The aim of this systematic review, meta-analysis, and meta-regression was to examine the effects of virtual reality–based training on global cognition and executive function compared with conventional training or information-based treatment in older adults, regardless of cognitive level. A systematic literature search was conducted using four databases. A total of 31 randomized controlled trials were identified. Pooled effect sizes were calculated, the risk of bias was assessed, and evidence was graded. The primary analyses showed a small but statistically significant effect of virtual reality–based training compared with control on global cognition (Hedges' g 0.42, 95% confidence interval [0.17, 0.68], I
2 = 70.1%, n = 876, 20 randomized controlled trials, low evidence) and executive function (Hedges' g 0.35, 95% confidence interval [0.06, 0.65], I2 = 68.4%, n = 810, 16 randomized controlled trials, very low evidence). Meta-regression yielded inconclusive results. Virtual reality–based training may be more effective than control in improving cognition in older adults; however, more high-quality studies are needed. 5. Problem-Based Learning in Professional Studies from the Physiotherapy Students' Perspective

- Abstract
The aim of the study was to investigate how physiotherapy students using a problem-based learning approach develop into experts during higher education, and answers the question: How do physiotherapy students at bachelor's level understand the problem-based learning approach while learning to become professionals? PBL is examined using interpretative phenomenological analysis (IPA) of longitudinal data written by 15 voluntary students from two different higher education institutions and collected during 3.5 years. The main results on the new way of learning strengthen earlier conceptions of the importance of reflection in the learning process. The PBL method activates a reflection process by allowing students to participate in something that differs from their previous experiences of teaching and learning methods, which creates confusion and forces them to critically reflect on their actions. There are two dimensions of reflection in this study: self-reflection (information-seeking and creative learning processes) and reflection together (peer-group working and the teacher), in which reflection together seems to be more powerful than in earlier experiential learning theories. This study brings out the directions for and the timing of the necessary scaffolding and support for learning.

This study aimed to explore added value of mcoach app and activity bracelet in 12-month cardiac rehabilitation on changes in rehabilitee's biopsychosocial functioning: cardiorespiratory fitness, depressive symptoms and social quality of life compared to similar rehabilitation without remote technology. Additionally, the study explored the factors explaining changes of biopsychosocial functioning. Six groups of coronary rehabilitees were cluster-randomised to experimental (n = 30) or control group (n = 29). Parametric and nonparametric statistical analyses and linear regression analyses were used. No differences between the groups regarding changes in VO2max (p = 0.474), DEPS (p = 0.315) or social quality of life (p = 0.592) were observed. Positive changes in the whole study population in VO2max were explained with shorter time from last cardiac operation and higher light activity at the baseline (R2 = 0.309). Smaller increase in DEPS was explained with positive changes in different aspects of social quality of life (R2 = 0.562). Remote technology had no added value for rehabilitation, but the results confirmed the importance of early rehabilitation, maintenance of socially meaningful life and the use of biopsychosocial and multidisciplinary approaches. Research defining the needs and types of technology, their timing, to whom and to what extent the remote technology would be most beneficial, is still needed. [ABSTRACT FROM AUTHOR]

The aim of this study was to gain deeper understanding of physiotherapists' clinical reasoning in examination of clients with low back pain (LBP) in direct access (DA) practice. Data were collected through individual semi-structured thematic interviews of ten physiotherapists who described their clinical reasoning in one of their LBP clients in direct access practice. The interviews were recorded and transcribed verbatim and then examined using a theory-driven qualitative content analysis. Four clinical reasoning models, hypothetico-deductive, pattern recognition, narrative and systematic, were used as theoretical frameworks to analyse the descriptions of physiotherapists' clinical reasoning in examination. In this study clinical reasoning in examination manifested as a process where physiotherapist proceeds from interview to clinical assessment and decision-making, simultaneously taking into consideration the DA setting and the specific role of the physiotherapist. As described by the physiotherapists working in DA practice clinical reasoning in examination presented by extracting facts from the interview in relation to symptoms or certain diagnosis, identifying and excluding red flags and identifying the factors that were important to consider in acute musculoskeletal pain in relation to individual client's problem. Physiotherapists described having improved differential diagnostic skills, wider range of physical assessment techniques and more precise assessment. Based on the data, the physiotherapists presented to use either the hypothetico–deductive or the systematic clinical reasoning model when examining an LBP client in DA practice. Clinical reasoning in examination with LBP clients in direct access practice started with anamnesis and proceeded by using either the hypothetico–deductive or the systematic clinical reasoning model. Tiivistelmä Tutkimuksen tarkoituksena oli kuvata sosiaali-, terveys- ja kuntoutusalan (soteku) opettajien itsearvioitua osaamista. Lisäksi selvitettiin koulutusalojen ja koulutusorganisaatioiden välisiä eroja opettajien itsearvioidussa osaamisessa. Tutkimus oli osa ”Osaavat opettajat yhdessä!” TerOpe– OKM -kärkihanketta. Kohderyhmä muodostui 21 ammattikorkeakoulun ja seitsemän ammatillisen oppilaitoksen opettajista (n=422). Tutkimusaineistona hyödynnettiin TerOpe-hankkeen HeSoEduCo-mittarilla (1–4 Likert-asteikko) hankkimaa aineistoa, joka mittasi opettajien itsearvioitua osaamista kahdeksalla osaamisalueella. Aineisto analysoitiin tilastollisin menetelmin Mann Whitneyn U- ja Kruskal Wallis-testeillä. Opettajat arvioivat osaamisensa hyvälle tasolle. Vahvin osaaminen muodostui opiskelijan ammatillisen kehittymisen ohjausosaamisesta ja heikoin osaaminen yhteisöllisestä digipedagogiikasta. Terveysalan opettajien näyttöön perustuvan toiminnan osaaminen oli sosiaalialan ja yhdistettyjen alojen opettajia parempi. Sosiaalialan opettajien substanssi- ja opetussuunnitelmaosaaminen sekä verkosto- ja yhteiskunnallinen osaaminen olivat terveysalan opettajien osaamista vahvempia. Yhdistettyjen alojen opettajien opiskelijalähtöinen pedagoginen sekä verkosto- ja yhteiskunnallinen osaaminen oli ter veysalan opettajien osaamista parempi. Ammattikorkeakoulujen opettajien osaaminen oli ammatillisten oppilaitosten opettajien osaamista parempi opiskelijalähtöisen pedagogiikan, yhteisöllisen digipedagogiikan, kulttuurisen osaamisen, johtamis- ja hallinnollisen sekä näyttöön perustuvan toiminnan osalta. Tuloksia voidaan hyödyntää ammattikorkeakoulujen ja ammatillisten oppilaitosten johtamisessa sekä soteku-alan perus- ja täydennyskoulutuksessa. Abstract The purpose of the study was to describe the competence of social-, health care, and rehabilitation educators based on their evaluations. Differences between educational organizations and work organizations in the competencies of educators were also investigated. The study was part of the ”Competence educators together!” TerOpe project which was funded by the Ministry of Education and Culture in Finland. The target group consisted of educators from 21 universities of applied sciences and seven vocational institutes (n=422). The data was collected with a HeSoEduCo instrument (1–4 Likert scale) in the autumn of 2018 by the team of TerOpe project. The instrument measured educators’ self-assessed competence in eight areas of competence. The data were analyzed using statistical methods. Educators evaluated their competence at a good level. The highest competence consisted of mentoring students into professional development competence and the lowest of competence of digital collaborative learning. The evidence-based competence of health care educators was better than the competence of social care and combined fields’ educators. Subject and curriculum competence, as well as collaboration and societal competence of social care educators, were higher than those of health care educators. The student-centered pedagogy, collaboration, and societal competencies of the educators of the combined fields were better than those of the health care educators. Competence of universities of applied sciences educators was higher than those of vocational college’ educators in terms of student-centered pedagogy, digital collaborative learning, culture, leadership and management, and evidence-based practice competence. The results can be used in the management of the university of applied sciences and vocational colleges, as well as in basic and continues education in the social-, health care, and rehabilitation fields.

The social and health care educator's role in educating future professionals need to be stronger emphasised and deserves international recognition. The purpose of this study was to develop and test an empirical model of social and health care educators' competence in higher and professional education. The presented research employed a cross‐sectional study design. Data were collected using HeSoEduCo‐instrument from 28 educational institutions in Finland. The model was empirically tested with confirmatory factor analysis through Structural Equation Modelling that applied the Full Imputation Maximum Likelihood estimator. A total of 422 social and health care educators participated in the study. The empirical model of social and health care educators including eight competence areas: leadership and management, collaboration and societal, evidence‐based practice, subject and curriculum, mentoring students in professional competence development, student‐centred pedagogy, digital collaborative learning, and cultural and linguistic diversity. All of the connections between concepts of the empirical model were found to be statistically significant. There were strong connections between most of the identified competence concepts; however, two weak connections were found, namely, the link between competence in evidence‐based practice and competence in subject and curriculum, along with the link between competence in digital collaborative learning and competence in student‐centred pedagogy. The presented empirical model can help stakeholders identify which areas of social and health care educators' curricula should be further developed. The model is also relevant for improving continuous education, allowing educators to assess their competence levels and evaluating educators' performance at the organisational level. Johdanto: Sosiaali-, terveys- ja kuntoutusalan (soteku) toimintaympäristö on jatkuvasti muuttuva ja kehittyvä. Jotta pätevien opettajien saatavuus turvataan myös tulevaisuudessa, tarvitaan jatkuvaa terveystieteiden opettajakoulutuksen uudistamista ja systemaattista sosiaali-, terveys- ja kuntoutusalan opettajien osaamisen kehittämistä. Tarkoitus: TerOpe-hankkeen tarkoituksena oli kehittää valtakunnalliset ja kansainvälisesti vertailukelpoiset soteku-opettajien osaamisvaatimukset, ja luoda osaamisen kehittämisen malli opettajien suunnitelmalliselle, uran aikaiselle osaamisen kehittymiselle. Lisäksi hankkeessa kehitettiin digipedagogiikan verkko-opintojakso ja digitaalinen osaamisverkosto alan opettajille, opettajankouluttajille ja opiskelijoille. Menetelmät: TerOpe-hanke oli kuuden yliopiston ja yhden ammatillisen opettajakorkeakoulun valtakunnallinen kärkihanke, jota rahoitti Opetus- ja kulttuuriministeriö vuosina 2017–2019. Hanke toteutettiin neljässä vaiheessa: 1) Opettajien osaamismallin ja osaamisvaatimusten kehittäminen; 2) Opettajien osaamisen kehittämismallin rakentaminen; 3) ”Digipedagogiikan perusteet sosiaali-, terveys- ja kuntoutusalan koulutuksessa 2 op” -verkko-opintojakson kehittäminen ja pilotointi; 4) Digitaalisen osaamisverkoston kehittäminen ja pilotointi. Tulokset: Hankkeessa kehitettiin näyttöön perustuvasti soteku-opettajien osaamismalli, osaamisvaatimukset, opettajien osaamisen kehittämisen malli, digipedagogiikan verkko-opintojakso sekä digitaalinen osaamisverkosto alan opettajille, opettajankouluttajille ja opiskelijoille. Lisäksi hankkeessa kehitettiin kolme itsearviointimittaria: Sosiaali-, terveys- ja sekä kuntoutusalan opettajan osaaminen (HeSoEduCo), Opettajan jatkuva ammatillinen kehittyminen (EduProDe) ja Opettajan digitaalinen osaaminen (DigCompEduF). Johtopäätökset: Soteku-opettajan osaamismallia ja osaamisvaatimuksia voidaan hyödyntää terveystieteiden opettajankoulutuksen opetussuunnitelmien uudistamisessa valtakunnallisesti, ja yhteisopetuksen ja ristiinopiskelun kehittämisessä terveystieteiden opettajankoulutuksessa. Opettajien osaamisen kehittämismallissa kuvataan monitasoinen lähestymistapa, jonka pohjalta opettajien osaamisen kehittäminen tapahtuu opettajankoulutuksessa, koulutuspoliittisessa päätöksenteossa, koulutusorganisaatioiden strategisella tasolla, opettajien osaamisen lähijohtamisessa sekä yksittäisten opettajien ja opettajatiimien osaamista kehittävässä toiminnassa. Introduction: The operating environment in the social-, health care, and rehabilitation sector is continuously changing and evolving. In order to ensure the availability of qualified educators in the future, constant reform of health science teacher education and systematic development of competence in social-, health care, and rehabilitation is needed. Purpose: The TerOpe project aimed to develop national and internationally comparable competence requirements for social-, health care, and rehabilitation educators, and to create a competence development model for educators’ systematic career development. Besides, the project developed an online course in digital pedagogy and a digital competence network for educators, educators’ trainers and students in the field. Methods: The TerOpe project was part of nationwide government key projects. The project involved six universities and one university of applied sciences, funded by the Ministry of Education and Culture in 2017–2019. It was implemented in four phases: 1) development of the educator competence model and its requirements; 2) building an educator competence development model; (3) development and piloting of a digital course in digital education for social-, health care, and rehabilitation education; 4) development and piloting of a digital competence network. Results: The project developed an evidence-based model for the competence of social-, health care and rehabilitation educators. The project also developed competence requirements for the educators, a model of educator competence development, an online course in digital pedagogy, and a digital competence network for educators, educators’ trainers and students in the field. Besides, the project developed and tested three self-assessment instruments: The Health and Social Care Educator’s Competence (HeSoEduCo), Continuous Professional Development of Educators (EduProDe), and Digital Competence of Educators (DigCompEduF). Conclusions: The social-, health care, and rehabilitation educators’ model and competence requirements can be utilized to revise health science education curricula nationwide, and to develop collaborative and cross-curricular education. - Abstract
The aim of the study was to investigate how physiotherapy students using a problem-based learning approach develop into experts during higher education, and answers the question: How do physiotherapy students at bachelor’s level understand the problem-based learning approach while learning to become professionals? PBL is examined using interpretative phenomenological analysis (IPA) of longitudinal data written by 15 voluntary students from two different higher education institutions and collected during 3.5 years. The main results on the new way of learning strengthen earlier conceptions of the importance of reflection in the learning process. The PBL method activates a reflection process by allowing students to participate in something that differs from their previous experiences of teaching and learning methods, which creates confusion and forces them to critically reflect on their actions. There are two dimensions of reflection in this study: self-reflection (information-seeking and creative learning processes) and reflection together (peer-group working and the teacher), in which reflection together seems to be more powerful than in earlier experiential learning theories. This study brings out the directions for and the timing of the necessary scaffolding and support for learning.

Introduction and aim: Cultural differences have significant impacts on classroom behaviours and communication in teaching. The aim of this study is to explore social and healthcare educators' cultural competence in transcultural education. Methodology: Data was collected from semi-structured focus group interviews at universities of applied sciences and vocational colleges. Inductive content analysis was used in the analysis process. Results: Educators' cultural competence in transcultural education emerges as generic categories: transcultural education, educatorship and ethical attitudes, and underpins by the sub-categories: language and linguistics, different learning styles, integrating multicultural students, cultural knowledge and sensitivity, collaborating and cooperating, self-awareness and openness, and respecting and caring. Conclusion: Cultural competence represents a core competence for social-and health care educators. Educators need to know students' background, master different learning styles, be flexible in their pedagogical approaches and have an open and ethical attitude. [ABSTRACT FROM AUTHOR]

Background and purpose: Educators' ethical competence is of crucial importance for developing students' ethical thinking. Previous studies describe educators' ethical codes and principles. This article aims to widen the understanding of health- and social care educators' ethical competence in relation to core values and ethos. Theoretical background and key concepts: The study is based on the didactics of caring science and theoretically links the concepts ethos and competence. Methods: Data material was collected from nine educational units for healthcare and social service in Finland. In total 16 semi-structured focus group interviews with 48 participants were conducted. The interviews were analysed with a thematic analysis according to Braun and Clarke. Ethical considerations: The study is approved by the Declaration of Helsinki, the legislation regarding personal data and the General Data Protection Regulation. The study received ethical permission from the University of Jyväskylä. Informed consent was obtained from all the educational units and participants in the study. Findings: The findings are presented based on three general patterns, an ethical basic motive, an ethical bearing and ethical actions. Subthemes are Humane view of students as unique individuals with individual learning, Bearing of tactfulness and firmness, Bearing of perceptiveness and accessibility, Bearing of satisfaction and joy over student learning, Valuing bearing towards each oneself and colleagues, Ability to interact and flexibility, Collegiality and a supportive work community and Educators as role models and inspirators. Conclusion: Educators' personal and professional ethos is crucial to student learning, personal growth and ethical reasoning. Tutkimuksen tavoitteena oli selvittää fysioterapian vaikuttavuutta ja merkityksellisyyttä aivoverenkiertohäiriötä (AVH) ja multippeliskleroosia (MS) sairastavilla kuntoutujilla keskittyen erityisesti ICF:n suoritukset ja osallistuminen -osa-alueeseen. Vaikuttavuutta koskevat määrälliset kirjallisuuskatsaukset (vaihteluväli: AVH 1/2008–5/2017; MS 1/2007–1/2017) kohdistuivat satunnaistettuihin kontrolloituihin terapeuttisen harjoittelun tutkimuksiin (RCT) painottuen tasapainoon ja kävelyyn. Laadulliset kirjallisuuskatsaukset fysioterapian merkityksellisyydestä (1/2000–11/2017) kohdistuivat kuntoutujan kokemuksiin ja käsityksiin fysioterapiasta. Kahdeksassa osajulkaisussa oli yhteensä 182 RCT-tutkimusta (6 856 kuntoutujaa) sekä 50 laadullista tutkimusta (711 kuntoutujaa). Tutkimukseen osallistujat ja interventiot olivat heterogeenisiä. Terapeuttisella harjoittelulla oli suurempi vaikutus kuntoutujien tasapainoon ja kävelyyn liittyvään suorituskykyyn vakioiduissa olosuhteissa, kun harjoittelua verrattiin ei-harjoitteluun tai tavanomaiseen hoitoon. Sen sijaan vaikutus suoritustasolla oli huomioitu vain muutamassa tutkimuksessa. Tuloksia voidaan hyödyntää etenkin kroonisessa vaiheessa olevien AVH-kuntoutujien sekä lievää tai keskivaikeaa MS-tautia sairastavien henkilöiden kuntoutuksessa. Kuntoutujat kokivat merkityksellisenä sen, että fysioterapiassa korostuivat ihmisarvon ylläpysyminen, yksilöllisenä ihmisenä kohdatuksi tuleminen, itsenäisyyden ja vapauden mahdollistuminen, tavalliseen elämään osallistuminen sekä tulevaisuuden uskon ja toivon herääminen ja ylläpysyminen. Kuntoutujat arvostivat tasavertaista, ammattitaitoista, laaja-alaista ja oikeudenmukaista fysioterapiaa, jossa huomioitiin kuntoutumisen merkityksellisyyden ylläpysyminen ja kuntoutumiseen liittyvien haasteiden voittamisen mahdollistuminen. Tulevaisuudessa tutkimuksissa tulisi huomioida paremmin arkisuoriutuminen ja osallistuminen sekä suomalainen kuntoutuksen monitahoinen palvelujärjestelmä. peerReviewed

Tämä Terveystieteiden opettajan andragoginen käsikirja on syntynyt lukuvuonna 2018-19 terveystieteiden opettajiksi opiskelleiden Tutkiva ja kehittävä opettajuus -opintokokonaisuuden töistä. Terveystieteiden opettajien ydinosaamista ovat kehollisuus ja liikkumisen ohjaaminen (Teema 1). Opettajan oma asiantuntijuus on oleellista tiedostaa, ja usein monitieteisissä ja monialaisissa tiimeissä on osattava myös kertoa omasta osaamisestaan. Tästä hyvänä esimerkkinä on Jyväskylän koulutuskuntayhtymä Gradian, Keski-Suomen Liikunnan ja Jyväskylän yliopiston yhteistyössä kehittämä paikallinen ammatillisen koulutuksen valinnainen tutkinnonosa. Opettajaopiskelijoiden kehittämistehtävissä muutos ja tulevaisuuden opettajuus ovat keskeisenä teemana (Teema 2). Tulevaisuuden näkymiä on kartoitettu sekä fysioterapiaopiskelijoilla että –opettajilla. Tulevaisuudessa digipedagogiikka haastaa sekä opettajia että opiskelijoita. Tätä tärkeää ja ajankohtaista teemaa on käsikirjassamme tarkasteltu sekä opiskelijoiden kokemana, että opettajuuden näkökulmasta. Vaikka opetus yhä enenemässä määrin siirtynee digipedagogiikkaa toteuttavaksi, ei opettajan läsnäoloa silloinkaan tule sivuuttaa. Opettajuudessa on tärkeätä huomioida opiskelijoiden jaksaminen ja pyrkiä edistämään heidän hyvinvointiaan ja toimintakykyään (Teema 3). Käsikirjassa pohditaan opettajan roolia mielenterveydellisissä ongelmissa sekä erityistä tukea tarvitsevien lasten ohjauksessa. Terveystieteiden opettajat toimivat laajasti erilaisissa ympäristöissä ja koulutuksen eri asteilla. Toivomme, että andragogiikan teoreettinen osaaminen yhdessä oman alan ydinosaamisen kanssa edesauttavat teitä vastavalmistuneet terveystieteiden opettajat työurallanne.
