Carbon regulation in Russia is at the stage of formation, which requires the development of an integrated approach based on the principles of sustainable development, taking into account international experience. With an understanding of the importance of the problem of climate change for the future of the country and the entire planet, as well as in fulfillment of international obligations, Russia has adopted a number of measures to nationally regulate greenhouse gas emissions reduction, as well as other mechanisms that help slow down global changes.The purpose of this article is to develop comprehensive legal solutions to optimize the system of regulatory legal regulation of issues of reducing greenhouse gas emissions. The research problem lies in the formation of such mechanisms of state climate regulation, which, on the one hand, will contribute to achieving the goals of reducing greenhouse gas emissions set by Russia, and on the other hand, will correspond to the current foreign policy challenges, the ambiguous geopolitical and difficult economic situation that affect public policy, the business community and the global interaction on climate issues.

Relevance. To control COVID-19, a net of special infection hospitals and departments was created, a system of medicaments and protective means’ supply was set up, necessary normative and regulatory documentation was developed and overall vaccination of population was performed in Moscow. Within frames of work for prophylaxis of COVID-19 spread in patients with tuberculosis at in-patient units of the State Budgetary Healthcare Institution of Moscow city «The Moscow Research and Clinical Center for Tuberculosis Control of the Moscow Government Department of Health” (hereinafter referred as the Center), observational departments for treatment of patients with tuberculosis from COVID-19 contact and convalescents after COVID-19. Aims. To evaluate specialized observational departments’ work as part of general anti-epidemic control measure system for COVID- 19 at tuberculosis hospitals. Materials and methods. The observational departments’ work within the period of 2020–2022 with 1075 patients treated including 400 patients from COVID-19 contact areas (37.2%) and 675 convalescent patients after COVID-19 (62.8%), was analyzed. The results obtained during the study were statistically treated by means of application program package Microsoft Office 2007. The quantitative parameters with presumably normal distribution were described with mean arithmetic values (M) and standard deviations (SD), and 95% confidence interval (CI 95%). The estimation of significance of differences between outcomes was evaluated with the Pearson χ2 test. The differences were considered to be statistically significant with p < 0,05. Results and discussion. The evaluation data on functioning efficacy of observational departments for patients with tuberculosis from COVID-19 contact and convalescent patients with tuberculosis after COVID-19 within the large medical institution hospital of physiatry profile. Among all patients, 153 of patients who fell ill with COVID-19 (14.2%; 95% CI [12.21–16.38%]) were determined, which lowered the number of COVID-19 areas at in-patient departments of the Center by 11.8%. The mean timeframes of COVID- 19 determination in general were 8.0 days: 5.4 days among contacts from new coronavirus infection areas and 12.4 days among convalescents at the department for the treatment of patients with tuberculosis and new coronavirus infection. The comparative analysis showed that COVID-19 was more frequently determined in tuberculosis patients from the contact with those who fell ill with COVID-19 (p < 0.001) than in convalescents. Conclusions. During medical care for patients of physiatry profile in Moscow city, a wide range of management decisions consisting in the change of routing system in specialized medical care, was taken. Among them, creation and functioning of observational departments for contact persons from COVID-19 areas and convalescents after COVID-19 play a particular role.Creation of such departments within a large medical institution enabled the contemporary isolation of contact persons from COVID- 19 areas and convalescents after COVID-19 and allowed the additional control of nosocomial infection spread

Early assessment of HIV incidence is an important public health tool for understanding the state of the epidemic in a particular area, identifying high-risk groups, and assessing the effectiveness of HIV prevention interventions.Objective. To assess the possibility of using the positivity rate (S/CO) in the HIV Ag/Ab immunoassay on the Alinity i analytical platform to determine the duration of infection during HIV screening.Materials and methods. The study included 316 HIV-infected patients with different infection durations. Immunochemical analysis was performed on an Alinity i automatic analyzer (Abbott Laboratories, USA) using the Alinity i HIV Ag/Ab Reagent Kit (Abbott Laboratories, Germany) in accordance with the manufacturer’s instructions.Results. Statistical analysis of 316 blood samples from HIV-infected patients at different stages of infection demonstrated the reactivity of the Alinity i HIV Ag/Ab test result and a dynamic increase in the positivity ratio during the first six months after the onset of the disease. Based on the data obtained, a threshold value (≤294 conventional units) was obtained for the positivity ratio, which allowed for a clear distinction between HIV-positive patients with a recent (

Analyzing many official documents of recent years, we can conclude that the reform of higher education is expected in our country. It is also necessary in the system of physical education of students, because over the years of study at university the physical condition of most students deteriorates, and rarely any students manages to form universal competence in self-assessment, self-organization and self-development. The authors of the article propose to get acquainted with the topics of theoretical issues that have been included in the work programs at the Department of Physical Education of the Ryazan State Medical University named after Academician I. P. Pavlov for more than 30 years. In their opinion, they allow students to assess their physical development, physical fitness level and study the impact of regular physical activity or a sedentary lifestyle on the main systems of a human body.

Abstract Background In Russia, the following two questionnaires are mainly used to study the burnout syndrome: Maslach Burnout Inventory (MBI) and Boyko’s Burnout Inventory (BBI). Despite the fact that the questionnaires are based on different theoretical models and composition of the scales, they evaluate essentially the same construct. A few published sources provide data on correlations between the results of measuring burnout using these methods. However, the presence of a correlation does not imply the comparability of the methods. The goal of our study was to compare the results of MBI and BBI as well as to develop a methodology for reciprocal recalculation of their burnout estimates. Methods MBI and BBI scales were employed to diagnose the burnout syndrome. Our study included 117 men aged 41–44 years. The total scores obtained by the two methods, as well as the subscale scores, were compared using the correlation analysis, the cross-comparative analysis, and the Bland–Altman plot method, while the associations between the results were estimated with odds ratio (OR) and 95% confidence interval (CI). Results The total scores (integral indicators based on summing the scores of all subscales and taking into account differences in the weights of scale scores) demonstrated a high similarity in measuring the severity of the burnout. All three dimensions of burnout sensu MBI correlated with the total BBI score. Conclusion The comparison (MBI vs. BBI) demonstrated the consistency of the results, which implied the possibility of comparing data yielded by the studies based on the two questionnaires (MBI and BBI).

Agomelatine is an antidepressant with a unique pharmacological action that is both a melatonin agonist and a selective serotonin antagonist. The drug's unique pharmacological profile includes the properties of a dual 5-HT2С receptor antagonist as well as the properties of a melatonin MT1 and MT2 receptor agonist. Through its effect on melatonin receptors, agomelatine resynchronizes disturbed circadian rhythms and has a positive effect on sleep architecture. In addition, agomelatine shows a novel and fundamentally different mechanism of anxiolytic action compared to other classes of drugs used to treat anxiety. The article presents studies of agomelatine that demonstrate good treatment results in terms of response rates and remission in major depressive disorder of varying severity and generalized anxiety disorder. Agomelatine in a daily dose of 25–50 mg not only reduces the degree of anhedonia, apathy, anxiety, reduces somatic symptoms of depressive and anxiety disorders, but is also well tolerated, normalizes sexual dysfunctions in patients in psychiatric and general medical networks, it is used in cardiovascular, neurological diseases, and post-COVID-19.

Anxiety disorders, except the manifestations of anxiety itself, are characterized by a broad spectrum of cognitive and somatic symptoms significantly reducing the individual’s quality of life and increasing the risk of somatic disorders. The rate of cardiovascular, respiratory, gastrointestinal, and other disorders throughout life is significantly higher in people suffering from anxiety. Furthermore, many symptoms of anxiety and panic/fear mimic manifestations of somatic disorders. Optimal treatment of anxiety disorders includes both pharmacological and psychotherapeutic interventions. At the same time, psychotherapy is not always available for patients, and internists not always decide to prescribe psychotropic medication, for example antidepressants. In such situations, nonbenzodiazepine anxiolytics showing high efficacy and tolerability can be used. Mebix, having an adaptogenic effect on the central nervous system and a stabilizing effect on the autonomic nervous system, that affects almost all neurotransmitters can be considered one of such drugs. The paper reports clinical cases of patients with anxiety disorders, who were treated with Mebix as monotherapy or in combination with antidepressants.

Pain is represented in the child and adolescent population all over the world, and many young adults have chronic pain. Chronic pain is accompanied by the lifestyle and functioning alteration, as well as by the development of depression and anxiety symptoms. The global prevalence of chronic pain among children varies between 20 and 40%, about 8% of children report frequent severe pain, and 5% of children and adolescents become disabled due to pain. Headache (HA) is a common complaint of children and adolescents: by the age of 3 years complaints of HA are reported in 3–8% of children, by the age of 5 years in 19.5%, by the age of 7 years in 37–51.5% of children. The prevalence of HA, regardless of the subtype, is almost the same in girls and boys under the age of 12 years. The main HA subtypes that occur in pediatric practice are as follows: migraine (with or without aura), tension-type HA, cluster HA. Pain, including HA, is often associated with mental disorders in children and adolescents. Thus, the study focused on assessing the relationship between HA and mental disorder in the representative sample (10,123 people) confirmed the association between HA, especially migraine, and mental disorders in adolescents. The strongest associations were reported between migraine and mood disorders and anxiety, behavioral disorders, and substance use. Young adults with HA, especially migraine, more often suffered from multiple mental disorders. The paper presents a clinical case of the 10-year-old girl with the autism spectrum disorder and the combination of migraine without aura and depressive disorder.

New risks make bank marketing especially relevant in modern conditions, e.g., COVID-19 restrictions, political issues, economic sanctions, etc. These risks affect the activities of banks, which have to look for new ways to create and promote their services and products. Bank products are quite peculiar: on the one hand, they are difficult to perceive by potential customers; on the other hand, digital technologies provide banks with additional promotion andexpansion opportunities. Global trends in bank marketing include such new technologies as artificial intelligence and digital platforms, new mobile applications, new sales channels and competition with other banks in this sphere, cybersecurity, winning the trust of customers, etc. New risks force banks to fight for customers and expand their market share using bank marketing concepts and tools. Digital marketing includes not only new promotion channels but also such new approaches as brand performance. Banks strive to meet customers halfway and to conquer new age groups by using gamification and financial education. The authors used the 7P marketing model to analyze AO Bank.DOM.RF as an average Russian bank and to recommend new options of bank marketing in practice. The analysis included strengths, weaknesses, opportunities, and threats.

More than half of the world’s population is infected with the herpes simplex virus. In most cases, infection is not accompanied by symptoms, but in some people the disease occurs as a chronic infection with frequent and severe relapses. One of the most likely reasons for this may be a dysregulation of the immune system. In recent years, the role of checkpoint molecules, in particular PD-1 and Tim-3, in the regulation of the immune response and the functions of immunocompetent cells has been actively studied. Activation of PD-1 and Tim-3 on T cells has previously been shown to suppress the immune response. PD-1 and Tim-3 are also expressed on other immune cells, in particular monocytes. However, the expression of these molecules on monocytes during chronic viral infections has not been previously studied. The study was aimed at assessing the level of PD-1 and Tim-3 expression on various populations of monocytes in patients with chronic often recurrent herpesvirus infection. Twenty-six patients were recruited into the study. All patients received antiviral and immunomodulatory therapy in the immunological department. The number of classical, intermediate, and non-classical monocytes and the expression of PD-1 and Tim-3 on monocytes, were assessed by flow cytometry before and after the therapy. Monocytes were isolated from peripheral blood, and subpopulations were divided according to the level of expression of CD14 and CD16. In patients with herpes, a reduced number of monocytes was observed in comparison with healthy donors. The relative number of PD-1-positive monocytes, the mean fluorescence intensity of PD-1 and Tim-3, and the number of double-positive cells were reduced in herpes patients in all three monocyte subpopulations examined. Three months after therapy, the response to the therapy was assessed; patients who did not have a single recurrence of herpes within 3 months were considered to respond. Responding patients had a lower initial content of double-positive cells among intermediate and non-classical monocytes. The decrease in the level of PD-1 and Tim-3 positive monocytes during herpesvirus infection revealed in the present study may indicate the involvement of monocytes deficient in the expression of checkpoint molecules in the pathogenesis of the disease.

The article presents the results of a clinical investigation on the administration of the Grippol Quadrivalent vaccine (influenza tetravalent inactivated subunit adjuvant vaccine) in children from 6 months to 5 years old. The vaccine has demonstrated no less effectiveness in comparison with the Grippol plus vaccine (influenza trivalent inactivated polymer-subunit vaccine).comparable results were obtained for all evaluated indicators of immunological efficacy against matching strains of the influenza virus, while for additional strain B (Yamagata line) there was a statistically significant difference in the increase in the immune response in the Grippol Quadrivalent group. The results of the assessment of the reactogenicity and frequency of systemic adverse events indicated a favorable and comparable safety profile of the vaccines Grippol Quadrivalent and Grippol plus in children from 6 months to 5 years old. The data obtained allowed us to conclude that the immunological efficacy of the Grippol Quadrivalent vaccine is no less when compared to the Grippol plus vaccine, as well as a comparable safety and reactogenicity profile. According to the results of the clinical investigation, on 08/04/2023, the Ministry of Health of the Russian Federation amended the “Instructions for the medical use of the drug”: Grippol Quadrivalent is indicated for children from the age of 6 months.

The study considers the phenomenology of the professional crisis of the teacher personality in the context of the transition to online learning. Based on the analysis of the results of the theoretical and empirical research involving 170 teachers, it was established that the features of the professional crisis of the teacher personality are the transformation of the value-semantic sphere of the personality and the actualization of reflexive and anticipation processes. The results of the empirical research allowed for the first time to reveal the features of the professional crisis of the teacher in the context of the transition. The experimental group had the life efficiency indicator on the level 47.92 out of 100 points, while the control group -- 69.59 points, which is not a much higher indicator. The results can become an empirical basis for the development of programs of psychological and pedagogical support for teachers experiencing professional crisis situations when implementing online learning programs.

In live organisms, cells are embedded in tissue-specific extracellular matrix (ECM), which provides chemical and mechanical signals important for cell differentiation, migration, and overall functionality. Careful reproduction of ECM properties in artificial cell scaffolds is necessary to get physiologically relevant results of in vitro studies and produce robust materials for cell and tissue engineering. Nanoarchitectonics is a contemporary way to building complex materials from nano-scale objects of artificial and biological origin. Decellularized ECM (dECM), remaining after cell elimination from organs, tissues and cell cultures is arguably the closest equivalent of native ECM achievable today. dECM-based materials can be used as templates or components for producing cell scaffolds using nanoarchitectonic approach. Irrespective of the form, in which dECM is used (whole acellular organ/tissue, bioink or hydrogel), the local stiffness of the dECM scaffold must be evaluated, since the fate of seeded cells depends on the mechanical properties of their environment. Careful dECM characterization is also necessary to reproduce essential ECM traits in artificial cell scaffolds by nanoparticle assembly. Atomic force microscopy (AFM) is a valuable characterization tool, as it allows simultaneous assessment of mechanical and topographic features of the scaffold, and additionally evaluate the efficiency of decellularization process and preservation of the extracellular matrix. This review depicts the current application of AFM in the field of dECM-based materials, including the basics of AFM technique and the use of flicker-noise spectroscopy (FNS) method for the quantification of the dECM micro- and nanostructure.

In the context of the modern dynamic socio-economic environment, career-related issues are becoming relevant to psychological and pedagogical sciences as it is professional development that enables people to realize their abilities, capabilities, and needs. Career orientation should be identified as early as possible in order to obtain significant achievements. An important issue is related to career orientation predictors, which determine future achievements and career choice. The study discusses conceptual provisions and methodological approaches to assessing the career orientations of college students with different levels of self-efficacy. The purpose of the research is to study theoretical and methodological approaches and aspects of the psychological assessment of self-efficacy as a personality predictor of the career orientations of college students. The study was conducted with the participation of a representative sample of 558 third-year students aged 18 to 20 on the campus of the Ural State College named after I. Polzunov (Ekaterinburg) (average age is 18.7 years). The practical significance of the study arises from the possibility to use the results obtained in order to implement programs aimed at improving self-efficacy in adolescents. The research is of practical value for organizational psychologists, educational psychologists, and teachers.

Objectives. The purpose of this work is to provide a clinical case study of Lemierre’s syndrome (LS) in order to raise awareness amongst doctors of various medical specialties regarding this rare cause of septic pulmonary embolism (SPE).Materials and methods. The patient in clinical case is a 25-year-old female who was hospitalized due to SPE, which had been complicated by respiratory distress, pneumothorax and septic shock. The initial source of embolus lung damage was the septic thrombosis of both the internal and external jugular veins. This thrombosis occurred against the backdrop of a carbuncle of the lower lip, an inflammatory infiltrate at the bottom of the mouth, an abscess in the soft tissue of the chin and neck cellulitis, which correspond to the diagnostic criteria for Lemierre Syndrome.Results. The literature data of the epidemiology, etiology, pathogenic mechanisms of the venous thrombosis, embolic and septic complications among patients with LS are presented. The article discusses the clinical features and course of the disease, as well as key aspects of diagnosis, including current diagnostic criteria, with regard to the typical and atypical variants of LS. The principles of antibiotic therapy and the possibility of anticoagulation therapy in cases of septic pulmonary emboli associated with head and neck vein thrombosis are also considered.Conclusion. LS is a rare, but clinically significant condition characterized by the development of septic thrombosis in the internal jugular vein in response to acute suppurative inflammation of the head and neck. In typical cases, the causative organism is Fusobacterium necrophorum, or other anaerobic bacteria. This pathological condition is often accompanied by multiple other septic complications, with the most significant being the development of septic pulmonary embolism. Clinical observation indicates the necessity for including LS in the differential diagnosis among causes of the septic pulmonary embolism, and for adopting a multidisciplinary approach in the treatment of these patients in a specialized multifield hospital. Identification and diagnosis of Lemierre Syndrome can be challenging due to its low incidence and variable clinical presentation. Although the condition is not widely recognized among physicians, an understanding of the basic principles of its diagnosis and management can significantly improve treatment outcomes and patients prognosis.

The paper examines conditions of occurrence and typical time scales of a polarization jet by measuring ionospheric parameters from DMSP satellites (h~830 km) and using data from the ground-based ionospheric station Yakutsk (YA462) during strong and long-term geomagnetic storms. The polarization jet at the ionospheric station Yakutsk was recorded in the dusk sector at the background of substorm disturbances when the geomagnetic index SME reached values from 1000 to ~3000 nT, and, according to data from the magnetic observatory Yakutsk (YAK), there was a positive bay ~50–100 nT in the geomagnetic field H component and ~100–200 nT in its Z component. We show that the lifetime of the polarization jet or narrow troughs in ionospheric plasma in the subauroral ionosphere during strong and long-term magnetic storms can be as long as 12 hrs.

Production of soluble dry extracts from sea buckthorn raw materials is relevant and promising for enterprises of the Republic of Buryatia. The authors have selected technological equipment for obtaining dry extracts: extractor, concentrator, dryer. The possibility of using vacuum-pulse action and the influence of technological factors on the yield of water-soluble substances from sea buckthorn leaves and shoots has been studied. The operating parameters of the technological process have been determined experimentally under production conditions: temperature 45–50 °C, total duration of extraction, concentration and drying 79–111 minutes. The process of obtaining dry extracts is carried out in three stages: water extraction; filtration and concentration; drying in a vacuum pulse dryer. The resulting experimental sample of dry extract from sea buckthorn leaves and shoots is a free-flowing crystalline powder with a moisture content of 4–6 %, highly soluble in water, having high organoleptic characteristics – a natural, well-defined aroma characteristic of sea buckthorn, light brown color. The use of low temperatures and vacuum-pulse modes at the stages of the technological process predicts high safety of thermolabile biologically active substances in the product. The process of obtaining dry extracts from sea buckthorn leaves and shoots has been studied. The results of the studies prove the possibility of producing a dry extract with high organoleptic characteristics using the developed by the authors' technology by equipment whose vacuum-pulse operating modes have a positive effect on the yield of extractives and help reduce time costs along with the intensification of heat and mass exchange processes in sea buckthorn raw materials.

Research on the development of environmentally friendly plant protection systems against fungal pathogens using symbiotic bacteria Xenorhabdus sp. – symbionts of entomopathogenic nematodes (EPN) has been a new direction in agricultural practice in recent years and undoubtedly represent relevance and scientific significance. The studies used suspensions of live and autoclaved cultures of symbiotic bacteria of symbionts of various types of EPN (Steinernema carpocapsae, S. feltiae and S. feltiae protense) with a bacterial cell titer of 107 CFU/ml in comparison with the biological preparation Phytosporin-M (dough) and water as a control. In laboratory conditions at a temperature of 25 oC, there were established differences in the antibiotic activity of the primary forms of producers of Xenorhabdus sp., isolated from different types of EPN. The greatest inhibition of the fungal growth zone on the 4th day was observed for metabolites of the S. carpocapsae strain against Alternaria solani. Biological effectiveness in suppressing this pathogen was 51 %. Field research conducted in 2022-2023 in the conditions of the Republic of Karelia on the mid-early potato variety ʻRed Scarlettʼ, showed that under epiphytotic conditions (low air temperatures and excessive waterlogging) double spraying of vegetative plants with a suspension of live and autoclaved cultures of symbiotic bacteria (EPN-1-1, EPN-2 and EPN-2 -1) reduced the development of rhizoctoniosis compared to the control variant by 50, 64 and 60 %, respectively. It was found that double treatment with a live and autoclaved aqueous suspension of bacteria, symbionts of the subspecies S. feltiae was more effective and ensured a reduction in the degree of development of scab symptoms by 1.3–2.8 times and the spread of rhizoctonia by 1.5–2.0 times. It has been also established that 2-fold spraying of plants with a live and autoclaved suspension of symbiotic bacteria S. feltiae during the growing season significantly increases the yield of tubers by 35–22 %, respectively. Thus, the use of biologically active secondary metabolites of Xenorhabdus sp. has significant potential as biological plant protection agents against potato pathogens.

The analysis of the textbooks published in recent years and devoted to the improvement of the physical education of students with disabilities (health limitations) shows that all the authors point out negative trends in the health of students in most universities of the country, while the number of first-year students with disabilities and those disabled since childhood increases annually, many of them having the pathology of two or more systems simultaneously. The main task facing the teachers of the departments of physical education is the development of such students’ ability to assess their physical condition for a given period and training them in choosing available physical activities, i.e. the formation of universal competencies in the field of health preservation and promotion. The actual organization of physical education for students in all the universities of the country that includes 1-2 classes per week in the 1st-2nd-3rd years does not provide students with disabilities with physical rehabilitation they need during the study period. And it is not possible to form their universal competencies connected with performing physical exercises during extracurricular time (with self-training) due to the limited number of hours in the 1st semester that does not allow students to acquire the necessary amount of knowledge on the impact of physical activity on human health. Referring to Order No. 245 of the Ministry of Science and Higher Education of the Russian Federation, which went into effect on September 1, 2022, the authors propose to form groups of students with disabilities starting with the first year and plan physical education classes for them for 45 minutes at least 4 times a week throughout their studies, given that according to the State Standard 400 hours are allotted for physical education. In addition, in order to form universal competencies in the field of health preservation and promotion, the authors propose to introduce a theoretical course (module) during the first 3 years of studies in all universities of the country that will be devoted to the basics of health-improving physical education (72 hours) at the expense of the course “Elective disciplines in physical education”.

The objective is to determine the complete nucleotide sequence and conduct a phylogenetic analysis of genome variants of the Puumala virus isolated in the Saratov region. Materials and methods. The samples for the study were field material collected in the Gagarinsky (formerly Saratovsky), Engelssky, Novoburassky and Khvalynsky districts of the Saratov region in the period from 2019 to 2022. To specifically enrich the Puumala virus genome in the samples, were used PCR and developed a specific primer panel. Next, the resulting PCR products were sequenced and the fragments were assembled into one sequence for each segment of the virus genome. To construct phylogenetic trees, the maximum parsimony algorithm was used. Results. Genetic variants of the Puumala virus isolated in the Saratov region have a high degree of genome similarity to each other, which indicates their unity of origin. According to phylogenetic analysis, they all form a separate branch in the cluster formed by hantaviruses from other subjects of the Volga Federal District. The virus variants from the Republics of Udmurtia and Tatarstan, as well as from the Samara and Ulyanovsk regions, are closest to the samples from the Saratov region. Conclusion. The data obtained show the presence of a pronounced territorial confinement of strains to certain regions or areas that are the natural biotopes of their carriers. This makes it possible to fairly accurately determine the territory of possible infection of patients and/or the circulation of carriers of these virus variants based on the sequence of individual segments of their genome.

In the article, the authors answered the following questions: How often do anxious patients occur in clinical practice? Why do most patients with anxiety see primary care physicians rather than a psychiatrist? What complaints and manifestations help to suspect anxiety in the patient?
