Silikozis, 2005 yılında tekstil alanında çalışanlarda meslek hastalığı olarak tanımlanmış ve ilk olarak Türkiye’de rapor edilmiştir. Kot pantolonların ağartılması, aşındırılması ve eskitilmiş bir görüntüye kavuşturulması için, elekten geçirilmiş deniz kumlarının kompresör yardımıyla kumaş üzerine püskürtülmesi işlemine “kumlama” denilmektedir. Çalışma ortamında kum tozuna maruz kalınması, işyerinde iş sağlığı ve güvenliği önlemlerinin alınmaması, kot kumlama işlemini gerçekleştiren çalışanlarda silikozis hastalığının görülmesine sebep olmaktadır. Bu tez kapsamında İstanbul’da belirli sürelerde kot kumlama işinde çalışmış ve sonrasında silikozis tanısı konulmuş 45 bireyin sosyo-ekonomik durumları, demografik özellikleri, çalışma şartları ve rahatsızlıklarıyla ilgili ana verilere ulaşmayı hedefleyen, tanımlayıcı epidemiyolojik bir çalışma gerçekleştirilmiştir. Ayrıca, hastaların çalışma koşulları ve hastalıkla alakalı yaşadıkları sürece ait bilgilere de yer verilmiştir. Tez çalışmasında yer alan bireylerin tamamı erkek ve ortalama 39,40 ± 8,65 yaşındadır. Çalışanların tamamına (%100, n=45) işe giriş muayenesi yapılmamıştır. Çalışanların tamamı haftada 6 gün olacak şekilde, %42,9’u iki vardiya, %57,1’i üç vardiya sistemiyle çalışmaktadır. Çalışanların günlük çalışma sürelerinin dağılımı ise %84,4’ünün 8-9 saat, %13,3’ünün 10-11 saat ve %2,2’sinin 12 saat’in üzerindedir. Sigorta girişi yapılmadan çalışanların oranı %93,3 (n=42), bir süre sigortalı çalışanların oranı ise %6,7 (n=3)’dir. Çalışanların %71,1’inin (n=32) işe başladıktan sonra, %17,8’inin (n=8), işi bıraktıktan sonra ve %11,1’i (n=5) askerlik vazifesi öncesinde gerçekleştirilen sağlık muayenesinde tanı almıştır. Ayrıca hastaların %8,9’una (n=4) tüberküloz tanısı konulmuştur. Hastalığın meslek hastalığı olduğunu belgeleyenlerin oranı ise %2,2’dir. (n=1). Tez kapsamında silikozis başta olmak üzere tüberküloz ve akciğer kanseri hastalıkları hakkında literatür bilgisi paylaşılmış olup, Türkiye’de toz maddeler ve kristal silikaya ilişkin güncel yasal düzenlemelere yer verilmiştir. Ayrıca tez kapsamında yapılan anket çalışması sonucunda elde edilen sonuçlar paylaşılmıştır. “Kumlama” işlemini uygulayan iş yerlerinde çalışanlarda, meslek hastalığına sebebiyet vermemek için iş sağlığı güvenliği kurallarının önemi vurgulanmıştır. Silicosis, in the textile sector in 2005, was first reported and defined as an occupational disease in Turkey. The process of spraying the sea sand through the fabric with the help of a compressor is called “sandblasting” to bleach, abrading, and aging the jeans. Exposure to sand dust in the working environment, the absence of occupational health and safety measures at the workplace, causes silicosis disease to be observed in the employees who perform denim sandblasting. Within the scope of this thesis, a descriptive epidemiological study was carried out aiming to reach with 45 persons who worked in Istanbul for a certain period in sandblasting and were subsequently diagnosed with silicosis. The main data of this thesis are the socio-economic conditions, demographic characteristics, working conditions as well as information about the working conditions of the patients, and the duration of the disease are also included. All the individuals in the thesis study are male and average 39.40 ± 8.65 years old. All employees (100%, n = 45) have not been subjected to an employment examination. All the employees work 6 days a week, 42.9% work in two shifts, and 57.1% work in three shifts. The distribution of the daily working time of the employees is 8-9 hours of 84.4%, 10-11 hours of 13.3%, and over 12 hours of 2.2%. The proportion of employees without insurance is 93.3% (n = 42) and the rate of insured employees is 6.7% (n = 3) for a while. During the medical examination, 71.1% (n = 32) of the employees started after work, 17.8% (n = 8) after quitting the job and 11.1% (n = 5) before the military service. It has received. Also, 8.9% (n = 4) of the patients were diagnosed with tuberculosis. The rate of those who document that the disease is an occupational disease is 2.2%. (N = 1). In the thesis, especially silicosis and tuberculosis has been shared literature about lung cancer diseases, powdery substance in Turkey and is given to current regulations for crystalline silica. Besides, the results obtained from the survey conducted within the scope of the thesis were shared. The importance of occupational health and safety rules has been emphasized to prevent occupational diseases among those working in workplaces applying the “sandblasting” process.

Silicosis which is one of the oldest known occupational diseases is characterized by an irreversible and progressive fibrotic reaction occuring in the lungs caused by inhaling crystalline silica dust. Effective treatment for silicosis is not available. Recently, denim sandblasting especially being made in uncontrolled small-scale workplace in primitive conditions has been a striking occupation leading to silicosis . Twenty and 28 years old male patients who work in denim sandblasting for 3 years were admitted to our clinics with complaints of dyspnea and loss of weight. Chest X ray and thorax computerized tomography revealed bilateral diffuse reticulonodular and nodular opacities which were prominent in the upper and middle zones and peripheric area of pulmonary parenchyma. Restrictive disorder was observed at their pulmonary function tests. Silicosis was diagnosed by performing transbronchial biopsy in one case. Because of the similarity of complaints, radiological findings and occupational history with the former patient, no other further and invasive procedure was performed and the other patient was also diagnosed as silicosis. In developing countries in which the labor force is very cheap, silicosis becomes a public health problem in denim sandblasters and can be prevented with improvement of working conditions. We believe that further precaution must be taken to stop darkened lives while jeans is bleaching. Key words: Denim Sandblasting; Silicosis., Bilinen en eski meslek hastalıklarından biri olan silikozis, solunabilir büyüklükteki silika kristallerinin akciğer dokusunda geri dönüşümsüz ve ilerleyici bir fibrotik reaksiyon oluşturması ile karakterize tedavisi olmayan bir hastalıktır. Son yıllarda özellikle küçük ve denetimsiz atölyelerde uygunsuz koşullarda yapılan kot kumlama işi hastalığın gelişiminde farklı bir işkolu haline gelmiştir. 3 yıldır kot kumlama işinde çalışan 20 ve 28 yaşlarında iki erkek hasta, nefes darlığı ve kilo kaybı şikayetleri ile kliniğimize başvurdu. Posteroanterior akciğer grafilerinde ve toraks bilgisayarlı tomografilerinde özellikle her iki üst ve orta zonlarda ve periferik bölgelerde yoğun yerleşimli, yaygın retikülonodüler ve nodüler opasiteler izlendi. Solunum fonksiyon testlerinde restriktif tipte ventilasyon bozukluğu saptandı. Birinci olgumuza transbronşiyal biyopsi ile silikozis tanısı kondu. İkinci olgu ise ilki ile benzer yakınmaları, radyolojik bulguları ve mesleksel maruziyet öyküsü nedeniyle ileri girişimsel inceleme yapılmadan tanı aldı. İş gücünün ucuz olduğu gelişmekte olan ülkelerde kot kumlama işinde çalışanlarda bir toplum sağlığı sorunu haline gelen silikozis çalışma şartlarının düzeltilmesi ile önlenebilir. Kotlar beyazlarken kararan hayatlara son vermek için daha fazla önlem alınması gerektiği inancındayız. Anahtar kelimeler: Kot Kumlama; Silikozis.
