Neuromyelitis optica spectrum disorders (NMOSDs) are autoimmune inflammatory disorders accompanied by central nervous system damage, widespread immunemediated demyelination, and axonal damage, involving mainly the optic nerves, spinal cord, and area postrema. The diagnostic capabilities, administration and routing of patients, and therapeutic approaches to this disease need to be improved. During several expert councils held in 2019–2021 in different regions of the Russian Federation, we discussed multiple issues related to various aspects of medical care for patients with NMOSDs. As a result, the experts developed further steps necessary to improve the medical care to these patients: to write and publish clinical guidelines for the diagnosis and treatment of NMOSDs; to consider the possibility of optimizing the NMOSDs diagnostic program including the aquaporin-4 antibodies (AQP4-IgG) testing; to evaluate the implementation of a set of measures aimed at including the corresponding laboratory investigations into the system of state guarantees (together with the institutions of the Ministry of Health of Russia), if there is clinical and economic feasibility; to include the issues of timely NMOSDs evaluation in educational programs initiated by the scientific medical community, in order to raise awareness of primary care neurologists in relation to the clinical and neuroimaging signs of probable NMOSDs; to assess the possibility of introducing routing schemes for patients with NMOSDs at the regional level; to work out a decision on the collection of NMOSDs epidemiological and clinical data in the Russian Federation.

Objective. To compare the efficacy of restoration of gait function in patients in the early recovery period of ischemic stroke (IS) using a lower limb exoskeleton and an active-passive pedal bicycle trainer. Materials and methods. An open randomized study of 47 patients in the early recovery period of IS was undertaken. Patients in group 1 received a single rehabilitation course including exoskeleton exercises using an ExoAtlet exoskeleton; those of group 2 used a pedal trainer for active-passive training (five days per week for two weeks). Treatment results were assessed using a series of tests, including the Hauser Ambulation Index, the 10-m walking test, the Berg balance test, stabilometry, and gait biomechanics. Training courses were completed by 20 patients in group 1 and 21 in group 2. Results. Completion of training courses was followed by statistically significant increases in strength in the paretic muscles, postural stability, functional level, and walking speed in patients of both groups, though recovery was statistically significantly more marked in patients of group 1 (p < 0.05). Patients of group 1 showed a statistically significant decrease in the level of disability and an increase in the activities of daily living, changes being greater than those in group 2. Analysis of the main indicators of the statokinesiogram showed that improvements in patients of group 1 were greater, though statistically significant differences were seen only in the dynamics of the length and area of the curve in the test with the eyes open. Studies of gait biomechanics showed that gait function was altered: there was a statistically significant 2.2-fold reduction in movement speed, a 1.6-fold decrease in double stride length, an a 1.3-fold reduction in tempo compared with normal values. After completion of exercises, patients of group 1 displayed statistically significant increases in double stride length and walking speed and rate and a contraction in the locomotor cycle period. Conclusions. The study identified improvements to locomotion using instrumented rehabilitation using both an exoskeleton and an active-passive pedal trainer, though use of the exoskeleton had advantages, in that it produced statistically significantly greater recovery of strength, stability, and walking speed and symmetry over an analogous period using the trainer. A significant increase in postural stability was obtained with retention of the vertical posture.

Loss of functional activity after strokes occupies the leading position among the main causes of disability in the adult population throughout the world. Particular attention is currently paid to poststroke cognitive disorders. Construction of rehabilitation programs increasingly employs approaches based on multimodal treatments, which allows comprehensive coverage of the whole spectrum of ongoing neurological deficits in patients and provides the opportunity for more effective restoration of functional activity after stroke.

Stroke is the leading cause of disability in adults. Depression after a stroke is detected in one third of patients, complicating physical rehabilitation, worsening functional outcome, increasing mortality rates. The question of the use of antidepressants in the treatment of post-stroke depression is currently not completely resolved, there is no consensus on the most optimal drug. The drugs of choice are selective serotonin reuptake inhibitors, the use of tricyclic antidepressants is possible. A number of clinical studies indicate the effectiveness of selective serotonin reuptake inhibitors in the treatment of post-stroke depression, including through mechanisms including increased neuroplasticity and stimulation of neurogenesis, while others disprove their effectiveness. The article presents a clinical case of the use of vortiroxetin in the patient’s neurorehabilitation in the early recovery period of a stroke, its safety and positive effect are shown.

The process of the functional rearrangement of the motor cortex of the brain after stroke is due to neuroplasticity, and this underlies motor recovery. Functional magnetic resonance imaging (fMRI) and electroencephalography (EEG) are currently recognized as the most informative methods for studying these processes. The course of the neuroplastic process can be evaluated from the power levels of EEG rhythms during imagination of movements in the paralyzed arm in right-handed patients after stroke in the left hemisphere monitored at different times – before and after courses of neurorehabilitation using a brain–computer interface controlling a wrist exoskeleton. Powerful excitatory interactions in the primary motor cortex and frontoparietal areas in the lesioned and “intact” hemispheres are initially seen, and these probably reflect reorganization of neural networks. Rehabilitation courses were followed by restoration of bioelectrical activity in the primary motor cortex due to recovery of efficient connections with the premotor and superior parietal zones and decreases in the pathological influences of the contralateral hemisphere.

The need for optimal treatment of the active forms of multiple sclerosis (MS), especially highly active MS (HAMS), poses a number of difficult problems for specialists, including not only the choice of a highly effective disease-modifying drug for MS (DMDMS), but also conditions for the timely and safe switching from other DMDMS. A group of expert neurologists from various clinics in Russia presents a consensus on the practical aspects of switching patients with HAMS from other DMDMS to cladribine tablets, which has been registered in Russia in March 2020. The paper discusses indications for changing therapy and gives indications, contraindications, and conditions for initiating cladribine therapy. It details the procedure and results of the expert consensus, on the basis of which the recommendations have been developed to switch to cladribine tablets from each of the DMDMS registered in Russia for the treatment of MS.

A brain–computer interface (BCI) used to control a hand exoskeleton provides a tool for rehabilitation of the arm motor function (MF) after stroke and has proven efficacy and a potential to stimulate brain neuroplasticity. A study was made to analyze the effect of repeated rehabilitation courses with a BCI + exoskeleton (2 to 9 months after the first course) on the MF restoration in the late recovery period. MF recovery was assessed using a biomechanical analysis of the patient’s movements and clinical scales: the Fugl-Meyer Assessment (FMA) scale, the Action Research Arm Test (ARAT), and the Medical Research Council Weakness Scale sum score (MRC-SS). A positive effect of repeated rehabilitation courses with a BCI + exoskeleton on the MF recovery was observed in both patients with moderate paresis in the late recovery period and patients with severe paresis. The data may be useful for developing an appropriate protocol for the rehabilitation procedures that employ the exoskeleton controlled via a BCI with kinesthetic imagination of movements.

Background: Robotic mechanotherapy is considered as a promising area of physical rehabilitation of multiple sclerosis patients, while it ensures high training efficacy. Aim: To study the effect of physical training using with the ExoAtlet exoskeleton for lower extremities the functioning of patients with multiple sclerosis. Materials and methods: This was a prospective, open, uncontrolled, single center study. The rehabilitation course with the ExoAtlet exoskeleton included 43 patients (14 male and 29 female, aged from 28 to 59 years, mean age 43,5 ± 9,12 years) with remitting multiple sclerosis in remission (RMS-R) (n = 20) and secondary progressive course (VPRS) (n = 23), with the EDSS scores from 3 to 8. One of the inclusion criteria was the presence of motor paresis of the lower extremities. Training with the ExoAtlet exoskeleton was performed 5 days a week for two weeks. The neurological deficits and functioning were assessed with the Kurtzke expanded disability status scale (EDSS), the multiple sclerosis functional composite (MSFC) test, including the assessment of walking (Timed 25 Footwalk), of upper limb functions (9-Hole PegTest, 9-HPT), and of mental functioning (Symbol Digit Modalities Test, SDMT) before and after the rehabilitation course. Cognitive functions were assessed by the Montreal Cognitive Assessment (MoCA) scale. Results: The rehabilitation course resulted in a significant decrease of neurological deficiency by EDSS (by 0.26 score, 5%, p < 0.001). The MSFC test showed an improvement in all subtests: SDMT by 2 points, or 4.9% (p = 0.018), Timed 25-Footwalk by 3.2 seconds, or 19.6% (p < 0.001), 9-HPT for the dominant hand by 1.6 seconds, or 5% (p = 0.004), and for the non-dominant hand by 2.1 seconds, or 6.2% (p = 0.006). The improvement in the MoCA test after the rehabilitation course was 1.6 points, or 6% (p < 0.001). Conclusion: The study confirmed the positive effect of the exoskeleton in the lower extremities, such as restoration of the walking function in multiple sclerosis patients. There was a positive trend towards restoring of hand motor skills and cognitive functions.

A clinical case of a 27-year-old patient with distal arthrogryposis of the 5th type – arthrogryposis with ophthalmoplegia, which was combined in a patient with polyneuropathy is presented. To assess tissue respiration (mitochondrial respiratory chain) and other types of metabolism in mitochondria, cytochemical analysis of lymphocytes in peripheral blood was carried out according to A. Pearse's method modified by R.P. Narcissov. The activity of four mitochondrial enzymes involved in carbohydrate metabolism (lactate dehydrogenase), amino acid metabolism (glutamate dehydrogenase), fatty acid metabolism (α-glycerophosphate dehydrogenase) and the second complex of the mitochondrial respiratory chain (succinate dehydrogenase) was assessed. A slight decrease in the activity of the enzyme succinate dehydrogenase, which is part of the second complex of the mitochondrial respiratory chain, was determined. The activity of the enzyme α-glycerophosphate dehydrogenase was more significantly reduced, an increase in the activity of lactate dehydrogenase was noted. In the presented observation, along with the typical manifestations of the disease (contractures of the hands and feet, ophthalmoplegia, ptosis of the eyelids, visual impairment), polyneuropathy with impaired sensitivity of the polyneuritic type was revealed. Thus, a patient with polyneuropathy had a hereditary type 5 arthrogryposis disease. Along with the typical manifestations of the disease, polyneuropathy with hyporeflexia and impaired sensitivity of the polyneuritic type was revealed. Secondary mitochondrial disorders were identified, which was the basis for the appointment of energotropic therapy.

The article presents the results of safety fi ndings during international multicenter randomized double-blind, active and placebo-controlled, comparative phase 3 trial. 158 patients with relapsing-remitting multiple sclerosis were randomly assigned into 3 groups: Timexon (glatiramer acetate, manufactured by JSC «BIOCAD», Russia), copaxone-Teva (Teva Pharmaceutical Enterprise Co., Ltd., Israel) and placebo, at a ratio of 2:2:1, respectively. At the second group 63 patients received Copaxone-Teva, after 48 weeks of therapy they received Timexon. Switching between therapy was not associated with adverse eff ect frequency. There was no clinically signifi cant diff erences in profi le and frequency of adverse eff ects between the groups of Copaxone-Teva and Timexon. Also, effi cacy analysis of therapy demonstrated no diff erences between timexone group and Copaxone-Teva group in both MRI parameters and frequency of relapses. The data obtained from the present study confi rm the equivalence in safety of Timexon (CJSC BIOCAD, Russia) and Copaxone-Teva, that is important for further implementation of glatiramer acetate generic in the clinical practice of multiple sclerosis therapy.

INTRODUCTION In this study an attempt was made to determine some reasons that prompted the urologist to make a choice in favor of particular drug or combination from a wide variety of available drugs. The prescribing habits and subjective evaluation of efficiency and safety of the drugs for the treatment of lower urinary tract symptoms/benign prostatic hyperplasia by the urologists in Russia was analyzed. MATERIALS AND METHODS a prospective multicenter epidemiological study "LUTS/BPH - who treats?" was carried out in 12 centers using data obtained from May 1 to July 31, 2018 with anonymous survey of 500 urologists living in 9 cities of the Russian Federation. The survey consisted of 46 questions reflecting a level of education, involvement in scientific life of urologic community, membership in various professional organizations, and subjective assessment of safety and efficiency of drugs used in Russian Federation for the treatment of urinary disorders. The obtained data was analyzed for a whole group and further in the subgroups. RESULTS Alpha-blockers are the most commonly prescribed drugs. These drugs constitute 63.4% of all appointments, followed by the 5-a-reductase inhibitors (23.98%) and combined therapy by the both drugs (24.68%). The most frequently used alpha-blocker is tamsulosin, but professors, MD and urologists, who read special literature at a regular basis, prescribed tamsulosin and silodosin equally. In primary cases and in patients with concomitant cardiac pathology, silodosin was prescribed more commonly in comparison with other alpha-blockers. CONCLUSION the results of the study indicate a concordance with global trends in the treatment of these patients. Our data reflect a significant influence of the level of education, work experience and involvement in scientific professional life of urologic community on the choice of a particular drug for the treatment of LUTS/BPH and prove the feasibility of further work which should be aimed at popularization of the modern medical knowledges among the urologists and organization of a process of continuous medical education.

The project is aimed at investigating efficacy of a BCI-controlled palm exoskeleton as a tool for motor function recovery in post-stroke patients. The idea of using the system is grounded on vast amount of data supported by physiologic literature and our own findings in healthy subjects, suggesting that kinesthetic motor imagery (MI) requires activation of the brain areas involved in motion planning, execution and control. Thus, the common idea of using a MI-based BCI for neurorehabilitation is to reinforce motor imagery of intention to move with visual, proprioceptive and\or tactile feedback. Results of a four-year multi-center randomized controlled study of post-stroke motor rehabilitation procedure with BCI-controlled hand exoskeleton complex are presented. The study has the largest number of participants so far. Statistical analysis of different clinical scales used to assess motor function recovery show that incorporating the BCI+exoskeleton procedure into rehabilitation significantly improves its outcome. The analysis also revealed non-monotonical dependency of motor function recovery rate on initial motor and sensory function status, as well as on age, and BCI control accuracy. Hopefully, the reported data combined with the results obtained by other groups in the world, would provide solid evidence supporting inclusion of the BCI-based systems into rehabilitation practice.

The rehabilitation course included exercises on an ExoAtlet exoskeleton in group 1 and exercises on a pedal simulator for active-passive training (5 days a week for 2 weeks) in group 2. Several tests were used to evaluate treatment results, including the Hauser walking index, the 10-meter walking test, the Berg balance scale, stabilometry, and biomechanics of walking. The complete training course was completed by 20 patients of group 1 and 21 of group 2.There was a significant increase in strength in paretic muscles, postural stability, functional level and walking speed in patients of both groups, but in patients of group 1, the dynamics of recovery was more pronounced (The study revealed a positive impact of hardware rehabilitation on locomotion, both with the use of an exoskeleton and an active-passive pedal simulator. The use of an exoskeleton, have the advantages resulting in a significantly greater recovery of strength, stability, speed and symmetry of walking over the same period of training. A significant increase in postural stability in vertical position was revealed.Сравнение эффективности восстановления функции ходьбы у пациентов в раннем восстановительном периоде ишемического инсульта (ИИ) с использованием экзоскелета для нижних конечностей и активно-пассивного педального велотренажера.Проведено открытое рандомизированное исследование 47 пациентов в раннем восстановительном периоде ИИ. В 1-й группе 1 реабилитационный курс включал занятия на экзоскелете ExoAtlet, во 2-й — занятия на педальном тренажере для активно-пассивных тренировок (5 дней в неделю в течение 2 нед). Для оценки результатов лечения использовали ряд тестов, включая индекс ходьбы Хаузера, 10-метровый тест ходьбы, шкалу баланса Берга, стабилометрию, биомеханику ходьбы. Полный курс тренировок закончили 20 пациентов 1-й группы и 21 — 2-й группы.После завершения курса тренировок отмечено статистически значимое нарастание силы в паретичных мышцах, постуральной устойчивости, функционального уровня и скорости ходьбы у пациентов обеих групп, однако у пациентов 1-й группы динамика восстановления была статистически более выражена (В результате проведенного исследования выявлено положительное воздействие аппаратной реабилитации на локомоцию с использованием как экзоскелета, так и активно-пассивного педального тренажера, но при использовании экзоскелета получены преимущества, проявляющиеся в статистически значимо большем восстановлении силы, устойчивости, скорости и симметричности ходьбы за аналогичный период тренировок. Выявлено существенное увеличение постуральной устойчивости при сохранении вертикальной позиции.

To study an effect of robotic mechanotherapy on the functional condition of patients with multiple sclerosis (MS) with impaired walking function using the ExoAtlet exoskeleton.A prospective open-source, uncontrolled, single-center study included 44 patients (main group, MG) with remitting course in remission (RRS) and secondary-progressive course (VPRS) of MS with 3 to 8 points on the Extended Disability Status Scale (EDSS). Eighteen people (subgroup 1) of this group were re-examined after 3 months and 8 people completed the second rehabilitation course (subgroup 2). Neurological deficits and functional condition of patients were assessed using the EDSS, the Multiple sclerosis functional composite (MSFC) test, which includes the assessment of walking - Timed 25- Footwalk, the assessment of upper limb functions - 9-HolePegTest (9-HPT), the assessment of mental abilities - Symbol Digit Modalities Test (SDMT). Additionally, cognitive functions were assessed by the Montreal Cognitive Assessment Scale (MoCA).The improvement in the EDSS score was 0.23 points (5%,The results have shown the effectiveness regarding the problems in MS patients and the prospects for further study of the possibilities of robotic assistance to walking and support a vertical posture using the ExoAtlet exoskeleton in order to restore walking function in MS patients with motor deficits.Изучение влияния роботизированной механотерапии с использованием экзоскелета ExoAtlet на функциональное состояние пациентов с рассеянным склерозом, имеющих нарушение функции ходьбы.Проспективное открытое неконтролируемое одноцентровое исследование проведено с участием 44 пациентов (основная группа — ОГ) с ремиттирующим течением в стадии ремиссии и вторично-прогредиентным течением рассеянного склероза с уровнем неврологического дефицита по шкале EDSS от 3 до 8 баллов. Из них 18 больных (1-я подгруппа) были осмотрены повторно через 3 мес после окончания курса реабилитации и 8 прошли повторный курс (2-я подгруппа). Неврологический дефицит и функциональное состояние пациентов оценивали, используя шкалу EDSS, комплексную функциональную шкалу рассеянного склероза (MSFC), включающую оценку ходьбы — ходьба на 25 футов (Timed 25-Foot walk), оценку функционального состояния верхней конечности — 9-луночный тест (9-HPT), оценку когнитивных функций — символьно-числовой тест (SDMT). Дополнительно когнитивные функции оценивали с помощью MoCA теста.Улучшение показателя EDSS в среднем составило 0,23 балла, 5% (Результаты данного исследования показали эффективность в отношении решения ряда проблем, которые имеются у пациентов с рассеянным склерозом, и перспективность дальнейшего изучения возможностей роботизированного ассистирования ходьбе и поддержанию вертикальной позы с помощью экзоскелета ExoAtlet с целью восстановления функции ходьбы.

Loss of functional activity after a stroke is a leading cause of disability in adults worldwide. Particular attention is currently being paid to post-stroke cognitive impairment. Approaches based on multimodal exposure are increasingly being used when planning rehabilitation programs, which makes it possible to comprehensively cover the entire spectrum of existing neurological disorders in patients and enables to achieve a more effective recovery of functional activity after a stroke.Потеря функциональной активности после инсульта занимает ведущие позиции среди основных причин инвалидизации у взрослого населения во всем мире. Особое внимание на сегодняшний день уделяется постинсультным когнитивным расстройствам. При построении реабилитационных программ все чаще используются подходы с применением принципа мультимодального воздействия, что позволяет комплексно охватить весь спектр имеющихся неврологических нарушений у больных и дает возможность добиться более эффективного восстановления функциональной активности после инсульта.

The aim of the present work was to study the effects of multiple courses of “BCI + hand exoskeleton” neurorehabilitation on the restoration of movements and resocialization of patients during the fi rst year after stroke. The study included seven patients with cerebral stroke who received two or more courses of sessions. All patients had poststroke hemiparesis of severity 1–4 points. Multiple courses of “BCI + hand exoskeleton” neurorehabilitation led to maintenance of the level of motor activity with further improvements, repeated courses yielding signifi cantly better results than single courses. Appropriate personal interpretation of illness, a positive attitude to recovery, and being motivated to take active part in the rehabilitation process were fundamental for effective rehabilitation of patients.

Objectives. To demonstrate equivalence of the efficacies of the drugs Teberif (BCD-033, interferon β-1a) and Rebif (interferon β-1a) in patients with remitting multiple sclerosis (RMS). Materials and methods. A multicenter, double-blind, placebo-controlled, comparative, randomized phase III trial included 163 patients with diagnoses of MS. Patients were randomized to the Teberif, Rebif, and placebo groups at a ratio of 1:1:1. Results and conclusions. Analysis of efficacy after 52 weeks of the trial demonstrated equivalence between the study drug Teberif and the original formulation Rebif in patients with RMS. Evaluation of primary endpoint results – CUA (combined unique active lesions, i.e., the total number of MRI T1 plaques and new T2 plaques or cases of increases in T2 plaques without double counting) – demonstrated that there were no significant differences (0.727 ± 1.042 and 0.652 ± 1.059, p = 0.7354, Student’s t test) between the Teberif and Rebif groups. There were no statistically significant between-group differences in other MRI indicators or measures associated with exacerbations. The safety profile and tolerance of Teberif were satisfactory and comparable with the safety and tolerance profile of Rebif. These data provide evidence of therapeutic equivalence of these drugs, which may provide grounds for the use of the interferon β-1a bioanalog in patients with RMS.

Objectives. To study the efficacy of complex therapy including daily speech therapy and two courses of Cellex in patients with aphasia in the early recovery period of ischemic stroke (IS). Materials and methods. Forty patients in the early recovery period of IS in the basin of the left middle cerebral artery with moderate or severe aphasia were studied. Twenty patients received complex therapy, including daily sessions with a speech therapist/aphasia specialist for 10 days using an improved method followed by self-study using educational materials and two 10-day courses of Cellex injections; the other 20 patients received only speech therapy. Treatment efficacy was evaluated using an automatic “Aphasia patient study program,” the Goodglass–Kaplan scale, and the modified Rankin scale. Results and conclusions. Patients of both groups achieved significant improvements in speech functions, communicative abilities, and functional recovery (p < 0.01); patients receiving complex therapy including two courses of Cellex injections showed significantly higher levels of rehabilitation (p < 0.05). These results allow the inclusion of Cellex into the complex restorative treatment of patients with poststroke speech impairments to be recommended.

Objective. To seek evidence that Timexon (BCD-063, glatiramer acetate, Biocad, Russia) and Copaxone-Teva (Teva Pharmaceuticals Ltd., Israel) have similar efficacies in patients with remitting multiple sclerosis. Materials and methods. A multicenter, double-blind, placebo-controlled, comparative, randomized, phase III study included 158 patients with confirmed diagnoses of remitting multiple sclerosis. Patients were randomized to the BCD-063, Copaxone-Teva, and placebo groups at a ratio of 2:2:1. Results and conclusions. Efficacy analysis at 48 weeks of treatment demonstrated that there were no differences between the BCD-063 and Copaxone-Teva groups in terms of MRI parameters or exacerbation frequency. Assessment of the primary endpoint (number of MRI-confirmed exacerbations per patient per year) showed that the mean number of exacerbations was 0.098361 (0.351422) in the BCD-063 group, 0.098361 (0.351422) in the Copaxone-Teva group, and 0.178571 (0.390021) in the placebo group. Assessments on the EDSS and MSFC also demonstrated that there were no differences between the BCD-063 and Copaxone-Teva groups. Both BCD-063 and Copaxone-Teva had favorable safety profiles. These data provide evidence of the therapeutic equivalence of BCD-063 (Biocad, Russia) and Copaxone-Teva, which is an important aspect for further introduction of the reproduced glatiramer acetate formulation into the treatment of multiple sclerosis.

Objective. To study the characteristics of the diagnosis and treatment of non-alcoholic-related Wernicke–Korsakov syndrome (WKS). Materials and methods. Eight patients (five men and three women) with mean age 38.9 ± 1.4 years with WKS were observed; WKS in three patients developed on the background of acute and two on the background of exacerbations of chronic gastrointestinal tract diseases with malabsorption, and in three after surgery to the upper segments of the gastrointestinal tract Results. Illness debuted with impaired consciousness. Symptoms of ataxia, oculomotor disorders, and bulbar syndrome started after 24–48 h. One patient developed a treatment-resistant state of tonic-clonic seizures. The brain MRI picture was characterized by the presence of hyperintense foci on T2-weighted images in the areas of the hypothalamus, mammillary bodies, brainstem, and cerebellum, with accumulation of contrast in the mammillary bodies. Patients received treatment with a complex of group B vitamins (Neurobion) and thiamine, with positive results Conclusions. Patients with gastrointestinal tract diseases accompanied by malabsorption are at risk of developing WKS. The appearance of changes in consciousness, ataxia, and oculomotor disorders in these patients is an indication to start thiamine therapy, thus preventing the development of stable cognitive deficit.

The rehabilitation potential of post-stroke patients was evaluated after a rehabilitation procedure using a hand exoskeleton controlled via a brain–computer interface (BCI). Examples are given for parameters describing the motor and cognitive functions and the capacity for kinesthetic movement imagination. It is emphasized that instrumental quantitative methods are important to use for adequate assessment of both the rehabilitation potential and the effectiveness of the BCI + exoskeleton procedure.

The two-year study of the efficacy of Ifnb-1B (infibeta) in patients with relapsing-remitting (RRS) and secondary progressive multiple sclerosis (SPMS) in the Moscow region with the criteria NEDA-3.Three hundred patients, 81 men and 219 women, aged from 18 to 67 years, including 227 with RRS and 73 with SPMS, were studied. The duration of disease was from 0.5 to 39 years, the level of neurological deficit on EDSS scale was from 1 to 6.5 points. All patients received infibeta.During the treatment with infibeta, the chance of exacerbation decreased by an average of 39.4 times (OR 39.4 95% CI; 16.6-122.7) during the first year, by 35.3 times (OR 35.3; 95% CI 13.5-131,2) during the second year. A statistically significant decrease in the probability of exacerbation is also confirmed in the conditional logistic regression model (p0.001). MRI revealed the signs of exacerbation and progression of the disease only in 11.5% of patients; 54.9% of the patients met the criteria NEDA-3. Материал и методы. Обследованы 300 пациентов (81 мужчина и 219 женщин) в возрасте от 18 до 67 лет, из них 227 с РРС и 73 - с ВПРС. Длительность заболевания - от 0,5 до 39,0 года, уровень неврологического дефицита по шкале EDSS - 1-6,5 балла. Все принимали препарат инфибета. Результаты и обсуждение. На фоне лечения инфибетой риск наступления обострения снизился в среднем в 39,4 раза (ОШ 39,4; 95% ДИ 16,6-122,7) в течение 1-го года, в 35,3 раза (ОШ 35,3; 95% ДИ 13,5-131,2) в течение 2-го года терапии. To assess the possibility of using the integral indicators of thrombodynamics to improve the safety of rehabilitation measures in patients in the recovery period of stroke.A prospective study included 52 patients in the recovery period of stroke. To compare the effect of the intensity of physical activity on the change in the integral indicators of thrombodynamics in accordance with the objectives of the study, the patients were randomized into two groups. In the first group, patients received treatment, including physiotherapy, stabilometric trainings, neuropsychological tasks. Patients of the second group received complex treatment, including more intense physical activity with the use of cyclic exercises, biomechanotherapy. To assess the state of hemostasis, an integral thrombodynamics test was used, and the numerical parameters of the spatial dynamics of the growth of a fibrin clot were calculated.The state of compensated hypercoagulability was revealed in the majority of patients before the start of medical rehabilitation, despite taking antithrombotic therapy, which was evidence of the ineffectiveness of the therapy. At the same time, it was noted that it was its action, probably during the ongoing program of physical rehabilitation, that ensured the stability of the integral indicators of thrombodynamics and the absence of clinically significant unwanted thromboembolic complications. As a result of the analysis of the data in dynamics before and after treatment, a direct relationship was traced between the intensity of physical activity and the tendency to disturb the hemostatic balance.The intensity of physical activity during treatment undoubtedly influenced the state of hemostasis. The expediency of monitoring the dynamics of coagulation activity in patients in the recovery period of stroke has been demonstrated.Оценка возможности использования интегральных показателей тромбодинамики для повышения безопасности проведения реабилитационных мероприятий у пациентов в восстановительном периоде церебрального инсульта.В проспективное исследование включены 52 пациента в восстановительном периоде инсульта. Методом простой рандомизации пациенты были разделены на две группы в соответствии с задачами исследования: сравнить влияние интенсивности физических нагрузок на изменение интегральных показателей тромбодинамики. В первой группе больные получали лечение, включающее физиотерапию, стабилометрические тренинги, нейропсихологические задания. Пациенты второй группы получали комплексное лечение, включающее более интенсивные физические нагрузки с использованием роботизированной биомеханотерапии. Для оценки состояния гемостаза использовали интегральный тест тромбодинамики, рассчитывали численные параметры пространственной динамики роста фибринового сгустка.В нашей работе было выявлено состояние компенсированной гиперкоагуляции у большинства больных до начала мероприятий медицинской реабилитации, несмотря на прием антитромботической терапии, что является свидетельством неэффективности проводимого лечения. При этом отмечено, что, вероятно, именно действие антитромботической терапии обеспечило стабильность интегральных показателей тромбодинамики и отсутствие клинически значимых нежелательных тромбоэмболических осложнений в ходе проводимой программы физической реабилитации. В результате анализа данных до и после лечения была прослежена прямая зависимость между интенсивностью физической нагрузки и тенденцией к нарушению гемостатического баланса.В результате проведенного исследования было выявлено, что интенсивность физических нагрузок в ходе лечения оказывала влияние на состояние гемостаза. Продемонстрирована целесообразность мониторинга динамики коагуляционной активности у пациентов в восстановительном периоде инсульта.

Stroke is the leading cause of disability and death in the adult population. Modern methods of treating patients with acute ischemic stroke include thrombolytic therapy with a narrow therapeutic window and endovascular thrombectomy. The development of other methods of treatment of brain hypoxia in the penumbra zone is relevant. Mitochondria, which are involved in immediate and delayed molecular mechanisms of adaptation to hypoxic stress in the cerebral cortex, primarily respond to hypoxia. Hypoxia induces reprogramming of the mitochondrial respiratory chain function and switching from oxidation of substrates of the respiratory chain complex I to succinate oxidation (complex II). The brain's need for succinate increases. Clinical studies have shown a positive effect of drugs containing succinates on the course of stroke. The study of mitochondrial function is carried out mainly in an experiment. In the present study of mitochondrial disorders in stroke and chronic brain ischemia in adult patients, the quantitative method proposed by A. G. Pearse was used to assess the activity of mitochondrial enzymes of peripheral blood lymphocytes, which are referred to as the "enematic mirror" of tissues. In acute cerebral ischemia, a compensatory increase in the activity of succinate dehydrogenase was observed on the first day, indicating the tension (increased activity) of the second complex of the mitochondrial respiratory chain.. These data confirm the need to prescribe succinic acid in the acute phase of stroke. At the same time, the dose of 250 mg per day is not sufficient for patients with increased body weight. The standard dose of the drug should be higher, taking into account the different body weight of patients. In patients with stroke, there was also a decrease in the activity of α-glycerophosphate dehydrogenase, which is involved in the fat metabolism of mitochondria, which is an indication for the appointment of carnitine. In chronic brain ischemia, the activity of succinate dehydrogenase and α-glycerophosphate dehydrogenase decreased, indicating indications for the appointment of idebenone and carnitine along with vasodilator therapy and endovascular thrombectomy. Thus, the results of a study of mitochondrial function in patients with acute and chronic brain ischemia are presented. Violations of complex II in the respiratory chain cycle and violation of fat metabolism were revealed, indicating indications for the appointment of energotropic therapy.

Objective. To study the potential for use of a bioengineered system consisting of an electroencephalograph, a personal computer running a program for the synchronous data transmission, recognition, and classification of electroencephalogram (EEG) signals, and formation of control commands in real time, combined with a hand exoskeleton (a bioengineered “brain–computer interface (BCI) with exoskeleton” system) for the motor rehabilitation of patients with poststroke upper limb paresis. Materials and methods. Brain–computer interfaces have potential for use in neurorehabilitation. A total of five patients with poststroke upper limb paresis received neurorehabilitation courses consisting of 8–10 sessions. All the patients had large foci of poststroke changes of cortical-subcortical locations as demonstrated by MRI scans. Results. Improvements in neurological status on the NIHSS were seen after courses of sessions, with significant increases in the volume and strength of movements in the paralyzed hand, improvements in the coordination of its movements, and minor decreases in the level of spasticity. There was an increase in daily activity on the Barthel index, mainly due to improvement in fi ne motor function. Levels of disability showed clear changes on the modified Rankin scale. Conclusions. Use of the “brain–computer interface (BCI) with exoskeleton” system in the rehabilitation of patients with poststroke paresis of the hand gave positive results, pointing to the need to continue these studies.

Background: Non-motor signs, including affective and cognitive ones, are important initial manifestations of Parkinsons disease, along with motor dysfunction. Some of them manifest even before motor disorders and affect patients’ quality of life. Aim: To evaluate prevalence and severity of affective and cognitive disorders at early stages of Parkinson’s disease, as well as efficacy of their correction with a dopamine receptor agonist treatment. Materials and methods: We assessed 33 treatment-naive patients with Parkinsons disease Hoehn & Yahr stage I and II. Degree of motor dysfunction (UPDRS, Unified Parkinson's Disease Rating Scale), depressive (MADRS, Montgomery Asberg Depression Rating Scale), anxiety (HARS, Hamilton Anxiety Rating Scale) and cognitive (Montreal Cognitive Assessment – MoCA, symbols and figures test, Schulte tables, Gorbovs black and red tables) was assessed at baseline and after 1, 3 and 6 months of treatment with pramipexole. Results: Patients with early signs of Parkinson’s disease had subdepressive disorders, moderate anxiety disorders, mild cognitive disorders of a neurodynamic type and represented mainly be delayed recall of information and attention disorders. Treatment with pramipexole decreased the degree of depressive symptoms by 82% and of anxiety symptoms by 74%. There was an improvement in cognitive functions, indicated by an increase in a delayed recall (+34%), better efficacy (+31%), a decrease in switching time (-38%) and attention distribution (-33%); however, age-related normal values were not achieved. Conclusion: Already early stages of Parkinsons disease are characterized not only by motor abnormalities, but also by affective and neurodynamic cognitive dysfunction. Medical treatment with pramipexole was effective in correction of motor and neuropsychiatric disorders in patients with Parkinsons disease Hoehn & Yahr stage I–II.

Cognitive impairment is common in poststroke patients. Today in rehabilitation programs the specialists use the vestibular stimulation including biological feedback to supporting reaction for treatment poststroke cognitive impairment. These studies show the relationship of vestibular function with memory, attention, spatial orientation, navigation, mental representation of three-dimensional space and other cognitive functions. It makes possible to build rehabilitation programs for patients with stroke.Когнитивные нарушения широко распространены у пациентов после инсульта. На сегодняшний день для коррекции этих расстройств в реабилитационных программах все чаще используются подходы с применением вестибулярной стимуляции, в том числе на основе биологической обратной связи по опорной реакции. Данные исследований свидетельствуют о связи вестибулярной функции с процессами обеспечения памяти, внимания, пространственной ориентации, навигации, мысленного представления трехмерного пространства и других когнитивных функций, что дает возможность построения реабилитационных программ для пациентов с инсультом.

Alemtuzumab (Lemtrada) is a recombinant humanized IgG1 kappa monoclonal antibody to the surface cell glycoprotein, a CD52 differentiation cluster. The drug is approved for use in more than 65 countries, including the Russian Federation. The drug is one of the most effective methods of treating patients with aggressive multiple sclerosis, but the risk management plan should be followed. The safety profile of the drug includes infusion-associated reactions, thyroid dysfunction, immune cytopenia, acute cardiovascular events, infections, and other autoimmune diseases. The main results of a medical-sociological study aimed at the evaluation of the level of satisfaction with treatment in patients with primary progressive multiple sclerosis (PPMS) are presented. The survey was conducted in 19 regions of the Russian Federation and involved 437 patients with confirmed diagnosis of PPMS and 80 neurologists specialized in multiple sclerosis. The research was carried out by the All-Russian Public Organization of Disabled People with Multiple Sclerosis with the participation of the Russian Committee for Treatment and Research in Multiple Sclerosis. This is the first medical-sociological study in the Russian Federation involving patients with PPMS and neurologists. The study revealed typical medical and social problems patients with PPMS faced. A significant decrease in quality-of-life of PPMS patients was shown.Представлены основные результаты медико-социологического исследования, проведенного с целью изучения уровня удовлетворенности терапией пациентов с первично-прогрессирующим рассеянным склерозом (ППРС). В опросе, проведенном в 19 регионах Российской Федерации, приняли участие 437 пациентов с установленным диагнозом ППРС и 80 неврологов, специализирующихся в области данного заболевания. Исследовательский проект был осуществлен Общероссийской общественной организацией инвалидов - больных рассеянным склерозом (ОООИБРС) при научном участии Российского комитета исследователей рассеянного склероза (РОКИРС). Это первый опыт проведения медико-социологического исследования на территории РФ, охватывающий и больных ППРС, и неврологов. Выявлены типичные медицинские и социальные проблемы, с которыми сталкиваются пациенты с ППРС, отмечено существенное снижение качества жизни данной группы больных.

To study the efficacy of the complex therapy, including cocarnit (group B vitamins, triphosadenine and nicotinamide), of diabetic neuropathy.Forty-one patients with diabetes mellitus type 2 and distal symmetric sensorimotor polyneuropathy were examined. Patients were divided into 2 groups. Patients of the main group (n=26) received complex therapy, including cocarnit, and patients of the comparison group (n=15) received standard treatment.The positive dynamics based on the VAS (р=0.0001), TSS (р=0.0001), NSS (р=0.001), NDS (р=0.0431), SF-36 (р=0.0008), electroneuromyographic results and glycated hemoglobin levels was observed in the main group. In patients of the comparison group, the positive dynamics was instable; the scores of clinical scales did not reach statistical significance. The results suggest the use of cocarnit in the complex treatment of patients with diabetic neuropathy.Цель исследования. Изучение эффективности комплексной терапии, включающей кокарнит, в состав которого входят витамины группы В, трифосаденин и никотинамид, у пациентов с диабетической нейропатией. Материал и методы. Обследован 41 пациент с наличием дистальной симметричной сенсомоторной полинейропатии на фоне сахарного диабета 2-го типа. Были созданы две группы: 26 пациентов основной группы получали в комплексном лечении препарат кокарнит, 15 больных группы сравнения - стандартную терапию. Результаты и заключение. У пациентов основной группы на фоне терапии была выявлена положительная динамика по данным визуальной аналоговой шкалы (р=0,0001), шкалам TSS (р=0,0001), NSS (р=0,001), NDS (р=0,0431), SF-36 (р=0,0008), электронейромиографических показателей, уровню гликированного гемоглобина. У пациентов группы сравнения отмечалась нестойкая положительная динамика субъективной неврологической симптоматики, при этом показатели клинических шкал не достигали степени достоверности. Полученные результаты позволяют рекомендовать использование препарата кокарнит в комплексном лечении пациентов с диабетической нейропатией.

To prove the equivalent efficacy of teberif (BCD-033, interferon beta-1) and rebif (interferon beta-1a) in patients with remitting multiple sclerosis (RMS).A multicenter double blind placebo-controlled comparative randomized III phase study included 163 patients with RMS. Patients were randomized into three equal groups (teberif, rebif or placebo).After 52 weeks, the equivalent efficacy of teberif and the brand drug rebif was shown. The result of assessment of the primary endpoint, which was combined unique active (CUA) lesion (the total of MRI T1-weighted lesions and new or newly enlarging T2-weighted lesions, without double counting of lesions with both activities), showed no significant differences (0.727±1.042 and 0.652±1.059 (p=0.7354, t-Student test) in the teberif and rebif groups, respectively. No between-group differences were found for other MRI indices and clinical parameters related with relapses. Teberif was shown to have a favorable safety and tolerability profile comparable to that of rebif. The results suggest the therapeutic equivalency of the drugs and form the basis for using the bioanalogue of interferon-beta 1 in patients with RMS.Цель исследования. Доказательство эквивалентной эффективности препаратов тебериф (BCD-033, интерферон бета-1а) и ребиф (интерферон бета-1а) у пациентов с ремиттирующим рассеянным склерозом (РРС). Материал и методы. В многоцентровое двойное слепое плацебо-контролируемое сравнительное рандомизированное исследование III фазы были включены 163 пациента с диагнозом РС. Рандомизация в группы тебериф, ребиф и плацебо проводилась в соотношении 1:1:1 соответственно. Результаты и заключение. При анализе эффективности после 52 недель исследования была установлена эквивалентность исследуемого препарата тебериф и оригинального препарата ребиф у пациентов с РРС. По результатам оценки первичной конечной точки - показателю CUA (совокупное количество очагов на МРТ в Т1-режиме и новых очагов в Т2-режиме или случаев увеличения очагов в Т2-режиме без двойного сложения), было установлено отсутствие статистически значимых различий (0,727±1,042 и 0,652±1,059; p=0,7354, t-критерий Стьюдента) в группах тебериф и ребиф соответственно. По другим показателям МРТ, а также по показателям, связанным с обострениями, не было выявлено статистически значимых различий в группах. Установлен благоприятный профиль безопасности и переносимости теберифа, сопоставимый с профилем безопасности и переносимости ребифа. Полученные данные свидетельствуют о терапевтической эквивалентности препаратов, что может служить основанием для применения биоаналога интерферона бета-1а у пациентов с РРС.

Systemic thrombolytic therapy (STLT) is an available method of treatment of ischemic stroke (II) with proven efficacy. The efficacy and safety of STLT in certain groups of patients, in particular with diabetes mellitus type 2 (T2DM), are being discussed. To compare the efficacy and safety of STLC in II patients with T2DM, hyperglycemia and normoglycemia.Eighty-six patients were examined, including 23 with T2DM (7 newly diagnosed), 27 with hyperglycemia at stroke onset and 36 with normoglycemia. Cardioembolic subtype of II was diagnosed in 35%, atherothrombotic in 29%, lacunar in 7%. In other patients, the subtype was unknown. STLT was administered to all patients 153.8±4.7 min after stroke onset.At admission, the NIHSS severity of II and the level disability assessed by the Rankin scale did not differ in the three groups. A significantly lower regression was observed in patients with T2DM (p0.01) in the 28th day. The fatality was higher in this group compared to the patients with normoglycemia (p0.05). The frequency of symptomatic hemorrhagic transformation (SHT) was higher (56.5%) as well. After treatment, status of surviving patients in all groups has improved to the end of hospitalization, which was reflected in a significant decrease in NIHSS compared to baseline (p0.01).The data indicate an increased risk of SGT after STLC in patients with T2DM. Significant positive changes in the surviving patients with T2DM confirm the feasibility of STLC in patients with T2DM.Цель исследования. Сравнение эффективности и безопасности системной тромболитической терапии (СТЛТ) у больных с ишемическим инсультом (ИИ) на фоне сахарного диабета 2-го типа (СД2), гипергликемии дебюта и нормогликемии. Материал и методы. Обследованы 86 пациентов, из них 23 - с СД2 (7 - впервые выявленный), 27 - с гипергликемией дебюта ИИ, 36 - с нормогликемией. Кардиоэмболический подтип ИИ выявлен у 35%, атеротромботический - у 29%, лакунарный - у 7%, у остальных - неизвестный. СТЛТ была проведена всем больным в среднем через 153,8±4,7 мин после дебюта инсульта. Результаты. Если при поступлении тяжесть инсульта по NIHSS и степень инвалидизации по модифицированной шкале Рэнкина не различались в трех группах, то к 28-му дню отмечен достоверно меньший регресс у больных СД2 (p0,01). У них же отмечен и достоверно больший уровень летальности по сравнению с пациентами с нормогликемией (p0,05), причем у них достоверно чаще (в 56,5% случаев) отмечена симптомная геморрагическая трансформация (СГТ). В результате проведенного лечения состояние выживших пациентов во всех группах к концу пребывания в стационаре улучшилось, что выражалось в достоверном снижении оценки по NIHSS по сравнению с исходными показателями (p0,01). Заключение. Полученные данные свидетельствуют о повышенном риске СГТ после СТЛТ у больных СД2. Достоверная положительная динамика у выживших пациентов с СД2 является аргументом, подтверждающим целесообразность проведения СТЛТ у больных СД2.Цель исследования. To study the prevalence, clinical features and treatment of diabetic asymmetric proximal neuropathy (DAPN).Four hundred and forty-five patients with diabetes mellitus (DM), 257 women and 188 men, mean age 47.6±0.5 years, including 163 patients with DM type I and 282 with DM type II, were examined.Distal symmetric sensory motor polyneuropathy was found in 62% of the patients, autonomic neuropathy in 25.6%, somatic mononeuropathy in 28.5%, DAPN in 7.9%. DAPN was more frequent in patients with DM type II (9.6), with poor control of glycemia and DM duration more than 5 years. Pain and paresthesia in a zone of innervation of several roots and nerves, amyotrophy that spread beyond the definite myotome were key symptoms of DAPN. The efficacy of double therapy of neuropathic pain (pregabalin and duloxetine) and vitamin B complex was shown. A role of glucocorticoids and normal human immunoglobulin in treatment of DAPN is discussed.Цель исследования. Изучение распространенности, особенностей клинических проявлений, диагностики и лечения диабетической асимметричной проксимальной нейропатии (ДАПН). Материал и методы. Обследовали 445 пациентов с сахарным диабетом (СД), 257 женщин и 188 мужчин (средний возраст - 47,6±0,5 года); 163 из них страдали СД 1-го типа, 282 - СД 2-го типа. Результаты и заключение. ДАПН наблюдалась чаще у пациентов с СД 2-го типа (9,6%) при неудовлетворительном контроле гликемии и длительности СД более 5 лет. Ведущими признаками ДАПН являются боль и парестезии в зоне иннервации нескольких корешков и нервов, амиотрофии, выходящие за пределы определенного миотома. Показана эффективность двойной терапии нейропатической боли (прегабалин и дулоксетин) и препаратов витаминов группы В. Обсуждается также роль в терапии глюкокортикоидов и нормального человеческого иммуноглобулина.Цель исследования. Изучение распространенности, особенностей клинических проявлений, диагностики и лечения диабетической асимметричной проксимальной нейропатии (ДАПН). Материал и методы. Обследовали 445 пациентов с сахарным диабетом (СД), 257 женщин и 188 мужчин (средний возраст — 47,6±0,5 года); 163 из них страдали СД 1-го типа, 282 — СД 2-го типа. Результаты и заключение. The article presents the results of international multicenter randomized double-blind, active and placebo-controlled, comparative phase 3 trial. The goal of the study was to demonstrate non-inferiority of BCD-063 (glatiramer acetate, manufactured by JSC «BIOCAD», Russia) to copaxone-Teva (Teva Pharmaceutical Enterprise Co., Ltd., Israel) in patients with relapsing-remitting multiple sclerosis.158 patients with relapsing-remitting multiple sclerosis were randomly assigned into 3 groups: BCD-063, copaxone-Teva and placebo, at a ratio of 2:2:1, respectively.Efficacy analysis after 48 weeks of therapy demonstrated no differences between BCD-063 group and copaxone-Teva group in both MRI parameters and frequency of relapses. The mean (SD) of number of MRI-confirmed relapses per patient per year (the primary endpoint) in BCD-063 group was 0.098361 (0.351422), in copaxone-Teva group - 0.098361 (0, 351 422) and in placebo group - 0.178571 (0.390021). There were also no differences between the groups for all other efficacy parameters (EDSS and MSFC). Both investigational BCD-063 and copaxone-Teva demonstrated a favorable safety profile. The data obtained from the present study confirm the therapeutic equivalence of BCD-063 (CJSC BIOCAD, Russia) and copaxone-Teva, that is important for further implementation of glatiramer acetate generic in the clinical practice of multiple sclerosis therapy.Цель исследования. Рандомизация в группы BCD-063, копаксон-тева и плацебо производилась в соотношении 2:2:1 соответственно. Результаты и заключение. При анализе эффективности после 48 недель терапии было установлено отсутствие различий между группами BCD-063 и копаксон-тева как на основании показателей МРТ, так и по частоте обострений заболевания. To evaluate efficacy, safety, and tolerability of the treatment with teberif/interferon β-1a, to analyze safety, tolerability and dynamics of key efficacy variables after switching from referent drug rebif to biosimilar teberif in patients with remitting multiple sclerosis (RMS).During the main period of the international multicenter randomized study patients were randomized to receive treatment with teberif for 52 weeks, or rebif for 52 weeks, or placebo for 16 weeks to evaluate efficacy and safety of treatment. Изучение эффективности, переносимости и безопасности терапии интерфероном β-1а (тебериф), а также анализ переносимости и безопасности переключения с терапии референтным препаратом ребиф на биоаналог тебериф и динамики основных показателей эффективности у пациентов с ремиттирующим рассеянным склерозом (РРС). Материал и методы. В основном периоде международного многоцентрового рандомизированного исследования по оценке эффективности и безопасности терапии РРС пациенты были рандомизированы для получения препаратов тебериф, ребиф в течение 52 нед и плацебо в течение 16 нед. После этого на протяжении дополнительных 48 нед пациенты, независимо от группы, переводились на открытый прием теберифа. Результаты и заключение. На основании результатов анализа параметров оценки эффективности на протяжении первого (заслепленного) и второго (открытого) периода исследования был сделан вывод об эквивалентной эффективности препарата тебериф по сравнению с препаратом ребиф, а также о сохраняющейся эффективности применения биоаналога на протяжении 2 лет наблюдения. Тебериф не имел существенных отличий ни по одному из параметров переносимости и безопасности от препарата ребиф. Переключение с ребифа на тебериф не оказывало влияния на эффективность терапии. Опыт использования биоаналогичного препарата тебериф в рамках 2-летнего исследования подтвердил его хорошую переносимость и ожидаемый для других препаратов интерферона β-1а профиль безопасности в лечении пациентов с РРС.

To study the pharmacokinetics, pharmacodynamics, and immunogenicity of two intravenous dosing regimens of the new anti-B-cells drug BCD-132 (JSC BIOCAD, Russia) at ascending doses in patients with remitting multiple sclerosis.Twenty-four patients with multiple sclerosis were sequentially randomized in the multicenter open-label uncontrolled multicohort phase I study (3+3 design) and assigned to 4 cohorts (8 groups). Patients in each cohort received an intravenous infusion of BCD-132 at a predefined dose ranging from 100 to 1000 mg based on the planned algorithm of dose escalation if no dose-limiting toxicities occurred.The assessment of the number of cells positive for the main B-cell antigens over time demonstrated a direct effect of BCD-132 on B lymphocytes when used at a wide range of doses (100 to 1000 mg) in patients with remitting multiple sclerosis. No significant variation of the number of T-cells was observed, which clearly proves strict specificity of BCD-132 exclusively to B lymphocytes.BCD-132 has an expected pharmacodynamic effect of long-term depletion of CD19Цель исследования. Материал и методы. В многоцентровое открытое несравнительное мультикогортное исследование I фазы (дизайн '3+3') было последовательно рандомизировано 24 пациента с РРС, распределенных в 4 когорты (8 групп). Пациентам каждой когорты была выполнена внутривенная инфузия BCD-132 в предустановленной дозе препарата в диапазоне 100-1000 мг с учетом запланированного алгоритма эскалации режима дозирования при отсутствии явлений дозолимитирующей токсичности. Продолжительность клинического исследования составила 6 мес. Результаты и заключение. При динамической оценке уровня клеток, положительных по основным В-лимфоцитарным антигенам, было установлено наличие прямого действия BCD-132 на В-лимфоциты при его применении в широком диапазоне доз (100-1000 мг) у пациентов с РРС. Уровень Т-клеток не изменялся, что служит наглядным доказательством строгой специфичности препарата BCD-132 исключительно в отношении В-лимфоцитов. The article presents preliminary results of iMove research study. By the time of this publication, the data of 47 patients have been processed. The patients in the experimental group (n = 36) were trained in kinesthetic motor imagery using brain-computer interface (BCI) and a controllable exoskeleton. In the control group, BCI imitation procedures were carried out. In average, the patients had 9 training sessions with a duration of up to 40 minutes. On completing the training, only the experimental group showed improvement in scores (results are presented as median and quartiles (25 %; 75 %)): grasp score increased from 0.5 (0.0; 13.0) to 3.0 (0.0; 15.5) points (р = 0.003) and pinch score increased from 0.5 (0.0; 7.5) to 1.0 (0.0; 12.0) points (р = 0.005) on ARAT scale. In the experimental group, a significant improvement in motor function was found in 33.3 % patients on ARAT scale, and in 30.5 % patients on Fugl-Meyer scale. In the control group, those scores were lower: 9.1 % and 18.2 % patients, respectively., В статье представлены предварительные результаты исследования iMove. На момент публикации получены данные по 47 пациентам. Основная группа (n = 36) пациентов проходила обучение кинестетическому воображению движения под контролем интерфейса мозг–компьютер (ИМК) с управляемым экзоскелетом. В контрольной группе проводили процедуры имитации ИМК. В среднем пациенты прошли 9 тренингов длительностью до 40 мин. По завершении тренингов только в основной группе выявлено улучшение по параметрам [Me (25 %; 75 %)]: шаровой захват кисти — c 0,5 (0,0; 13,0) до 3,0 (0,0; 15,5) балла (р = 0,003) и щипковый захват пальцев кисти — с 0,5 (0,0; 7,5) до 1,0 (0,0; 12,0) балла (р = 0,005) по шкале ARAT. В основной группе клинически значимое улучшение двигательной функции по шкале ARAT показали 33,3 % пациентов, а по шкале Fugl-Meyer — 30,5 %. В контрольной группе эти показатели были меньше: 9,1 и 18,2 % пациентов соответственно.

To determine the mitochondrial dysfunction in multiple sclerosis (MS).Fourteen adult patients with MS and 23 healthy people were examined. Cytochemical analysis of lymphocytes in peripheral blood was carried out. The activity of the mitochondrial enzymes involved in metabolism of carbohydrates (lactate dehydrogenase, LDH), amino acids (glutamate dehydrogenase, GDH), fatty acids (alpha-glycerophosphate, α-GPDH) and II complex of the mitochondrial respiratory chain (succinate dehydrogenase, LDH), and the level of lactate in the blood were measured.The activity of α-GPDH was reduced in 62.5% patients and increased in 37.5%. GDH activity in all patients was lowered. LDH activity was reduced in 71.4% patients and compensatory increased in 28.6%. In 66.7% patients, LDH activity was reduced and in 33.3% compensatory increased. Increased blood lactate was observed in 33.3% patients before meal and 44.4% after carbohydrate loading. After carbohydrate loading, in 33.3% patients lactate levels in the blood increased and in 11.1% increased above normal values. Therefore, the enzyme activity have decreased in most patients that suggests the decompensation of mitochondrial function. The results indicate the advisability of administering the energotrophic drugs carnicetin and coenzyme QЦель исследования. Изучить нарушение функции митохондрий при рассеянном склерозе (РС). Материал и методы. Обследованы 14 взрослых больных РС и 23 - без РС. Проведен цитохимический анализ лимфоцитов периферической крови. Оценивали активность фермента митохондрий, участвующих в углеводном обмене (лактатдегидрогеназа - ЛДГ), обмене аминокислот (глутаматдегидрогеназа - ГДГ), жирных кислот (α-глицерофосфатдегидрогеназа - α-ГФДГ), и второго комплекса дыхательной цепи митохондрий (сукцинатдегидрогеназа - СДГ), а также уровень лактата в крови. Результаты и обсуждение. Активность α-ГФДГ у 62,5% больных была снижена, у 37,5% - повышена, активность ГДГ была снижена у всех. У 71,4% выявили снижение активности СДГ, у 28,6% - компенсаторное повышение. У 66,7% активность ЛДГ была снижена, а у 33,3% - компенсаторно повышена. Наиболее частое снижение активности фермента, свидетельствующее об истощении компенсаторных возможностей, отмечено в отношении ГДГ, на 2-м месте - СДГ, на 3-м - ЛДГ, на 4-м - α-ГФДГ. Увеличение содержания лактата в крови отмечено у 33,3% больных до еды и у 44,4% - после нагрузки углеводами (в 33,3% случаев уровень лактата повысился, в 11,1% - был выше нормы). Таким образом, у большинства больных отмечено снижение активности ферментов, свидетельствующее о декоменсации функции митохондрий. To compare the efficacy and safety of systemic thrombolytic therapy (STLT) in patients with cardioembolic stroke (CE) versus other pathogenic subtypes of ischemic stroke (IS).The study included 147 patients, 62 women and 85 men (mean age - 62.9±0.8 years) including 37 patients (25.2%) with CE subtype of IS (group 1) and 110 patients with other pathogenetic subtypes of IS (group 2). NIHSS and Rankin scale were used to assess patient's neurological status.One hundred and twenty-six patients were discharged, 21 (14.3%) died. In 11 patients, the cause of death was the development of symptomatic hemorrhagic transformation (SHT). There were no significant differences in the lethality between groups 1 and 2. Tolerability to STLT in these groups did not differ as well. As a result of treatment, the condition of patients surviving to the end of the hospital stay improved, which was reflected in a significant decrease in the NIHSS scores, despite the higher NIHSS scores in group 1.The results confirm the efficacy of STLT in patients with CE IS and indicate the increase in the frequency of favorable functional recovery in these patients.Цель исследования. Сравнение эффективности и безопасности системной тромболитической терапии (СТЛТ) у больных с кардиоэмболическим (КЭ) ишемическим инсультом (ИИ) и другими патогенетическими подтипами ИИ. Материал и методы. Обследовали 147 пациентов (62 женщин и 85 мужчин, средний возраст 62,9±10,8 года), которым была проведена СТЛТ. У 37 (25,2%) пациентов был установлен КЭ ИИ (1-я группа), у 110 пациентов - другой патогенетический подтип ИИ (2-я группа). Состояние оценивали по шкале NIHSS и модифицированной шкале Рэнкина при поступлении и при выписке. Результаты и заключение. Были выписаны 126 пациентов, в 21 (14,3%) случае имел место летальный исход. У 11 пациентов причиной смерти стало развитие симптомной геморрагической трансформации. Достоверного различия летальности в 1-й и 2-й группах при использовании точного критерия Фишера получено не было. Переносимость СТЛТ у выживших пациентов двух групп также существенно не отличалась. В результате проведенного лечения состояние выживших пациентов к концу пребывания в стационаре улучшилось, что выражалось в достоверном снижении оценки по шкале NIHSS, несмотря на то что у пациентов 1-й группы исходно тяжесть состояния по шкале NIHSS была несколько больше. Полученные данные подтверждают эффективность СТЛТ у пациентов с КЭ ИИ и свидетельствуют о увеличении частоты благоприятного функционального восстановления у этих больных.

Objective. To investigate the possibility of using a bioengineering system, which includes an electroencephalograph and a personal computer with a software for synchronous data transmission, recognition and classification of EEG signals, development of directions for intended actions in real time in the combination with the hand exoskeleton (the bioengineering system "brain-computer interface + exoskeleton"), in motor rehabilitation of post stroke patients with paresis of the upper extremity. Material and methods. Brain-computer interface is a promising field of neurorehabilitation. Rehabilitation treatment, including 8-10 sessions, was conducted in 5 patients with paresis of the upper extremity. All patients had large MRI lesions in cortical/subcortical areas. Results. Positive changes in neurological status measured with the NIHSS, a significant increase in the volume and power of movements in the paretic hand, improvement of coordination and slight decrease in the level of spasticity were found after the treatment. There was an increase in daily activities measured with the Barthel index, mostly due to the improvement of fine motor skills. The level of disability assessed by the modified Rankin scale was changed significantly. Conclusion. The use of "brain-computer interface + exoskeleton" in the rehabilitation of post stroke patients with hand paresis provided positive results that would need to be verified in further studies.Цель исследования - изучение возможности использования биоинженерного комплекса, состоящего из электроэнцефалографа, персонального компьютера с программным обеспечением для синхронной передачи данных, распознавания и классификации сигналов электроэнцефалограммы (ЭЭГ) и формирования управляющей команды в реальном времени в сочетании с экзоскелетом кисти руки (биоинженерный комплекс "интерфейс мозг-компьютер (ИМК) + экзоскелет" для проведения двигательной реабилитации у пациентов с постинсультными парезами верхней конечности. Материал и методы. Перспективным направлением нейрореабилитации является использование интерфейса "мозг - компьютер". Реабилитационные курсы, состоящие из 8-10 занятий, проведены 5 пациентам с постинсультными парезами верхней конечности. У всех включенных в исследование больных имелись обширные очаги постинсультных изменений корково-подкорковой локализации по результатам магнитно-резонансной томографии. Результаты. После курса занятий отмечены положительная динамика в неврологическом статусе (по шкале NIHSS), значимое увеличение объема и силы движений в паретичной руке, улучшение в ней координации движений, незначительное снижение уровня спастичности. Обнаружено увеличение показателей повседневной активности по данным индекса Бартел, преимущественно в результате улучшения мелкой моторики. Отчетливо изменился и уровень инвалидизации пациентов по данным модифицированной шкалы Рэнкина. Заключение. Использование комплекса "ИМК + экзоскелет" в реабилитации пациентов с постинсультными парезами руки дало положительный результат, что указывает на необходимость продолжить исследования.

The pathologic role of autoantibodies in autoimmune disease is widely accepted. Recently, we reported that anti-myelin basic protein (MBP) serum Abs from multiple sclerosis (MS) patients exhibit proteolytic activity toward the autoantigen. The aim of this study is to determine MBP epitopes specific for the autoantibodies in MS and compare these data with those from other neuronal disorders (OND), leading to the generation of new diagnostic and prognostic criteria. We constructed a MBP-derived recombinant “epitope library” covering the entire molecule. We used ELISA and PAGE/surface-enhanced laser desorption/ionization mass spectroscopy assays to define the epitope binding/cleaving activities of autoantibodies isolated from the sera of 26 MS patients, 22 OND patients, and 11 healthy individuals. The levels of autoantibodies to MBP fragments 48–70 and 85–170 as well as to whole MBP and myelin oligodendrocyte glycoprotein molecules were significantly higher in the sera of MS patients than in those of healthy donors. In contrast, selective reactivity to the two MBP fragments 43–68 and 146–170 distinguished the OND and MS patients. Patients with MS (77% of progressive and 85% of relapsing-remitting) but only 9% of patients with OND and no healthy donors were positive for catalysis, showing pronounced epitope specificity to the encephalitogenic MBP peptide 81–103. This peptide retained its substrate properties when flanked with two fluorescent proteins, providing a novel fluorescent resonance energy transfer approach for MS studies. Thus, anti-MBP autoantibody-mediated, epitope-specific binding and cleavage may be regarded as a specific characteristic of MS compared with OND and healthy donors and may serve as an additional biomarker of disease progression.

Vision impairment is one of the most typical characteristics of multiple sclerosis (MS). This pathology develops due to the lesions of optic nerves and optic tracts in the brain or eye movement dysfunction. Low contrast vision impairment is very frequent and early detected. We assessed low contrast non-color vision in patients with MS.We examined 56 MS patients and 33 healthy controls. Computed optometric testing was used (grey-colored digits of the standard size were shown against the background of the different shade of grey on the computer monitor).The results demonstrated low-contrast vision dysfunction while high-contrast vision remained intact. The level of low-contrast vision in MS patients differed significantly from that in the controls. The higher severity of visual disturbances was identified during high illumination. The direct correlation between the degree of low-contrast vision dysfunction and the level of disability on EDSS was found.Цель исследования. Снижение зрения является одним из наиболее характерных проявлений рассеянного склероза (РС). Эта патология возникает вследствие поражения зрительных нервов, зрительных путей в веществе мозга или нарушения функции глазодвигательной системы. Чаще всего раньше обнаруживается нарушение низкоконтрастного зрения. Цель исследования - оценка состояния низкоконтрастного черно-белого зрения у больных РС. Материал и методы. Обследовали 56 пациентов с РС и контрольную группу из 33 здоровых. Для исследования использовали метод компьютерного оптометрического тестирования (в качестве оптических стимулов применяли цифры серого цвета стандартного размера на фоне другого оттенка серого цвета на мониторе компьютера). Результаты и заключение. Полученные данные свидетельствуют о нарушении низкоконтрастного зрения у больных РС при сохранности высококонтрастного. Выявлено достоверное отличие уровня низкоконтрастного зрения при РС от здоровых лиц. Большая выраженность зрительных нарушений отмечена в условиях высокой освещенности. Обнаружена прямая связь степени нарушения низкоконтрастного зрения с уровнем инвалидизации по шкале EDSS.

To develop a technique of physical rehabilitation of stroke based on the gradual increase of motor/cognitive loading using biofeedback to supporting reaction based on stabilometry with a focus on the changes in peripheral afferentation with the help of an axial loading suit and simple non-apparatus vestibular and respiratory exercises. The efficacy of standard scheme of restorative treatment and a new technique of optimized complex rehabilitative treatment of acute ischemic stroke in the vertebral-basilar system was compared.We examined 28 patients with acute stroke in the vertebrobasilar area manifesting with dizziness, disturbances of equilibrium and stability. The patients were randomized into two equal groups (n=14). In the main group, a new technique was added to standard treatment. In the control group, traditional pharmacological and physical treatment was used. We administered clinical scales and instrumental stabilometric assessment as well.A significant improvement of the vertical stability assessed with clinical scales (Bohannon, Perry, Stolyarova, Berg Balance Scale) and stabilometric parameters (the Romberg's test) was shown in the main group compared to the control one.The new technique expands the potentials of rehabilitation in the acute stage of stroke and allows to prepare the patient to independent gait.Цель исследования - оценка эффективности новой методики физической реабилитации больных после церебрального инсульта на основе постепенного увеличения двигательно-когнитивной нагрузки с помощью костюма аксиального нагружения, а также простых безаппаратных упражнений вестибулярной и дыхательной гимнастики. Материал и методы. Обследовали 28 пациентов с головокружением и нарушением равновесия в остром периоде ишемического инсульта. Они были разделены на две группы. В основной (n=14) в схему лечения дополнительно были включены новые методики; в контрольной (n=14) применялась только традиционная терапия. При обследовании больных помимо общего неврологического обследования были использованы клинические шкалы и инструментальная оценка методом стабилометрии. Результаты. Выявлено достоверное улучшение показателей вертикальной устойчивости пациентов основной группы по сравнению с контрольной по клиническим шкалам и стабилометрическим показателям. Заключение. Примененная новая методика расширяет возможности реабилитации больных в остром периоде инсульта и позволяет лучше подготовить пациента к самостоятельной ходьбе.

Introduction Multiple sclerosis (MS) is the socio-significant disease, which leads to disability of patients. Known, that in MS etiopatogenesis are involved the viruses, functional capacity disorders of cells the innate and adaptive immunity. The IFN inducer drugs, in particular, cycloferon,have anti-inflammatory, antiviral and immunomodulatory properties, which could have beneficial effects in immune mediated neurological disorders. Methods We report the results observed on 22 patients with the relapsing-remitting MS (in stages of remission 17 patients and an exacerbation 5 patients) after 1 months of therapy with cycloferon. All the subjects were recruited and treated at the neurological department of MRSRCI. Our patients consisted basically in women with a mean disease duration of 3.35 ± 2.85 years, with mild to moderate severity (EDSS average power at remission was 4 points, in aggravation- 9.2 points). The cycloferon scheme in MS: 1 injection (0.25 g) every other day for a month, on a course of injections15 +vascular therapy. The control group was represented by 17 patients with MS cerebrospinal shape received only the vascular therapy. Results All MS patients IFN-producing capacity by peripheral blood leukocytes (PBL) compared with healthy subjects was significantly reduced: a-IFN/g-IFN in the 5.49/3.1 times in the acute stage and 3.24/2.4 times in remission remitting form of the disease. It should be noted that the most significant decrease in a- and g-IFN-producing ability of PBL was determined in the acute stage relapsing-remitting forms of MS, which correlated with the clinical manifestations of disease. The content of IgG-antibodies to MBP in MS patients with any form of the disease was significantly increased. As a result of treatment clinical improvement was noted with the positive dynamics of neurologic status in cycloferon treated patients. Our results show a EDSS decrease in 7 (32%) patients, EDSS increase in 2 (9%) patients, with the remaining 13 (59%) patients had EDSS did not change. It may be noted that the deficit was initially detectable product a-IFN blood leukocytes of MS patients during treatment to correct the increase of production capacity for a-IFN, reduction of IgG-antibodies to MBP. 15 patients in remission were followed up after treatment with cycloferon 2 times a week for 1.3 years. All patients had pre- treatment exacerbation of the disease in 4, 6, 8 or 12 months. After treatment of 6 patients who had exacerbation in six months, in 4 (66.7%) -long-term remission during the observation period. Among the 9 patients who had exacerbation of disease during the period from 6 to 12 months in 7 patients (77.8%) on cycloferon had stable neurological condition for 15 months. Special attention should be reducing the frequency of exacerbations and longer duration of remission in patients in cycloferon therapy. Long-term observation period of 4 years had 3 patient, 12 years have 1 patient who received cycloferon constantly 2 times a week showed the stability of neurological status. Conclusion Stabilization of clinical current MS, improvement of the IFN-status parameters, decrease in a level of antibodies to myelin basic protein are achieved, that is the basis to consider application low-molecular IFN inducer cycloferon as the alternative approach in MS.
