Abstract

Suspected perioperative allergic reactions are often severe. To avoid potentially life-threatening re-exposure to the culprit drug, establishing a firm diagnosis and identifying the culprit is crucial. Drug provocation tests are considered the gold standard in drug allergy investigation but have not been recommended in the investigation of perioperative allergy, mainly because of the pharmacological effects of drugs such as induction agents and neuromuscular blocking agents. Some specialised centres have reported benefits of provocation testing in perioperative allergy investigation, but the literature on the subject is limited. Here we provide a status update on the use of drug provocation testing in perioperative allergy, including its use in specific drug groups. This review is based on a literature search and experiences of the authors comprising anaesthesiologists and allergists with experience in perioperative allergy investigation. In addition, 19 participating centres in the International Suspected Perioperative Allergic Reaction Group were surveyed on the use of provocation testing in perioperative allergy investigation. A response was received from 13 centres in eight European countries, New Zealand, and the USA. Also, 21 centres from the Australian and New Zealand Anaesthetic Allergy Group were surveyed. Two centres performed provocation routinely and seven centres performed no provocations at all. Nearly half of the centres reported performing provocations with induction agents and neuromuscular blocking agents. Drug provocation testing is being used in perioperative allergy investigation in specialised centres, but collaborations between relevant specialties and multicentre studies are necessary to determine indications and establish common testing protocols.

Abstract

Suspected perioperative allergic reactions are rare but can be life-threatening. The diagnosis is difficult to make in the perioperative setting, but prompt recognition and correct treatment is necessary to ensure a good outcome. A group of 26 international experts in perioperative allergy (anaesthesiologists, allergists, and immunologists) contributed to a modified Delphi consensus process, which covered areas such as differential diagnosis, management during and after anaphylaxis, allergy investigations, and plans for a subsequent anaesthetic. They were asked to rank the appropriateness of statements related to the immediate management of suspected perioperative allergic reactions. Statements were selected to represent areas where there is a lack of consensus in existing guidelines, such as dosing of epinephrine and fluids, the management of impending cardiac arrest, and reactions refractory to standard treatment. The results of the modified Delphi consensus process have been included in the recommendations on the management of suspected perioperative allergic reactions. This paper provides anaesthetists with an overview of relevant knowledge on the immediate and postoperative management of suspected perioperative allergic reactions based on current literature and expert opinion. In addition, it provides practical advice and recommendations in areas where consensus has been lacking in existing guidelines.

Abstract

Perioperative immediate hypersensitivity reactions are rare. Subsequent allergy investigation is complicated by multiple simultaneous drug exposures, the use of drugs with potent effects and the many differential diagnoses to hypersensitivity in the perioperative setting. The approach to the investigation of these complex reactions is not standardized, and it is becoming increasingly apparent that collaboration between experts in the field of allergy/immunology/dermatology and anaesthesiology is needed to provide the best possible care for these patients. The EAACI task force behind this position paper has therefore combined the expertise of allergists, immunologists and anaesthesiologists. The aims of this position paper were to provide recommendations for the investigation of immediate-type perioperative hypersensitivity reactions and to provide practical information that can assist clinicians in planning and carrying out investigations.

35. 1749-P: A Case Report of IgE-Mediated Insulin Allergy Successfully Managed with a Fixed Combination of Degludec and Liraglutide.

Abstract

Background: Insulin allergy is a rare condition that poses a great challenge particularly in diabetics who are insulin dependent. If feasible, switching to different antidiabetic agents or immunotherapy is the common approach, but the universal consensus on insulin allergy management in diabetics is lacking. Here we present a clinical case of a diabetic with allergy to insulin successfully managed with a combination of degludec and liraglutide. Case Report: A 51-year-old female patient with type 2 diabetes presented with poorly controlled glycaemia (HbA1c 8.6%) on dual oral antidiabetic treatment, metformin and sitagliptin. Due to pronounced fasting hyperglycaemia the NPH insulin before bedtime, but soon began to complain of urticariform skin lesions on insulin injection sites. Despite the adverse reaction the patient continued to up-titrate the insulin, but the effect on glycaemia was negligible. After switching to different NPH insulin manufacturer, to no avail, the insulin analogs detemir and glargin were introduced, respectively, but produced same adverse reaction. The elevated levels of specific IgE immunoglobulins to human insulin (2.58 klU/L) confirmed the allergy to insulin. Conversely, the novel insulin analog degludec yielded no skin reaction. The patient was then titrated to a fixed combination of degludec and GLP-1 receptor agonist liraglutide which provoked no allergic reaction and promptly improved glycemic control (HbA1c 6.7% 4 months after treatment commencement). Understandably, treatment with sitagliptin was stopped, whereas metformin was continued. Conclusion: The patient with confirmed IgE mediated allergy to insulin exhibited allergic reaction to all types of basal insulin but degludec. It can be assumed that the unique structure of degludec has different epitopes than other insulin analogues and therefore provides safe and combined with GLP-1 receptor agonist effective therapeutic approach to patient with allergy to insulin. Abstract

DOT-FH-11-8814, This report summarizes the major findings of a detailed, comprehensive report which has received only a limited distribution. The full report, FHWA-RD-78-209, is available upon request. An interdisciplinary team of researchers has conducted an in-depth review of the literature, supplemented with expert opinions, research experience, and limited tests, to develop a state-of-the-art on the topics of (a) properties of aggregates, (b) pavement texture and skid resistance, (c) tire-pavement noise, (d) tire wear, (e) light reflection and glare, (f) splash and spray, (g) rolling resistance and fuel consumption, and (h) optimal aggregates and pavement surfacing systems. A cost-benefit analysis is also summarized. The information on aggregates and their contribution to pavement texture, coarse and fine, is being used in on-going FHWA staff and contract research studies to investigate currently and potentially available ceramic and impregnation materials and processes for the manufacture or treatment of aggregates to provide extremely durable and skid-resistant pavement surfaces.

Abstract

Introduction: The incidence of allergy to penicillin is highly overestimated. Many patients are labeled as penicillin-allergic, which is often unconfirmed. We report an analysis of patients that were referred for evaluation of suspected penicillin allergy in 2007 and 2008., Methods: In a 2-year period, 606 patients were referred: 460 (76%) female, average age 42 (14-85) years. The diagnostic procedure started with specific IgE (sIgE) measurement, followed by skin prick and intradermal tests with PPL, MDM, and the suspected antibiotic. If all tests were negative, a drug provocation test was performed (DPT). If more than 3 years had passed from the reaction and if the reaction was not severe, the DPT followed serological tests., Results: In 49 (8%) patients, sIgE to penicillin was detected. Skin testing was performed on 274 (45%) sIgE-negative patients, with positive results in 14 (5%) patients. In 426 (70%) patients, DPT with the suspected drug was performed, which was positive in 19 (4.5%) patients. Diagnosis of penicillin allergy was established in 82 (13.5%) patients., Conclusions: Tests of immediate hypersensitivity to penicillins were positive in a minority of patients referred. It is important to confirm or exclude suspected allergy to antibiotics because unnecessary use of more expensive broad-spectrum agents also contributes to the development and spread of certain types of drug-resistant bacteria.
