Background: Children with intellectual disabilities (IDs) have a severe delay in syntactic development compared with other language abilities. This study investigated conditions of syntactic development in native Japanese‐speaking children with ID. Methods: Children with ID [N = 51; 18 autism spectrum disorders (ASD), 18 Down syndrome (DS) and 15 ID without ASD and DS] were compared with typically developing children (N = 78) with the same mental age (MA). The development of syntax in spoken language was examined by receptive and production tasks. Results: The development of syntax in children with ID was significantly delayed than in typically developing children with the same MA. However, when reaching the MA of 7–9, syntax abilities started to develop remarkably. Moreover, children with ASD had significant difficulties in acquiring passive voice, whereas children with DS showed a significant delay in syntactic development. Conclusions: The development of syntax in children with ID might be affected by MA and the type of disability. A large-volume (ϕ 12.0 cm × 12.7 cm) LaBr 3 scintillation detector equipped with a specially-designed radiation shield was evaluated for neutron resonance capture analysis at the neutron time-of-flight (TOF) facility GELINA. By using the LaBr 3 detector with and without the shield, measurements were carried out at a 13-m TOF station with three metallic samples, namely, Ni, Cr, and Fe. In addition, Monte Carlo simulations with Geant4 were performed, and the results were compared with the measurements to analyze the observed energy spectra and TOF spectra. Energy spectra obtained with the shield showed that prompt γ -ray peaks emitted from each sample can be used to identify the isotopes. Moreover, the signal-to-noise ratios of resonance peaks in a TOF spectrum with the shield were enhanced 1.5–2.5 times in comparison with those without the shield. Furthermore, simultaneous measurements conducted using the three samples demonstrated that the shield employed herein was indispensable for identifying impurities in a composite sample such as particle-like fuel debris. Lattice nuclei of III-V type semiconductors can be self-polarized through the contact hyperfine interaction with artificially excited conduction electrons at extremely low temperatures. In the self-polarization process, a positive feedback between the nuclear magnetization and the expansion of the energy splitting of the conduction electron states due to this nuclear magnetization takes an essential role. Extending the original theoretical work on the self-polarization with linearly polarized (LP) light by Dyakonov and Perel,1 we examined the effect of the circularly polarized (CP) light on the phenomenon of the nuclear self-polarization. While no polarization is obtained with LP light above the critical temperature, the nuclear polarization is enhanced also above the critical temperatures with CP light. More important finding in the application of CP light is that the direction of the nuclear polarization can be controlled by the orientation of the excited conduction-electron polarization, which is easily manipulated by changing the degree of polarization of the excitation light. Related to this polarization change, it is also remarked that the nuclear polarization shows a hysteresis phenomenon below the critical temperature when the degree of the electron polarization is swept up and down. We report on the quantitative nondestructive analysis of the natural isotopic abundances of hafnium and tungsten elements using nuclear resonance fluorescence. Metallic samples of hafnium and tungsten were irradiated to six quasi-monochromatic γ -ray beams generated by laser Compton scattering in the energy range of 2.4–3.2 MeV. Multiple nuclei were simultaneously excited at each of the six γ -ray beam energies. A high-purity germanium detector array detected deexcitations of the nuclei. In total, 51 transitions were unprecedentedly employed to estimate the isotopic abundances of heavy elements nondestructively. The estimated abundances of three hafnium isotopes and three tungsten isotopes are consistent with standard known natural abundances within the experimental uncertainties. Purpose: To examine updated evidence on the efficacy and safety of mesh non-fixation in patients undergoing laparo-endoscopic repair of groin hernias. Methods: We searched MEDLINE, Cochrane Central Library, Embase, ClinicalTrials. gov, and ICTRP databases to identify randomized controlled trials. The primary outcomes were recurrence, chronic pain, and return to daily life. The certainty of evidence (CoE) was assessed by grading recommendations, assessments, developments, and evaluations. We performed a subgroup analysis based on the surgical type. This study was registered with PROSPERO (CRD 42022368929). Results: We included 25 trials with 3,668 patients (4,038 hernias) were included. Mesh non-fixation resulted in little to no difference in hernia recurrence (relative risk [RR]:1.40, 95% confidence interval [CI]:0.59–3.31; I2 = 0%; moderate CoE) and chronic pain (RR:0.48, 95% CI:0.13–1.78; I2 = 77%; moderate CoE), but reduced return to daily life (mean difference [MD]: − 1.79 days, 95% CI: − 2.79 to –0.80; I2 = 96%; low CoE). In subgroup analyses, the transabdominal preperitoneal approach (TAPP) (MD: − 2.97 days, 95% CI: − 4.87 to − 1.08; I2 = 97%) reduced return to daily life than total extraperitoneal inguinal approach (MD: − 0.24 days, 95% CI − 0.71 to 0.24; I2 = 61%) (p = 0.006). Conclusions: Mesh nonfixation improves the return to daily life without increasing the risk of hernia recurrence or chronic pain. Surgeons and patients may discuss mesh nonfixation options to accommodate a patient's desired return to daily life. Further trials focusing on TAPP are required to confirm these findings. A fast neutron detector system that can figure out neutron source direction is developed. The system consists of two rod-shaped fast neutron detectors and one polyethylene moderator. Pulse shape discrimination plastic scintillators are applied for reduced gamma-ray background and improved system sensitivity for fast neutron detection. The neutron detectors are placed on the back and front sides of the moderator facing a neutron source. Experiments are conducted for a single rod-shaped detector and the system changing the PuBe neutron source direction. Simulation-based studies are also performed using the Monte Carlo simulation code, PHITS (Particle and Heavy ion Transport code System). Taking the ratio of counts of the front and back system detectors, linear change with an increased detector angle is observed, except for near 0° and 90°. This enables us to deduce neutron source direction from several measurements. The method is probably useful for a quick and efficient survey of neutron sources or special nuclear materials. Purpose: This retrospective study evaluates the clinical course and outcomes of patients who underwent surgery for strangulated hernias. Methods: Among 520 groin hernias from 2001 to 2012, 51 inguinal and 42 femoral hernias were strangulated and operated emergently at a tertiary referral center. Perioperative factors, patient profiles, and time interval to surgery ( T = time from onset to surgery, T = time from onset to initial evaluation, T = time from the first hospital to the tertiary center, T = time from admission at the tertiary center to surgery, T = T + T + T) were analyzed in patients with strangulation, then compared between two groups, the bowel resection (BR) group and the non-bowel resection (NBR) group. Results: T, T and T in the bowel resection group were significantly longer than those in the non-bowel resection group ( P < 0.05). Patients who presented initially to the tertiary center ( T = 0) had a significantly lower resection rate than patients transported from other hospitals (24 vs. 44 %, P = 0.048). There was no significant difference in morbidity between the BR and NBR groups (35 vs. 24 %, P = 0.231). Conclusions: The elapsed time from onset to surgery, especially T and T, is the most important prognostic factor in patients with strangulated groin hernias. Abstract: We report on the development of a front-end ASIC for high spatial resolution PET detectors with time-of-flight capability based on LYSO scintillator arrays coupled with position-sensitive avalanche photodiode (APD) arrays. The ASIC is designed based on the open-IP LSI project led by JAXA and realized in TSMC CMOS technology. It consists of an 8-channel charge-sensitive amplifier, band-pass filters, differentiators, pulse-height and timing discriminators, and two-channel time-to-amplitude converters. As a result, energy resolution of 9.7% (FWHM) is obtained at 511keV, with a time resolution below 970ps . We will also report on the current status of developing a second-version ASIC designed to have 32-channel analog circuits with improved time resolution. [Copyright &y& Elsevier]

Abstract: Avalanche photodiodes (APD) offer advantages in terms of weak scintillation detection, fast time response, and magnetic field insensitivity. We have developed new types of large-area, reverse-type APD-arrays specifically designed for high-resolution positron emission tomography (PET). Each device has a monolithic (or ) pixel structure with an active area of 1.0 (or 4.0, 0.25) for each pixel. An excellent gain uniformity () and low dark noise () have been achieved, measured at room temperature. Energy resolution of (FWHM) was obtained for the direct detection of 5.9keV X-rays, while (FWHM) was obtained for 662keV gamma-rays when coupled with a LYSO scintillator matrix. An excellent time resolution of 102ps (FWHM) was obtained for a monolithic, APD pixel. These results suggest that APD-arrays could be a promising device for future applications in nuclear medicine. Abstract: Resonance enhanced multi-photon ionization (REMPI) of neutral aluminum atoms sputtered with gallium focused ion beam (Ga-FIB) was studied in terms of substrate temperature and chemical state of the surfaces. Aluminum has the lowest excitation state (3p 2P3/2) at 112cm−1 above the ground state (3p 2P1/2). The results showed that the total REMPI signal intensity of neutral aluminum atoms and the ratio of REMPI signal intensities attributed to 2P1/2 to 2P3/2 were increased at higher temperature. On the other hand, the REMPI signal and the ratio were decreased in the case of partially oxidized aluminum surfaces. Considering the result on Al2O3, it was confirmed that the REMPI signals of ground state 2P1/2 and the first excited spate 2P1/2 could be affected with surface oxidation state. Abstract: We comprehensively re-analyzed the toxicity data for 18 industrial chemicals from repeated oral exposures in newborn and young rats, which were previously published. Two new toxicity endpoints specific to this comparative analysis were identified, the first, the presumed no observed adverse effect level (pNOAEL) was estimated based on results of both main and dose-finding studies, and the second, the presumed unequivocally toxic level (pUETL) was defined as a clear toxic dose giving similar severity in both newborn and young rats. Based on the analyses of both pNOAEL and pUETL ratios between the different ages, newborn rats demonstrated greater susceptibility (at most 8-fold) to nearly two thirds of these 18 chemicals (mostly phenolic substances), and less or nearly equal sensitivity to the other chemicals. Exceptionally one chemical only showed toxicity in newborn rats. In addition, Benchmark Dose Lower Bound (BMDL) estimates were calculated as an alternative endpoint. Most BMDLs were comparable to their corresponding pNOAELs and the overall correlation coefficient was 0.904. We discussed how our results can be incorporated into chemical risk assessment approaches to protect pediatric health from direct oral exposure to chemicals. [Copyright &y& Elsevier]

Background and objective: As pharmacokinetic drug interactions frequently cause adverse events, it is important that the relevant information is given in package inserts (PIs). We previously analysed the provision of PIs for HMG-CoA reductase inhibitors and Ca antagonists, for which metabolism by cytochrome P450 could be a major interaction mechanism. In this article, we focus on interactions involving glucuronoconjugates because many drugs and their metabolites undergo this conjugation. Methods: We reviewed clinical drug interactions related to glucuronoconjugates, focusing on reports of adverse events. Then, we picked out three important drugs (zidovudine, valproic acid and lamotrigine), and examined how the literature information is reflected in the relevant PIs in Japan, UK and USA. Results and discussion: Pharmacokinetic interactions related to glucuronoconjugates were found with 33 drug combinations. Of these, five combinations induced clear adverse events: (i) severe anaemia due to zidovudine and caused by interaction with valproic acid, (ii) recurrence/increased frequency of seizure or increased manic states from a reduction in therapeutic effects of valproic acid caused by panipenem, (iii) meropenem or (iv) ritonavir and (v) of lamotrigine caused by oral contraceptives. Analysis of PIs showed a lack of description of the interaction of zidovudine with valproic acid in the Japanese PI. The UK PI mentioned this interaction without quantitative data, whereas full information was given in the US PI. A lack of description was also present on the interaction between valproic acid with ritonavir, reported in 2006, in the PIs of all three countries. For the interactions involving valproic acid and panipenem or meropenem, even though marked reduction of blood valproic acid level has been reported, no quantitative data were provided in any of the PIs. Conclusion: Five combinations were identified to cause severe adverse events because of interactions related to glucuronoconjugates. This information, including quantitative data, is not always properly provided in the relevant PIs in Japan, UK or USA. Statins (HMG-CoA reductase inhibitors) are one of the most widely prescribed classes of drugs throughout the world, because of their excellent cholesterol-lowering effect and overall safety profile except for rare but fatal rhabdomyolysis arising either directly or indirectly by pharmacokinetic interactions with certain other drugs. As package inserts in pharmaceuticals are the primary source of information for health care providers, we carried out a literature search to examine how crucial information was provided in package inserts of five statins approved in Japan (simvastatin, atorvastatin, fluvastatin, pravastatin and pitavastatin).A MEDLINE search from 1996 to June 2004 was carried out to identify studies on clinical pharmacokinetic drug interactions for the five statins. We mainly collected information on area under plasma concentration (AUC) following co-administration of statins with other drugs. The current package inserts used in Japan were obtained from the website of the Pharmaceutical and Medical Device Agency whereas USA package inserts were obtained from the Food and Drug Administration website.The majority of package inserts listed the drugs that interacted with statins with most describing the risk of rhabdomyolysis because of the possibility of increases in blood concentration. However, quantitative information such as change in AUC was provided in only a few cases. Instructions for dosage adjustment are seldom provided in the Japanese package inserts. USA package inserts list almost identical drug interactions as the Japanese package inserts, although they contain more quantitative data, especially for typical cytochrome P450 (CYP) inhibitors.All pharmacokinetic drug interactions including relevant quantitative data for potential effectors and details on mechanisms of interaction need to be given in package inserts as soon as the information becomes available, to ensure safe and proper use of the drugs concerned. Coulomb excitation experiment was carried out with a 68Zn beam bombarding a natPb target. Two E2 matrix elements and the quadrupole moment of the 21+ state were newly derived with the least-squares search code GOSIA. The potential energy surface (PES) was calculated with the Nilsson–Strutinsky model, showing two shallow minima: the first minimum does not contain the 1g9/2 orbit below the Fermi surface, while the second minimum does. The ground state band and the intruder band seem to be constructed on the first and the second minimum, respectively. As for the ground state band, the asymmetric rotor model and the IBM in O(6) limit reproduced the experimental values rather well. The shallow PES may suggest instability of the shape. A Coulomb excitation experiment was carried out with a 66Zn beam bombarding a matPb target. Four E2 matrix elements and the quadrupole moment of the 2+1 state were derived with the least-squares search code GOSIA. According to the B(E2) values, the ground band can be interpreted as a quasirotational band. An ohmic-heating laser ion source with a thin ionizer of thickness of 30 μm has been developed for the TIARA-ISOL. It can form an electric field of 4–5 V/cm inside the ionizer. The properties of the laser ion source were tested on- and off-line with aluminum isotopes. The FWHM of the time distribution of the bunched photoions from the ion source was about 4 μs for 27Al. In on-line experiment, a photoionization efficiency of about 0.1% for 25Al was obtained. [Copyright &y& Elsevier]

Objective To assess the level of a bone-formation marker, the amino-terminal propeptide of type I procollagen (PINP), for its utility in indicating bone metastasis in patients with prostate cancer. Patients and methods Several bone formation markers, i.e. PINP, the carboxy-terminal propeptide of type I procollagen (PICP), bone-specific alkaline phosphatase (BALP), and bone Gla protein (BGP), a bone resorption marker (pyridinoline cross-linked carboxy-terminal telopeptide, ICTP), and prostate specific antigen (PSA) were measured in 40 patients without and 25 patients with bone metastasis. No patient had undergone previous treatment, except for six who developed bone metastasis while undergoing hormone therapy. Results All markers except BGP were significantly higher in patients with bone metastasis than in those without. The levels of PINP correlated best with the extent of disease, although the levels of PSA, BALP and ICTP also correlated well. While PINP had the largest area under the receiver-operator characteristic curve, PSA, BALP and ICTP also produced useful curves. Conclusion The bone formation marker PINP seems to be useful for discriminating patients with and without bone metastasis. Background: The effects of isoflurane and halothane anesthesia on hepatic circulation and oxygen metabolism during hepatolobectomy were investigated in the dog, in an attempt to assess which of the anesthetics was the better one for hepatic resection.Methods: Mongrel dogs (n=24) were divided into two groups and accordingly anesthetized with isoflurane (n=12) or halothane (n = 12). Each test anesthetic was administered in air. Electromagnetic flowmeters were used to measure hepatic arterial and portal venous blood flows 1) before the inhalation of each anesthetic (baseline); 2) 1 h inhalation of 1.5 MAC (minimum alveolar concentration) of each anesthetic; and 3) 1 h after hepatolobectomy with each anesthesia. Measurements of systemic hemodynamics, blood gas tensions, and the arterial ketone body ratio were made at the same time.Results: Isoflurane maintained portal venous, hepatic arterial and total hepatic blood flows better than halothane anesthesia before and after hepatolobectomy. With halothane anesthesia, hepatolobectomy decreased prominently hepatic arterial blood flow. Hepatic arterial and mesenteric vascular resistance increased in the halothane group, but remained constant in the isoflurane group after hepatolobectomy. Hepatic oxygen delivery was significantly suppressed in the halothane group, but did not change in the isoflurane group. No significant difference was found in hepatic oxygen consumption between the two groups, but the arterial ketone body ratio decreased significantly only in the halothane group before and after hepatolobectomy.Conclusion: The present data indicate that isoflurane has less adverse effect than halothane anesthesia on hepatic circulation, oxygen delivery and energy charge in hepatolobectomy cases. The distribution of fluctuation of sugars in germination barley seeds were examined by 13C nuclear magnetic resonance (NMR) spectroscopy, ¹H-NMR imaging and ¹H-NMR localized spectroscopy in relation to morphology. Maltose, sucrose, fructose and oils were detected in intact imbibed seeds by 13C-NMR spectra. During the first 6d of germination, the maltose content increased and the oil content gradually decreased, whilst the levels of sucrose and fructose remained constant. Sugars were located by ¹H-NMR images and ¹H-NMR localized spectra in the vascular bundle of the seeds as well as in the solubilized endosperm. They are also detected in the shoots. The sugars detected in an 80% ethanol shoot extract were sucrose and glucose, which were located in the vascular bundles but not in the mesophyll cells of the coleptile. They were also located in the basal part of the shoot, but not above 7 mm from the scutellum. The data suggest that the sugars are primarily transported through the vascular bundles and, at the same time, rapidly incorporated into mesophyll cells in the leaves. [ABSTRACT FROM AUTHOR]

Activation spectra of photochemical reactions were measured by a flash spectrophotometer in leaves having varying chlorophyll contents at different stages of greening. The increase of chlorophyll concentration up to 30 nmol cm-2 elevated the rates of photochemical reactions at all wavelengths of light used, and was round to be produced by an increase in the amounts of reaction centres. Further accumulation of chlorophyll up to 40 nmol cm-2 was associated with an increase in light-harvesting chlorophyll, an improved rate of photochemical reactions around 600 nm and at 700 nm, and self-absorption and screening effects where chlorophyll absorbed maximally (400-450 nm and around 680 nm). In order to improve the sensitivity for the quantification of trace elements, we propose the combination of prompt gamma-ray analysis (PGA) and a multiple gamma-ray detection method. A new Ge detector system for multiple prompt gamma-ray analysis (MPGA) was constructed at the neutron guide-hall of the JRR-3M reactor of the Japan Atomic Energy Agency (JAEA). The first demonstration of this system was given with a plastic sample containing traces of cadmium. The quantification limit of cadmium in a plastic sample was found to be about 0.1 ppm. Intravascular malignant lymphocytosis (IML) is a rare systemic disease characterized by proliferation of malignant B (rarely T) lymphoid cells within the lumina of small arteries, veins, and capillaries. Diagnosis requires skin, liver, renal, meningeal, or brain biopsy, but is rarely made ante mortem. In this report, we describe a patient who had an ante mortem diagnosis of IML as a result of a skin biopsy. Autologous peripheral blood stem cell transplantation (auto-PBSCT) was successfully performed after chemotherapy. The patient has survived for more than 30 months since the onset of the disease and maintains complete remission on the 450th day post PBSCT. We propose neutron resonance fission neutron analysis (NRFNA), an active nondestructive assay (NDA) technique, to improve the capability to identify and quantify a small amount of fissile material in a sample. NRFNA uses pulsed neutrons to induce fission reactions in the sample. Fission neutrons are detected by a neutron-gamma pulse shape discrimination (PSD) scintillation detector with time-of-flight (TOF) technique. The obtained nuclide-specific resonance peaks in the neutron energy spectrum provide information to identify and quantify a fissile material in the sample. The possibility of using PSD for NRFNA was confirmed through a test experiment using a natural uranium sample. External causes and hence rates, of morbidity and mortality have been increasing substantially in Brazil and resulting in heavy economic and social burdens, among them hospital costs. To investigate and quantify hospitalizations for injuries and poisoning in the Unified Health System (UHS) hospitals or those licensed by it in Brazil and in the state of Sao Paulo, in order to determine the epidemiological characteristics of these patients and the hospital costs to the government, based on variables considered important. Hospitalizations in UHS or UHS licensed hospitals in the year 2000 were studied.

The impact of systematic effects on the areal density derived from a neutron resonance transmission analysis (NRTA) is investigated by measurements at the time-of-flight facility GELINA. The experiments were carried out at a 25 m station using metallic natural Cu discs with different thicknesses. To derive the areal density from a fitting to the experimental transmission, the resonance shape analysis code REFIT was used. Large bias effects were observed using recommended resonance parameters. Therefore, neutron resonance parameters, in particular resonance energies and neutron widths, were derived from the transmission data obtained with a 0.25 mm thick Cu metallic sample. These parameters were used to study the impact of the resonance strength and sample thickness on the accuracy of the areal density derived by NRTA. [ABSTRACT FROM AUTHOR]

Abstract: Neutron resonance densitometry (NRD) has been proposed to quantify nuclear materials in melted fuel (MF) that will be removed from the Fukushima Daiichi nuclear power plant. The problem is complex due to the expected presence of strong neutron absorbing impurities such as 10B and high radiation field that is mainly caused by 137Cs. To identify the impurities under the high radiation field, NRD is based on a combination of neutron resonance transmission analysis (NRTA) and neutron resonance capture analysis (NRCA). We investigated with Geant4 the performance of a gamma-ray detector for NRCA in NRD. The gamma-ray detector has a well shape, consisting of cylindrical and tube type LaBr3 scintillators. We show how it measures 478keV gamma rays derived from 10B(n, ) reaction in MF under a high 137Cs-radiation environment. It was found that the gamma-ray detector was able to well suppress the Compton edge of 662-keV gamma rays of 137Cs and had a high peak-to-Compton continuum ratio, by using the tube type scintillator as a back-catcher detector. Then, we demonstrate that with this ability, detection of 478-keV gamma rays from 10B is accomplished in realistic measuring time. [Copyright &y& Elsevier]

Introduction Laparoscopic splenectomy using pneumoperitoneum has been performed since 1992. The gasless abdominal wall-lifting method for laparoscopic splenectomy was introduced as an alternative. This retrospective study was undertaken to compare results using the two techniques. Methods Between 1995 and 2010, 54 patients underwent laparoscopic splenectomy at a single institution; 30 underwent the procedure using the gasless technique and 24 using pneumoperitoneum. There were no significant differences between the two groups regarding age, sex or BMI, but more patients underwent concurrent operations in the pneumoperitoneum group. The abdominal wall-lift system with subcutaneous K-wires was used for the gasless method. Results Intraoperative blood loss was similar in the two groups (193.0 ± 196.7 mL gasless, 217.3 ± 296.6 mL pneumoperitoneum; P > 0.05), but operative time (182.1 ± 92.1 min, 135.1 ± 46.1 min; P < 0.05), and resected spleen weight (306.1 ± 297.7 g, 138 ± 81.0 g; P < 0.05) were significantly different. In the gasless group, additional procedures included conversion ( n = 1), mini-laparotomy ( n = 2), and CO2 insufflation ( n = 2). Excluding the concurrent living-related kidney donor patients, hospital stay was similar (6.9 ± 2.5 days, 6.3 ± 2.0 days, P > 0.05). Conclusion Although gasless laparoscopic splenectomy is feasible, there are disadvantages, particularly the restricted operative working space in some patients. BACKGROUND: Wires and cables have been used extensively for spinal sublaminar wiring, but damages to the spinal cord due to compression by metal wires have been reported. We have used more flexible ultra-high-molecular-weight polyethylene cable (Tekmilon tape) instead of metal wires since 1999 and have obtained good clinical outcomes. Although the initial strength of Tekmilon tape is equivalent to metal wires, the temporal changes in the strength of Tekmilon tape in the body should be investigated to show that sufficient strength is maintained over time until bone union is complete. METHODS: Tekmilon tape was embedded into the paravertebral muscle of 10-week-old male Japanese white rabbits. Samples were embedded for 0, 1, 3, 6 or 12 months. At the end of each period, sequential straight tensile strength and sequential knot-pull tensile strength were measured. FINDINGS: The initial strength of Tekmilon tape in muscle tissue was maintained over time, with 92% straight tensile strength and 104% knot-pull tensile strength at 6months, and values of 77% and 100% at 12 months, respectively. Since single knot is clinically relevant, it is very important that the knot-pull tensile strength did not decrease over a 12-month period. This suggests that temporal changes in the tensile strength of Tekmilon tape are negligible at 1 year. INTERPRETATION: Tekmilon tape maintains sufficient strength in vivo until bone union has occurred. Background: No known physical findings are available to differentiate between bacterial pneumonia (BP) and atypical pneumonia (AP) in patients with community-acquired pneumonia (CAP). Objective: To evaluate the possible differences in phasic characteristics of inspiratory crackles between BP and AP in patients with CAP. Methods: Retrospective chart reviews were conducted to obtain phasic characteristics of inspiratory crackles (early, early-to-mid, late and pan-inspiratory crackles) in AP and BP groups in a community teaching hospital in Japan (n = 183). Results: 100 patients with BP and 83 patients with AP were evaluated. Patients with BP were significantly more likely to present with pan-inspiratory crackles (49 (49.0) vs 5(6.0); p<0.0001), whereas patients with AP were more likely to present with late inspiratory crackles (28 (33.7) vs 9(9.0); p<0.0001) (mean (SD)). Among pneumonia patients with audible crackles, the sensitivity and specificity of pan-inspiratory crackles for BP were 83.1% and 85.7%, respectively, and the sensitivity and specificity of late inspiratory crackles for AP were 80.0% and 84.7%, respectively. Discussion: In patients with CAP and audible crackles, phasic characteristics of inspiratory crackles may be used to distinguish AP from BP. Prospective studies are needed to confirm these findings. In order to improve the limits of quantification for trace elements, we applied the multiple gamma-ray detection method in the prompt gamma-ray analysis (PGA). The Clover Ge detector system was constructed for multiple prompt gamma-ray analysis (MPGA) at the neutron guide hall of the JRR-3M reactor of the Japan Atomic Energy Agency (JAEA). The MPGA system consists of three Clover detectors and a BGO Compton suppressor. The instrumentation of the MPGA beam line and the detector system were developed. The estimated limit of quantification of cadmium in the plastic sample is approximately 0.1 ppm. Abstract: The Cd concentration in food is a public concern related to the human health. In order to remove Cd-polluted food, the development and validation of a rapid and sensitive method of Cd analysis is required. By applying the multiple γ-ray detection method to prompt γ-ray analysis (PGA), the influence from nuclei which emit only one prompt γ-ray at a time at every neutron capture reaction can be reduced, therefore the quantification limit of Cd is improved significantly. The limit of Cd contained in rice in the case of MPGA was evaluated, and under our proposed experimental conditions, it may be possible to quantify Cd content in rice to within 0.2ppm in 10min. Background: We sought to validate computed tomographic virtual pancreatoscopy (CT-VP) created by multidetector row CT (MD-CT) in the clinical diagnosis of intraductal papillary mucinous neoplasm (IPMN) of the pancreas. Methods: Five cases of pancreatic IPMNs were included in this study. A nasopancreatic drainage tube was inserted and the pancreatic duct was filled with contrast medium, after which an upper abdominal scan was performed by MD-CT. CT-VP and three-dimensional (3D) CT pancreatographic images were created using a workstation and compared with images by conventional diagnostic techniques. All cases were evaluated by endoscopic retrograde pancreatography (ERP) and three cases of main duct type were assessed by intraoperative real pancreatoscopy (RP). Results: In the main duct cases, papillary projections in the main pancreatic duct and branch orifices were clearly detected by CT-VP. These lesions and structures were confirmed by intraoperative RP, and the CT-VP images were clearer than RP images. In branch cases, a surface-rendering method allowed protruding lesions to be clearly detected in the dilated branches. Conclusion: Compared with conventional ERP or RP, CT-VP and 3D-CT pancreatographic images were finer in quality, and the procedures were less invasive, faster, and less expensive. The potential shown by CT-VP with 3D-CT pancreatography in the clinical diagnosis of pancreatic IPMNs suggests that this approach may replace ERP in the near future. [ABSTRACT FROM AUTHOR]

Fermentation processes of bread doughs were traced by MRI (magnetic resonance imaging), which suggests that punching and rounding treatment after the first fermentation promotes the development of elasticity and extensibility of gluten fibrils that makes thin grain walls of the pores connecting between adjacent pores with small holes, and a thin smooth crust after baking. While pore generation was small, expansion of the dough was low, gluten networks formed were large and coarse, and gluten fibrils were undeveloped in the frozen dough. Prefermentation could not essentially alleviate the inferior properties of the frozen dough. We report that antisense phosphorothioate oligodeoxyribonucleotides (PS-ODNs) against cyclic AMP response element-binding protein (CREB) induce the death of human leukemia cell lines including HL-60, Kasumi-1 and K562, OCI-AML1a and also primary leukemia cells isolated from patients with acute myelocytic leukemia and chronic myelocytic leukemia in blastic crisis. In contrast, normal human bone marrow CD34+ cells and normal peripheral blood lymphocytes were resistant to the antisense-mediated cell death. We found that antisense-treated HL-60 cells had prominent nuclear fragmentations but lacked apoptotic features including internucleosomal DNA cleavage and TUNEL positivity. Cell cycle analysis demonstrated a remarkable reduction in G1 phase population along with a mild accumulation of S phase and good preservation of G2/M phase, indicating cells died at G2/M without cycling into G1 phase. None of the sense-sequenced PS-ODNs induced cell death. Further, neither the expression nor the message of CREB protein was reduced by antisense treatment, indicating that cell death was mediated by a non-antisense mechanism. On the other hand, no consensus oligonucleotide sequence for cell death induction was detected. Rather, we found a good correlation between the melting temperatures and the anti-proliferative activities of the oligonucleotides. To establish an irradiation system to enable the ultra-high dose-rate (uHDR; FLASH) scanned carbon-ion irradiation with a compact type medical synchrotron. After technical adjustments of the accelerator operation parameters, the medical synchrotron extracted ≥1.0 × 109 carbon-ions (2.0 nC/spill) at 208.3 MeV/u (86 mm in range) within a maximum 100 ms. The uHDR carbon-ion beam delivery system was designed to prevent damage to monitors in the nozzle due to the uHDR beam by avoiding passage through the nozzle. The dose amount was adjusted by the extraction time of the total spill with a function generator. The carbon-ion beam was scanned once to create a field within the extraction time. Advanced Markus chamber and radiographic film were used to measure the absolute dose and the field size respectively, at a plateau depth. Operation voltage of the chamber was set to 400 V at the uHDR. The FLASH irradiation utilizing beam scanning was confirmed with a dose of 5.4 Gy (homogeneity of ±2%) for the beam in a volume of at least 16 mm × 16 mm in a square field and a corresponding dose-rate of 95 Gy/s (± 1%). The dose was changed to about 0.7, 1.5, 2.9 Gy for the dose-rate and the field size. Details of the results are shown in Table1. The FLASH dose-rates >40 Gy/s at different dose levels were achieved in a useful field size for research created by the scanned carbon-ion beams with the compact type medical synchrotron. This study reported promising beam characteristics of uHDR scanned carbon-ion beams, at different dose levels, and can be used for further investigation of carbon-ion FLASH research. This study investigated intermediate-spin states of 92Zr via the inverse reaction 9Be(86Kr,3n) 92Zr. Seven transitions were newly observed, and a lifetime was extracted for the 10+1 state by analysis of Doppler-broadened line shapes of decay γ rays. A large B(E2) value was obtained for the transition from 10+1 to 8+1, and the magnitude was comparable to that for the deformed excited configurations in 94Zr that have recently been established. A possible origin for such collectivity is discussed qualitatively based on a phenomenological deformed rotor model. Purpose: The purpose of this study was to examine the efficacy of an ovarian tissue transportation network for fertility preservation (FP) for cancer patients in Japan. Methods: PubMed was searched for papers on transportation of human ovarian tissue for FP. We analyzed population, area, number of cancer patients for ovarian tissue cryopreservation (OTC), quality control/assessment and safety, cost of a cryopreservation center for the building for 30 years, and medical fees of cancer patients (operation, cryopreservation, and storage of ovarian tissue). Results: More than twenty babies have been born in Denmark and Germany through a transportation system. Up to 400 new patients a year need OTC. The fees for removal, cryopreservation, and storage for 5 years, and transplantation of ovarian tissue are around €5,000, €4,000, and €5,000, respectively. It costs more than €5 million to establish and maintain one cryopreservation center for 30 years. If we have a few cryopreservation centers in Japan, we can cryopreserve 400 patients' ovarian tissue per year by safer slow freezing and maintain quality control/assessment. We need to lighten the patients' burden for easy to use FP by a government subsidy and medical insurance coverage. Conclusions: This model has been termed the Danish model ('the woman stays - the tissue moves'). This is truly patient-centered medicine. We can have maximum effects with the minimum burden. A transportation network like those of Denmark and Germany is the best strategy for FP in Japan. Hypertestosteroneemia can be caused by a variety of diseases and drugs, some of which can cause male infertility. We report a case of spinal and bulbar muscular atrophy (SMBA) diagnosed by hypertestosteronemia during infertility treatment. The patient was a 28-year-old man. His wife had no cause of infertility, did not become pregnant after 10 cycles of timed-intercourse. His semen test pointed out insufficient semen volume and asthenozoospermia. Although varicocele surgery was performed at the neighboring general hospital, semen findings did not improve. Even though two cycles of intrauterine insemination (IUI) were performed, she did not become pregnant. The couple were introduced to our clinic in November 2019. Semen findings at the first visit were same as first time. Serum hormone levels were LH 12.0 mIU / mL, FSH 3.9 mIU / mL and total testosterone 19.47 ng / mL. The testicular volume was 13 ml on both sides. SHIM: 22 points, EHS: Grade 3; the erectile function was a slight decreased. Drug-induced hypertestosteroneemia was negative. Microscopic pituitary adenoma revealed at the general hospital, but no pituitary dysfunction. The examination was conducted at the local national hospital, and a high value of CK 621 IU / L was pointed out. SMBA was suspected and the patient visited the neurology department. Finally, he had an allele of 49 CAG repeats and SMBA was diagnosed with mild bulbar palsy, weakness of the extremities Even with 3 courses of IUI, she did not become pregnant and was scheduled for in vitro fertilization (IVF). At the same time as the start of IVF, the patient had a definitive diagnosis of SMBA and was scheduled for LHRH agonist treatment. Because LHRH agonists were predicted to exacerbate semen findings, we performed sperm freezing for the purpose of fertility preservation. In the initial treatment cycle of ovum pick-up, 11 oocytes were collected, and 7 mature oocytes were obtained. Five oocytes were fertilized by ICSI using frozen sperm, and four blastocysts were vitrified. After that, the second vitrified-warmed blastocyst transplantation led to pregnancy, and the pregnancy is currently ongoing. SMBA, also known as Kennedy's Disease, affects adult men aged 30-60 years. It is a slowly progressive neuromuscular disease that causes muscle weakness, muscle atrophy, and fasciculation due to degeneration of lower motor neurons. This is a genetic disorder of X-linked recessive inheritance. It is caused by a trinucleotide repeat expansion; >40 CAG repeats, in the androgen receptor gene located on the X chromosome. Patients are mildly desensitized to androgens, causing gynecomastia, testicular atrophy, and diminished fertility. It is also accompanied by mild hypertestosteroneemia. These findings must have been the same for this patient. This report is the world's first report of fertility-preserving sperm freezing in SMBA patients using ejaculated sperm, and is also the first report of ICSI-induced pregnancy in frozen sperm in SMBA patients.
