Abstract

Objectives Racial factors play a significant role in SSc. We evaluated differences in SSc presentations between white patients (WP), Asian patients (AP) and black patients (BP) and analysed the effects of geographical locations. Methods SSc characteristics of patients from the EUSTAR cohort were cross-sectionally compared across racial groups using survival and multiple logistic regression analyses. Results The study included 9162 WP, 341 AP and 181 BP. AP developed the first non-RP feature faster than WP but slower than BP. AP were less frequently anti-centromere (ACA; odds ratio (OR) = 0.4, P < 0.001) and more frequently anti-topoisomerase-I autoantibodies (ATA) positive (OR = 1.2, P = 0.068), while BP were less likely to be ACA and ATA positive than were WP [OR(ACA) = 0.3, P < 0.001; OR(ATA) = 0.5, P = 0.020]. AP had less often (OR = 0.7, P = 0.06) and BP more often (OR = 2.7, P < 0.001) diffuse skin involvement than had WP. AP and BP were more likely to have pulmonary hypertension [OR(AP) = 2.6, P < 0.001; OR(BP) = 2.7, P = 0.03 vs WP] and a reduced forced vital capacity [OR(AP) = 2.5, P < 0.001; OR(BP) = 2.4, P < 0.004] than were WP. AP more often had an impaired diffusing capacity of the lung than had BP and WP [OR(AP vs BP) = 1.9, P = 0.038; OR(AP vs WP) = 2.4, P < 0.001]. After RP onset, AP and BP had a higher hazard to die than had WP [hazard ratio (HR) (AP) = 1.6, P = 0.011; HR(BP) = 2.1, P < 0.001]. Conclusion Compared with WP, and mostly independent of geographical location, AP have a faster and earlier disease onset with high prevalences of ATA, pulmonary hypertension and forced vital capacity impairment and higher mortality. BP had the fastest disease onset, a high prevalence of diffuse skin involvement and nominally the highest mortality.

Abstract

Objective In patients with SSc, peripheral vasculopathy can promote critical ischaemia and gangrene. The aim of this study was to investigate the prevalence, incidence and risk factors for gangrene in the EUSTAR cohort. Methods We included patients from the EUSTAR database fulfilling the ACR 1980 or the ACR/EULAR 2013 classification criteria for SSc, with at least one visit recording data on gangrene. Centres were asked for supplementary data on traditional cardiovascular risk factors. We analysed the cross-sectional relationship between gangrene and its potential risk factors by univariable and multivariable logistic regression. Longitudinal data were analysed by Cox proportional hazards regression. Results 1757 patients were analysed (age 55.9 [14.5] years, disease duration 7.9 [10.3] years, male sex 16.7%, 24.6% diffuse cutaneous subset [dcSSc]). At inclusion, 8.9% of patients had current or previous digital gangrene, 16.1% had current digital ulcers (DUs) and 42.7% had ever had DUs (current or previous). Older age, DUs ever and dcSSc were statistically significant risk factors for gangrene in the cross-sectional multivariable model. During a median follow-up of 13.1 months, 16/771 (0.9%) patients developed gangrene. All 16 patients who developed gangrene had previously had DUs and gangrene. Further risk factors for incident gangrene were the dcSSc subset and longer disease duration. Conclusion In unselected SSc patients, gangrene occurs in about 9% of SSc patients. DUs ever and, to a lesser extent, the dcSSc subset are strongly and independently associated with gangrene, while traditional cardiovascular risk factors could not be identified as risk factors.

11. Preterm birth phenotypes in women with autoimmune rheumatic diseases: a population-based cohort study.

Abstract

Objective: To investigate preterm birth (PTB) phenotypes in women with different autoimmune rheumatic diseases in a large population-based cohort.Design: Retrospective cohort study.Setting: California, USA.Population: All live singleton births in California between 2007 and 2011 were analysed. Patients with autoimmune disease at delivery were identified by International Classification of Diseases, Ninth Revision , Clinical Modification (ICD-9-CM), codes for systemic lupus erythematosus (SLE), systemic sclerosis (SSc), rheumatoid arthritis (RA), polymyositis/dermatomyositis (DM/PM), and juvenile idiopathic arthritis (JIA).Methods: Maternally linked hospital and birth certificate records of 2 481 516 deliveries were assessed (SLE n = 2272, RA n = 1501, SSc n = 88, JIA n = 187, DM/PM n = 38). Multivariable Poisson regression models estimated the risk ratios (RRs) for different PTB phenotypes (relative to term deliveries) for each autoimmune disease compared with the general obstetric population, adjusting for maternal age, race/ethnicity, body mass index, smoking, education, payer, parity, and prenatal care.Main Outcome Measures: Preterm birth (PTB) was assessed overall (20-36 weeks of gestation) and by subphenotype: preterm prelabour rupture of membranes (PPROM), spontaneous birth, or medically indicated PTB. The risk of PTB overall and for each phenotype was partitioned by gestational age: early (20-31 weeks of gestation) and late (32-36 weeks of gestation).Results: Risks for PTB were elevated for each autoimmune disease evaluated: SLE (RR 3.27, 95% CI 3.01-3.56), RA (RR 2.04, 95% CI 1.79-2.33), SSc (RR 3.74, 95% CI 2.51-5.58), JIA (RR 2.23, 95% CI 1.54-3.23), and DM/PM (RR 5.26, 95% CI 3.12-8.89). These elevated risks were observed for the majority of PTB phenotypes as well.Conclusions: Women with systemic autoimmune diseases appear to have an elevated risk of various PTB phenotypes. Abstract

Two trials with Atlantic salmon ( Salmo salar) were conducted to evaluate the potential of krill meal to improve feed intake. In the first experiment, after transfer to sea water, salmon smolts were fed diets added 75 or 150 g kg−1 Antarctic krill meal in substitution for fish meal for 13 weeks. The apparent digestibility coefficient for crude protein and the majority of the amino acids was significantly lower in the feeds added krill meal (around 83.5%) than in the control diet (84.9%), whereas the digestibility of crude lipids, dry matter and energy was not significantly different among the three diets. Krill meal addition resulted in higher feed intake, which led to higher growth rates and final body weights. In the second experiment, large salmon were fed a diet containing 100 g kg−1 krill meal for 6 weeks before slaughter. Their feed intake and growth performance were assessed, and fillet and visceral fat contents were measured. Salmon fed the 100 g kg−1 krill meal diet tended to eat more, resulting in significantly increased growth rates, when compared to control fish. 42. A prospectively randomized double-blind study on the effect of initial graft tension on knee stability after anterior cruciate ligament reconstruction.

Abstract

BACKGROUND: No consensus exists on the amount of tension that should be applied to anterior cruciate ligament grafts to best facilitate graft incorporation and re-create normal knee mechanics. HYPOTHESIS: Differences in initial graft tension will affect postoperative knee stability. STUDY DESIGN: Prospective, randomized, double-blind clinical trial. METHODS: Forty-nine patients undergoing bone-patellar tendon-bone autograft anterior cruciate ligament reconstruction by a single surgeon were randomized into high-tension (n = 27) and low-tension (n = 22) groups. Grafts were set at 90 N or 45 N. Arthrometric measurements (KT-1000 arthrometer manual maximum) of anterior tibial displacement and knee range of motion were made before surgery and at 1 week and an average of 20 months after surgery. Knee outcome scores were collected before and after surgery, and a single-leg hop test was also performed at final follow-up. RESULTS: After anterior cruciate ligament reconstruction, anterior tibial displacement was significantly greater in the patients in the low-tension group (P < .05). The side-to-side difference in anterior tibial displacement in the high-tension and low-tension groups was 1.1 +/- 1.7 mm versus 2.4 +/- 2.4 mm 1 week after surgery and 2.2 +/- 1.6 mm versus 3.0 +/- 2.2 mm at follow-up. Five patients had abnormal anterior tibial displacement (>5 mm side-to-side difference), and all were in the low-tension group (P <.05). Knee outcome scores improved with surgery (P <.01), with similar results for low-tension and high-tension groups. Hop test deficits were not different between groups. CONCLUSIONS: Initial graft tension affects the restoration of knee stability. A graft tension of 45 N was not sufficient for restoring knee stability. [ABSTRACT FROM AUTHOR]

43. The Cone hip stem: a prospective study of 13 patients followed for 5 years with RSA.

Abstract

We operated on 13 patients (14 hips) with dysplastic hips, mean age 42 (28-58) years, with a cementless Cone stem and followed them for 5 years, using the Merle d'Aubigne clinical score, conventional radiography and repeated radiostereometry analyses. The clinical scores improved markedly at 4 months and still more throughout the study. None of the patients complained of thigh pain. No stem showed radiographic subsidence, but 3 stems had radiolucent zones probably indicating fibrous ingrowth. Micromigration was measured at 4 months, 1, 2 and 5 years. The mean subsidence after 5 years was 0.27 mm and the mean posterior micromigration of the head was 0.74 mm. Most of the micromigration took place within the first 4 months. We conclude that the uncemented Cone stem used in dysplastic hips has shown a good clinical outcome so far and was found to be stable on conventional radiographs and using RSA technique. 47. Characterization of pig genotypes for growth modeling

Abstract

The models dealt with herein are driven by descriptors of pig growth potential and environment, predicting growth from their interaction. Growth potential parameters relate to resource intake and partitioning to maintenance, protein (P) deposition (PD), and lipid (L) deposition (LD); these parameters quantify genotype (breed, etc.). Simulation of a pig's growth requires characterization of its potential in terms of the associated model parameters. This requires a set of parameters that fully describe the potential, measurement of resource input, and partitioning in a genotype, and using these measurements to quantify those parameters for that genotype. Resource partitioning is commonly covered by potential PD, required LD, and MEm. Description of the first two features commonly requires three parameters. The MEmhere is restricted to a neutral environment without functions for coping with stressors, which would require extra parameters. Nutrient intake is best modeled as resulting from nutrient requirements and from constraints to physical uptake, be they external or genetic. Intake and partitioning observations must reflect potential; environmental load must be minimized. Repeatedly measuring whole-body P and L and ad libitum ME intake over a sufficiently wide maturity range (for example from 10 to 175 kg of BW) requires serial slaughter trials with chemical analysis or in vivo techniques such as ultrasound. The latter allow for the description of individual growth patterns and for quantification of variation in addition to mean levels. Parameters can be estimated in three ways. First, P and L observations can be fitted to P and L growth functions. Then, MEmcomes out as the remainder of the ME budget, given valid assumptions about PD and LD efficiency. Second, observed feed intake, growth, and body composition can be fitted to their simulations (parameter calibration, inverted modeling) to avoid P or L measurement. This requires serial data and iteration to match resource requirements to allowance. Third, differential nutrient restriction techniques can be used.

48. All-polyethylene versusmetal-backed and stemmed tibial components in cemented total knee arthroplasty

Abstract

We studied the quality of fixation of the tibial component using radiostereometric analysis (RSA) in 40 patients who had undergone a cemented Freeman-Samuelson total knee arthroplasty. They were prospectively randomised to either a stemmed metal-backed (MB) or non-stemmed all-polyethylene (AP) tibial component. The articulating geometry of the implants was identical, as was the operative technique and the postoperative regime.The study showed no complications of fixation using AP tibial components, and the migration was the same as that of their metal-backed counterparts. There was no bony collapse or increased subsidence of any part of the tibial component or increased incidence of radiolucent lines in the knees with AP components. Most AP implants were stable between one and two years after surgery, a finding known to be of positive prognostic significance when predicting future aseptic loosening.
