Background Patients with stage 4/5 chronic kidney disease (CKD) suffer from various symptoms. The retention of uremic solutes is thought to be associated with those symptoms. However, there are relatively few rigorous studies on the potential links between uremic toxins and symptoms in patients with CKD. Methods The EQUAL study is an ongoing observational cohort study of non-dialyzed patients with stage 4/5 CKD. EQUAL patients from Germany, Poland, Sweden and the UK were included in the present study (n = 795). Data and symptom self-report questionnaires were collected between April 2012 and September 2020. Baseline uric acid and parathyroid hormone and 10 uremic toxins were quantified. We tested the association between uremic toxins and symptoms and adjusted P-values for multiple testing. Results Symptoms were more frequent in women than in men with stage 4/5 CKD, while levels of various uremic toxins were higher in men. Only trimethylamine N-oxide (TMAO; positive association with fatigue), p-cresyl sulfate (PCS) with constipation and 3-carboxy-4-methyl-5-propyl-2-furanpropionic acid (negative association with shortness of breath) demonstrated moderately strong associations with symptoms in adjusted analyses. The association of phenylacetylglutamine with shortness of breath was consistent in both sexes, although it only reached statistical significance in the full population. In contrast, TMAO (fatigue) and PCS and phenylacetylglutamine (constipation) were only associated with symptoms in men, who presented higher serum levels than women. Conclusion Only a limited number of toxins were associated with symptoms in persons with stage 4/5 CKD. Other uremic toxins, uremia-related factors or psychosocial factors not yet explored might contribute to symptom burden.

In hemodialysis patients, non-adherence to fluid restriction is associated with high interdialytic weight gain (IDWG) and adverse outcomes. Monitoring drinking behaviour and direct feedback to the patient can lead to better adherence. We developed the MySleeve, a device that can be wrapped around a drinking glass to monitor fluid intake throughout the day. The MySleeve will also provide a subtle vibration on the glass when the amount drunk exceeds target. The information about drinking behaviour can be found in the accompanying application on a mobile phone. In this study, we investigate the effect of direct feedback and information to the patient on fluid restriction adherence, measured by the IDWG

- Abstract
Both in dialysis patients and non-uremic patients heart failure is associated with an adverse prognosis. In a state of abrupt worsening of cardiac function, acute cardiogenic shock or decompensated congestive heart failure, acute kidney injury may occur, whereas in a more chronic worsening of cardiac function chronic kidney injury may occur. Recently, the term cardiorenal syndrome was adopted and defined as "a pathophysiological disorder of the heart and kidneys whereby acute or chronic dysfunction in one organ may induce acute or chronic dysfunction in the other organ". Despite better treatment techniques and the continuous development of new medications volume overload in patients with cardiorenal syndrome is difficult to treat. Especially treatment of cardiorenal syndrome type I and II is notoriously difficult. Peritoneal dialysis might be, because of the gradual fluid removal, a therapeutic option in these patients. However, data on the effect of peritoneal dialysis in patients with heart failure with fluid overload and/or renal impairment are scarce. In this reviewe, the role of peritoneal dialysis in the treatment cardiorenal syndrome type I, II and IV will be discussed.

27. Evolution of protein-bound uraemic solutes during predilution haemofiltration

- Abstract
Background: Protein-bound uraemic toxins provoke multiple biological changes involved in uraemia. Few if any dialytic strategies remove these compounds. Methods: In this post hoc analysis of remnant samples from a randomised controlled trial, we evaluate whether predilution haemofiltration (HF) decreases the pretreatment concentration of protein- bound uraemic solutes. Patients treated with low-flux haemodialysis (HD) were enrolled into a group continuing this strategy (group A, n=8) over 6 months, whereas group B (n=12) was switched to predilution online HF. Blood was sampled at baseline and after 6 months to determine total and free concentration and percentage binding of indoxyl sulfate (IS), indole-3-acetic acid (IAA), hippuric acid (HA), p-cresol (PC) and 3-carboxy- 4-methyl-5-propyl-2-furanpropionic acid (CMPF). Results: Comparing concentrations at start versus 6 months of treatment by paired analysis, HD had no impact. In contrast, at the end of the HF period, we found a decrease in total and free PC, free IAA and total CMPF. In addition, the percentage protein binding of IAA increased significantly. However, unpaired analysis revealed no statistical difference between HD and HF, both at baseline and after 6 months of treatment for all compounds. Conclusions: Paired analysis showed a beneficial impact of predilution online HF for several proteinbound uraemic solutes. Unpaired analysis, however, showed no statistical difference.

A lowering of the glomerular filtration rate (GFR) and/or the presence of albuminuria are signs of chronic renal disease. Both variables are for the most part independently associated with an increased risk of cardiovascular morbidity and mortality. Albuminuria is a marker of endothelial dysfunction. A decrease of the GFR is associated with non-traditional risk factors, e.g. renal anaemia, uraemic toxins due to a decrease of the renal clearance, hyperhomocysteinaemia caused by a diminished homocysteine metabolism, excessive activation of the sympathetic nervous system which is related to sleep apnoea syndrome, oxidative stress and dyslipidaemia associated with the formation of vasotoxic, oxidised LDL cholesterol. These non-traditional risk factors may, alone or in combination with traditional atherogenic risk factors (e.g. age, male gender, smoking, hypercholesterolaemia, hypertension, obesity, positive family history and diabetes mellitus), partially via endothelial dysfunction, result in harmful effects on arterial function, increasing cardiovascular morbidity and mortality. Different stages of chronic kidney disease are associated with specific risk factors, making a specific therapeutic approach essential.

- Abstract
Differences in vascular reactivity between isolated ultrafiltration (i-UF) and hemodialysis (UF + HD) have been attributed to various factors, including differences in core temperature (CT) and energy transfer (ET). However, the relative importance of these thermal factors is not known. The aim of this study was to elucidate to what extent differences in ET are responsible for the divergent vascular response between i-UF and UF + HD. During four different dialysis treatments in 15 patients, four measurements were performed that consisted of 1 h of i-UF, UF + HD at a dialysate temperature (T-d) of 37.5 degrees C (UF + HD37.5), UF + HD at T-d 35.5 degrees C (UF + HD35.5) and UF + HD with a similar ET as during i-UF(UF + HDET-set). The UF rate in all sessions was 1 L/h. CT (degrees C) decreased significantly during i-UF and UF + HDET-set (P < 0.05), increased significantly during UF + HD37.5 (P < 0.05), and remained unchanged during UF + HD35.5 (NS). Forearm vascular reactivity increased significantly during i-UF, UF + HDET-set, and UF + HD35.5 (P < 0.05), but not during UF + HD37.5 (NS). Venous tone increased significantly during i-UF, UF + HD35.5, and UF + HDET-set (P < 0.05), and decreased significantly during UF + HD37.5 (P < 0.05). When i-UF and UF + HD are matched for ET, all differences in vascular response disappear, showing that differences in ET are the single most important factor for the observed difference in vascular response between i-UF and UF + HD. In contrast to UF + HD37.5, vascular reactivity was improved when the increase in CT was prevented during UF + HD35.5 and appeared to increase more when CT was lowered. Preventing the increase in CT during UF + HD appears to be mandatory for optimization of hemodynamic stability during dialysis.

Contains fulltext : 49762.pdf (publisher's version ) (Closed access), BACKGROUND: Sodium profiling improves haemodynamic tolerance in haemodialysis (HD) patients but may also influence sodium homeostasis. Changes in blood volume and plasma conductivity (PC) during HD can be modelled by feedback technology, but their effects on sodium homeostasis are not widely studied. METHODS: This randomized crossover study compared PC and ionic mass balance (IMB) as surrogate markers of sodium balance between standard HD [dialysate conductivity (DC) 14.0 mS/cm], sodium profiling (DC 15.0-->14.0 mS/cm), blood volume (BV)-controlled and PC-controlled feedback (target: post-HD PC: 14.0 mS/cm) in 10 HD patients with frequent hypotension. RESULTS: 440 treatments were studied. Pre-dialytic PC was significantly higher during SP (14.4+/-0.2 mS/cm) compared to standard HD (14.2+/-0.3 mS/cm), and was not different between the other manoeuvres: PC-controlled (14.1+/-0.3 mS/cm), and BV-controlled feedback (14.2+/-0.2 mS/cm). Except for the first treatment, during which IMB was lower during the sodium profile, IMB did not differ significantly between the various manoeuvres and was strongly dependent upon ultrafiltration volume and the difference between pre-dialytic PC and DC. Symptomatic hypotensive episodes occurred least frequently during BV-controlled feedback (8%) compared to the other manoeuvres (standard HD, 16%; sodium profile, 14%; PC-controlled feedback, 17%), but differences were not significant. Inter-dialytic weight gain and pre-dialytic systolic blood pressure did not differ. CONCLUSIONS: Pre-dialytic PC increased during the sodium profile, and did not differ between BV- or PC-controlled feedback compared to standard HD. Thus, it appears that both BV- and PC-controlled feedback can be safely prescribed without substantial salt- and water-loading, at least in the short term. Analysis of IMB is useful to assess differences in sodium balance between single treatment sessions but appears of less value in a steady-state situation.

47. Morphological changes of the venous system in uremic patients. A histopathologic study

- Abstract
Because of the decreased venous compliance in hypertensive dialysis patients, it was investigated whether their venous system exhibited structural abnormalities. Venous samples were taken during transplantation from the common and external iliac vein in 12 hypertensive and 6 normotensive uremic patients and from the distal inferior caval vein and the common iliac vein in 7 kidney donors and 5 autopsy patients without history of cardiovascular disease. The thickness of the venous media was significantly increased in hypertensive uremic patients as compared to controls, but did not differ between normotensive patients and controls. The quantity of medial collagen did not differ among the various groups. The smooth muscle content of the media was increased in 5 uremic patients (2 normotensive and 3 hypertensive patients), whereas almost no smooth muscle was observed in the media of controls. Intimal thickening was observed neither in uremic patients nor in controls. In conclusion: increased medial thickness in hypertensive dialysis patients could be an explanation for the decreased venous compliance previously found in these patients
