Abstract

Zusammenfassung: Unfälle mit chemischen, biologischen oder radioaktiven Gefahrenstoffen sind selten. Aus diesem Grund haben die meisten Krankenhäuser keine Routine in der Versorgung dieser Patienten. Prinzipiell besteht in einigen Fällen Unklarheit über die mögliche Gefährdung Dritter. Aus Sicherheitsgründen sollte die medizinische Versorgung dieser Patienten in gesonderten Räumlichkeiten unter besonderen Schutzvorkehrungen durchgeführt werden, bis eine weitere Gefährdung ausgeschlossen werden kann. Die Schutzvorkehrungen erstrecken sich sowohl auf das Personal als auch auf unbeteiligte Dritte. Außerdem ist in jedem Fall eine möglichst frühe Dekontamination anzustreben. Anhand des Falles eines nach einer Explosion mit rotem Phosphor kontaminierten Patienten, welcher in unseren Schockraum gebracht wurde, wollen wir ein mögliches Konzept vorstellen und mit der (spärlich vorhandenen) Literatur diskutieren.

Abstract

The selection pressures that drove dramatic encephalisation processes through the mammal lineage remain elusive, as does knowledge of brain structure reorganisation through this process. In particular, considerable structural brain changes are present across the primate lineage, culminating in the complex human brain that allows for unique behaviours such as language and sophisticated tool use. To understand this evolution, a diverse sample set of humans' closest relatives with varying socio-ecologies is needed. However, current brain banks predominantly curate brains from primates that died in zoological gardens. We try to address this gap by establishing a field pipeline mitigating the challenges associated with brain extractions of wild primates in their natural habitat. The success of our approach is demonstrated by our ability to acquire a novel brain sample of deceased primates with highly variable socio-ecological exposure and a particular focus on wild chimpanzees. Methods in acquiring brain tissue from wild settings are comprehensively explained, highlighting the feasibility of conducting brain extraction procedures under strict biosafety measures by trained veterinarians in field sites. Brains are assessed at a fine-structural level via high-resolution MRI and state-of-the-art histology. Analyses confirm that excellent tissue quality of primate brains sourced in the field can be achieved with a comparable tissue quality of brains acquired from zoo-living primates. Our field methods are noninvasive, here defined as not harming living animals, and may be applied to other mammal systems than primates. In sum, the field protocol and methodological pipeline validated here pose a major advance for assessing the influence of socio-ecology on medium to large mammal brains, at both macro- and microstructural levels as well as aiding with the functional annotation of brain regions and neuronal pathways via specific behaviour assessments.

Abstract

Genetic alterations in signaling pathways that control cell-cycle progression, apoptosis, and cell growth are common hallmarks of cancer, but the extent, mechanisms, and co-occurrence of alterations in these pathways differ between individual tumors and tumor types. Using mutations, copy-number changes, mRNA expression, gene fusions and DNA methylation in 9,125 tumors profiled by The Cancer Genome Atlas (TCGA), we analyzed the mechanisms and patterns of somatic alterations in ten canonical pathways: cell cycle, Hippo, Myc, Notch, Nrf2, PI-3-Kinase/Akt, RTK-RAS, TGFβ signaling, p53 and β-catenin/Wnt. We charted the detailed landscape of pathway alterations in 33 cancer types, stratified into 64 subtypes, and identified patterns of co-occurrence and mutual exclusivity. Eighty-nine percent of tumors had at least one driver alteration in these pathways, and 57% percent of tumors had at least one alteration potentially targetable by currently available drugs. Thirty percent of tumors had multiple targetable alterations, indicating opportunities for combination therapy. Abstract

A study assesses primary (after hatching) and secondary (after juvenile mortality) outcrossing rates of two mixed-mating snail populations in order to investigate the variation in mating-system parameters and estimate significance of inbreeding depression for secondary outcrossing rate. Results show that outcrossing rate is much more variable among families in the population with the lower average outcrossing rate, suggesting that individuals differ considerably in the mating system.

Abstract

Context: Twin-bearing ewes may be unable to produce enough colostrum following birth to support the survival of two lambs. This causes the newborn lambs to be at risk of death due to starvation, mismothering, cold exposure and infection. Niacin has previously been demonstrated in cattle to increase immunoglobulin concentrations in colostrum, possibly due to niacin increasing blood flow, allowing the greater accumulation of nutrients and immunoglobulins in colostrum. High producing animals, such as the late gestation twin-bearing ewe, may not be able to produce enough niacin to meet their production requirements. Aims: This experiment evaluated whether supplementation of niacin (vitamin B3) to late gestation twin-bearing ewes would increase colostrum production and composition (concentration of fat, lactose, protein and immunoglobulin G), and postnatal lamb growth. Methods: A total of 24 artificially inseminated Merino ewes were weighed, body condition scored, aged and randomly allocated to a treatment – control treatment or with 5 g/day niacin supplemented for 2 weeks before lambing until 2–8 days post-lambing. Key results: Colostrum production and immunoglobulin G concentration were similar between treatments (P > 0.05). Ewes supplemented with niacin produced significantly higher (P = 0.039) colostral protein at 12.54% compared with the control group ewes at 11.78%. Early lamb growth rates were unaffected (P > 0.05) by treatment. Conclusions: Supplementation of ewes with 5 g/day niacin is not recommended to increase the production of colostrum and immunoglobulin G or to increase lamb growth rates following birth. Implications: Further research into niacin supplementation, supplementation time and supplementation amount for production effects in late gestation ewes is warranted. Colostrum production following birth is integral for lamb survival, providing energy and preventing infection by supplying immunoglobulins. Abstract

Context: Lamb survival can be compromised by twin-bearing ewes being unable to produce enough colostrum and immunoglobulins following parturition. Without sufficient colostrum production, newborn lambs are at risk of death due to starvation, mismothering, exposure and infection. These deaths can be reduced by altering the ewe's nutrition in the final weeks of pregnancy. Starch, a glucose precursor, has increased colostrum production; however, the digestibility of starch can be increased with protein supplement, which may increase colostrum production. Aims: Two separate experiments evaluated if protein supplementation, to a high-starch diet could improve colostrum production and immunoglobulin G (IgG) concentration in twin-bearing ewes. It was hypothesised that the addition of protein to a high-starch diet would have a cumulative effect on colostrum production and IgG. Methods: Ewes in both experiments were randomly allocated to one of two treatments –control and protein (120 g/day canola meal) – with both treatments receiving 600 g/head.day barley grain. Feeding began from Day 130 of gestation until 2–10 days post-lambing. Key results: Colostrum IgG concentration at 2–10 days old did not differ between treatments (P > 0.05) in both experiments. In Experiment 2, colostrum (3–7 h post-lambing) and milk production (2 days post-lambing) did not differ between treatments (P > 0.05), although metabolisable energy intake was affected by treatment (P = 0.018) with protein supplemented ewes consuming 12.91 ± 0.612 MJ ME/day and control ewes consuming 10.78 ± 0.854 MJ ME/day. Conclusions: Supplementation of ewes with protein when consuming high-starch diets did not increase colostrum production nor the IgG concentration of colostrum in ewes with pre-lambing body condition scores above industry recommendations. Implications: Supplementation of protein and starch combined has limited commercial viability and would not be recommended for late gestation ewes to increase colostrum production. However, supplementation of ewes on low-quality feed with canola meal can increase metabolisable energy intake, which may have other production benefits. Colostrum production and colostral immunoglobulin concentration can compromise twin lamb survival, reducing productivity and profitability on-farm. Abstract

Context: Twin-bearing ewes may be unable to produce enough colostrum following birth to support the survival of two lambs. This causes the newborn lambs to be at risk of death due to starvation, mismothering, cold exposure and infection. Niacin has previously been demonstrated in cattle to increase immunoglobulin concentrations in colostrum, possibly due to niacin increasing blood flow, allowing the greater accumulation of nutrients and immunoglobulins in colostrum. High producing animals, such as the late gestation twin-bearing ewe, may not be able to produce enough niacin to meet their production requirements. Aims: This experiment evaluated whether supplementation of niacin (vitamin B3) to late gestation twin-bearing ewes would increase colostrum production and composition (concentration of fat, lactose, protein and immunoglobulin G), and postnatal lamb growth. Methods: A total of 24 artificially inseminated Merino ewes were weighed, body condition scored, aged and randomly allocated to a treatment – control treatment or with 5 g/day niacin supplemented for 2 weeks before lambing until 2–8 days post-lambing. Key results: Colostrum production and immunoglobulin G concentration were similar between treatments (P > 0.05). Ewes supplemented with niacin produced significantly higher (P = 0.039) colostral protein at 12.54% compared with the control group ewes at 11.78%. Early lamb growth rates were unaffected (P > 0.05) by treatment. Conclusions: Supplementation of ewes with 5 g/day niacin is not recommended to increase the production of colostrum and immunoglobulin G or to increase lamb growth rates following birth. Implications: Further research into niacin supplementation, supplementation time and supplementation amount for production effects in late gestation ewes is warranted. Abstract

Survival of lambs affects the profitability of Merino enterprises, with mortality from birth to marking the most prominent factor leading to poor reproductive performance. Survival of lambs following parturition is dependent on the ability to access colostrum and milk, which can be altered by the provision of barley grain to promote lactogenesis and colostrum production when feed availability is suboptimal. The present study evaluated whether the survival of lambs could be improved by supplementing ewes grazing abundant quality pasture with barley grain. Naturally joined Merino ewes (n = 240, 65.5 kg, body condition score (CS) 3) bearing twin fetuses grazing spring pastures (1712 kg DM/ha green, 12.3 MJ ME/kgDM, and 18.5% crude protein at the commencement of the experiment) were allocated to a supplemented or a control diet, with two replicates of each. Ewes were supplemented with 550 g/ewe.day barley grain (12.6 MJ ME/kgDM, 10% crude protein) for 20 days before and 2 weeks following the commencement of lambing (supplemented), or were unsupplemented (control). Ewes gained 0.5 CS and 10.6 kg liveweight from pre-lambing to lamb marking age in both treatments, indicating that their energy intake was high. The proportion of lambs surviving from birth to marking was 0.79 for control and supplemented treatments. Lamb survival was affected by the interaction (P = 0.039) between treatment and sex, with control female lambs having a significantly higher survival than control male lambs. Supplemented lambs reared as singles (17.1 ± 0.95 kg) had significantly (P < 0.05) heavier marking weights thanthe control lambs reared as singles (14.8 ± 1.1 kg), supplemented lambs reared as twins (14.7 ± 0.71 kg) and control lambs reared as twins (15.5 ± 0.70 kg). Lamb survival was not increased by provision of barley grain in the final weeks before lambing and the first 2 weeks of lambing when ewes were in good CS and grazed ample high-quality pastures. Lamb survival in Merino ewes, especially those bearing twins, can be very low leading to reduced production and animal-welfare concerns. The present study investigated whether providing barley grain in late gestation can improve lamb survival when pasture availability is high. Abstract

BACKGROUND:Thrombin potently activates platelets through the protease-activated receptor PAR-1. Vorapaxar is a novel antiplatelet agent that selectively inhibits the cellular actions of thrombin through antagonism of PAR-1.METHODS:We randomly assigned 26,449 patients who had a history of myocardial infarction, ischemic stroke, or peripheral arterial disease to receive vorapaxar (2.5 mg daily) or matching placebo and followed them for a median of 30 months. The primary efficacy end point was the composite of death from cardiovascular causes, myocardial infarction, or stroke. After 2 years, the data and safety monitoring board recommended discontinuation of the study treatment in patients with a history of stroke owing to the risk of intracranial hemorrhage.RESULTS:At 3 years, the primary end point had occurred in 1028 patients (9.3%) in the vorapaxar group and in 1176 patients (10.5%) in the placebo group (hazard ratio for the vorapaxar group, 0.87; 95% confidence interval [CI], 0.80 to 0.94; P
