Abstract

Aim: The formation of a local vegetation mosaic may be attributed to local variation in abiotic environmental conditions. Recent research, however, indicates that self‐facilitating organisms and negative species interactions may be a driving factor. In this study, we explore whether heterogeneous geohydrological conditions or vegetation feedbacks and interactions could be responsible for a vegetation mosaic of rich and poor fen species. Location: Lake Aturtaun, Roundstone Bog, Ireland. Methods: In a floating fen, transects were set out to analyze the relation between vegetation type and rock–peat distance and porewater electrical conductivity. Furthermore, three distinct vegetation types were studied: rich fen, poor fen and patches of poor fen within rich fen vegetation. Biogeochemical measurements were conducted in a vertical profile to distinguish abiotic conditions of distinct vegetation types. Results: Geohydrological conditions may drive the distribution of poor and rich fen species at a larger scale in the floating fen, due to the supply of minerotrophic groundwater. Interestingly, both rich and poor fen vegetation occurred in a mosaic, when electrical conductivity values at 50 cm depth were between 300 µS/cm and 450 µS/cm. Although environmental conditions were homogeneous at 50 cm, they differed markedly between rich and poor fen vegetation at 10 cm depth. Specifically, our measurements indicate that poor fen vegetation lowered porewater alkalinity, bicarbonate concentrations and pH. No effects of rich fen vegetation at 10 cm depth on biogeochemistry was measured. However, rich fen litter had a higher mineralization rate than poor fen litter, which increases the influence of minerotrophic water in rich fen habitat. Conclusions: These results strengthen our hypothesis that species can drive formation of vegetation mosaics under environmentally homogeneous conditions in a floating fen. Positive intraspecific self‐facilitating mechanisms and negative species interactions could be responsible for a stable coexistence of species, even leading to local ecosystem engineering by the species, explaining the local vegetation mosaic at the microscale level in a floating fen. Abstract

We investigated rivastigmine effectiveness in 84 Alzheimer outpatients, with a special focus on behavioural problems. Cognition, activities in daily living (ADL) and behaviour were assessed during 30 months. Changes in test results between 6 months and baseline were compared with a historical control cohort of Alzheimer patients (n = 69) by performing t-tests and calculation of Cohen's d and standardised response mean (SRM). During 6 months, rivastigmine showed effect on cognition (p < 0.001, Cohen's d = 0.33, SRM = 0.78), ADL (p < 0.001, Cohen's d = -0.43, SRM = -0.54) and memory-related behaviour (p = 0.006, Cohen's d = -0.28, SRM = -0.28). Depressive behaviour worsened (p = 0.001, Cohen's d = 0.30, SRM = 0.37) and disruptive behaviour (p = 0.369, Cohen's d = -0.07, SRM = -0.09) was not effected by rivastigmine. During 30 months, a gradual decline was shown in most domains. Most RMBPC items showed stabilization during 30 months. Improvement on disruptive behaviour items and depression items was shown after 6 months of treatment in a large proportion of patients in whom behavioural problems were present at baseline. In conclusion, a huge discontinuation rate is experienced within the first half year of treatment. In the subpopulation of patients who continued rivastigmine for 6 months, it shows modest effectiveness on cognition, functionality and memory-associated behaviour compared with historical control patients. Unfortunately, disruptive behaviour is not altered by rivastigmine therapy, and depressive behaviour worsened slightly after initial treatment. During 30 months, rivastigmine showed stabilization on numerous behaviour items as measured by the RMBPC.

Abstract

OBJECTIVE: To compare and evaluate drug notations in outpatient medical records and in pharmacy records in a cohort of HIV-1-infected patients treated with antiretroviral drugs. METHODS: Data on 103 patients were obtained from January 1, 1998, through December 31, 1999, by medical chart review and collection of pharmacy records. Two analyses were performed. First, antiretroviral drugs and comedication in the pharmacy records were documented and compared with their appearance in the outpatient medical records. Second, a detailed comparison was performed at 5 time points during the study period for the antiretroviral drugs. Generic name, formulation, strength, and frequency of dosing as registered in the outpatient medical records were compared with those registered in the pharmacy records. RESULTS: Total drug dispensation was 1607 (366 and 1241 antiretroviral drugs and comedication, respectively). The first screening resulted in a total discrepancy of 55.1% (n = 885), of which 97.1% (n = 859) was attributed to the comedication and 2.9% (n = 26) to the antiretroviral drugs. The discrepancy for the antiretroviral drugs at the specific time points ranged from 5.1% to 12.6% when the generic name only was used, and from 7.1% to 17% when formulation, strength, and frequency of dosing were also taken into account. CONCLUSIONS: The observed discrepancy between outpatient medical records and pharmacy records mainly concerns the comedication. For the antiretroviral drugs fewer, but still substantial, discrepancies were observed. These results indicate that full exchange of information conceming drug use in this population between general practitioners and specialists (infectious disease) is lacking.
