and LOQ 5.3 ng/mL, with a linear range of 1.52–48.83 ng/mL. N protein heat pretreatment (56 °C, 1 h) decreased, while pretreatment with 1% Triton X-100 increased analytical ELISA sensitivity. The diagnostic specificity of ELISA was 100% (95% CI, 91.19–100.00%) and sensitivity was 52.94% (95% CI, 35.13–70.22%) compared to rtRT-PCR (Ct < 40). Profoundly higher sensitivity was obtained using patient samples mostly containing Wuhan-similar variants (Wuhan, alpha, and delta), 62.50% (95% CI, 40.59 to 81.20%), in comparison to samples mostly containing Wuhan-distant variants (Omicron) 30.00% (6.67–65.25%). The developed product has relatively high diagnostic sensitivity in relation to its analytical sensitivity due to the usage of polyclonal antibodies from two species, providing a wide repertoire of antibodies against multiple N protein epitopes. Moreover, the fast, simple, and inexpensive production of polyclonal antibodies, as the most expensive assay components, would result in affordable antigen tests.

- Abstract
S1.1. Checking of N protein purity Recombinant N protein purity was checked after imidazole removal and buffer exchange by SDS PAGE (Figure 6.). For comparison, commercial high-purity HSA was also analyzed. S1.2. Identification of N protein Tandem mass spectrometry identification of proteins in an in-gel digested band of N protein (Figure S1, lane 3), confirmed the identity of N protein with high scores and peptide coverage (Fig. S2.). S2. Purification of polyclonal antibodies from mice and rabbit sera For the development of an ELISA test specific for the detection of SARS-CoV-2 N protein, recombinantly produced N protein was used for the immunization of mice and rabbits. Sera obtained from rabbits and mice were then tested for titer and specificity (Figure S3 and Figure 1). To determine the titer of polyclonal sera required to detect N protein in samples, we use wells coated with N protein and serial dilution of sera pools from different animals. After multiple washing steps, we detected the binding of rabbit and mice antibodies using secondary biotinylated antibodies and streptavidin-alkaline phosphatase chimaera or secondary antibodies with previously coupled alkaline phosphatase, where the amount of enzymes’ substrate converted to the product was measured as an increase in absorbance at 405 nm. As shown in Figure S3A, unpurified sera pools from both animals showed very high titers and expected logarithmic decrease of signal with dilution. Based on the obtained data titer for unpurified sera was determined to be X. The same trend was observed for pools purified using AS precipitation and rabbit sera purified using protein A affinity chromatography (Figure S3B and S3C). As shown in Figure S3D, clear bands from antibodies could be observed in both full and purified samples. 6. Modulation of T-Cell-Dependent Humoral Immune Response to Influenza Vaccine by Multiple Antioxidant/Immunomodulatory Micronutrient Supplementation.

Notwithstanding prevalence gaps in micronutrients supporting immune functions, the significance of their deficits/supplementation for the efficacy of vaccines is underinvestigated. Thus, the influence of supplementation combining vitamins C and D, zinc, selenium, manganese, and N-acetyl cysteine on immune correlates/surrogates of protection conferred by a quadrivalent influenza vaccine (QIV) in mice was investigated. The supplementation starting 5 days before the first of two QIV injections given 28 days apart increased the serum titres of total and neutralizing IgG against each of four influenza strains from QIV. Accordingly, the frequencies of germinal center B cells, follicular CD4+ T helper (Th) cells, and IL-21-producing Th cells increased in secondary lymphoid organs (SLOs). Additionally, the supplementation improved already increased IgG response to the second QIV injection by augmenting not only neutralizing antibody production, but also IgG2a response, which is important for virus clearance, through favoring Th1 differentiation as indicated by Th1 (IFN-γ)/Th2 (IL-4) signature cytokine level ratio upon QIV restimulation in SLO cell cultures. This most likely partly reflected antioxidant action of the supplement as indicated by splenic redox status analyses. Thus, the study provides a solid scientific background for further research aimed at repurposing the use of this safe and inexpensive micronutrient combination to improve response to the influenza vaccine. Tema ovog diplomskog rada je izrada aplikacije za Nacionalni park sa interaktivnom kartom. U teoretskom dijelu rada obradit će se osnove izrade aplikacije odnosno predprodukcija i produkcija te istraživanje tržišta postojećih aplikacija na tu temu. Proći će se kroz postupak određivanja komponenti aplikacije, fontova te palete boja i samog stila aplikacije. U praktičnom će dijelu rada biti izrađen dizajn sa postojećim idejnim rješenjima logotipa Nacionalnog parka te će biti ukomponiran u vlastiti dizajn aplikaciije. Po završetku izrade prototipa u alatu Figma bit će provedeno testiranje aplikacije na određenoj skupini ljudi koji će potom popuniti popratnu anketu kako bi se dobile povratne informacije o funkcionalnosti navigacije te preglednosti same aplikacije. Aplikacija će se bazirati na interakrivnoj karti koja će prikazivati atrakcije koje se nalaze u parku, mogućnost odabira lokacija te stvaranje željene rute obilaska parka. Na karti će također biti označena biljna i životinjska staništa te osnovne informacije o istima. Isto tako će biti prikazane lokacije za odmor, piknik, toaleti, štandovi sa autohtinim domaćim proizvodima, restorani, kafići, ulazi u park kao i prodajna mjesta ulaznica. Na karti će biti vidljiva mjesta polaska turističkih brodova i autobusa te vozni red u parku. Aplikacija će sadržavati povijest parka, kontakt te osnovne inforamcije koje su potrebne za obilazak parka.

Folklor i glazba koja se veže uz njega sastavni su dio tradicije koja je važna u stvaranju nacionalnog identiteta jednog naroda. Da se ona ne bi izgubila, bitno je istraživati i čuvati tradicijsku građu. Upravo tome je i posvećen ovaj završni rad. Sam naziv rada govori da će se on usmjeriti na folklor i glazbu dvaju mjesta – Gradišta, odakle je i autorica rada, i Županje. Kroz osam poglavlja, prikazana su najvažnija obilježja dvaju mjesta. Na početku je objašnjen sam pojam folklora, nakon čega slijedi prikaz njegovih segmenata: glazbe, tradicijske odjeće, bećarcaca.. Predstavljena su i mjesta o kojima se radi te skupine i udruge koje se bave folklornim aktivnostima. Folklore and the music associated with it are part of a tradition that is very important for creating national identity of Croatian people. In order for it not to be lost, it is important to research and preserve traditional structure. This final paper is dedicater exactly to that. The very title of the work says that it is directed to folklore and the music of two places – Gradište, from wich the author comes, and Županja. Through the eight chapters, the most important folklore characteristics of the two places are shown. At the very beginning, the concept of the folklore is explained, followed by a presentation of its segments: music, traditional clothing, „bećarci“.. The mentioned places are also presented, as well as groups engaged in folklore activities.

Electromagnetic radiation of all frequencies represents one of the most common and fastest growing environmental influence. All populations are now exposed to varying degrees of electromagnetic radiation and the levels will continue to increase as technology advances. An electronic or electrical product should not generate electromagnetic radiation which may impact the environment. In addition, electromagnetic radiation measurement results need to be accompanied by quantitative statements about their accuracy. This is particularly important when decisions about product specifications are taken. This paper presents an uncertainty budget for disturbance power measurements of the equipment as part of electromagnetic radiation. We propose a model which uses a mixed distribution for uncertainty evaluation. The evaluation of the probability density function for the measurand has been done using the Monte Carlo method and a modified least-squares method (combined method). 22. Transient Pseudohypoaldosteronism Secondary to Urinary Tract Infection in a Male Infant with Unilateral Hydronephrosis Due to Primary Obstructive Megaureter: A Case Report

- Abstract
We present a case of transient form of type 1 pseudohypoaldosteronism (S-PHA) in a 1.5-month-old male infant who presented with lethargy, failure to thrive, severe hyponatremia (Na=118 mmol/L), hypochloremia (Cl=93 mmol/L) and fever due to urinary tract infection. Potassium levels were normal. Markedly elevated serum aldosterone level and elevated serum renin confirmed the diagnosis of pseudohypoaldosteronism. Renal ultrasound showed grade III hydronephrosis on the left kidney while contrast-enhanced voiding urosonography excluded the existence of vesicoureteral reflux, which raised suspicion of obstructive uropathy at the level of vesicoureteral junction. Serum sodium normalized after several days of intravenous fluids and antibiotic therapy, after which oral supplementation of sodium was introduced. The levels of 17-hydroxyprogesterone, adrenocorticotropic hormone, cortisol and thyroid-stimulating hormone were normal. Functional magnetic resonance urography conducted at the age of 3 months confirmed the diagnosis of primary congenital obstructive megaureter and the infant was referred to a pediatric surgeon. Although a rare occurrence, S-PHA can be a potentially life-threatening condition in infants if not recognized and treated appropriately. Therefore, serum concentrations of electrolytes should be obtained in every child diagnosed with obstructive anomaly of the urinary tract and/or acute cystopyelonephritis. On the other hand, every child diagnosed with S-PHA should be evaluated for obstructive anomaly of the urinary tract., Prikazujemo slučaj tranzitornog oblika pseudohipoaldosteronizma tip 1 u muškog dojenčeta u dobi od dobi mjesec i pol koje se prezentiralo letargijom, nenapredovanjem na tjelesnoj masi, teškom hiponatremijom (Na=118 mmol/L), hipokloremijom (Cl=93 mmol/L) te febrilitetom u sklopu akutne infekcije mokraćnog sustava. Serumska razina kalija je bila uredna. Značajno povišene vrijednosti aldosterona i renina u plazmi potvrdile su dijagnozu pseudohipoaldosteronizma. Ultrazvukom mokraćnog trakta utvrđena je hidronefroza III. stupnja lijevo, dok je mikcijskom urosonografijom pojačanom kontrastom isključena prisutnost vezikoureteralnog refluksa te je postavljena sumnja na opstrukciju na razini vezikoureteralnog spoja. Serumska razina natrija se normalizirala nakon nekoliko dana parenteralne rehidracije i antibiotske terapije, nakon čega je uvedena oralna suplementacija natrija. Razina 17-hidroksiprogesterona, ACTH, kortizola i TSH su bile uredne. U dobi od 3 mjeseca učinjena je funkcionalna MR urografija koja je potvrdila dijagnozu primarnog kongenitalnog opstruktivnog megauretera te je dojenče upućeno dječjem kirurgu. Iako rijedak, pseudohipoaldosteronizam je potencijalno životno ugrožavajuće stanje ako nije na vrijeme prepoznato i primjereno zbrinuto. Sukladno tome, serumske koncentracije elektrolita treba provjeriti kod svakog djeteta s poznatom opstruktivnom anomalijom mokraćnog sustava i/ili cistopijeloneferitisom. S druge strane, kod svakog djeteta s pseudohipoaldosteronizmom treba isključiti anomaliju mokraćnog sustava.

Evaluation of probability density function for the measurand has been done using Monte Carlo method and a modified least-squares method (combined method). We present a case of transient form of type 1 pseudohypoaldosteronism (S-PHA) in a 1, 5-month- old male infant who presented with lethargy, failure to thrive, severe hyponatremia (Na=118 mmol/L), hypochloremia (Cl=93 mmol/L) and fever due to urinary tract infection. Potassium levels were normal. Markedly elevated serum aldosterone level and elevated serum renin confirmed the diagnosis of pseudohypoaldosteronism. Renal ultrasound showed grade III hydronephrosis on the left kidney while contrast-enhanced voiding urosonography excluded the existence of vesicoureteral reflux which raised suspicion of opstructive uropathy on the level of vesicoureteral junction. Serum sodium normalized after several days of intravenous fluids and antibiotic therapy after which oral supplementation of sodium was introduced. Levels of 17-hydroxyprogesterone, ACTH, cortisole and TSH were normal. Functional MR urography conducted at the age of 3 months confirmed the diagnosis of primary congenital obstructive megaureter and the infant was referred to a pediatric surgeon. Although a rare occurence, S-PHA can be a potentially life threatening condition in infants if not recognized and treated adequately. Therefore, serum concentrations of electrolytes should be obtained in every child with diagnosed obstructive anomaly of the urinary tract and/or acute cystopyelonephritis. On the other hand, every child with diagnosed S-PHA should be evaluated for obstructive anomaly of the urinary tract.

Cilj rada je analizirati izvore financiranja privatnog sektora kulturnih i kreativnih industrija u Republici Hrvatskoj. U teorijskom dijelu dan je pregled mogućih izvora financiranja privatnog sektora kulturnih i kreativnih industrija, u obliku kredita, programa, natječaja, sponzorstva i crowdfunding kampanja te je analizirano stanje za vrijeme pandemije COVID-19. Pritom su postavljena dva istraživačka pitanja: kako je pandemija uzrokovana virusom COVID-19 utjecala na poslovanje gospodarskih subjekata u kulturnim i kreativnim industrijama te koliki je udio samofinanciranja u poslovanju poslovnih subjekata kulturnih i kreativnih industrija od 2016. do 2020. godine. U okviru empirijskog dijela rada pomoću financijske analize provedena je analiza uspješnosti poslovanja poslovnih subjekata privatnog sektora kulturnih i kreativnih industrija te su prikazani rezultati. Zaključak istraživanja je kako je pandemija imala negativan utjecaj na poslovanje tvrtki te kako su se u najvećoj mjeri financirali iz rezervi i vlastitih sredstava. Korištene metode u radu su: deskriptivna metoda, metoda komparacije, induktivna metoda, metoda analize i sinteze, metoda financijske analize i statistička metoda The purpose of this paper is to analyse the sources of funding for the private sector of cultural and creative industries in Croatia. The theoretical part provides an overview of possible sources of funding for the private sector of cultural and creative industries, in the form of loans, programs, tenders, sponsorships and crowdfunding campaigns, and analyzes the situation during the COVID-19 pandemic. Two research questions were set: how the pandemic caused by the COVID-19 virus affected the business of economic entities in the cultural and creative industries, and what is the share of self-financing of business entities in the cultural and creative industries from 2016. to 2020.. Within the empirical part of the paper, the analysis of business performance of business entities of the private sector of cultural and creative industries was performed using financial analysis, and the results are presented. The conclusion of the research is that pandemic had a negative impact on business in cultural and creative industries and that they have been financed by their own reserves and self-financing. The methods used in the paper are: descriptive method, comparison method, inductive method, analysis and synthesis method, financial analysis method and statistical method.

ECMO (Extra Corporeal Membrane Oxygenation - izvantjelesna membranskaoksigenacija) je metoda liječenja koja omogućava privremenu mehaničku cirkulacijsku potporubolesnicima s respiracijskom i/ili srčanom insuficijencijom. Implantacija ECMO-a je indicirana kod bolesnika kod kojih su iscrpljene druge mogućnosti liječenja. Može služiti kao most premaoporavku, drugoj mehaničkoj potpori ili transplantaciji.Indikacije za liječenje ECMO potporom imaju bolesnici s navedenom patologijom kod kojih je stanje reverzibilno i očekuje se poboljšanje u funkciji i radu vitalnih organa koji su ugroženi. Osnovni cilj liječenja ECMO potporom je održavanje srčane i plućne funkcije kako bi se održala hemodinamska stabilnost bolesnika i osigurala zadovoljavajuća dostava kisika svim tkivima i organima.Uloga medicinske sestre u skrbi za bolesnika na ECMo potpori je ključna jer je medicinska sestra u jedinici intenzivnog liječenja 24 sata uz bolesnika te uz standardne sestrinske postupke primjenjuje i sva znanja u svrhu otkrivanja i spriječavanja nastalih komplikacija od strane bolesnika i ECMO sustava tijekom liječenja. Izuzetno je važna kontinuirana edukacija medicinskih sestarau jedinici intenzivnog liječenja i obnavljanje usvojenih znanja kako bi bile u mogućnosti pratit napredak tehnologije i metoda liječenja u svrhu pružanja što kvalitetnije skrbi bolesniku. U ovom diplomskom radu dati će se uvod u metode i oblike ECMO liječenja te će se opisati komplikacije koje mogu nastati tijekom ecmo liječenja od strane bolesnika i komplikacije od strane ecmo sustava. Posljednje poglavlje biti će posvećeno ulozi medicinske sestre u skrbi za bolesnika na ECMO potpori. Za kraj rada biti će dat prikaz slučaja koji je uspješno liječen ECMO potporom., ECMO (Extra Corporeal Membrane Oxygenation) is a treatment method that provides temporary mechanical circulatory support to patients with respiratory and / or heart failure. ECMO implantation is indicated in patients in who mother treatment options have been exhausted. It can serve as a bridge to recovery, other mechanical support, or transplantation. Indications for ECMO-supported treatment are in patients with this pathology in whom the condition is reversible an dimprovement in the function and function of vital organs that are at risk is expected. The primary goal of ECMO support treatment is to maintain cardiac and pulmonary function to maintain the hemodynamic stability of patients and to ensure satisfactory oxygen delivery to all tissues and organs. The role of the nurse inpatient care on ECMO support is crucial because the nurse is in the intensive care unit 24 hours a day with the patient and with standard nursing procedures applies all knowledge to detect and prevent complications by patients and ECMO system during treatment. It is extremely important to continuously educate nurses in the intensive care unit and renew the a cquired knowledge in order to beable to monitor the progress of technology and treatment methods in order to provide the best possible care to the patient. This thesis will provide an introduction to the methods and forms of ECMO treatment and will describe the complications that may occur during ecmo treatment by patients and complications by the ecmo system. The last chapter will be devoted to the role of the nurse in patient care on ECMO support. At the end of the paper, a case report that has been successfully treated with ECMO support will be presented.

Cilj istraživanja bio je ispitati odnos dimenzija privrženosti (izbjegavajuća i anksiozna), dimenzija ljubomore (kognitivna, ponašajna i emocionalna), interpersonalnog nadzora na društvenim mrežama i zadovoljstva romantičnim odnosom. Također se željela ispitati mogućnost predviđanja zadovoljstva vezom na temelju dimenzija privrženosti, dimenzija ljubomore te interpersonalnog nadzora. U istraživanju je sudjelovalo ukupno N = 387 sudionika (158 žena i 229 muškaraca) u dobi od 18 do 30 godina koji su trenutno u romantičnom odnosu. Sudionici su ispunjavali Sociodemografski upitnik konstruiran za potrebe ovoga istraživanja, Inventar iskustava u bliskim vezama – skraćena verzija (Fraley, Waller i Brennan, 2000; Kamenov i Jelić, 2003), Skalu interpersonalnog elektronskog nadzora (Tokunaga, 2011), Multidimenzionalnu skalu ljubomore – kratka forma (Elphinston, Feeney i Noller, 2011) te Upitnik procjene zadovoljstva vezom (Hendrick, 1988). Rezultati korelacijske analize pokazali su da postoji značajna povezanost između zadovoljstva vezom i obje dimenzije nesigurne privrženosti te kognitivne i ponašajne ljubomore. Emocionalna ljubomora i zadovoljstvo odnosom su u pozitivnoj korelaciji. Utvrđena je pozitivna korelacija između interpersonalnog nadzora i svih dimenzija ljubomore te negativna između nadzora i zadovoljstva odnosom. Provedena hijerarhijska regresijska analiza pokazala je da su dimenzija izbjegavajuće privrženosti te kognitivna i ponašajna dimenzija ljubomore značajni negativni prediktori zadovoljstva odnosom. Konačno, opisani su dobiveni nalazi, ograničenja istraživanja te prijedlozi za buduća istraživanja. The aim of this study was to examine relations between attachment dimensions (avoidance and anxiety), jealousy dimensions (cognitive, emotional and behavioural), interpersonal surveillance and relationship satisfaction. We also wanted to investigate the possibility of predicting relationship satisfaction based on attachment dimensions, jealousy dimensions and interpersonal surveillance. A total of N = 387 participants (158 female, 229 male) between ages of 19 to 30 who are in a romantic relationship completed Sociodemographic questionnaire that was designed for the purpose of this research, Experiences in Close Relationships Scale – Revised (Fraley, Waller and Brennan, 2000; Kamenov and Jelić, 2003), Interpersonal Electronic Surveillance Scale (Tokunaga, 2011), The Short Form of the Multidimensional Jealousy Scale (Elphinston, Feeney and Noller, 2011) and Relationship Assessment Scale (Hendrick, 1988). Correlation analysis was conducted to establish relationship between relationship satisfaction and both (avoidance and anxiety) attachment dimensions, jealousy dimensions and interpersonal surveillance. The results showed that relationship satisfaction was negatively related to insecure attachment, cognitive and behavioural jealousy and interpersonal surveillance. The emotional jealousy was positively related to relationship satisfaction. Also, all three jealousy dimensions were positively correlated to interpersonal dimension. Furthermore, regression analysis has shown that avoidance attachment dimension, cognitive and behavioural jealousy negatively predicted relationship satisfaction. Finally, findings of this research are further explained, as well as limitations and recommendations for future studies.

Predstavljamo slučaj tranzitornog oblika pseudohipoaldosteronizma tipa1 u muškog dojenčeta dobi mjesec i pol koje se prezentiralo letargijom, nenapredovanjem na tjelesnoj masi te teškom hiponatremijom (Na=118 mmol/L), hipokloremijom (Cl=93 mmol/L) i vrućicom u sklopu urinarne infekcije. Razina kalija je bila uredna. Značajno povišena koncentracija aldosterona i povišena aktivnost serumskog renina potvrdili su dijagnozu pseudohipoaldosteronizma. Ultrazvuk mokraćnog sustava utvrdio je hidronefrozu III stupnja lijevog bubrega dok je kontrastom pojačana mikcijska urosonografija isključila postojanje vezikoureteralnog refluksa te postavila sumnju na opstruktivnu uropatiju na razini vezikoureteralnog ušća. Razina serumskog natrija normalizirala se nakon nekoliko dana parenteralne rehidracije i antibiotske terapije nakon čega je uvedena peroralna suplementacija natrija. Koncentracije 17-hidroksiprogesterona, ACTH, kortizola i TSH su bile uredne. Iako rijedak, S- PHA može biti potencijalno životno ugrožavajuće stanje ukoliko se ne prepozna na vrijeme i ne liječi adekvatno. Sukladno tome, serumska koncentracije elektrolita trebaju se provjeriti kod svakog djeteta s opstruktivnom anomalijom urinarnog sustava i/ili cistopijelonefritisom. S druge strane, svako dijete s dijagnosticiranim S- PHA treba obraditi u svrhu utvrđivanja podležeće anomalije mokraćnog sustava.

35. A rare case of polycystic kidney disease and multicystic dysplastic kidney in a pediatric patient

INTRODUCTION: Cystic kidney disease (CyKD) is one of the most important causes of chronic kidney disease (CKD) in children. Multiple kidney cysts can occur unilaterally (e.g. multicystic dysplastic kidney- MCDK) or bilaterally (e.g. autosomal dominant or autosomal recessive polycystic kidney disease) due to genetic or non-genetic (developmental or acquired) disorders. Here we present a rare case of combined polycystic kidney disease (PCKD) and MCDK in a pediatric patient. CASE REPORT: The patient was admitted to our hospital for the first time at the age of two. She was normally developed (weight: 10.8 kg – 19th percentile, height: 90 cm- 80th percentile) with blood pressure (BP) within normal limits. Estimated glomerular filtration rate (eGFR) at that time was 70.2 mL/min with elevated albumin/creatinine ratio 5, 2 mg/mmol and she was diagnosed with G2A2 stage of CKD. During her prenatal period oligohydramnios and polycystic kidneys were detected. At birth a palpable mass of the left abdomen was found. First sonography after birth showed hydronephrosis of the right kidney and multiple cysts on the left kidney. Micturating cystourethrography was negative for vesicoureteral reflux. Initial magnetic resonance imaging (MRI) showed enlargement of both kidneys with multiple cysts of various sizes on the left kidney and lesser number of diffuse renal cysts on the right kidney. Cysts weren’t found neither on the brain nor on the other abdominal organs. Echocardiography was normal. Family history was negative for kidney diseases. The abdominal and kidney ultrasound of both parents and older brother was normal. At the age of two dynamic scintigraphy with Tc-99m revealed unfunctional left kidney leading to diagnosis of MCDK. Follow-up ultrasound showed progressive involution of the MCDK left and multiple very small cysts that generate abnormal parenchymal echogenicity (e.g., „salt-and-pepper sign”) on the right kidney. Functional MRI urography showed nonfunctional MCDK left and functional right kidney with lesser number of small cysts. At the age of four arterial hypertension (130/70 mmHg) was diagnosed and ACE inhibitor was introduced. There were no signs of liver disease. Abdominal ultrasound revealed no cysts of liver, spleen or pancreas. Gynecological ultrasound and ophtalmological examination were also normal. Due to parental disapproval, genetic testing wasn’t performed. CONCLUSION We conclude that, although rare, different types of CyKD can be associated and we should consider it when setting the diagnosis. Due to vast differential diagnosis and overlapping clinical presentations of CyKD genetic testing should be performed whenever possible.

Introduction: Crohn’s disease (CD) is a chronic inflammatory bowel disease that affects the entire digestive tube. Orofacial granulomatosis describes changes in the oral cavity without the involvement of the gastrointestinal tract, although some patients may have active subclinical disease or potential for developing CD. In patients with diagnosed CD changes of the oral cavity are called oral Crohn’s disease, and they can occur as side effects of pharmacological therapy, malabsorption, or nutritional deficits. Treatment is complex and involves treatment of primary intestinal disease combined with dental treatment. Methods: In this report, we describe the case of a girl with CD who developed severe chronic lesions in the oral cavity. Results: A 17-year- old girl suffers from an extensive form of CD which initially presented as pancolitis with sideropenic anemia. Over the years she had frequent relapses and pharmacotherapy was often modified. Clinical course seemed favorable, but typical changes persisted in the ileocaecal region (intersphinteric fistula). At the age of 16, she developed severe gingivitis and periodontitis with ulcer-like lesions. She was initially treated by periodontologist, but progressive changes aroused suspicion for oral Crohn’s disease. Antibiotic therapy showed partial effect and significant improvement was recorded only during short periods of corticosteroid therapy (local or systemic). Histologically the changes were described as hyperplastic gingivitis with chronic inflammatory changes, without granuloma. Conclusion: With this case, we wanted to recall another (extra- intestinal?) CD manifestation which can significantly impair the patient’s quality of life and requires cooperation between gastroenterologists and dental specialists.

Functional MR urography conducted at the age of 3 months confirmed the diagnosis of primary congenital obstructive megaureter and the infant was referred to a pediatric surgeon. Conclusions: Although a rare occurrence, S-PHA can be a potentially life- threatening condition in infants if not recognized and treated adequately. Therefore, serum concentrations of electrolytes should be obtained in every child with a diagnosed obstructive anomaly of the urinary tract and/or acute cystopyelonephritis. On the other hand, every child with diagnosed S-PHA should be evaluated for the obstructive anomaly of the urinary tract.

Refeeding syndrome (RFS) describes a potentially fatal shift in electrolytes in severely malnourished patients receiving rapid and excessive food re-introduction. It is a result of hormonal and metabolic disturbances. There are various clinical and laboratory features with hypophosphatemia being the most common one. Elevation in liver function tests is also frequently seen. Patients with anorexia are a high-risk group for developing RFS. The aim of the study was to investigate the incidence and clinical features of RFS among hospitalized patients, as well as the severity of malnutrition (Z-score, BMI). This study is a retrospective analysis of medical documentation of patients diagnosed with anorexia nervosa (restricting (ANRT) and binge eating/purging (ANBP) subtype), eating disorder NOS (EDNOS), and avoidant/ restrictive food intake disorder (ARFID) who were admitted to our Centre for eating disorders in children and adolescents during a 5 year period (2014-2018). We analyzed the age, gender, duration of the disease before admittance, anthropometric data (BMI and Z-score), average weight loss, the need for nasogastric (NG) tube feeding, and phosphate supplementation. For statistical analysis we used t-test. 256 patients (232 female) aged 6-20 years (median 15+/-2.06) of which 43% were diagnosed with ANRT, 10% ANBP, 8% ARFID, 39% with EDNOS were included in the study. The average duration of the disease at the time of admittance was 13.25+/-13.43 months. The average BMI Z-score was -1.97 +/-1.63 average weight loss was 20+/-9.76% initial body weight (IBW). Hypophosphatemia was found in 15.6% of patients, of which 65% received phosphate supplements by oral or intravenous route depending on phosphate serum concentration. Elevated liver enzymes due to RFS were found in 9.3% of patients. In total RFS in some form developed in 23% of patients. The average BMI Z-score of patients that developed RFS was -2.6+/-1.89, average weight loss was 23.9+/-9.85% of IBW. Both variables were significantly different (p

In this study, Lactobacillus reuteri B2was isolated fromthe feces of C57BL/6 mice and assessed on probiotic activity. L. reuteri B2was identified by 16S rDNA sequencing, which the cell viability in acidic conditions at pH 2.0was 64% after 2 h, and in the presents of 0.30% of the bile salts, after 6 h, was 37%. Antimicrobial assay with L. reuteri B2 showed maximumdiameters against Klebsiela oxytoca J7 (12.5±0.71mm).Wefurther hypothesized if L. reuteri B2 strain in the free form can survive all conditions in the gastrointestinal tract (GIT) then the utilization of the appropriate biomaterials would ameliorate its stability and viability in GIT. L. reuteri B2 was microencapsulated into sodium alginate-(Na-alg) and different content of Na-alg and sodium maleate (SM) beads. Characterization materials enveloped their thermal characteristics (TGA/DTA analysis) and structure using: scanning electron microscopy (SEM), FTIR, and particle size distribution. The high survival rate of L. reuteri B2 at lowpH from2.0 to 4.0 and in the presence of the bile salts, at concentrations up to 0.30%, was obtained. L. reuteri B2 showed strong antimicrobial activity and the best protection microencapsulated with Na-alg + SM in simulated gastric juices (SGJ).

Introduction: Chronic obstructive pulmonary disease (COPD) is a global health issue characterized by progressive airflow limitation and respiratory symptoms1,2. Recent research have shown that patients with COPD often have a higher prevalence of cardiovascular diseases (CVD), including hypertension, coronary artery disease, and congestive heart failure3,4. This retrospective study aims to compare prevalence of CVD based on COPD severity. Patients and Methods: Data were collected from patients with COPD attending Pulmonology Clinic between April 2023, and October 2023. We divided patients into Global Initiative for Chronic Obstructive Lung Disease (GOLD) stages I or II (FEV1 >50%) and GOLD stages III or IV (FEV1 <50%). We compared groups regarding the frequency of cardiovascular comorbidities, gender, age, treatments, current smoking status, COPD exacerbations in the past year, and hospitalizations. Statistical analysis used IBM SPSS Statistics 26. Categorical variables were analysed with Fisher’s exact test, while numerical variables underwent Student’s t-test. All p-values were two-tailed, with significance set at p = 0.05. Results: In this study of 109 COPD patients, those in GOLD stages I or II (n=56) had a mean age of 67.7±12.9 years, and those in GOLD stages III or IV (n=53) had a mean age of 70.6±6.9 years (p=0.2). In GOLD stage III or IV group, had more male patients, higher exacerbations rate, and increased long-term oxygen therapy usage compared to GOLD stage I or II (Table 1). Mean FEV1 in GOLD I or II patients was 65.1±11.1%, while in GOLD III or IV patients, 33.9±10.6% (p<0.001). Hospitalization rates were significantly lower for the former group (0.09±0.3 vs. 0.4±0.6, p=0.005). Figure 1 shows the frequency of CVD depending on GOLD stage, no statistical significance was found between the two. GOLD stage I or II patients significantly more often have LAMA and LABA+LAMA in their therapy, while GOLD stage III or IV patients have SABA+SAMA and IC+LABA+LAMA (Figure 2). Notably, the choice of cardiovascular therapy did not differ significantly between these groups (Figure 3). Conclusion: Despite prior research, we could not confirm the COPD severity-CVD link. Introduction: The pericardium is a double-walled sac (consisting of visceral and fibrous layers) between which lies the pericardial space, enveloping the heart and the roots of blood vessels entering or exiting the heart1 . Although pericardial effusion can arise from various pathological conditions, its etiology is typically presumed based on clinical presentation and comorbidities, and an accurate diagnosis is established through biochemical, microbiological, and cytological analysis of the effusion. However, pericardiocentesis is an invasive procedure indicated when effusion onset is symptomatic or accompanied by tamponade, or when its etiology is unclear2,3. Patients and Methods: The retrospective analysis included 48 patients with echocardiographically confirmed cardiac tamponade of various etiologies in the period from 2016 to 2021. Descriptive statistical data are presented as a percentage. Due to a small sample size and uneven distribution, the examination of intrahospital mortality between patient groups, based on etiology and effusion treatment, was performed using the Fisher’s exact test and statistical significance was indicated as p-value < 0,05. The statistical analysis was conducted using IBM SPSS Statistics. Results: The median age was 66.5 years (IQR 59-72 years), with a minimum value of 21 years and a maximum of 87 years (Table 1). No statistically significant difference in intrahospital mortality was found among patients with different etiologies of cardiac tamponade (p > 0.05). However, patients treated with a combination of cisplatin and pericardiocentesis had a lower mortality rate compared to those treated with pericardiocentesis alone, p < 0.05 (Figure 1). Conclusion: Malignant diseases are one of the leading causes of death worldwide, and when combined with pericardial effusion and tamponade, the most common ones are lung and breast cancer, melanoma, and lymphoma. The therapy of choice for acutely occurring pericardial effusion is pericardiocentesis, which alleviates symptoms and provides additional diagnostic possibilities. The effectiveness of cisplatin administration in combination with pericardiocentesis is independent of hemodynamic instability parameters and inflammatory markers in patients with recurrent pericardial effusion. Introduction: Atrial fibrillation (AF) is one of the most common arrhythmia in clinical practice. Apart from pharmaceutical, one of the most successful methods for achieving sinus rhythm is electrocardioversion (ECV). Although we can quickly achieve sinus rhythm with ECV, maintaining it is a challenge. Data from the literature suggest that there are certain predictors associated with the recurrence of AF, such as the duration of AF and long-standing arterial hypertension1 . Recurrence usually does not occur within 24 hours of cardioversion2. An early catheter ablation strategy may potentially reduce the recurrence of AF3,4. We made a retrospective analysis of patients who underwent successful ECV through the emergency hospital admission and the Cardiology Day Hospital at the General Hospital “Dr. Josip Benčević“. Aim: To determine the relationship between gender, left ventricular ejection fraction, use of antiarrhythmic drugs, and comorbidities with recurrence of AF after successful ECV. Patients and Methods: We used data from the hospital information system according to the ECV procedure performed from January 1, 2021 to December 31, 2022. Age, gender, use of antiarrhythmic (amiodarone/dronedarone), left ventricular ejection fraction, and recurrence of AF were analyzed. Categorical variables were analyzed with Fisher’s exact test, while numerical variables underwent Student’s t-test with Mann Whitney U test for correlation. P<0.05 was considered statistically significant. Results: In the study, 91 patients participated, including 64 (70.3%) males and 27 (29.7%) females. The median age was 66 years with an interquartile range of 58 to 73 years. Out of the total, 39 (42.9%) patients experienced a recurrence, while 52 (57.1%) did not. There was no correlation between patient age and recurrence (median age at recurrence 66, IQR 60-74 vs without recurrence 67.5, IQR 58-73; p=0.6, Mann Whitney U test). We did not find statistically significant differences according to gender, left ventricular ejection fraction, use of antiarrhythmics, and comorbidities (Figure 1, 2, 3, and Table 1). Conclusion: Although ECV is a successful method for achieving sinus rhythm, it does not predict or affect its maintenance. Cilj ovoga rada bio je ispitati utjecaj dodatka antioksidansa na aromatski sastav crnog vina Cabernet Sauvignon nakon 12 mjeseci starenja u bocama. Aromatske komponente i njihova koncentracija u vinu određeni su pomoću plinske kromatografije s masenom spektrometrijom uz prethodnu mikroekstrakciju na čvrstoj fazi (SPME-GC/MS). Cabernet Sauvignon karakteriziraju sljedeći spojevi arome: hlapive masne kiseline, viši alkoholi, terpeni, acetatni i etilni esteri te norizoprenoidi. Smanjena koncentracija sumporovog dioksida rezultirala je smanjenjem koncentracije određenih aromatskih spojeva, dok je dodatkom glutationa njihova koncentracija u vinu ostala očuvana. The aim of this study was to investigate the effect of antioxidants addition (sulfur dioxide and glutathione) on the aroma composition of Cabernet Sauvignon after 12 months of bottle ageing. Aromatic compounds and their concentration in wine were analyzed by gas chromatography-mass spectrometry with solid-phase microextraction (SPME-GC/MS). Cabernet Sauvignon is characterized by aroma compounds: volatile fatty acids, higher alcohols, terpenes, acetate and ethyl esters and norisoprenoids. The reduced concentration of sulfur dioxide resulted in a decrease of aromatic compounds, while glutathion addition preserved their concentration in wine.

Uvod: Entezitisu pridruženi artritis (ErA) posebni je oblik juvenilnog idiopatskog artritisa (JIA) koji predstavlja nediferencirani juvenilni spondiloartritis (jSpA). Uz artritis perifernih zglobova, jedna od najvažnijih značajki jSpA je upala enteza i/ili aksijalnog skeleta. I dok se u odraslih bolesnika sa SpA crijevo sve češće navodi kao polazište i/ ili ishodište subkliničke upale, učestalost i raširenost upalnog procesa u crijevima djece s ErA još uvijek nije poznata, uglavnom zbog izazova vezanih uz provođenje invazivnih pretraga neophodnih za otkrivanje upale crijeva u djece. Cilj našeg istraživanja stoga je bio procijeniti koncentraciju fekalnog kalprotektina (fCAL), surogatnog markera upale crijeva, u djece s različitim oblicima artritisa. Ispitanici i metode: U ovom presječnom istraživanju sudjelovalo je ukupno 71 dijete s postavljenom dijagnozom oligo i poliartikularnog oblika JIA (N=29), ErA (N=26), PsA (N=4) te neupalnim muskuloskeltnim stanjima (N=12). Svi bolesnici s ErA uključeni su neovisno o prisutnosti gastrointestinalnih simptoma, dok su ostali uključeni u slučaju izraženih simptoma. Uz standardne laboratorijske pretrage, svim sudionicima izmjeren je fCAL, a svim bolesnicima s ErA, kao i ostalima koji su se žalili na upalnu bol u leđima, učinjen je MR SIZ- ova. U svih bolesnika s ErA izmjerena je aktivnost bolesti pomoću jSpADA, a u ostalih oligo i poli JIA bolesnika pomoću JADAS-a. Rezultati: Medijan koncentracije fCAL bio je najviši u skupini bolesnika s ErA (33.2 mg/kg, p=0.043), s značajnom razlikom između bolesnika s aktivnom i inaktivnom bolesti (20.0 mg/kg vs 57.4 mg/kg, p=0.01), kao i između bolesnika s prisutnim znakovima upale te bolesnika s urednim nalazom MR-a (22.6 mg/kg vs 54.3 mg/ kg, p=0.048). Koncentracije fCAL nisu se značajno razlikovale između bolesnika koji su koristili i koji nisu koristili NSAIL-ove (23 mg/kg vs 20 mg/kg, p=0.18), mada je uočena slaba koleracija s trajanjem korištenja (r=0.25, p=0.03). Zaključak: Subklinička upala crijeva nije vezana samo uz odrasle bolesnike sa SpA, već i uz djecu s nediferenciranom oblikom jSpA, osobito ako je uz to prisutna aktivna bolest i/ili znakovi upale SIZ-ova na MR-u.

Any discrimination or exclusion based on sexual orientation for the purpose of reducing equali- ty before the law, as well as providing preferential treatment on these grounds, constitutes sexual orientation discrimination . This paper will, inter alia, address the rights of same-sex partners vis-à-vis the rights of heterosexual partners, with the aim of analyzing the current stage of the European perspective in the area where the legal arrangements of certain EU Member States EU AND COMPARATIVE LAW ISSUES AND CHALLENGES SERIES (ECLIC) – ISSUE 41272(hereinafter: MS) will be presented by a comparative method. It is precisely these issues that have been decided by the Court of Justice of the European Union (hereinafter: CJEU), from which the major problems in the area of sexual orientation discrimination are identified. In the view of the CJEU, in order for a treatment to be characterized as discrimination on the basis of sexual orientation, a community of same-sex partners must be legalized in a MS. Only when such a legal framework exists can the CJEU decide whether there is a basis for discrimi- nation. Such an interpretation begs the question, what about states that do not have any legal framework that recognizes the same- sex life community? The first part of the paper will analyze international and European standards of protection against sexual orientation discrimination, and after that, the current level of same-sex rights in the EU will be briefly presented. The sec-ond part of the paper will be based on analysis of the impact of CJEU decisions on changes in Romanian legislation in the field of recognizing the effect of same-sex marriage, concluded in another MS, as well as an analysis of the process of recognizing the effect of same-sex marriage in the Republic of Croatia (hereinafter: Croatia) In addition, the paper will also analyze the judgment of the Constitutional Court of the Republic of Croatia (hereinafter: CCRC) regard-ing discrimination on the grounds of sexual orientation observed in the positive regulation gov-erning the issue of foster care. Based on these analyzes, de lege ferenda guidelines will be given at the end of the paper in order to reduce the possibility of sexual orientation discrimination

- Abstract
Vaccines can have heterologous effects on the immune system, i.e., effects other than triggering an immune response against the disease targeted by the vaccine. We investigated whether monoclonal antibodies (mAbs) specific for tetanus could cross-react with Chlamydia and confer heterologous protection against chlamydial infection. The capability of two tetanus-specific mAbs, namely mAb26 and mAb51, to prevent chlamydial infection has been assessed: (i) in vitro, by performing a neutralization assay using human conjunctival epithelial (HCjE) cells infected with Chlamydia trachomatis serovar B, and (ii) in vivo, by using a guinea pig model of Chlamydiacaviae-induced inclusion conjunctivitis. The mAb26 has been superior in comparison with mAb51 in the prevention of chlamydial infection in HCjE cells. The mAb26 has conferred ≈40% inhibition of the infection, compared to less than 5% inhibition in the presence of the mAb51. In vivo, mAb26 significantly diminished ocular pathology intensity in guinea pigs infected with C. caviae compared to either the mAb51-treated or sham-treated guinea pigs. Our data provide insights that tetanus immunization generates antibodies which induce heterologous chlamydial immunity and promote protection beyond the intended target pathogen.

- Abstract
Investigation of a CBRN incident has to be conducted in a systematic, objective, and timely manner in order to collect, preserve and analyse physical evidence from the associated site to be admissible in a criminal or civil proceeding, as well as in prosecution of acts of terrorism and war crimes. The inherently hazardous character of a crime scene involving CBRN agents adds complexity to investigative actions and requires additional expertise, equipment and conditions. The specific investigation strategies are necessary, together with the adapted CBRN detection equipment and forensics methods and procedures. This paper deals with the management of a CBRN crime scene and the specifics of investigation process, including interfaces between CBRN safety and security, and emergency response. The necessary framework, the conduct of operations, and an integrated command structure are outlined. The role of the technical support organisations with adequate CBRN expertises is emphasised.
