In orthopedic surgery, patient-specific bone plates are used for fixation when conventional bone plates do not fit the specific anatomy of a patient. However, plate failure can occur due to a lack of properly established design parameters that support optimal biomechanical properties of the plate. This review provides an overview of design parameters and biomechanical properties of patient-specific bone plates, which can assist in the design of the optimal plate. A literature search was conducted through PubMed and Embase, resulting in the inclusion of 78 studies, comprising clinical studies using patient-specific bone plates for fracture fixation or experimental studies that evaluated biomechanical properties or design parameters of bone plates. Biomechanical properties of the plates, including elastic stiffness, yield strength, tensile strength, and Poisson’s ratio are influenced by various factors, such as material properties, geometry, interface distance, fixation mechanism, screw pattern, working length and manufacturing techniques. Although variations within studies challenge direct translation of experimental results into clinical practice, this review serves as a useful reference guide to determine which parameters must be carefully considered during the design and manufacturing process to achieve the desired biomechanical properties of a plate for fixation of a specific type of fracture. Staatsbosbeheer is eigenaar van natuurgebieden de Zunasche Heide, Middelveen, Overtoom en Venegge. In 2012 is door DLG landschapsecologisch onderzoek uitgevoerd om de natuurpotenties, knelpunten en maatregelen te kunnen vaststellen. In 2014 (Zunasche Heide, Middelveen en Overtoom) en 2021 (Zunasche Heide) is een groot deel van deze maatregelen uitgevoerd. De wens is om het effect van deze maatregelen tegen het licht te houden en te constateren in hoeverre de natuurdoelen zijn gehaald. Daartoe heeft Staatsbosbeheer aan de Bosgroepen gevraagd samen met hen een evaluatie uit te voeren, op basis van een landschapsecologische systeemanalyse (LESA). Het onderzoek uit 2012 is intussen deels verouderd gezien de uitgevoerde maatregelen. Bovendien blijft, ondanks de goede ontwikkelingen in een groot deel van het gebied, op een aantal plekken de vegetatie-ontwikkeling achter. Daarom is onderzoekscentrum B-WARE betrokken om deze locaties biogeochemisch te onderzoeken. De nieuwe LESA geeft bovendien ruimte voor een verdiepingsslag met betrekking tot de vormingsprocessen, wat meer inzicht geeft in de samenhang tussen de gebieden, en het hydrologisch systeem, waardoor meer inzicht wordt verkregen in de hydrologische kansen en knelpunten voor het gebied.

Background: A common complication after a DIEP flap reconstruction is the occurrence of fat necrosis due to inadequate flap perfusion zones. Intraoperative identification of ischemic zones in the DIEP flap could be optimized using indocyanine green near-infrared fluorescence angiography (ICG-NIR-FA). This randomized controlled trial aims to determine whether intraoperative ICG-NIR-FA for the assessment of DIEP flap perfusion decreases the occurrence of fat necrosis. Design/methods: This article describes the protocol of a Dutch multicenter randomized controlled clinical trial: the FAFI-trial. Females who are electively scheduled for autologous breast reconstruction using DIEP or muscle-sparing transverse rectus abdominis muscle (msTRAM) flaps are included. A total of 280 patients will be included in a 1:1 ratio between both study arms. In the intervention arm, the intraoperative assessment of flap perfusion will be based on both regular clinical parameters and ICG-NIR-FA. The control arm consists of flap perfusion evaluation only through the regular clinical parameters, while ICG-NIR-FA images are obtained during surgery for which the surgeon is blinded. The main study endpoint is the difference in percentage of clinically relevant fat necrosis between both study arms, evaluated two weeks and three months after reconstruction. Conclusion: The FAFI-trial, a Dutch multicenter randomized controlled clinical trial, aims to investigate the clinical added value of intraoperative use of standardized ICG-NIR-FA for assessment of DIEP/msTRAM flap perfusion in the reduction of fat necrosis. 20 juli 2022, Contains fulltext : 252219.pdf (Publisher’s version ) (Open Access), Between 2009 and 2020, beam trawlers in the North Sea switched to electrical stimulation to target sole (Solea solea). The transition to pulse trawling raised widespread concern about possible adverse effects of electrical stimulation on marine organisms. Environmental NGO’s and small scale fishers claimed that it would electrocute marine life and create a ‘graveyard’ in the wake of pulse trawlers. This paper uses realistic field experiments to investigate the ‘graveyard’ hypothesis. In cooperation with fishers, a field experiment was designed where we simultaneously sampled marine organisms in the wake of pulse trawlers and in untrawled control areas. The impact was quantified by estimating the direct mortality among three dominant fish species and four dominant invertebrate species. In total, nine experimental tows were conducted in two years. Direct mortality among fish and invertebrates was low (0-10%) and did not differ between the pulse trawl track and the untrawled controls. Equally, no impact of the pulse trawl was found on external damages and vitality scores. The limited effects observed are likely due to the mechanical impact of the pulse and the sampling gear. The results of experiment do not support the claim that pulse trawling results in mass mortality among marine organisms in the trawl track. Throughout the research period, the concerns of small-scale fishers on pulse fishing shifted from being focused on biological effects to political and managerial issues. This can partly be attributed to the engagement in and the results of our research and has increased its credibility and salience. By integrating fishers’ knowledge and examining their perceptions through transdisciplinary research, we were able to show the importance of untangling the intricate relation between perceived knowledge gaps and political or management related concerns.

Onderzoekcentrum B-WARE is door Prolander gevraagd om een bodem- en hydrochemisch onderzoek uit te voeren om natuurpotenties en geschikte inrichtingsmaatregelen in kaart te brengen voor een aantal (voormalige) landbouwgronden van deelgebied Roodzanden. Het onderzoek is gericht op de bodem- en grondwaterkwaliteit. Het uitvoeren van een (ecohydrologische) systeemanalyse en het opstellen van een inrichtingsplan maken geen deel uit van het onderzoek. Met behulp van de resultaten van dit onderzoek kan de opdrachtgever gericht keuzes maken bij de gebiedsontwikkeling. Het deelgebied Roodzanden ligt op de flanken van het beekdal van de Drentsche Aa, gelegen ten noordoosten van Assen. Het betreft een onderzoeksgebied van circa 39 hectare. Dit onderzoek is onderdeel van het programma ‘Natuurlijk Platteland’, waarbij de Provincie Drenthe aan de ontwikkeling en realisatie van het Natuurnetwerk Nederland (NNN) werkt. Met de realisatie van het NNN wordt ook invulling gegeven aan de doelstellingen vanuit Natura 2000, KRW en het Klimaatakkoord. Er wordt gestreefd naar ecohydrologisch herstel. Dit houdt in dat zoveel mogelijk sloten worden gedempt tot aan het maaiveld dan wel verondiept of afgedamd.

Image-to-patient registration in navigated mandibular surgery is complex due to the mobile nature of the mandible compared with other craniofacial bones. As a result, surgical navigation is rarely employed in the mandibular region. This systematic review provides an overview of the different registration methods that are used for surgical navigation of the mandible. A systematic search was performed in the MEDLINE Ovid, Scopus, and Embase databases on March 25, 2021. Search terms included synonyms for mandibular surgery, surgical navigation, and registration methods. Articles about navigated mandibular surgery, where the registration method was explicitly mentioned, were included. The database search yielded a total of 2952 articles, from which 81 articles remained for analysis. Four main registration methods were identified: point registration, surface registration, hybrid registration, and computer vision-based registration. The mobility of the mandible is accounted for by either keeping the mandible in a fixed position during preoperative imaging and surgery, or by tracking the mandibular movements. Although different registration methods are available for navigated mandibular surgery, there is always a trade-off between accuracy, registration time, usability, and invasiveness. Contains fulltext : 243955.pdf (Publisher’s version ) (Open Access), PURPOSE: Pathogenic variants in SETD1B have been associated with a syndromic neurodevelopmental disorder including intellectual disability, language delay, and seizures. To date, clinical features have been described for 11 patients with (likely) pathogenic SETD1B sequence variants. This study aims to further delineate the spectrum of the SETD1B-related syndrome based on characterizing an expanded patient cohort. METHODS: We perform an in-depth clinical characterization of a cohort of 36 unpublished individuals with SETD1B sequence variants, describing their molecular and phenotypic spectrum. Selected variants were functionally tested using in vitro and genome-wide methylation assays. RESULTS: Our data present evidence for a loss-of-function mechanism of SETD1B variants, resulting in a core clinical phenotype of global developmental delay, language delay including regression, intellectual disability, autism and other behavioral issues, and variable epilepsy phenotypes. Developmental delay appeared to precede seizure onset, suggesting SETD1B dysfunction impacts physiological neurodevelopment even in the absence of epileptic activity. Males are significantly overrepresented and more severely affected, and we speculate that sex-linked traits could affect susceptibility to penetrance and the clinical spectrum of SETD1B variants. CONCLUSION: Insights from this extensive cohort will facilitate the counseling regarding the molecular and phenotypic landscape of newly diagnosed patients with the SETD1B-related syndrome.

Contains fulltext : 232014.pdf (Publisher’s version ) (Open Access), Following the Paris Agreement, investing in renewable energy production at sea is perceived as a promising development. In addition, food security challenges place increased focus on utilizing seas and oceans while biodiversity and ecosystem integrity must be safeguarded. In the North Sea, these three challenges are combined by the Dutch government in a deliberative governance agreement, involving key stakeholders, aimed at establishing a marine spatial allocation strategy. This paper outlines the development and first outcomes of this agreement. Using concepts of sustainability transitions and deliberative governance, we examined the role of the Dutch government in creating sustainability solutions through cross-sectoral participation processes. Based on our results, we argue that only the government can bring together parties that do not actively seek cooperation, like fisheries organizations and wind farm developers. This is in line with the government’s role to ensure compliance with international and national agreements, while also representing local (national) stakes. By opening up a discussion between parties that in the past were competing for marine space, the Dutch government has managed to make progress in obtaining transition goals. One of these transitions is the transition of the governance system itself, moving towards a more egalitarian style of policy making.

Abstract Antimicrobial stewardship (AMS) in Australia is supported by a number of factors, including enabling national policies, sectoral clinical governance frameworks and surveillance programmes, clinician-led educational initiatives and health services research. A One Health research programme undertaken by the National Centre for Antimicrobial Stewardship (NCAS) in Australia has combined antimicrobial prescribing surveillance with qualitative research focused on developing antimicrobial use-related situational analyses and scoping AMS implementation options across healthcare settings, including metropolitan hospitals, regional and rural hospitals, aged care homes, general practice clinics and companion animal and agricultural veterinary practices. Qualitative research involving clinicians across these diverse settings in Australia has contributed to improved understanding of contextual factors that influence antimicrobial prescribing, and barriers and facilitators of AMS implementation. This body of research has been underpinned by a commitment to supplementing ‘big data’ on antimicrobial prescribing practices, where available, with knowledge of the sociocultural, technical, environmental and other factors that shape prescribing behaviours. NCAS provided a unique opportunity for exchange and cross-pollination across the human and animal health programme domains. It has facilitated synergistic approaches to AMS research and education, and implementation of resources and stewardship activities. The NCAS programme aimed to synergistically combine quantitative and qualitative approaches to AMS research. In this article, we describe the qualitative findings of the first 5 years.

Climate change and attendant weather events are global phenomena with wide-ranging implications for migration and health. We argue that while these issues are inherently interrelated, little empirical or policy attention has been given to the three-way nexus between climate vulnerability, migration, and health. In this Review, we develop a conceptual model to guide research on this three-way nexus. In so doing, we apply our conceptual model to a range of case studies, including Bangladesh, Mexico, Myanmar, and the USA. They illustrate that climate vulnerability-migration-health interlinkages are context specific, varying by political, economic, demographic, social, and environmental factors unique to each population and place. Even so, the case studies also demonstrate that overarching themes amenable to policy can be identified. Global organizations and researchers from a multiplicity of disciplinary backgrounds have strong imperatives and unique but often overlooked capacity to innovate and experiment in addressing climate vulnerability-migration-health interlinkages. We call for research and policy focus on these issues and suggest targeted efforts to begin mitigating migration and health issues associated with global climate change.

The pharmacist's role in hospital antimicrobial stewardship (AMS) programs is known to improve patient safety and the quality of care. Despite this, many Australian hospitals struggle to provide adequate pharmacy AMS program resourcing and need to explore newer models of care. The Pharmacy Board of Australia's Guidelines for Dispensing Medicines permit suitably qualified, competent and experienced pharmacy technicians to assist pharmacists in 'tasks in a pharmacy department'. The pharmacy technician workforce is expanding, and there is growing interest in career advancement and expansion of the pharmacy technician role. We propose that the pharmacy technician, a well-integrated member of many Australian hospital pharmacy departments, can play an important role in hospital AMS programs. To bolster AMS initiatives in Australian hospitals, this paper explores the existing evidence for pharmacy technicians in AMS programs and describes how this role may be better supported in Australia.
