Abstract

Metal cutting, a complex process in manufacturing, involves various factors that significantly affect the quality of the final product. Notably, the turning process is crucial, with outcomes that heavily depend on multiple machining parameters. These parameters encompass speed, depth of cut, feed rate, the type of coolant used (specifically, high heat transfer MXene coolant), and insert types, among others. The material of the workpiece is also a critical factor in the metal-cutting operation. This study focuses on achieving optimal surface quality and minimizing cutting forces in the turning process. It recognizes the substantial impact of numerous process parameters, directly or indirectly affecting the product's surface roughness and cutting forces. Understanding these optimal parameters can lower machining costs and improve product quality. Our research concentrates on turning a stainless-steel alloy workpiece using a carbide insert tool. We employ the Response Surface Method (RSM) to optimize cutting parameters within a set range of cutting speed (100, 125, 150 m/min), feed rate (0.1, 0.2, 0.3 mm/rev), and depth of cut (0.4, 0.8, 1.2 mm). Additionally, we use various tool geometries and the RSM design of experiments to enhance and analyze the multi-response parameters of surface roughness and tool life. Optimal machining parameters for MXene-NFC involve a cutting speed of 140 m/min, a feed rate of 0.05 mm/rev, and a depth of cut of 0.5 mm. These settings ensure minimal surface roughness, maximum tool life, and the greatest total length of cut, achieving a composite desirability of 0.695. [ABSTRACT FROM AUTHOR]

Abstract

Background Longitudinal cohort studies provide important information about the clinical effectiveness of an intervention in the routine clinical setting, and are an opportunity to understand how a population presents for treatment and is managed. Methods INTEREST IN CD2 (NCT01753349) is a prospective, international, 3-year, longitudinal, observational study following the course of adult idiopathic cervical dystonia (CD) treated with botulinum neurotoxin type A (BoNT-A). The primary objective is to document long-term patient satisfaction with BoNT-A treatment. Here we report baseline data. Results This analysis includes 1036 subjects (67.4% of subjects were female; mean age was 54.7 years old; mean TWSTRS Total score was 31.7). BoNT-A injections were usually given in line with BoNT-A prescribing information. The most commonly injected muscles were splenius capitis (87.3%), sternocleidomastoid (82.6%), trapezius (64.3%), levator scapulae (40.9%) and semispinalis capitis (26.9%); 35.5% of subjects were injected using a guidance technique. Most subjects (87.8%) had been previously treated with BoNT-A (median interval between last pre-study injection and study baseline was 4 months); of these 84.8% reported satisfaction with BoNT-A treatment at peak effect during their previous treatment cycle and 51.5% remained satisfied at the end of the treatment. Analyses by geographical region revealed heterogeneity in the clinical characteristics and BoNT-A injection practice of CD subjects presenting for routine treatment. Conclusions These baseline analyses provide sizeable data regarding the epidemiology and clinical presentation of CD, and demonstrate an international heterogeneity of clinical practice. Future longitudinal analyses of the full 3-year study will explore how these factors impact treatment satisfaction. Water: het is één van de belangrijkste elementen voor de akkerbouw, maar ook de andere sectoren krijgen met verschillende vraagstukken te maken. Hoewel er afgelopen zomer op sommige plaatsen in Nederland te veel neerslag viel, was er tijdens andere periodes weer een tekort. De agrarische sector staat rondom dit thema voor grote uitdagingen. Naast het teveel of tekort aan water, zijn ook de waterkwaliteit, waddenagenda, verzilting en de toegang tot water belangrijke onderwerpen. Samen met onze partners en leden gingen we in discussie over dit ‘upcoming’ onderwerp. Of we het nu willen of niet: als agrarische sector kunnen we simpelweg niet om het thema water heen!

Een natuurbrand op de heide is nauwelijks te voorkomen. Maar heidebeheerders kunnen wel een belangrijke bijdrage leveren om een moeilijk beheersbare natuurbrand te voorkomen door een heideveld op een natuurlijke wijze te compartimenteren met brandstroken. Hierdoor ontstaan zogenaamde (af)brandvakken. Dat reduceert de bestrijdings- en herstelkosten en de brandweer blijft paraat voor andere incidenten in de omgeving.

43. Effect of text messaging on depression in patients with coronary heart disease: A substudy analysis from the TEXT ME randomised controlled trial

Abstract

Objective We aimed to evaluate the effects on depression scores of a lifestyle-focused cardiac support programme delivered via mobile phone text messaging among patients with coronary heart disease (CHD). Design Substudy and secondary analysis of a parallel-group, single-blind randomised controlled trial of patients with CHD. Setting A tertiary hospital in Sydney, Australia. Intervention The Tobacco, Exercise and dieT MEssages programme comprised four text messages per week for 6 months that provided education, motivation and support on diet, physical activity, general cardiac education and smoking, if relevant. The programme did not have any specific mental health component. Outcomes Depression scores at 6 months measured using the Patient Health Questionnaire-9 (PHQ-9). Treatment effect across subgroups was measured using log-binomial regression model for the binary outcome (depressed/not depressed, where depressed is any score of PHQ-9 ≥5) with treatment, subgroup and treatment by subgroup interaction as fixed effects. Results Depression scores at 6 months were lower in the intervention group compared with the control group, mean difference 1.9 (95% CI 1.5 to 2.4, p<0.0001). The frequency of mild or greater depressive symptoms (PHQ-9 scores≥5) at 6 months was 21/333 (6.3%) in the intervention group and 86/350 (24.6%) in the control group (relative risk (RR) 0.26, 95% CI 0.16 to 0.40, p<0.001). This proportional reduction in depressive symptoms was similar across groups defined by age, sex, education, body mass index, physical activity, current smoking, current drinking and history of depression, diabetes and hypertension. In particular, the rates of PHQ-9 ≥5 among people with a history of depression were 4/44 (9.1%) vs 29/62 (46.8%) in intervention vs control (RR 0.19, 95% CI 0.07 to 0.51, p<0.001), and were 17/289 (5.9%) vs 57/288 (19.8%) among others (RR 0.30, 95% CI 0.18 to 0.50, p<0.001). Abstract

Introduction Simple and scalable strategies are needed to improve a € out-of-hospital' support and management for people living with cardiovascular disease (CVD) and respiratory disease. Text messaging via mobile phones has been shown to be effective in helping promote lifestyle change and is supported by quantitative and qualitative evidence. The aim of this study is to test the effectiveness and implementation of a 6-month text messaging support programme for people with CVD and respiratory disease as an addition to cardiac and pulmonary outpatient rehabilitation. Methods and analysis Pragmatic randomised controlled trial (n=310) to test the effectiveness of a 6-month text message support programme on clinical outcomes in people with CVD and chronic respiratory disease who are attending outpatient cardiac and pulmonary rehabilitation. The study includes a nested process evaluation to inform scalability and implementation across settings. The intervention group will receive a text message support programme comprising five messages per week for 26 weeks and the control group will continue with standard care. The primary outcome is exercise capacity (6 min walk distance). Secondary outcomes include clinical measures (proportion of people meeting the Australian guideline-recommended blood pressure and cholesterol targets), lifestyle outcomes (smoking rates, achievement of national guidelines for nutrition and physical activity), quality of life, mood (Hospital Anxiety and Depression Scale), medication adherence and attendance at and completion of rehabilitation. Ethics and dissemination Primary ethics approval was received from the Sydney Local Health District Hospital Human Research Ethics Committee and associated Governance committees at sites.
