Abstract

The latest World Health Organization (WHO) classification of central nervous system tumors (WHO2021/5th) has incorporated molecular information into the diagnosis of each brain tumor type including diffuse glioma. Therefore, an artificial intelligence (AI) framework for learning histological patterns and predicting important genetic events would be useful for future studies and applications. Using the concept of multiple-instance learning, we developed an AI framework named GLioma Image-level and Slide-level gene Predictor (GLISP) to predict nine genetic abnormalities in hematoxylin and eosin sections: IDH1/2, ATRX, TP53 mutations, TERT promoter mutations, CDKN2A/B homozygous deletion (CHD), EGFR amplification (EGFRamp), 7 gain/10 loss (7+/10−), 1p/19q co-deletion, and MGMT promoter methylation. GLISP consists of a pair of patch-level GLISP-P and patient-level GLISP-W models, each pair of which is for a genetic prediction task, providing flexibility in clinical utility. In this study, the Cancer Genome Atlas whole-slide images (WSIs) were used to train the model. A total of 108 WSIs from the Tokyo Women's Medical University were used as the external dataset. In cross-validation, GLISP yielded patch-level/case-level predictions with top performances in IDH1/2 and 1p/19q co-deletion with average areas under the curve (AUCs) of receiver operating characteristics of 0.75/0.79 and 0.73/0.80, respectively. In external validation, the patch-level/case-level AUCs of IDH1/2 and 1p/19q co-deletion detection were 0.76/0.83 and 0.78/0.88, respectively. The accuracy in diagnosing IDH-mutant astrocytoma, oligodendroglioma, and IDH-wild-type glioblastoma was 0.66, surpassing the human pathologist average of 0.62 (0.54–0.67). In conclusion, GLISP is a two-stage AI framework for histology-based prediction of genetic events in adult gliomas, which is helpful in providing essential information for WHO 2021 molecular diagnoses. 20. Intraluminal washout in rectal and sigmoid colon cancer surgeries with double‐stapling technique anastomosis: A single‐institution prospective study.

Abstract

Aim: This study aimed to determine the necessity of intraluminal washout through cytological assessment to prevent implantation of exfoliated cancer cells (ECCs) in patients with rectal and sigmoid cancers. Methods: We studied 140 patients with either sigmoid or rectal cancer who underwent anastomosis surgery using a double‐stapling technique. An intraluminal washout sample was collected before and after irrigation with 1000, 1500, or 2000 mL of physiological saline or distilled water. Cytological assessments were conducted using the Papanicolaou classification system, where classes IV and V indicated positive cytological findings. Results: Initially, 46.4% of the patients (65 out of 140) had positive ECCs. Patients with cancer cells had a significantly shorter distal free margin (DM) from the tumor (p < 0.001). The length of the DM was significantly associated with the tumor distance from the anal verge (p < 0.001). After irrigation with 2000 mL, ECCs were found in only 7.3% of patients. Logistic regression analysis showed that DM (≤50 mm) and tumor size (≥50 mm) were independent risk factors for positive ECCs after intraluminal washout, regardless of the type of irrigation solution used. Conclusion: In patients with sigmoid colon cancer, adequate preoperative bowel preparation, a long DM, and a small tumor size, a 1000 mL intraluminal washout may be sufficient. By contrast, in patients with rectal cancer with a short DM and a large tumor size, a ≥2000 mL intraluminal washout is required. The different types of irrigation solution did not affect the ECCs. 38. Rice Straw Cellulose Nanofibrils via Aqueous Counter Collision and Differential Centrifugation and Their Self-Assembled Structures

Abstract

Rice straw cellulose was completely defibrillated via aqueous counter collision (ACC) at a low energy input of 15 kWh/kg, then fractionated by differential centrifugation into four increasing weight fractions of progressively thinner cellulose nanofibrils (CNFs): 6.9% in 80-200 nm, 14.4% in 20-80 nm, 20.3% in 5-20 nm, and 58.4% in less than 5 nm thickness. The 93.1% less than 80 nm or 78.7% less than 20 nm thick CNFs yields were more than double those from wood pulp by other mechanical means but at a lower energy input. The smallest (3.7 nm thick and 5.5 nm wide) CNFs were only a third or less in lateral dimensions than those obatined through ACC processed from wood pulp, bamboo, and microbial cellulose pellicle. The less than 20 nm thick CNFs could self-assemble into continuous submicron (136 nm) wide fibers by freezing and freeze-drying or semitransparent (13-42% optical transmittance) film by ultrafiltration and air-drying with excellent mechanical properties (164 MPa tensile strength, 4 GPa Young's modulus, and 16% strain at break). ACC defibrillated CNFs retained essentially the same chemical and crystalline structures and thermal stability as the original rice straw cellulose and therefore were much more thermally stable than TEMPO oxidized CNFs and sulfuric acid hydrolyzed cellulose nanocrystals from the same rice straw cellulose.
