Newer-generation DESs have been developed to overcome this limitation using novel stent platforms, new drugs, more biocompatible durable polymers, and bioabsorbable polymers or backbones. To date, new-generation DESs have virtually replaced the use of first-generation DESs worldwide. In this review article, we discuss in detail the design, pharmacology, and mechanism of action of the newer-generation permanent and bioresorbable everolimus-eluting platforms. Furthermore, we present and evaluate the current evidence on the performance and safety of these devices compared to those of other available stent platforms. Abstract

Pain radiating from the spine into the leg is commonly referred to as “sciatica,” “Sciatica” may include various conditions such as radicular pain or painful radiculopathy. It may be associated with significant consequences for the person living with the condition, imposing a reduced quality of life and substantial direct and indirect costs. The main challenges associated with a diagnosis of “sciatica” include those related to the inconsistent use of terminology for the diagnostic labels and the identification of neuropathic pain. These challenges hinder collective clinical and scientific understanding regarding these conditions. In this position paper, we describe the outcome of a working group commissioned by the Neuropathic Pain Special Interest Group (NeuPSIG) of the International Association for the Study of Pain (IASP) which was tasked with the following objectives: (1) to revise the use of terminology for classifying spine-related leg pain and (2) to propose a way forward on the identification of neuropathic pain in the context of spine-related leg pain. The panel recommended discouraging the term “sciatica” for use in clinical practice and research without further specification of what it entails. The term “spine-related leg pain” is proposed as an umbrella term to include the case definitions of somatic referred pain and radicular pain with and without radiculopathy. The panel proposed an adaptation of the neuropathic pain grading system in the context of spine-related leg pain to facilitate the identification of neuropathic pain and initiation of specific management in this patient population.

29. Clinical effectiveness of thrombus aspiration during percutaneous coronary intervention for stent thrombosis in a contemporary setting

Abstract

Objective: The impact of adjunctive manual thrombus aspiration (TA) in patients with stent thrombosis (ST) treated with percutaneous intervention has not been evaluated in the current era of potent P2Y12 agents and new-generation drug-eluting stents. We sought to assess the effect of TA using data from a large contemporary registry. Methods: The study population was derived from the Essex ST Investigation Registry (ESTHIR), which contains all consecutive cases of angiographically determined definite ST undergoing interventional treatment in a tertiary cardiac centre between November 2015 and June 2018. Propensity score matching was performed to match patients who underwent TA (TA group) to those who did not (n-TA group). The study endpoints were final TIMI flow and survival free of cardiovascular death (CD) or target lesion revascularisation (TLR). Results: A total of 128 ST patients were included in the present analysis. The mean age was 65 ± 11 years, and 84% were male. About 90% of the patients presented with STEMI, and 85% had very late ST. Seventy-two patients (56%) underwent TA. After propensity score matching, 30 patients were included in each study group. A higher rate of final TIMI III flow was observed in the TA group (TA vs n-TA group, 100% vs 83%), but this difference did not reach statistical significance (p = 0.052). At 1000 days of follow-up, survival free of CD or TLR was not different between the two groups (p = 0.8). Conclusion: In a propensity-matched population of ST patients undergoing PCI in a contemporary setting, TA was not associated with improved final TIMI flow or long-term cardiovascular outcomes. © 2022 Hellenic Society of Cardiology

33. Moment tensor inversion of microearthquakes along the Santorini-Amorgos zone: Tensile faulting and emerging volcanism in an extensional setting

Abstract

The Santorini-Amorgos zone is an area rich in microseismicity at the center of the Hellenic volcanic arc. The microseismicity of the zone is distributed along the Santorini-Amorgos ridge and Kolumbo submarine volcano. In this study, we utilized crustal events that were recorded by temporary networks during September 2002 to July 2004 and October 2005 to March 2007, and also by the permanent network from 2011 to 2020. These events were inverted for their moment tensors by using P-wave polarities as well as SV/P and SH/P amplitude ratios, yielding 74 well-constrained moment tensor solutions. Most of these moment tensors have significant CLVD and isotropic components that are positively correlated to each other (R2 = 0.68). Tensile faulting due to high pore pressure is considered as the most likely cause of the observed non-DC components. The positive and negative non-DC components observed in Kolumbo may be generated by the opening and closing of cracks beneath the shallow (6–7 km) magma chamber due to a steady migration of magmatic fluids from the deep reservoir into the chamber. In Anydros, most of the microearthquakes have positive non-DC components associated with the opening of cracks. It is possible that the extensional deformation and high pore fluid pressure in the area opens subvertical cracks that become pathways for upward migrating fluids. The upward migration of magmatic fluids in an extensional regime such as the Santorini-Amorgos zone can also be viewed as an indication of emerging volcanic activity in this area.

Abstract

Phase-change memory materials are promising candidates for beyond-silicon, next-generation non-volatile-memory and neuromorphic-computing devices; the canonical such material is the chalcogenide semiconductor alloy Ge2Sb2Te5. Here, we describe the results of an analysis of glassy molecular-dynamics models of this material, as generated using a newly developed, linear-scaling (O(N)), machine-learned, Gaussian approximation potential. We investigate the behaviour of the glassy models as a function of different quench rates (varied by two orders of magnitude, down to 1 K ps−1) and model sizes (varied by two orders of magnitude, up to 24 300 atoms). It is found that the lowest quench rate studied (1 K ps−1) is comparable to the minimum cooling rate needed in order completely to vitrify the models on quenching from the melt.
