1. THE EFFECT OF PEDIATRIC SURGICAL PRACTICE UPON THE MANAGEMENT OF CHILDREN WITH APPENDICITIS

Purpose: Acute appendicitis in children is managed by both general surgeons (GS) and pediatric surgeons (PS). Our objective was to investigate the results of surgical care provided by either GS or PS for appendicitis. Methods: The outcome of children, within our state, undergoing operative treatment for appendicitis (1/94 to 6/97) by board certified GS were compared to the results of PS associated with the three state children's hospitals. Data were sorted according to patient age and ICD-9 diagnosis. ANOVA was performed upon continuous data and Chi-square analysis upon nominal data; data are depicted as mean [+ or -] SEM. Results: GS (n = 2178) managed older children when compared to PS (n = 832; 11.0 [+ or -] 0.1 vs 9.5 [+ or -] 0.1 yrs, p [is less than] 0.01) and more frequently treated simple appendicitis (81.2% vs 60.3%, p [is less than] 0.01). However, older children (13-16 yrs) appeared to have comparable outcome. Conclusions: Younger children with appendicitis have reduced hospital days and charges when managed by pediatric surgeons. Payers may wish to consider data such as these when planning care for their constituents.
