Performing craniotomy on patients while they are “awake” requires a very specific anesthesia management. It is the anesthetist’s responsibility to apply sedation and analgesia when necessary to ensure optimal comfort of the patient during the surgery, and to continue electrophysiological monitoring and patient cooperation. In addition to specific anesthesia management, good teamwork, where communication is very important, is an absolute necessity for the awake craniotomy procedure. The aim of this review is to discuss in detail the different anesthesia techniques that can be applied during awake craniotomy and the intraoperative complications that may develop. Objective: We evaluated patients who underwent excisional peripheral lymph node biopsy in terms of histopathologic findings, palpability of lymph nodes, incision depth, and surgery duration, together with demographic and clinical characteristics of the patients. Methods: This prospective study was conducted over an 18-month period at a university research hospital. The total number of 42 patients who attended the oncologic surgery outpatient clinic for excisional lymph biopsy was included in the study. Results: Reactive lymph nodes were found in most benign cases (n=15/25), but the most common malignancy in the malignant group was B-cell lymphoma (n=5/17). When the lymph node was clinically non-palpable and had a size of up to 3.5 cm, the incision was deeper (p<0.05). Conclusion: It would be a useful approach to add the depth of peripheral lymph nodes to the ultrasound reports together with other necessary data to assist the surgeon so that the exact location of the node can be more precisely predicted, and unnecessary incision size, depth, elongation of the duration of the surgery, and complications can be avoided. Collaboration between the surgeon, radiologist, and pathologist is a very important corner stone not to underdiagnose a malignancy and to avoid complications. [ABSTRACT FROM AUTHOR]

Introduction: Research; This study was planned retrospectively in order to compare the problems experienced by women diagnosed with epilepsy during pregnancy, seizure status, drug use, and maternal and fetal outcomes with non-epileptic pregnant women. Materials and Methods: The study was carried out retrospectively on the records of pregnant women with and without epilepsy. The total sample group in the study consisted of 445 pregnant women (49 in the study group, 396 in the control group). The data of the study were collected by examining all birth records in the Obstetrics and Gynecology Clinic of a Medical Faculty Hospital in Konya between March 2015 and February 2020. The data of the study were recorded using the data collection form. Results: No statistically significant difference was found between the epileptic and non-epileptic pregnant women included in the study in terms of age, gravida and parity (p>0.05). It was determined that epileptic pregnants made their first doctor visits later in pregnancy, while the total number of antenatal visits was lower (p<0.05). It has been determined that the antenatal and postnatal complications of epileptic pregnant women are at low levels, the drug is used at a high rate, and although there is a small increase in seizures with pregnancy, this is not statistically significant. Conclusion: Epileptic pregnant women do not have worse antenatal, natal and postnatal outcomes compared to non-epileptic pregnant women. The importance of antenatal follow-up should be explained to epileptic pregnant women starting from the preconceptional period, and their awareness should be increased about the importance of using drugs that can prevent epileptic seizures and closely following dose settings. Objective: As one of the most frequent procedures, breast surgery incurs high healthcare costs. The significance of surgical quality and complications before and after breast surgery entailed the use of methods with the highest outcomes and the least complications. Methods: Throughout this research, postoperative complications and factors affecting the quality of surgery were evaluated in the Kowsar Hospital between 2019 and 2020 for patients requiring breast surgery. Patients undergoing breast surgery were randomly categorized into two groups: electrocautery (A) and harmonic scalpel (B). Results: Of the 56 patients, 28 were in group A, and the remaining 28 were in group B. Intraoperative bleeding, drainage rate, period of drainage, and duration of hospitalization in group B were significantly lower (p<0.05). Furthermore, the extent of pain in both groups was different (p<0.001), so that there was less postoperative pain in women who experienced harmonic breast surgery. Patients throughout group A reported more complications than in group B, such that 7 (25%) patients underwent limb anesthesia and 3 (10.7%) patients experienced seroma, suggesting a higher risk of complications in group A; however, the difference between the two groups was not significant in terms of limb anesthesia (p=0.069) and serum (p=0.075). Conclusion: Harmonic scalpel usage in breast surgery has fewer complications, better operation quality, and shorter hospital stay than other older methods; therefore, the use of harmonic scalpels may substitute for different surgical strategies. This article discusses a study that compared the outcomes of liver resection and ablation for treating liver malignancies in patients aged 65 and older. The study found that patients who underwent ablation had better outcomes, with lower mortality rates and a reduced risk of complications such as abdominal abscess and pneumonia. Despite being a sicker patient population, older patients with primary malignant liver tumors had improved outcomes with ablative techniques. Objective: Acute promyelocytic leukemia (APL) is associated with an elevated risk of developing disseminated intravascular coagulation (DIC). The purpose of this study was to assess the outcomes of hospitalizations related to DIC in APL and their impact on healthcare. Materials and Methods: This study entailed a cross-sectional and retrospective analysis of the US National Inpatient Sample database. We identified adults with APL and categorized them into groups of patients with and without DIC. Our focus areas included in-hospital mortality, length of stay, charges, and complications associated with DIC. Unadjusted odds ratios/coefficients were computed in univariate analysis, followed by adjusted odds ratios (aOR)/coefficients from multivariate analysis that accounted for confounding factors. Results: Our analysis revealed that APL patients with DIC had a substantially higher aOR for mortality (aOR: 6.68, 95% confidence interval [CI]: 4.76-9.37, p<0.001) and a prolonged length of stay (coefficient: 10.28 days, 95% CI: 8.48-12.09, p<0.001) accompanied by notably elevated total hospital charges (coefficient: $215,512 [95% CI: 177,368-253,656], p<0.001), thereby emphasizing the reality of extended medical care and economic burden. The presence of DIC was associated with increased odds of sepsis, vasopressor support, pneumonia, acute respiratory failure, intubation/mechanical ventilation, and acute kidney injury, reflecting heightened vulnerability to these complications. Patients with DIC demonstrated significantly higher odds ratios for major bleeding, intracranial hemorrhage, gastrointestinal bleeding, red blood cell transfusion, platelet transfusion, fresh frozen plasma transfusion, and cryoprecipitate transfusion, highlighting the pronounced hematological risks posed by DIC. Conclusion: This study has revealed the significant associations between DIC in APL and various outcomes, underscoring the clinical and economic implications of these conditions. Objective Within the patients who were referred to our outpatient clinic, the number of those detected with a ventricular septal defect location and size of the defect, the complications it caused, the possibility of medical, surgical or spontaneous closure were examined and it has been compared with our pre-pandemic data whether there is a difference in the number of routine outpatient clinic examinations and the rate of diagnosis of ventricular septal defect under the conditions of the Covid-19 pandemic. Material and Method The results of transthoracic echocardiography made between the years 2018-2022 were evaluated which were performed due to hearing a cardiac murmur during routine examination of patients with symptoms such as chest pain, palpitations, fatigue, difficulty breathing, fainting, or any non-cardiac complaint. Results Ventricular septal defect was observed in 91 of 16362 patients (group 1) who underwent echocardiography between 2018 and 2020 before the pandemic, and in 202 of 14248 patients (group 2) who underwent echocardiography between 2020 and 2022, during the pandemic period. It is significant in that the patient rate is higher (p <0.001). Various complications may develop during the follow-up of ventricular septal defects. Considering the total number of complications, they were statistically significantly higher in the pre)pandemic period (p=0.003). However, when the developing complications were evaluated separately, a significant difference was detected only in terms of ventricular septal aneurysm development, with a higher rate in the pre-pandemic group (p=0.002). No significant difference was found in terms of other complications. Conclusion The similar number of patients has proven to us that there is no serious decrease in the number of pediatric cardiology outpatient clinic admissions, even under pandemic conditions. However, the higher frequency of diagnosis of ventricular septal defect during the pandemic period may be due to the fact that patients who actually require pediatric cardiology examination applied to the hospital during this period. The mediastinum is the region within the thoracic cavity that contains the heart, great vessels, esophagus, trachea, thymus, lymph nodes and neural structures. The mediastinum is divided into three sections: anterior, middle and posterior. The presence of so many different tissues in this area makes it prone to the development of many different types and numbers of malignant/benign tumors. Materials left behind during surgical operations can mimic mediastinal tumors in all three compartments of the mediastinum. Gossypiboma or textileoma are the terms used for granulomatous inflammations caused by surgical gas packs or foreign bodies left in the body after surgery. The present study relates to a case of gossypiboma originating from the paravertebral sulci and extending into the visceral mediastinum, mimicking a mediastinal tumor, in the light of current literature. Epigenetics are traits that are inherited and reflected in the phenotype, independent of the DNA sequence. There is a genetic predisposition to hyperglycemia; however, the environment plays critical roles in its development and progress. Epigenetic changes often translate environmental stimuli into changes in gene expression. Epigenetic factors are mainly DNA methylation, histone modifications and microRNAs. Epigenetic changes, which play a role in the regulation of all biological processes, are closely related to diseases such as autoimmune/inflammatory, cardiovascular, cancer, obesity and type 2 diabetes, which are among the most important health problems in the world and in our country. In particular, chronic hyperglycemia, which plays a role in the pathogen of diabetes and diabetes-related complications, affects gene transcription through epigenetic mechanisms such as DNA methylation, histone modifications and microRNAs. Purpose: The aim of our study is to obtain data that will shed light on the effect of the distal locking type of proximal femoral nails (PFN) on fracture healing, causes of general complications, especially those in the distal of the nails, and reduction of possible complication rates. Materials and Methods: Patients who underwent surgery with the diagnosis of trochanteric hip fracture between 01.01.2015 and 01.01.2020 and completed the second year follow-up time were examined from the medical records. Patients were grouped first according to AO/OTA sub-types (Three groups as AO A1, A2 and A3), then grouped according to distal locking type (Two groups as group 1: dynamic, group 2: static distal locking with 2 screws). The fracture type, healing status, fracture healing time, complication rates and reoperation because of complication development data were evaluated. Results: 339 patients enrolled in the study [189 women, 150 men, mean age: 76.24 (60-102) years]. The overall complication rate was 15.6%. AO A3 type fractures had long fracture healing times in each distal locking group compared to the other fracture sub-types. AO A3 type fractures had more frequent complication rates than the other subtype groups. AO A3 type fractures in group 2 had higher overall complication rates than in group 1. There was a significant correlation between group 1 and lag screw cut-out complication occurrence in females and between group 2 and pseudoarthrosis occurrence in males. In addition, there was a statistically significant relationship between the AO fracture sub-type and total rate of complication occurrence, AO A2 fracture type and lag screw cut-out complication occurrence and AO A3 fracture type and pseudoarthrosis complication occurrence. Conclusion: Patients with AO/OTA 31 A3 type fractures with static distal locking have a longer fracture healing time and a higher complication rate compared to dynamic locking. However, dynamic or static distal locking is not associated with other implant-related complications, especially those in the distal to the tip of PFN’s.

Aim: Percutaneous endoscopic gastrostomy (PEG) is a common method for patients who cannot be oral-fed. This study aims to evaluate the demographic characteristics, indication of PEG and early ( days) complications of PEG patients performed in our hospital over four years. Material and Method: The study is a retrospective study. This study includes patients who underwent percutaneous endoscopy gastrostomy between 2016-2020 in the endoscopy unit and intensive care units in the general surgery clinic of our hospital. The age, gender, comorbidities, length of hospital stay, PEG indications, the day of hospitalization, the complications, if any, and the day the complications developed were recorded on the computer. Complications before 30 days were divided into groups as early complications and those developing after 30 days of late complications. Results: A total of 207 patients the PEG procedure. When PEG indications were examined, it was observed that the most common cause was cerebrovascular events with a rate of 44.93%. Complications were observed in 19 (9.18%) of the patients after the procedure. 68.42% (13) of complications were seen before 30 days. In the evaluation, which was grouped as non-complicated and complicated patients, no significant difference was observed between age, gender, systemic diseases, time to PEG procedure, endoscopic or surgical opening, and mortality rates. Conclusion: Although PEG is a more invasive method compared to other methods in terms of enteral nutrition, it is the most preferred feeding method due to its low complication rate, fast and easy application, and low cost. PEG is recommended for eligible patients who are scheduled for long-term enteral nutrition.

Objective: The use of ureteral catheters in renal transplantation is controversial. We aimed to investigate the effect of intraoperative catheter use on bacteriuria and urological complications in deceased donor renal transplantation. Material and Method: Study design is cross sectional study. A total of 150 patients who underwent deceased donor renal transplantation in the Istanbul University Istanbul Faculty of Medicine Transplantation Unit were included in the study. Ureteral catheters were used in 72 patients but not in the remaining 78. The two patient groups were compared in terms of the incidence of early postoperative urological complications and bacteriuria in the first month after transplantation. Result: Ureteral catheter usage significantly reduced the frequency of early postoperative urological complications (p=0.004). The frequency of bacteriuria in the first month after transplantation was significantly lower in patients using ureteral catheters (p<0.001). In patients with ureteral catheters, the duration of anti-thymocyte globulin administration (p=0.003) and Foley catheter usage (p<0.001) was found to be significantly shorter than in the group without ureteral catheters. Conclusion: The routine use of ureteral catheters results in fewer urological complications in patients with deceased donor renal transplantation. Catheter use does not increase, and even lowers, the risk of early bacteriuria. The significant reduction in early bacteriuria in patients with ureteral catheters is associated with significantly shorter durations of anti-thymocyte globulin and Foley catheter usage. Aim: It is aimed to examine the indication distributions and clinical results of penetrating keratoplasties (PKP) performed with the first experience of surgeons in a center where PKP was started for the first time. Material and Method: In this retrospective study, 64 eyes of 63 patients who underwent PKP at Van Training and Research Hospital between 2017 and 2020 were evaluated. Demographic characteristics, PKP indications, and additional pathologies of the patients were obtained from the files of the patients and their records in the Van Training and Research Hospital eye bank. In addition, preoperative, postoperative first, third, and sixth-month visual acuities of the patients were evaluated, and postoperative complications were examined. The surgeries were performed by three different surgeons (EM, MSD, YE) who had just started performing PKP. Results: Penetrating keratoplasty indications are respectively pseudophakic bullous keratopathy (32.8%), keratoconus (17.1%), corneal scar due to previous herpes keratitis (14%), corneal dystrophy (10.9%), aphakic bullous keratopathy (9.3%), traumatic scar (9.3%), graft with rejection reaction (4.6%), and a corneal abscess that did not regress with medical treatment (1.5%). Postoperative complications were glaucoma (9.3%), rejection reaction (7.8%), cataract (6.2%), suture loosening (4.6%), keratitis (3.1%), traumatic perforation (1.5%) and endophthalmitis (1.5%). There was a statistically significant increase in visual acuity in the first, third, and sixth months compared to the preoperative value (for all; p<0.01). However, there was no significant increase in visual acuity in the sixth postoperative month compared to the third month (p>0.05). Conclusion: The most common indication for PKP was pseudophakic bullous keratopathy. In this patient series, which consisted of the first experiences of surgeons, the postoperative complication rate was 34.3%. Despite this, a significant improvement in visual acuity and graft survival was achieved in 95.3% of patients in early postoperative follow-ups. [ABSTRACT FROM AUTHOR]

Purpose: This study aims to investigate the effect of complications on survival after lung cancer surgery. Materials and Methods: This is a retrospective study of 802 patients who underwent surgical operations for lung cancer at two centers between January 2002 and December 2019. All patients were retrospectively evaluated according to age, gender, cancer stage, surgical technique, biochemical tests, pulmonary function tests, pulmonary risk index, length of the hospital stay, and the existing number of complications. Results: In total, 802 patients (%84 males, %16 females) with a mean age of 60 years (range 19-88 years) were included in the study. 5-year overall survival rate was 62.3%. 224 complications occurred in 180 patients (22.4%). The ten most common complications, respectively, were prolonged air leak (54), pneumonia (22), arrhythmia (19), respiratory failure (15), hemorrhage (17), atelectasis (10), pneumothorax-expansion defect (7), arrest (7) bronchopleural fistula (7), subileus-ileus (6). Cancer stage, the existing number of complications, advancing age, and tumor size are parameters that significantly affect survival. The occurs of one or more complications significantly adversely affects survival. Conclusion: Although the morbidity and mortality of lung cancer surgery have decreased in recent years, it is still high. Advanced stage lung cancer, advancing age, enlargement of tumor size and occurs of complications that negatively affect survival. Careful preoperative preparation may play a role in increasing long-term survival, as well as preventing complications, and decreasing early mortality.

Bu yaralanmaların şiddete yönelik olması ve adli olayların gençlerde daha sık görülmesi tedaviyi daha da zorlaştırmaktadır. Bu çalışmadaki amacımız, ateşli silahlarla ilişkili ekstremite yaralanmalarında nörovasküler, yumuşak doku yaralanmaları ve kemik kırıklarına aşamalı bir yaklaşımın nihai tedavi sonuçlarına etkisini değerlendirmektir. Gereç ve Yöntem: Bu çalışmada 2015-2020 yılları arasında ateşli silahla ilişkili ekstremite yaralanması olan ve cerrahi tedavi uygulanan 18 yaş üstü hastalar retrospektif olarak değerlendirildi. Hastaların demografik özellikleri, yaralanma bölgeleri, eşlik eden nörovasküler yaralanmalar, ilk müdalede yapılanlar, son tedavi sonuçları, gelişen komplikasyonlar ve komplikasyonların yönetimi değerlendirildi. Bulgular: Çalışmaya katılan 54 hastanın (52 erkek, 2 kadın) yaş ortalaması 28.3 (yaş aralığı 19-61) idi. Otuz iki (%59.2)’si alt ekstremite, 22 (%40.7)’si üst ekstremite ve 5 (%9.2)’i hem alt hem üst ekstremite yaralanması tespit edildi. Üç (%5.5) hastada damar yaralanması, 3 (%5.5) hastada amputasyon ve 6 (%11.1) hastada sinir yaralanması tespit edildi. Altı hastanın beşine (%9,2) fasyotomi uygulandı. En sık karşılaşılan komplikasyon olarak 7 (%12,9) hastada eklem hareket açıklığı kaybıydı. Sonuç: Ateşli silah yaralanmalarında multidisipliner ve hasar kontrollü bir yaklaşımla erken dönemde antibiyoterapi, kemik tespiti ve seri debridmanlar sonrası nihai tedaviye geçilmesi tedavi başarısını olumlu yönde etkilemektedir. Background: Our aim was to identify risk factors and clinical correlates of endocrine complications in β-thalassemia major (BTM) patients. Materials and methods: This was a retrospective study carried out in the pediatric hematology and pediatric endocrinology departments of a tertiary healthcare institution with the medical data of 249 children with BTM (108 females, 43.4%; 141 males, 56.6%) with a median age of 7.17 (2 -17.8) years. Baseline descriptive, clinical features including endocrine complications and laboratory data were noted. Correlation between the presence of endocrine complications and demographic, clinical, and laboratory variables were sought. The effects of age, gender, race, height, weight, and splenectomy on endocrine complications were evaluated separately in each complication group. Results: Vitamin D deficiency/insufficiency is the most common endocrine complication (41.7%). According to Turkish children, Syrian children had also significantly lower vitamin D concentration (p=0.001). At least one endocrinopathy was reported in the majority of BTM patients (67.9%). Accordingly, pubertal status (p=0.014) and Syrian nationality (p=0.007) had significant impacts on TSH levels. Syrian children and those with delayed puberty had greater likelihood for subclinical or evident hypothyroidism. The likelihood of suffering from at least one endocrine complication was higher in older children (p=0.042) and those with Syrian nationality (p=0.025) Conclusion: Disorders of endocrine and metabolic nature are common in children with BTM. Early detection and protocol-based multidisciplinary management of these disorders constitute the most suitable strategies to increase patients' quality of life. Surveillance, early detection and treatment, and collaborative follow-up with a multidisciplinary team are the key points in the reduction of the severity and frequency of endocrine complications as well as optimization of therapeutic outcomes. Aim Primary cleft lip and palate (CLP) repair is the most critical stage throughout lifetime. CLP surgery involves risks of surgery and anaesthesia related complications. In this study, it was aimed to evaluate complications occurred in early period after primary CLP surgery and to determine early factors that trigger these conditions. Materials and Methods In this study 328 surgeries of 271 CLP patients were included. Complications were classified as minor complications such as fever occurring within first 3 days. Complications that cannot be treated with palliative care or required transfer to intensive care were considered as major complications. Epidemiology and causal link of early complications were statistically evaluated. Results Early complications were seen in 19% (n: 63) of all cases that operated for cleft lip and palate. Among them rate of minor complications were 9% (n: 25). It was determined that prolongation of anaesthesia lead to rising of minor complications such as deficiency in oral feeding. Major complications were observed in 11% (n: 38). These major complications were more common in bilateral cleft lip, syndromic, palate and male patients. Amaç: Tonsillektomi cerrahisi, Kulak Burun Boğaz (KBB) pratiğinde en sık yapılan cerrahi işlemlerden biridir. Ameliyat sırasında cerrahın görüş alanını arttırmak için kullanılan ağız açacağı ve dil basacağı, hastanın dil dokusuna yüksek basınç uygulayarak çeşitli komplikasyonlara neden olabilir. Bu çalışmanın amacı, tonsillektomi yapılan erişkin hastalarda, cerrahi esnasında kullanılan dil basacağının dil üzerinde oluşturduğu basınca bağlı olarak gelişmesi muhtemel dil ödeminin, ultrasonografi (USG) ile araştırılmasıdır. Gereç ve yöntem: Çalışmaya 18-40 yaş arası hastalar dahil edildi ve iki gruba ayrıldı. Çalışma grubu (n = 31), KBB kliniğinde tonsillektomi yapılan hastalardan oluşmakta iken kontrol grubu (n = 30), genel anestezi altında tonsillektomi dışında başka bir cerrahi yapılan, dil dokusuna herhangi bir baskı uygulanmayan ve genel anestezi süresi, tonsillektomi cerrahisinin süresine yakın olan hastalardan oluşturuldu. Her hastanın, submental koronal plan Ultrasonografi (USG) ile dil kesit alanı (TA) iki defa ölçüldü. Çalışma grubunda ilk ölçüm (TA1) endotrakeal entübasyondan hemen sonra, ancak ağız açacağı ve dil basacağı takılmadan önce yapıldı. İkinci ölçüm, (TA2) tonsillektomi cerrahisinin sonunda, dil basacağı çıkartıldıktan sonra ancak ekstübasyondan hemen önce yapıldı. Kontrol grubunda, ilk ölçüm (TA1) endotrakeal entübasyondan hemen sonra, ikinci ölçüm (TA2) ise, cerrahi bitiminde ancak ekstübasyondan hemen önce yapıldı. Dil ödemi, dil kesit alanları farkı (TA2 - TA1) olarak tanımlandı. Gruplar dil kesit alanları açısından ve dil ödemi açısından karşılaştırıldı. Bulgular: Gruplar arasında, demografik açıdan ve genel anestezi süreleri açısından fark yoktu. TA1 değerleri bakımından, tonsillektomi (3.93 ± 1.29 cm2) ve kontrol grupları (3.80 ± 1.20 cm2) arasında anlamlı bir fark bulunmadı (p = 0.212). TA2 değerleri bakımından, tonsillektomi grubu (4.63 ± 0.28 cm2) ile kontrol grubu (3.91 ± 0.23 cm2) arasında anlamlı fark bulundu (p = 0.000). Tonsillektomi grubu dil ödemi değerleri (0.90 ± 0.26 cm2), kontrol grubu değerlerine (0.11 ± 0.07 cm2) göre yüksek bulundu (p = 0.000). Sonuç: Çalışmamız ile, erişkin hastalarda gerçekleştirilen tonsillektomi cerrahisinde kullanılan ağız açacağı ve dil basacağının dil üzerinde oluşturduğu yüksek basınca bağlı olarak dil ödemi gelişmiş ve dil ödemi USG ile tespit edilmiştir.

Objective: In the surgical treatment of supracondylar humerus fractures (SCHF), different treatment methods and surgical incisions are depending on the direction of displacement of the distal fragment and the surgeon’s experience. The aim of this study was to compare SCHF treatment results and complications according to closed reduction percutaneous pinning (CRPP) and the surgical incision type in open surgery and to determine the ideal approach thereby. Material and Methods: A retrospective examination was made of a total of 224 patients for this study which was comprising 142 (63.4%) males and 82 (36.6%) females with a mean age of 5.95±2.79 years (range, 1-13 years). According to the Gartland classification, 58 (25.8%) were Type 2, 166 (74.2%) were Type 3. We divided the patients included in the study into four groups: medial, lateral, posterior incision and those who underwent surgery with CRRP. Results: The most frequent complications observed were neurological deficit (n:34, 15.1%), scarring (n:32, 14.2%), subcutaneous pin migration (n:24, 10.2%), cubitus varus (n:20, 8.9%), pin tract infection (n:15, 6.6%), flexion restriction (n:13, 5.8%), extension restriction (n:12, 5.3%), myositis ossificans (n:12, 5.3%), and cubitus valgus (n:5, 2.29%). There were determined to be differences between the group applied with open reduction internal fixation and the closed reduction percutaneous pinning group in respect of complications. Conclusion: There is no difference between functional results between closed reduction percutaneous pinning and open surgery groups. In patients who cannot be closed reduction, open surgical incisions may be preferred depending on the surgeon's experience and preference.

Objective: Mortality and complication rates are increasing in emergency and delayed cancer surgeries during the COVID-19 pandemic. However, curing can be provided when colorectal cancer surgery is performed without delay. In this study, it was aimed to evaluate the results in patients who underwent colorectal cancer surgery after having COVID-19 infection during the pandemic process. Methods: Data on patients undergoing colorectal cancer surgery after or without COVID-19 infection were recorded from the hospital’s electronic database and file records. It was investigated whether there was a difference in 30-day complication and mortality rates in patients with COVID-19 infection. Results: Of the 77 patients between the ages of 20 and 85 who underwent colorectal cancer surgery, 34 were female, 43 were male. Thirty six of the patients who were infected with the COVID-19 (Group-C), 41 of the patients had undergone surgery without infection (Group-N). There was no difference between the groups in terms of lenght of stay in Hospital and Intensive Care Unit. The 30-day mortality rate in Group-C was 11.1%. The 30-day complication rate in Group-C was significantly higher than in Group-N (p=0.048). The most common complication in Group-C was acute kidney injury (11.1%). Being infected with COVID-19 virus within 7 weeks before the operation had no effect on postoperative results. Conclusion: While the surgical procedure is optimized to prevent complications in patients with COVID-19, delaying surgery can lead to local-remote metastases due to waiting and losing the possibility of operation. Amaç: Bu çalışmada meme küçültme amelyiatı sonrası, hasta demografik özellikleri, ameliyat teknikleri ve ameliyat sonrası bakımın hasta memnuniyetine etkisi değerlendirilmiştir.Gereç ve Yöntem: Meme küçültme ameliyatı geçiren hastalar arasından, ameliyat öncesi ve ameliyat sonrası altıncı aylık dönemde Breast-Q meme redüksiyonu modülü anketi doldurmayı kabul eden hastalar çalışmaya dahil edildi. Hastaların evlilik durumları, çocuk sahibi olup olmadıkları, sigara kullanımı, boy ve kiloları, yaşları ameliyat öncesinde sorularak kaydedildi. Ameliyat sonrası dren kullanılıp kullanılmadığı, cilt dikişleri olup olmadığı, herhangi bir komplikasyon gelişip gelişmediği, postoperatif yatış gün sayısı, hastanede kaldığı odada başka hasta bulunup bulunmadığı sorgulanarak kayıt altına alındı. Bulgular: Çalışmaya 76 hasta dahil edildi. Ortalama yaş 45.8 ve ortalama vücut kitle indeksi (VKİ) 29.2 idi. Ameliyat sonrası memelerden tatmin, psikososyal, cinsel ve fiziksel iyilik hali puanları ameliyat öncesi puanlardan anlamlı derecede yüksekti. Yaşlı kadınlarda memelerden memnuniyet ve psikososyal iyilik hali ameliyat öncesi puanlarda daha yüksekti. Genç hastalar ve normal kilolu hastalar ameliyat sonrası fiziksel iyilikten daha yüksek puan aldı. Ameliyat sonrası komplikasyonu olan hastalarda memelerden memnuniyet, cinsel ve fiziksel iyilik hali, bilgi memnuniyeti, cerrah ve tıbbi malzeme puanları anlamlı olarak düşüktü.Sonuç: Komplikasyon olmayan cerrahinin, hasta memnuniyetini artıran en güçlü faktör olduğu görüldü. Yaş ve VKİ gibi hasta demografisi bilgileri, hasta memnuniyetini arttırmak için hasta seçiminde kullanılabilecek kriterlerdir.

Objective: Totally implantable venous access ports (TIVAPs) are widely preferred for oncology patients who need chemotherapy. Although peripheral cannula or catheter in a large vein may help chemotherapy treatment; some complications such as vascular irritation, thrombosis may interrupt the treatment. To avoid this interruption, TIVAPs are usually preferred. The aim of the study was to evaluate device related complications and safety with anesthesia team implanted TIVAPs. Methods: This retrospective study was conducted on patients who underwent procedure for implantation of TIVAPs in our hospital between the dates of January 2017 - December 2020. The demographic data, vascular access side, complications seen related to TIVAPs insertion procedure, the number of removed catheters, reasons of removal of catheters were recorded. Results: During this study period 700 TIVAPs procedure have been performed in our clinic. While 646 patients (92.3%) had right sided TIVAPs, 678 patients (96.8%) had TIVAPs inserted to subclavian vein. When the most common cancer seen in this patient group was colorectal cancer, hepatobiliary tract cancer group came second. Total number of removed catheters for some reasons was 107 which is 15.2% of all patients. When the causes for catheter removal were examined, it was discovered that although systemic infection was the most common reason (47 patients, or 6.7%), the number of patients with positive culture from the port was extremely rare (19 patients) (2.71%). Conclusion: We think that when TIVAPs are inserted under ultrasound guidance and fluoroscopy control in the operating room, the incidence of complications will be relatively low.
